Experimental study on the treatment of mouse burn injury combined with smoke inhalation injury using thrombopoietin.

BACKGROUND: Burn injuries complicated by smoke inhalation significantly increase clinical severity and can lead to acute respiratory distress syndrome (ARDS). Inhalation injury is predominantly caused by exposure to toxic combustion gases, with thermal damage from steam being relatively uncommon. Thrombocytopenia is frequently observed in severe burns and is associated with poor outcomes, whereas platelet transfusion can carry clinically important risks. Recombinant human thrombopoietin (rhTPO), a hematopoietic growth factor used clinically for thrombocytopenia, may represent an alternative st

Inhaled antibiotics in critical care: State of the art and future perspectives.

Administration of inhaled antibiotics in intensive care units (ICU) remains confidential compared to bronchodilators, not withstanding extensive pre-clinical and clinical research and potential indications associated with the emergence of bacterial antibiotic resistances. Inhaled antibiotic administration enables delivery of high doses of antibiotics directly to the lung. Local antibiotic concentrations are pronouncedly higher than the minimum inhibitory concentration of causative pathogens of lung infection, and also higher than the minimal concentrations preventing resistant emergence, with

PK-PD Evaluation of Inhaled Microparticles loaded with Ciprofloxacin-Copper complex in a Rat Model of Chronic Pseudomonas aeruginosa Lung Infection.

The potential gain in efficacy of pulmonary administration over IV administration of some antibiotics such as ciprofloxacin (CIP) may be limited by the short residence time of the drug at the site of infection after nebulization. Complexation of CIP with copper reduced its apparent permeability in vitro through a Calu-3 cell monolayer and greatly increased its pulmonary residence time after aerosolisation in healthy rats. Chronic P. aeruginosa lung infections in cystic fibrosis patients result in airway and alveolar inflammation that may increase the permeability of inhaled antibiotics and alt

Cardiopulmonary Hemodynamic Alterations During Early Resuscitation in Burn Patients with Inhalation Injury: Do They Impact Prognosis?

Inhalation injury remains a major contributor to morbidity and mortality in critically ill burn patients, although its hemodynamic impact during early resuscitation is incompletely understood. Patients with inhalation injury could exhibit distinct hemodynamic alterations compared with those without inhalation injury, potentially contributing to worse outcomes. We conducted a prospective, single-center observational study over four years including adult patients with major burns. Hemodynamic variables, fluid administration, and analytical biomarkers during initial resuscitation were analyzed ac

Quercetin inhalation inhibits the asthmatic responses by exposure to aerosolized-ovalbumin in conscious guinea-pigs.

Effects of quercetin inhalation on immediate (IAR), late-phase (LAR) and late late-phase (LLAR) asthmatic responses by exposure to aerosolized-ovalbumin (AOA) (2w/v% in saline, inhalation for 3 min) were studied in conscious guinea-pigs sensitized with AOA. We measured specific airway resistance (sRaw), and recruitment of leukocytes, histamine and protein contents and phospholipase A2 (PLA2) activity in bronchoalveolar lavage fluid (BALF). Effects of quercetin (10 mg/mL, inhalation for 2 min) compared with cromolyn sodium, salbutamol, and dexamethasone inhalations, respectively. Quercetin inha

[Clinical use of arfonad and hygronium aerosols in emergency hypotensive therapy].

During clinical testing of the administration of ganglion-blocking agents by inhalation (175 persons) it was established that drug aerosols of arfonad and hygronium possess high effectiveness, act rapidly, and are relatively safe. The inhalation method of the management of hypertensive crises may find wide use in out-patient practice, under conditions of the admission room, cardiological and resuscitation departments, as well as in obstetrics when controlled hypotension is necessary.

Lack of adrenomedullin on antigen-induced bronchoconstriction in guinea pigs in vivo.

Antigen challenge can provoke acute bronchoconstriction, recognized as immediate asthmatic response (IAR), but the evolving events in this reaction are not well defined. Recently, a novel peptide, designated adrenomedullin, was isolated from human pheochromocytoma, and has been shown to have potent systemic and pulmonary vasodilator activity. The purpose of this study was to elucidate the influence of adrenomedullin in the development of IAR. Passively sensitized guinea pigs were anesthetized and treated with diphenhydramine hydrochloride, and then artificially ventilated. Ovalbumin was inhaled

The effect of a clinically effective and non-effective dose of lorazepam on 7.5% CO₂-induced anxiety.

Symptoms of anxiety induced by 7.5% CO₂ inhalation can be attenuated by acute administration of GABA(A) receptor anxiolytics such as lorazepam and alprazolam. This study investigated if these effects are dose-related, by comparing a 0.5 mg dose (considered non-clinically effective) and a 2 mg dose of lorazepam (clinically effective) on 7.5% CO₂ inhalation. Eighteen healthy males (mean age 20.6 years, SD 1.29), judged physically and mentally fit, attended three visits, each one week apart, to take each treatment in a randomised double-blind crossover design. Drugs were given 60 min prior to 20

Should acute treatment with inhaled beta agonists be withheld from patients with dyspnea who may have heart failure?

In patients with dyspnea, prehospital and emergency providers make therapeutic decisions before a diagnosis is established. Inhaled beta-2 agonists are frontline treatment for patients with dyspnea due to asthma or chronic obstructive pulmonary disease (COPD) exacerbations. However, these agents have been associated with increased adverse events when administered chronically to heart failure patients. Our goal was to determine the safety and efficacy of acute administration of inhaled beta-2 agonists to patients with heart failure. MEDLINE and EMBASE searches were performed using the terms "be

Acute tolerance to the cardiovascular effects of volatilized cocaine free base in rats.

The cardiovascular effects elicited by volatilized cocaine were compared to those produced by i.v. drug administration. All of the subjects exposed to volatilized cocaine exhibited an initial increase in mean arterial pressure (MAP), profound bradycardia, and heart blocks. Intravenous administration of cocaine (1.5 mg/kg i.v.), a dose equivalent to the amount of drug subjects received from inhalation exposure, led to a transient increase in MAP but failed to produce any obvious ECG abnormalities or bradycardia; however, increasing the dose to 4.5 mg/kg resulted in considerable toxicity. Oral toxicokinetics of the indoor air pollutant, α-pinene, and its genotoxic metabolite, α-pinene oxide, in rodents and comparison to inhalation route of exposure.

α-Pinene is a monoterpene with potential human exposure via oral and inhalation routes and has been associated with bladder hyperplasia and decrease in sperm counts in rodents following inhalation exposure. The toxicokinetics of α-pinene and α-pinene oxide was investigated following 7-day oral administration of α-pinene in rats (10, 50, and 150 mg/kg) and mice (50 mg/kg). α-Pinene was absorbed following oral administration. In rats, α-pinene systemic exposure (maximum concentration (Cmax) and area the under the concentration versus time curve (AUC)) increased less than dose-proportionally, exc

Some effects of trichloroethylene on mouse lungs and livers.

Repeated administration of trichloroethylene (TCE) to mice by either i.p. injection or by inhalation increased the activity of hepatic microsomal NADPH cytochrome-c reductase. The NADPH cytochrome c reductase activity in microsomes isolated from lungs of animals treated with TCE by inhalation was decreased relative to controls (untreated animals). TCE inhalation was associated with pathologic changes in lungs, but not in livers of the treated animals. The duration of exposure is probably an important factor however, since animals exposed for only 1 hr per day exhibited neither pathologic chang

Smoke inhalation injury in a pregnant patient: a literature review of the evidence and current best practices in the setting of a classic case.

For smoke inhalation injury of a pregnant woman, one must treat two patients and be aware of the potential effects of carbon monoxide (CO) and cyanide (CN) poisoning on both the mother and the fetus. In a pregnant woman, the size and age of the fetus and the degree of poisoning allow for tremendous variability in the toxicity of CO and CN and their respective treatment options. The authors will review a case of a 32-year-old woman who was at 37 weeks of gestation and admitted to the Evans-Haynes Burn Center after a house fire and received hydroxocobalamin (Cyanokit) for suspected CN poisoning.

Hydrocarbon toxicity: A review.

CONTEXT: Clinical effects of hydrocarbon exposure have been reported since 1897. These substances are ubiquitous, and their exposures are common. The specific hydrocarbon and route of exposure will determine the clinical effect, and an understanding of this is helpful in the care of the hydrocarbon-exposed patient. OBJECTIVE: To complete a comprehensive review of the literature on hydrocarbon toxicity and summarize the findings. METHODS: Relevant literature was identified through searches of Medline (PubMed/OVID) and Cochrane Library databases (inclusive of years 1975-2013), as well as from

Pulmonary delivery of low-dose dexamethasone significantly reduces toxicity in acute lung injury therapy.

Dexamethasone (DEX), a potent glucocorticoid with anti-inflammatory and immunosuppressive properties, exhibits dose-limiting systemic toxicity during long-term use. Inhalation therapy enables targeted pulmonary delivery, enhancing therapeutic efficacy while minimizing systemic exposure compared to oral routes. This study designed a DEX dry powder inhaler (DPI) formulation exhibiting excellent aerodynamic performance with a good fine particle fraction (FPF = 49.01%), which achieved approximately 7.1-fold higher pulmonary exposure relative to intravenous injection at an equivalent dose. Pulmonary administration of a dry powder formulation of the antifibrotic drug tilorone reduces silica-induced lung fibrosis in mice.

The aim of this work was to study the antifibrotic effect of pulmonary administration of tilorone to lung fibrosis. L-leucine coated tilorone particles were prepared and their aerosolization properties were analyzed using two dry powder inhalers (Easyhaler and Twister). In addition, the biological activity and cell monolayer permeation was tested. The antifibrotic effect of tilorone delivered by oropharyngeal aspiration was studied in vivo using a silica-induced model of pulmonary fibrosis in mice in a preventive setting. When delivered from the Easyhaler in an inhalation simulator, the emitte

Comparison of inhaled metaproterenol via metered-dose and hand-held nebulization in prehospital treatment of bronchospasm.

INTRODUCTION: Although the efficacy of the administration of beta-adrenergic bronchodilators has been demonstrated, the best method available for the delivery of these drugs in the prehospital setting has not been defined. This paper compares the effects of administration of metaproterenol when administered by paramedics using either a metered-dose inhaler (MDI) or a hand-held nebulizer (HHN). HYPOTHESIS: There is no difference in the effects produced in patients suffering from smooth bronchiolar muscle spasm by metaproterenol when delivered either by a standard metered-dose inhaler or with a
