Aggregation and analysis of 25 years of prion disease natural history extracted from published literature.

BACKGROUND AND OBJECTIVES: Prion disease is an untreatable, typically rapidly progressive dementia. New drug candidates designed to lower prion protein are now entering clinical trials. To support future pivotal trials, we sought to assemble and analyze a public dataset of natural history data characterizing the symptomatic course of prion disease in order to provide a quantitative basis for modelling and rational trial design. METHODS: We extracted data from medical publications between 2000 and 2024 reporting on n≥5 prion disease patients. Information about demographics, ascertainment, and

[Capsular pseudobulbar mutism in a patient of lacunar state].

A patient of mutism with pseudobulbar palsy and frontal lobe syndrome resulting from lacunar state was reported. The patient, a 64-year-old man, was admitted to Gifu University Hospital because of a decrease in spontaneous activity, lack of volition and anorexia. The CT scan, performed on July 29, 1987, demonstrated lacune in the right internal capsule (IC), periventricular lucency especially around the anterior horn of lateral ventricles, and ventricular dilatation. He was transferred to a medical ward because of repeated aspiration pneumonia. Neurological examination revealed mutism, pseudob

Catatonia: diagnosis and management.

The number of cases of catatonic schizophrenia has decreased in the past 50 years, but a few patients with the syndrome are still seen. The author describes the excited and withdrawn types of catatonia, reviews some organic and psychiatric conditions that resemble them, and gives suggestions for making the differential diagnosis. He discusses the use of the barbiturate interview, ECT, and phenothiazines in treatment, using case examples.

Tacrolimus-induced Central Pontine Myelinolysis in a Pediatric Liver Transplant Recipient.

Central pontine myelinolysis (CPM) is a rare neurological complication reported in liver transplant recipients. A 16-year-old boy with Wilson disease underwent a living donor liver transplant for acute-on-chronic liver failure. On postoperative day 7, he was noted to have diplopia, dysphagia, and bilateral lower limb weakness with wide base gait with gradual progression to akinetic mutism. Magnetic resonance imaging (MRI) brain was performed which was suggestive of CPM, and it was attributed to tacrolimus. We stopped tacrolimus, and he was started on ciclosporin. His neurological symptoms star

Creutzfeldt-Jakob disease associated with a V203I homozygous mutation in the prion protein gene.

We report a Japanese patient with Creutzfeldt-Jakob disease (CJD) with a V203I homozygous mutation of the prion protein gene (PRNP). A 73-year-old woman developed rapidly progressive gait disturbance and cognitive dysfunction. Four months after the onset, she entered a state of an akinetic mutism. Gene analysis revealed a homozygous V203I mutation in the PRNP. Familial CJD with a V203I mutation is rare, and all previously reported cases had a heterozygous mutation showing manifestations similar to those of typical sporadic CJD. Although genetic prion diseases with homozygous PRNP mutations oft

Third ventricular tumors.

Because of the uncertainty of a histological diagnosis without biopsy in tumors which lie within the third ventricle, we have advocated a surgical exploration of all cases. Those located in the posterior third ventricular or pineal region present a special problem and have been discussed elsewhere. Tumors lying in anterior portions of the third ventricle have been approached through a transcallosal route which we have preferred over the transfrontal route for the reasons indicated. The approach is relatively simple and associated with low morbidity. Problems have been encountered from forceful

Anterior cerebral artery and Heubner's artery territory infarction.

Anterior cerebral artery (ACA) territory strokes account for 0.5-3% of all ischemic strokes. The etiological mechanisms of ACA territory strokes vary by race; ACA dissection is a frequent cause in Japan. The most prevalent symptom of such strokes is contralateral hemiparesis or monoparesis, usually affecting the leg predominantly. Predominant leg weakness is attributed to damage in the paracentral lobule, and weakness of the arm and face is associated with involvement of Heubner's artery and the medial striate arteries. Hypobulia, typically 'akinetic mutism', is also common. Several behavioral

Lateralized clinical and diffusion-weighted MRI abnormalities in a probable case of sporadic Creutzfeldt-Jakob disease.

The usual clinical profile of sporadic Creutzfeldt-Jakob disease (sCJD) is subacute dementia, motor dysfunction and myoclonus. Occasionally, some patients present atypical clinical features. We report a case of probable sCJD in a 73-year-old man with a rapidly progressive lateralized neurologic dysfunction of the left hemisphere. In a few weeks the clinical picture deteriorated dramatically to akinetic mutism and myoclonus. The 14-3-3 protein was positive in the cerebrospinal fluid. Diffusion-weighted (DWI) magnetic resonance imaging (MRI) revealed increased signal in the left cortical ribbon

[A case of juvenile cerebral infarction caused by bilateral anterior cerebral artery dissection].

A 38-year-old man was admitted to our hospital with headache, dysarthria and paraparesis. Brain CT and diffusion MRI disclosed cerebral infarction at bilateral anterior cerebral artery (ACA) territories. His symptoms and signs deteriorated in several days despite intensive antithrombotic therapy, resulting in right hemiparesis, akinetic mutism, memory disturbance, change of personality, urinary incontinence, forced grasping, and starting delay of speech and motion. Cerebral angiography demonstrated occlusion with contrast pooling at the right ACA A2 portion. Stenosis and dilatation were found

[Creutzfeldt-Jakob disease with a point mutation at codon 232 of prion protein--a case report].

We report a 50-year-old female with sporadic Creutzfeldt-Jakob disease who revealed a point mutation at codon 232 of prion protein (Met to Arg). The initial symptom was visual disturbance. The patient then developed progressive dementia, cerebellar ataxia and myoclous. About eight months after the onset, the patient went into the state of akinetic mutism. The electroencephalogram showed periodic synchronous discharges. From the prion protein's DNA sequencing of the patient's family members, the 84-year-old father without any neurological symptoms was also detected to have a point mutation at c

[When does the brain atrophy in Creutzfeldt-Jakob disease? (author's transl)].

Chronological changes of computerized tomography (CT) findings and clinical signs and symptoms were studied in 3 patients of Creutzfeldt-Jakob disease. CT revealed no brain atrophy in the stage when the patients showed frequent myoclonus and fell rapidly into the state of akinetic mutism. One autopsied case of this stage showed no macroscopical brain atrophy at all. Microscopical examination revealed extensive spongiform change in cerebral cortices (except for hippocampus), thalamus, basal ganglia, midbrain and a part of cerebellar cortex, but neuronal loss and gliosis were not severe. In othe

Multiple system degeneration and involving thalamus, reticular formation, pallido-nigral, pallido-luysian and dentato-rubral systems. A case report.

An autopsy case of multiple system degeneration is characterized by the following; (1) progressive dementia and abnormal sleep patterns, followed by Parkinsonian symptoms with terminal akinetic mutism; (2) severe symmetrical degeneration in the thalamus, particularly nucl. medialis thalami, the reticular formation of the brain stem, also the pallidonigral, pallido-Luysian and dentato-rubral systems. As far as we known, there is no case in the literature, of combined system degeneration, which shows such a wide anatomical range of lesions as the present one. Clinico-pathological correlation bet

Case report: Evolution of catatonic mutism and psychotic symptoms in an adolescent with Down syndrome: transition from Down syndrome disintegrative disorder to anti-N-methyl-D-aspartate receptor encephalitis.

During her first year of junior high school, a 12-year-old Japanese girl with Down syndrome experienced dizziness, gait disruption, paroxysmal weakness in her hands, and sluggish speaking. Regular blood tests and a brain MRI revealed no abnormalities, and she was tentatively diagnosed with adjustment disorder. Nine months later, the patient experienced a subacute sickness of chest pain, nausea, sleep problem with night terrors, and delusion of observation. Rapid deterioration then developed with simultaneous fever, akinetic mutism, loss of facial expression, and urine incontinence. These catat

A case of Creutzfeldt-Jakob disease in a patient on hemodialysis.

We report an unusual case of probable Creutzfeldt-Jakob disease (CJD) in hemodialysis patient. A woman 59 years of age with a past history of hypertension and end-stage renal disease presented with a stuporous state preceded by rapidly progressive cognitive dysfunction, myoclonus, and akinetic mutism. At first, the cause of the altered mental status was assumed to be uremic or hypertensive encephalopathy combined with fever. Proper managements, however, did not improve the neurologic symptoms. Diffusion-weighted magnetic resonance imaging revealed bilaterally asymmetric high signal intensity i

[A case of spongiform encephalopathy with ataxia and amyloid plaques].

A 63-year-old man was admitted to our hospital because of gait disturbance. His daughter died of the same disorder at age 29. In 1974, at age 59, the patient noticed mild recent memory disturbance and clumsiness in handwriting. In 1976, his gait was markedly unstable, but he could walk without assistance, and his speech became dysarthric. In spring, 1977, he was unable to walk without assistance. From Jan. 9, 1978, through Feb. 14, 1978, he was hospitalized in our hospital. Neurological examination revealed an ataxic gait and scanning speech. Early Diagnostic Measures to Confirm the Diagnosis of Human Prion Diseases.

Human prion diseases are a group of rare and fatal diseases without a cure. Symptoms include rapidly progressive dementia, ataxia, myoclonus, akinetic mutism, and visual disturbances. A broad differential is required to consider prion disease as a diagnosis and rule out other conditions. Historically, to confirm the diagnosis of prion disease, a brain biopsy was needed. Over the past few decades, brain MRI, video electroencephalogram, lumbar puncture results, and a thorough clinical assessment have helped arrive at a probable diagnosis. We present the case of a 60-year-old female with a rapidl
