INTRODUCTION: Vascular injuries in contaminated fields secondary to intra-abdominal trauma present significant management challenges. When primary repair is not feasible, surgical dogma encourages avoidance of prosthetic grafts due to risk of infectious complications. Limited data exist characterizing the patterns of injury, reconstructive techniques utilized, and resultant outcomes.
METHODS: We conducted a retrospective review of adult patients who presented with traumatic intra-abdominal gastrointestinal and major vascular injury requiring emergent operative intervention from January 2012 t

UNLABELLED: Bowel injuries following blunt abdominal trauma (BAT) are evasive to detect clinically and with current imaging modalities. These injuries require immediate recognition and operative intervention. Short diagnostic delays can result in significant morbidity and mortality. The goal of this study was to develop a point-based scoring system for the detection of bowel injuries in BAT in order to decrease the incidence of missed and delayed bowel injury diagnoses and to guide clinical decision-making.
METHODS: A unique scoring system based on imaging, physical exam findings, and laborat

BACKGROUND: Penetrating injuries to the inferior vena cava and/or iliac veins are a source of hemorrhage but may also predispose patients to venous thromboembolism (VTE). We sought to determine the relationship between iliocaval injury, VTE and mortality.
METHODS: The National Trauma Data Bank was queried for penetrating abdominal trauma from 2015-2017. Univariate analyses compared baseline characteristics and outcomes based on presence of iliocaval injury. Multivariable analyses determined the effect of iliocaval injury on VTE and mortality.
RESULTS: Of 9,974 patients with penetrating abdom

Fatal bicycle-pedestrian collisions do not occur frequently and thus are rarely reported in literature. Pedestrians in bicycle-pedestrian accidents often sustain severe craniocerebral injuries caused by a collision induced fall with head impact on the road surface. We describe a case where a pedestrian crossing a road was hit by a bicycle. Hematomas of the left lower leg and of the left flank/abdomen were found to be caused by the primary impact. However, the fatal injuries were found to be contralateral with a rupture of the right renal pedicle, a rupture of the right lobe of the liver and a

BACKGROUND: Many trauma care providers often disregard the abdominal clinical examination in the presence of extra-abdominal distracting injuries and mandate abdominal computed tomographic scan in these patients. Ignoring the clinical examination may incur undue expense and radiation exposure. The purpose of this study was to assess the efficacy of abdominal clinical examination in patients with distracting injuries.
METHODS: During a 1-year period, all awake and alert blunt trauma patients with Glasgow Coma Scale (GCS) score of 14 or 15 were entered into a prospective study. Abdominal clinic

Adrenal injuries following blunt abdominal trauma are uncommon. Adrenal hemorrhage in children associated with multiple organ injury, which has received little attention in the past, is an increasingly recognized phenomenon in modern trauma centers with the widespread use of abdominal computed tomography. Adrenal trauma occurs in the setting of multisystem organ injury. Isolated adrenal injury is exceedingly rare. We report two children with blunt adrenal trauma (one isolated and one with associated injuries), who were admitted during the last two years to our Pediatric Surgery Department afte

Recently, in our country we experience an increase incidence of motor vehicle accidents, which often leads to some severe cases brought to the A&E's departments, requiring prompt complex surgical treatment, with the surgeon facing out of ordinary situations. This is the case for the combined trauma of the chest and abdomen. The authors present the case of a 32 years old female patient, victim of a MVA, who had suffered a deep penetrating wound of her right chest and right upper quadrant, caused by a woden pole which penetrated the anterior V and VI intercostal spaces, went through the right in

Injuries of the superficial veins on the ventral abdomen of dairy cows can result in life-threatening haemorrhage. Treatment of these injuries can be difficult because of the characteristics of the veins. Five cows aged 6.5 to 12 years were referred to the Animal Hospitale Zurich, because of intermittent haemorrhage from a ventral abdominal vein. Four of the cows were near term and one had calved two weeks before referral. Intermittent bleeding was seen from the highly convoluted and dilated subcutaneous tributaries to the cranial superficial epigastric vein (milk vein). The haematocrit of 2 c

PURPOSE: Epidemiologic studies of vascular injuries are usually limited to those caused by trauma. The purpose of this study was to review the management and clinical outcome in patients with operative injuries to abdominal and pelvic veins.
METHODS: Clinical data and outcome in all patients with iatrogenic venous injuries during abdominal and pelvic operations between 1985 and 2002 were reviewed.
RESULTS: Forty patients (21 men, 19 women; mean age, 51 years [range, 27-87 years]) sustained 44 venous injuries. Injuries occurred during general (30%), colorectal (23%), orthopedic (20%), gynecol

PURPOSE: The possibility of detecting contrast agent extravasation (i.e., active hemorrhage) with dynamic conventional Computed Tomography (CT) in patients with abdominal trauma has already been reported in small series. We report our experience in the demonstration of contrast material extravasation using helical CT; we also investigate the diagnostic and clinical value of this finding.
MATERIAL AND METHODS: January 1997 to July 1998, we examined 41 consecutive patients with upper abdominal trauma. Twelve patients (29%) had contrast material extravasation. The examinations were performed wit

OBJECTIVES: The purpose of this study was to determine if intra-abdominal pressure (IAP) could predict acute renal injury (AKI) in the postoperative period of abdominal surgeries, and which would be its cutoff value.
PATIENTS AND METHODS: A prospective observational study was conducted in the period from January 2010 to March 2011 in the Intensive Care Units (ICUs) of the University Hospital of Botucatu Medical School, UNESP. Consecutive patients undergoing abdominal surgery were included in the study. Initial evaluation, at admission in ICU, was performed in order to obtain demographic, clin

We report an unusual case of a posttraumatic evisceration of small and large bowel through 2 holes in the anterior abdominal wall after a motor vehicle accident. Prompt adequate management consists of reestablishing the perfusion of the eviscerated organ if the blood supply is compromised, performing a full laparotomy to exclude intra-abdominal organ injury and meticulous cleaning of the eviscerated organs before reducing them in the abdomen and closing the abdomen in layers.

INTRODUCTION AND IMPORTANCE: Blunt abdominal aortic injury (BAAI) resulting from blunt abdominal trauma is rare; therefore, there are no standard guidelines for its treatment. Herein, we report the successful treatment of BAAI via endovascular aortic repair (EVAR) performed immediately after emergency laparotomy to repair a bowel injury.
CASE PRESENTATION: A 78-year-old man was injured after being caught between a shovel car and the bumper of his own car for approximately 15 s. Upon arrival at the hospital, the patient was conscious and had stable vital signs, abdominal and low back pain, and

AIM: To assess whether thoracic and abdominal skin lesions predict CT-detected intrathoracic and intra-abdominal injuries in adults with blunt torso trauma.
METHODS: This single-center retrospective study included 1523 adults (June 2014-June 2024) with blunt thoracic and/or abdominal trauma who underwent both chest CT and contrast-enhanced abdominal CT. Skin lesions and their anatomical locations were extracted from forensic examination forms. Outcomes were any pathological finding on chest CT and abdominal CT. Associations were evaluated using logistic regression, and discrimination was asse
