[Proliferative vitreoretinopathy: curative treatment].

Proliferative vitreoretinopathy (PVR), which causes contractile fibrocellular membranes that may prevent retinal reattachment, remains one of the most severe complications of rhegmatogenous retinal detachment (RD), with an incidence of 5-11%, and one of the most frequent causes of surgical failure (50-75%). Its severity is due to the complexity of the surgery required to treat patients, and to its uncertain anatomic and functional prognosis. Curative treatment of PVR includes vitrectomy, sometimes associated with phacoemulsification or scleral buckling; systematic peeling of epiretinal membran

Risk factors for proliferative vitreoretinopathy.

Proliferative vitreoretinopathy (PVR) is one of the major causes of failure in retinal detachment surgery. To prevent PVR, it is necessary to determine factors predisposing its development. In primary PVR, large retinal tears, long duration of retinal detachment, vitreous hemorrhages, aphakia and choroidal detachment were demonstrated as clinical risk factors for PVR. In postoperative PVR, it was revealed that large breaks, pre- and postoperative choroidal detachment, minor intra- or postoperative hemorrhages, signs of uveitis, extensive retinal detachment, vitrectomy, cryopexy, air injection

Pathogenic mechanisms in proliferative vitreoretinopathy.

Landmarks in the development of treatment of retinal detachment include the recognition of the significance of retinal breaks that resulted in the distinction between rhegmatogenous retinal detachment (RRD) and exudative retinal detachment, the development of indirect ophthalmoscopy and scleral depression that allow better visualization of the peripheral area of the retina and better identification of retinal breaks, the recognition of the need to seal all retinal breaks, and the realization that vitreous traction is the major underlying cause of retinal tears and RRD. Scleral buckling procedu

[Clinical diagnosis of proliferative vitreoretinopathy].

The clinical features of a proliferative vitreoretinopathy (PVR) are systematically described. The patient's history reveals moderate lost of vision and metamorphopsia. Ophthalmoscopy, biomicroscopy of the fundus with the contact lens, and black-and-white photography usually lead to the correct diagnosis. Early stages of PVR are characterized by increased reflectance and a cellophane appearance of the inner retinal surface and tortuosity of both small and larger vessels. Advanced epiretinal tissue formation causes development of surface wrinkling and single or multifocal star-folds. In cases o

[Horseshoe tears and proliferative vitreoretinopathy].

Eyes at the risk of developing severe PVR postoperatively are limited to those affected with retinal detachment associated with horseshoe tears. This study is conducted to determine the relationship of horseshoe tears and PVR. 101 failure cases following surgery of retinal detachment with simple horseshoe tears were reviewed. The PVR group was compared with the group without PVR. The results indicate that vitreous gel liquefaction and posterior vitreous detachment are essential reasons of retinal detachment with horseshoe tears. Vitreous hemorrhage, uveitis, choroidal detachment and multiple o

[Risk factors of proliferative vitreoretinopathy].

Presented is the personal material comprising cases treated surgically for primary retinal detachment; analysed is the causal connection between postoperatively occurring proliferative vitreoretinopathy and the risk factors. The comparison of personal results with results of the other authors shows that in case of diagnosis of risk factors the reduction of the surgical trauma reduces the threat of proliferative vitreoretinopathy after operation of retinal detachment.

[Peripheral retinectomy in proliferative vitreoretinopathy].

In most cases of advanced proliferative vitreoretinopathy (PVR) relief of anterior traction can be achieved by careful dissection of the vitreous base. In more complicated cases, a peripheral, usually inferior retinectomy is necessary. We have analyzed retrospectively 51 cases of advanced PVR, in which we performed a combined vitrectomy-silicone oil procedure with peripheral retinectomy. In 90% of the cases, the retinectomy was done during a reoperation. In 24 of 51 patients (47%) the crystalline lens could be preserved until removal of the silicone oil. At the end of follow-up (more than 6 mo

Proliferative vitreoretinopathy and chemotherapeutic agents.

Proliferative vitreoretinopathy (PVR) is a disease process that occurs in eyes with rhegmatogenous retinal detachments and accounts for the majority of failures following retinal detachment surgery. PVR involves the uncontrolled proliferation of non-neoplastic cells capable of forming membranes, which may occur on either surface of the retina or along the detached surface of the vitreous gel. Contraction of these membranes creates tractional forces that can distort or detach the retina. Various surgical procedures have been used to repair retinal detachments associated with PVR. The results ha

Surgical treatments of proliferative vitreoretinopathy.

Four hundred ten eyes with retinal detachment and proliferative vitreoretinopathy underwent closed vitrectomy with membrane peeling, scleral buckling, and intraocular air injection. The retina was reattached in 243 eyes (59.3%). Useful vision was attained in 223 eyes. The preoperative proliferative vitreoretinopathy was clinically graded in six groups of increasing severity (C-1, C-2, C-3, D-1, D-2, D-3). From grades C-2 to D-3, a significant trend related a decreased rate of retinal reattachment with increased severity. However, grade C-1 showed a significantly lower success rate than did gra

A new model of proliferative vitreoretinopathy.

PURPOSE: To design a new model of proliferative vitreoretinopathy (PVR) that would not rely on the addition of exogenous cells. The release of endogenous cells from surrounding attachments seems to be an early event in the pathogenesis of PVR. Because the proteolytic enzyme dispase dissociates tissues, the hypothesis was that an intraocular injection of dispase could trigger events that would cause PVR. The requirement for a surgical retinal break at the time of dispase injection was also examined. METHODS: One eye of Dutch Belted rabbits was injected with 0.003 U to 1.0 U dispase in the subr

[Periostin in the Pathogenesis of Proliferative Vitreoretinopathy].

Proliferative vitreoretinopathy (PVR) is a serious complication of retinal detachment and vitreoretinal surgery. The hallmark of PVR is the formation of subretinal and epiretinal fibrotic membranes that can lead to traction retinal detachment due to their contractile properties. Surgical removal of the fibrotic membranes and retinal detachment repair are the first choice treatments for PVR. Despite recent progress in surgical techniques, recurrent detachment can lead to irreversible damage and a poor prognosis for visual acuity. Therefore, it is important to develop new molecular targeting the

Fulminant proliferative vitreoretinopathy in syphilitic uveitis.

BACKGROUND: Syphilis is a reemerging sexually transmitted disease that can lead to any type of intraocular inflammation. Prognosis of syphilitic uveitis after appropriate therapy is classically regarded as favorable. However, visual threatening complications may develop, rarely including rhegmatogenous/tractional retinal detachment (R/T RD) and proliferative vitreoretinopathy. FINDINGS: We report 4 patients presenting with complex R/T RD and fulminant proliferative vitreoretinopathy despite treatment among 19 patients with syphilitic posterior uveitis consecutively seen at our uveitis service

[Proliferative vitreoretinopathy: pathophysiology and clinical diagnosis].

Proliferative vitreoretinopathy (PVR) remains one of the most common causes of failed retinal detachment (RD) surgery. Many histological and clinical studies have highlighted the chain of events leading to PVR: cellular migration into the vitreous cavity, cellular differentiation, myofibroblast proliferation and activation, synthesis of extracellular matrix proteins, then contraction of preretinal tissues. The development of PVR can be explained schematically by cellular exposure to growth factors and cytokines (particularly retinal pigment epithelial cells and glial cells), in the context of

Clinical risk factors for proliferative vitreoretinopathy.

We reviewed the records of all patients with rhegmatogenous retinal detachments examined and treated by a single surgeon (B.P.C.) at the University of Virginia, Charlottesville, between 1978 and 1984. Of the 607 eyes that satisfied the selection criteria, a preliminary chart review of outcomes found that 65 (10.7%) had proliferative vitreoretinopathy and 34 had macular puckers. From the remaining 508 eyes, 325 controls were randomly selected to match each case from within a time window. Thirty-six (55.4%) of the 65 patients with proliferative vitreoretinopathy had had unequivocal reattachment

Proliferative Vitreoretinopathy: Pathophysiology and Therapeutic Approaches.

Proliferative vitreoretinopathy (PVR) is a complication of retinal detachment (RD) that is characterized by the development of retinal stiffness and contractile membranes on the surface or underside of the retina. It can occur in primary RD and make repair more challenging, or it can occur following initial successful RD repair and lead to re-detachment. Though our understanding of the pathophysiology underlying PVR membrane formation has grown based on cellular and animal models, there remains no currently approved medical therapy for treatment or prevention of PVR. Though some pharmacologic
