[Wounds and injuries to the colon].

On the basis of a retrospective analysis of results of treatment of 1097 patients with wounds and injuries of the colon of the peace and war time as well as of experiments in 160 dogs it has been established that the level of lethal outcomes and amount of complications are dependent on the size, number and localization of the colon wounds, severity of peritonitis by the moment of primary operation, degree of traumatic shock, blood loss volume, severity of the coexisting injuries and the chosen method of surgical treatment. A classification of the wounds according to the volume of injuries of t

Major venous injuries and bullet wounds.

A review of the reported incidence, management and results of treatment of major vein injuries is presented and a brief account given of the mechanism of vascular damage by high-velocity bullet wounds. This paper reports 3 cases of isolated major venous injury caused by high-velocity bullets. Repair was performed in each case. Two patients survived with a good short-term clinical result. The third patient died during operation. The advantages of repairing injured veins are discussed.

Stab-wounds and other penetrating injuries of the abdomen and thorax.

A series of 100 penetrating wounds of the abdomen and thorax is reviewed. Eighty-three were due to stab-wounds. The recent increase in incidence is confirmed and the distribution throughout the day and months of the year is given. The results and methods of treatment of thirty-six abdominal, ten thoraco-abdominal and thirty-seven thoracic wounds are given and described. An operative approach to abdominal wounds is supported. Conservative measures, including more frequent use of intercostal-tube drainage for pneumothorax and haemothorax, are satisfactory for the majority of chest wounds. Cardia

Practical Management of Common Skin Injuries, Lacerations, Wounds, Trigger Fingers, and Burns.

Primary care clinicians encounter many conditions during their day-to-day visits with patients. A few of these common presentations include burns, lacerations, trauma to the hand, and wounds, some of which do not require an evaluation by a specialist and can be managed outpatient by primary care clinicians. In this article, we share evidence-based tips to avoid common pitfalls in primary care recognition and management of such presentations as well as guide them to manage many of these conditions themselves. We also provide guidance in the decision to refer the patient to a plastic surgeon or

Determining injuries from posterior and flank stab wounds using computed tomography tractography.

Unlike anterior stab wounds (SW), in which local exploration may direct management, posterior SW can be challenging to evaluate. Traditional triple contrast computed tomography (CT) imaging is cumbersome and technician-dependent. The present study examines the role of CT tractography as a strategy to manage select patients with back and flank SW. Hemodynamically stable patients with back and flank SW were studied. After resuscitation, Betadine- or Visipaque®-soaked sterile sponges were inserted into each SW for the estimated depth of the wound. Patients underwent abdominal helical CT scanning,

Gunshot wounds to the thigh. Evaluation of vascular and subclinical vascular injuries.

A retrospective review of 173 patient charts, angiograms, surgical reports, and plain radiographs were performed for all patients admitted with gunshot wounds to the thigh from May 1988 to January 1991 at Martin Luther King, Jr. Hospital. A zonal classification of gunshot wounds to the thigh was created and defined to determine if a zonal classification was predictive of a positive angiogram. Associations and relationships of patients with vascular injury are presented.

Concomitant injuries of the hip joint and abdomen resulting from gunshot wounds.

Septic arthritis is a devastating complication of transabdominal gunshot wounds to the hip. Five patients sustained gunshot wounds to the hip which violated the alimentary tract. Diagnosis was established with plain radiographs in three patients, arthrogram in one patient, and a gastrointestinal series in one patient. Three patients had an exploratory laparotomy with diverting colostomy followed by immediate hip arthrotomy within 24 hours and no joint infections occurred. In the other two patients, hip involvement was identified late after septic arthritis occurred. Early diagnosis, diverting

Gunshot wounds to the upper extremity. Evaluation and management of vascular injuries.

Gunshot wounds to the upper extremity are uncommonly life threatening but are associated with a high incidence of significant long-term disability. The identification of vascular injuries is dependent on careful clinical examination and the liberal use of arteriography. Standard techniques of vascular repair should be followed for arterial reconstruction. The rate of successful vascular repair is in excess of 90%, but long-term function is limited by a high incidence of associated nerve injury.

Management and outcomes of cerebrovascular injuries after gunshot wounds to the cervical spine.

STUDY DESIGN: This study was a multicenter retrospective analysis of cervical spine gunshot wound (GSW) patients. OBJECTIVE: The present study was conducted to evaluate the management and outcomes of vascular injuries following GSW involving the cervical spine. SUMMARY OF BACKGROUND DATA: Gunshot wounds (GSW) injuring the cervical spine are associated with high rates of vascular injury. METHODS: Charts of patients with GSW involving the cervical spine at two Level 1 trauma centers were reviewed from 2010 to 2021 for demographics, injury characteristics, management and follow-up. Necrotizing Mucormycosis of Wounds Following Combat Injuries, Natural Disasters, Burns, and Other Trauma.

Necrotizing mucormycosis is a devastating complication of wounds incurred in the setting of military (combat) injuries, natural disasters, burns, or other civilian trauma. Apophysomyces species, Saksenaea species and Lichtheimia (formerly Absidia) species, although uncommon as causes of sinopulmonary mucormycosis, are relatively frequent agents of trauma-related mucormycosis. The pathogenesis of these infections likely involves a complex interaction among organism, impaired innate host defenses, and biofilms related to traumatically implanted foreign materials. Effective management depends upo

Radial nerve injuries from gunshot wounds and other trauma: comparison of electrodiagnostic findings.

OBJECTIVE: To identify the difference and similarity of radial nerve injury in gunshot wounds compared to blunt trauma with regard to level of injury, completeness of injury, and other associated nerve injury. DESIGN: This study was a retrospective review of electrophysiologic data performed in an electromyographic laboratory of a county hospital. A total of 67 consecutive patients had gunshot wounds or other trauma to the radial nerve during a 7-yr period (1992-1998). Forty patients met the criteria for this study. The radial nerve injury was categorized according to the level of involvement

Detection and evaluation of aerodigestive tract injuries caused by cervical and transmediastinal gunshot wounds.

BACKGROUND: Aerodigestive tract penetrations occurring with gunshot wounds to the neck and thorax are uncommon but are frequently associated with multiple organ injury and contribute to significant morbidity. METHODS: The selective management strategy used at our institution for suspected aerodigestive tract involvement with cervical, thoracic inlet, and transmediastinal gunshot wounds is reviewed with reference to eight clinical cases from 1989 to 1995. RESULTS: Seven pharyngoesophageal and four laryngotracheal injuries are described with three patients sustaining combined aerodigestive org

Civilian gunshot wounds to the genitourinary tract: incidence, anatomic distribution, associated injuries, and outcomes.

OBJECTIVES: Gunshot wounds (GSW) affecting the genitourinary (GU) system in civilians are uncommon. This study describes the incidence, anatomic distribution, demographics, associated injuries, management, and outcomes after civilian GU GSW. METHODS: A Level 1 Trauma Center Registry was used to retrospectively identify all patients who sustained GU GSW (January 1997-December 2008). Patient information was abstracted from the Registry, medical, and autopsy records. Multivariate regression detected significant factors associated with mortality. RESULTS: Of 2941 civilian GSW patients, 309 (10.5
