BACKGROUND: The most common type of carcinoma ex pleomorphic adenoma (CPA) is histologically equivalent to salivary duct carcinoma, which has an apocrine phenotype. Invasive CPA is often accompanied by non-invasive or in situ carcinoma, an observation that suggests the presence of precursor lesions. Pleomorphic adenomas most frequently affect the parotid gland, primarily the superficial lobe. The cystic change, which can resemble squamous cell carcinoma, mucoepidermoid carcinoma, mucocele, or carcinoma ex pleomorphic adenoma, presents a diagnostic challenge in pleomorphic adenomas. Pleomorphic adenoma is the most common benign salivary tumor involving the parotid gland. However, malignant transformation to carcinoma expleomorphic adenoma (CXPA) is an exceedingly rare occurrence. INTRODUCTION: Carcinoma ex pleomorphic adenoma (CXPA) is a rare malignant tumor of the parotid gland. We analyzed the clinical characteristics and treatment outcomes of CXPA of the parotid gland in patients managed for 11 years at this hospital.
METHODS: The study included 17 cases of CXPA of the parotid gland from January 2010 to December 2020.
RESULTS: Over 11 years, CXPA was the fourth most common parotid carcinoma, accounting for 9.4% of the 180 cases finally diagnosed as parotid carcinoma. Of the 17 cases of CXPA of the parotid gland, 12 lesions were removed by superficial parotidectomy

Pleomorphic adenoma is a benign tumour with variable cytomorphological and architectural elements. It is the most common salivary gland tumour in children and adults. We report a case of a 32-year-old Malay woman who presented with a slow-growing, painless, firm, and mobile nodule of 1 cm x 1 cm in size at the angle of her mandible on the right side which could be palpated bimanually. Pleomorphic adenoma is a benign mixed tumor, which is composed of myoepithelial and epithelial cells. A fibrous capsule separates these cells from the surrounding tissues. INTRODUCTION: Pleomorphic adenoma (PA) is the most common benign salivary gland tumor. When neglected on some occasions, its size and weight can enormously augment.
CASE PRESENTATION: We report a case of a giant submandibular pleomorphic adenoma in a 75 years old female patient which measured 34 × 26 × 20 cm and weighed 8.1 kg.
CONCLUSION: Neglecting such tumors without treatment, can cause severe facial disfigurement and can even hinder the airway.

Pleomorphic adenomas arising from nonsalivary glands are not common, whereas they are the most common of all salivary gland tumors. They have been reported to arise from the nasal septum, maxillary sinus, lacrimal gland, etc. However, no cases of pleomorphic adenoma involving the nasal ala have been reported. We present a case of pleomorphic adenoma arising from a nasal ala. The adenoma was excised without causing any deformity and there was no recurrence during a 6-month follow-up.
