Identification of hazardous/harmful drinking among subcritically injured patients.

OBJECTIVES: To examine the relationship between a saliva alcohol test (SAT) and hazardous/harmful drinking, as measured by the Alcohol Use Disorders Identification Test (AUDIT), among a sample of subcritically injured patients. METHODS: Patients (n = 78) seeking treatment for a subcritical injury were saliva-tested for alcohol and interviewed regarding their drinking behaviors and related difficulties. Associations of SAT values with AUDIT results were determined. RESULTS: SAT results and hazardous/harmful drinking were not independent events (p < 0.001). Estimates of sensitivity and specifi

Recurrent Pneumothorax Following Multilevel Posterior Spinal Fixation in a Patient With Pre-Existing Pulmonary Contusion: A Case Report.

Recurrent pneumothorax following posterior spinal fixation is a rare but life-threatening complication, particularly in polytrauma patients with underlying pulmonary contusion. We report a 19-year-old male involved in a high-speed maritime accident who sustained a T12 fracture and right-sided pulmonary contusion. Initial pneumothorax was managed with a chest tube, which was removed after radiographic resolution. Subsequently, the patient underwent posterior T10-L2 fixation. Immediately postoperatively, he developed acute dyspnea and massive recurrent pneumothorax requiring urgent re-interventi

Benefits and Liabilities of Benzodiazepines and Z-Drugs: What We Know and What We Don't Know.

Benzodiazepines, first introduced in the 1960s as effective and safer alternatives to barbiturate sedative-hypnotics, are now widely viewed as having a high risk of overdose, abuse, addiction, and "dependency". These beliefs are contrary to extensive scientific evidence. Systematic research has shown that prescribed benzodiazepines are not prone to tolerance, dose-escalation, abuse, or addiction. Benzodiazepine abuse and overdose fatalities occur largely in the context of established polysubstance abuse. They are as effective as antidepressants and have fewer adverse effects but similar withdr

Anesthetic management of a patient with closed tracheal transection.

Closed tracheal injury is a rare clinical emergency that is prone to misdiagnosis and missed diagnosis. Improper management can be life-threatening. This report describes the anesthetic management of a patient with closed tracheal transection undergoing airway reconstruction surgery. The patient, a 33-year-old male, presented with head and neck pain and dyspnea 4 hours after a car accident. CT with 3-dimensional airway reconstruction revealed upper tracheal transection. Emergency cervical exploration and end-to-end tracheal anastomosis were performed under general anesthesia. The anesthetic ap

Locally advanced endometrial cancer with multiple immune-related adverse events coinciding with the complete response to radiotherapy after immune checkpoint inhibitor therapy: A case report.

We report a case of a 70-year-old female patient with locally advanced endometrial cancer with primary empty sella who developed multiple immune-related adverse events (irAEs), including hypopituitarism coinciding with the complete response to radiotherapy after receiving immune checkpoint inhibitors. A computed tomography scan acquired after a traffic accident led to the discovery of endometrial cancer that had invaded the vulva and primary empty sella. Following adriamycin and cisplatin, pembrolizumab was administered for three cycles. No irAEs were observed during treatment, but the tumor w

Ultrasound evaluation of extracranial carotid artery lesions in Parkinsonian patients.

The purpose of this investigation was to determine the atherosclerotic changes in patients with vascular parkinsonism and in patients with idiopathic Parkinson's disease, in order to evaluate the possible influence of the extracranial pathology of carotid arteries in developing lacunar cerebral infarcts. Degree of stenosis and plaque morphology of the extracranial part of carotids in both group of patients were evaluated by color Doppler flow imaging ultrasound investigation and the results were compared. We selected two matched groups of patients with parkinsonism: 22 patients with vascular p

[Engel's "psychogenic pain and the patient susceptible to pain": a retrospective, controlled clinical study].

Engel's hypothesis of pain-prone patients having a distinct pattern of developmental psychosocial experiences was tested in a controlled design including four groups of 20 patients each: a) psychogenic pain, b) organic pain, c) psychogenic bodily symptoms, d) organic disease. On admission an open-ended interview, including childhood experiences was tape-recorded. Measures were taken to minimize observer-bias. Patients in group a) had, compared to the other groups, significantly increased prevalence of: "Parents, verbally and/or physically abusive of each other", "parents, abusive of the child"

Atypical cerebral fat embolism syndrome in an elderly patient with comorbidities following fracture: A case report.

RATIONALE: Cerebral fat embolism (CFE) is a rare but serious complication of fractures, most commonly affecting young males. However, atypical cases in elderly patients with underlying comorbidities are easily misdiagnosed. PATIENT CONCERNS: We report a case of a 61-year-old male patient with diabetes and hypertension who developed impaired consciousness and fever on the 2nd day after sustaining multiple fractures in a traffic accident. His condition deteriorated after receiving local treatment. DIAGNOSES: CFE was diagnosed based on the characteristic "starry sky sign" on brain magnetic reso

[Sleep disorders in psychiatry and their treatment].

Sleep disturbances are frequent and multifaceted and have serious consequences. They play an important role within psychiatric symptoms and disorders. On the one hand they may appear as a symptom of a disorder, which may also be a diagnostic criterion, as for example in affective disorders, on the other hand they may be independent disorders or last but not least sequelae of psychiatric disorders or their pharmacological therapy, as with antidepressants or neuroleptics, which may cause or deteriorate nocturnal movement disorders. They may aggravate psychiatric disorders, perpetuate them or pre

Infant sleep position and associated health outcomes.

BACKGROUND: The incidence of sudden infant death syndrome has decreased in the United States as the percentage of infants sleeping prone has decreased, but persisting concerns about the safety of supine sleeping likely contribute to prone sleeping prevalence rates that remain higher than 10%. OBJECTIVE: To document health outcomes in infants aged 1 to 6 months in relation to sleep position. DESIGN: Prospective cohort study. SETTING: Massachusetts and Ohio, from February 21, 1995, to December 31, 1998. STUDY PARTICIPANTS: A total of 3733 infants with consistent sleep positions at ages 1, 3,

Urine Prevalence of Psychoactive Substances, Nicotine Exposure and Associated Factors Among Commercial Drivers. An Exploratory Study.

Earlier studies have documented psychoactive substance use and nicotine exposure among commercial drivers in Ghana. However, those studies relied solely on self-reported questionnaires, which are prone to recall and response bias. This study aimed to provide a more accurate estimate by using a chromatographic immunoassay urine drug test. This exploratory cross-sectional study was conducted between January and August 2024 in the Tamale metropolitan area of Ghana. The study included 70 consenting commercial drivers aged from 26 to 78 years. Sociodemographic, anthropometric and occupational facto

Manual Reduction and Plaster External Fixation for the Treatment of Closed Total Talus Dislocation: Case Report and Literature Review.

BACKGROUND: Total dislocation of the talus from all its surrounding joints (talonavicular, tibiotalar, subtalar) is one kind of serious injury of the lower extremity with rare occurrence. It is usually accompanied by fractures of the talus and its periphery, as well as severe soft tissue injury, which is difficult to reset. Complications such as skin necrosis and infection are prone to occur in the early stage, and talus necrosis are prone to occur in the late stage, all of which aggravate disease severity and increase difficulties for its treatment. CASE PRESENTATION: Herein, we reported a c

The risk associated with organophosphorus nerve agents: from their discovery to their unavoidable threat, current medical countermeasures and perspectives.

The first organophosphorus nerve agent was discovered accidently during the development of pesticides, shortly after the first use of chemical weapons (chlorine, phosgene) on the battlefield during World War I. Despite the Chemical Weapons Convention banning these substances, they have still been employed in wars, terrorist attacks or political assassinations. Characterised by their high lethality, they target the nervous system by inhibiting the acetylcholinesterase (AChE) enzyme, preventing neurotransmission, which, if not treated rapidly, inevitably leads to serious injury or the death of t

Positive Outcomes of Physiotherapy Intervention in a Wedge Compression Fracture of the L1 Vertebra: A Case Report.

The thoracolumbar spine is prone to vertebral compression fractures (VCFs). An injury mechanism known as flexion compression is responsible for thoracolumbar spine compression fractures. Usually, this mechanism affects the longitudinal ligament at the front and the front part of the vertebral body as the first components. Pain is the first and foremost symptom; here we present a case report of a 34-year-old male, who came to the hospital with complaints of back pain, and difficulty in breathing followed by a road traffic accident (RTA). MRI and X-ray investigations were done. The patient was d

Hyperhaemolytic Syndrome in Sickle Cell Disease: Clearing the Cobwebs.

Sickle cell disease (SCD) presents with a dynamic background of haemolysis and deepening anaemia. This increases the demand for transfusion if any additional strain on haemopoiesis is encountered due to any other physiological or pathological causes. Patients with cerebrovascular accidents are placed on chronic blood transfusion; those with acute sequestration and acute chest syndrome are likewise managed with blood transfusion. These patients are prone to develop complications of blood transfusion including alloimmunization and hyperhaemolytic syndrome (HHS). This term is used to describe hae

Myasthenia gravis with superimposed spinal cord injury: a case report.

Both myasthenia gravis and traumatic spinal cord injury are uncommon disorders and their concurrence is extremely rare. We report here the case of a man with stable myasthenia gravis with spinal cord injury due to a motor vehicle accident. His muscle strength and sensory function in all 4 limbs partially recovered during the initial hospitalization. However, after a later episode of pneumonia and urinary tract infection, muscle strength deteriorated and weakness of the ocular muscles occurred. A relapse of myasthenia gravis was confirmed by the elevated anti-acetylcholine receptor antibody tit

Obstructive sleep apnea syndrome and hypothyroidism - merely concurrence or causal association?

The prevalence of obstructive sleep apnea-hypopnea syndrome (OSAHS) ranges from 4 to 7% in men and from 2 to 5% in women. Its deleterious consequences such as traffic accidents, cardiovascular complications increasing morbidity and mortality, make it a major health problem. Apart from obesity (a major risk factor for OSAHS), hypothyroid patients are prone to reveal this phenotype. Although hypothyroidism seems an acknowledged risk factor for OSAHS, some authors report the lack of clinically relevant association. The argument partly depends on the increased prevalence of hypothyroidism in OSAHS
