Islamic Jurisprudence, Biomedical Ethics, and the Social Construct in Relation to Intersex People: A Scoping Review.

In Islamic jurisprudence, intersex people are known as khuntha-those who have both male and female multiplicative organs. Taking into account the fragmented evidence on the role of science in the treatment of the khuntha, the socioethical demands, and expectations of people depending on their gender, this study aims to better understand the existing literature on the khuntha people in the Middle East and North Africa region (MENA)-the perception, acceptance, and treatment from families, communities, and health care providers. This study used a scoping review framework and was guided by the Pre

Smart Systems, Saved Lives: Using AI to Identify Deteriorating Sepsis Patients in Adult Critical Care.

Sepsis remains one of the leading causes of mortality in adult critical care, representing a dysregulated host response to infection. Despite advances in therapeutics and supportive care, it continues to account for over 31 million deaths annually. Its unpredictable trajectory and variable physiologic presentations make early recognition and timely intervention crucial to improving outcomes. Early warning systems were initially developed to standardize the recognition of physiologic instability and guide early recognition. The Modified Early Warning Score, National Early Warning Score 2, and Q

The anesthesiologist's perspective on emergency aortic surgery: Preoperative optimization, intraoperative management, and postoperative surveillance.

The management of emergencies related to the aorta requires a multidisciplinary approach involving various health care professionals. Despite technological advancements in treatment methods, the risks and mortality rates associated with surgery remain high. In the emergency department, definitive diagnosis is usually obtained through computed tomography angiography, and management focuses on controlling blood pressure and treating symptoms to prevent further deterioration. Preoperative resuscitation is the main focus, followed by intraoperative management aimed at stabilizing the patient's hem

Assessing Concordance Across Nonprofit Hospitals' Public Reporting on Housing as a Community Health Need in the Era of the Affordable Care Act.

Although the Affordable Care Act requires nonprofit hospital organizations to report how they identify and invest in community health needs, the utility of mandated reporting documents for tracking investments in the social determinants of health has been questioned. Using public reporting documents and focusing on housing as a social determinant of health, we describe how nonprofit hospital organizations in 5 communities with the highest rates of homelessness document needs and investments related to housing on their Community Health Needs Assessments, Implementation Strategies, and Schedule

[Locomotive syndrome].

In Japan one of the major reasons the elderly need nursing care is disorders of the locomotive organs, which can make it difficult for a person to walk. People aged 65 and over now account for 23% of the population and by 2055 this figure will reach 41%. This situation will have great impact on the country's social systems. Japanese Orthopaedic Association (JOA) has coined the term "locomotive syndrome" to bring attention to the importance of locomotive organs. JOA developed a simple questionnaire to self-assess an individual's locomotive ability and easily practicable home exercises involving

Just Culture and Accountability in Nursing Leadership: Closing the Experience Gap, Rebuilding Morale, and Hardwiring Reliability.

Leadership transitions in nursing have left many first-time nurse leaders underprepared for the most challenging aspects of their role: holding courageous conversations, maintaining accountability, and sustaining civility under stress. These trends were further accelerated by the disruptions of the pandemic. This gap, combined with heightened workplace violence, burnout, and workforce instability, has threatened the culture of safety across health care. Post-COVID-19, restoring trust and resilience requires intentionally integrating Just Culture, the Highly Reliable Experience training, and nu

Neurological Aspects of Foreign Accent Syndrome in Stroke Patients.

Foreign Accent Syndrome (FAS) is an intriguing motor speech disorder which has captured the interest of the scientific community and media for decades. At the moment, there is no comprehensive model which can account for the pathophysiology of this disorder. This paper presents a review of 112 FAS cases published between 1907 and October 2016: these were analyzed with respect to demographic characteristics, lesion location, associated neurocognitive symptoms, and comorbid speech and language disorders. The analysis revealed that organic-neurogenic FAS is more frequent in women than in men. In

Prevalence and epidemiology of microbial pathogens causing bloodstream infections: results of the OASIS multicenter study.

Beginning on April 2007, a prospective multicenter study was performed to investigate prevalence and epidemiology of microbial pathogens causing bloodstream infections (BSIs). Twenty microbiology laboratories participated to the survey over a 1-year period. A total of 11,638 episodes of BSI occurred in 11 202 patients, with 8.5% (n=985) of episodes being polymicrobial. Of 12 781 causative organisms, aerobic Gram-negative bacteria were 47.4% (n=6058), whereas Gram-positives accounted for 43.9% (n=5608). The remaining organisms included fungal species (n=924, 7.2%) and anaerobes (n=191, 1.5%). T

In-Hospital Mortality Risk Among Transfers to Cardiac Intensive Care Units.

BACKGROUND: Patients treated in tertiary cardiac intensive care units (CICUs) often arrive via transfer from other hospitals. This "transfer population" is poorly described and has been largely excluded from trials in critical care. OBJECTIVES: The aim of this study was to characterize patients who are transferred from other hospitals to advanced CICUs. METHODS: The Critical Care Cardiology Trials Network is an investigator-initiated network of predominantly North American CICUs. Consecutive CICU admissions during annual 2-month snapshots were submitted to the coordinating center (TIMI Study

Beyond physical access: disability-inclusive primary health care as a test of health equity in Sub-Saharan Africa.

Disability inclusion in primary health care is not a peripheral issue but a fundamental test of whether health systems in sub-Saharan Africa are advancing health equity and universal health coverage. Persons with disabilities experience persistent and overlapping barriers across the care pathway, including physical inaccessibility, unaffordable services, inaccessible information, poor provider communication, stigma, weak referral systems, and limited access to rehabilitation and assistive technology. These barriers are intensified by poverty, gender inequality, rural residence, and under-resou

Transplantation for childhood liver disease: an overview.

The advances made in surgical technique, postoperative care, and immunosuppression during the 1980s have permitted orthotopic liver transplantation (OLT) to evolve into an effective and widely accepted therapy for infants and children with end-stage liver disease. Biliary atresia, a progressive, obliterative disease of the bile ducts, is the most common indication for OLT in children, accounting for approximately 50% of cases. Metabolic liver disease (MLD) accounts for 20% to 25%; other common indications for OLT include fulminant hepatic failure (FHF) and forms of intrahepatic cholestasis. Th

Green dentistry: Organic toothpaste formulations. A literature review.

Dentistry as a profession should take into account the goals of sustainable development in daily practice and encourage the transition to a green economy. Consumers are becoming more conscious about the impact of self-care products, including toothpastes, on the environment. Organic toothpastes are considered very safe. A broad literature review was conducted to: (i) identify the ingredients in available organic toothpastes; (ii) classify them into active and inactive; and (iii) evaluate each ingredient's purpose and the adverse events that may be associated with its use. A comprehensive list

Organization and delivery of long-term care in Taiwan.

Taiwan reached the World Health Organization (WHO) benchmark of 7% aged 65 and over for defining an aging population only as recently as 1993. With this proportion projected to double to 14% by 2020, Taiwan faces a rapid increase in need for long-term care. This article presents an account of the current service delivery system, which is divided between health and social affairs administrations, with a substantial role also taken by the Veteran Administration, and growing provision of facilities that operate outside the government-registered system. While a basic level of both institutional an
