Risk Factors for Pneumococcal Colonization of the Nasopharynx in Alaska Native Adults and Children.

BACKGROUND: Alaska Native children have high invasive pneumococcal disease (IPD) rates, and lack of in-home running water has been shown to have a significant association with infection. Pneumococcal conjugate vaccines reduced IPD; however, this population saw substantial replacement disease and colonization with nonvaccine serotypes. We evaluated risk factors for nasopharyngeal pneumococcal colonization in Alaska Native adults and children. METHODS: We conducted annual surveys from 2008 through 2011 of residents of all ages in 8 rural Alaskan villages. Interviews were conducted, medical char

Emerging infectious diseases in Alaska and the Arctic: a review and a strategy for the 21st century.

Emergence of new, previously unknown, and drug-resistant infectious diseases pose a major threat to global health. The emergence of infectious diseases in Alaska and the Arctic parallels the resurgence of infectious diseases worldwide. The Centers for Disease Control and Prevention has developed a strategy to revitalize the capacity to protect the public from emerging infectious diseases by improving four major public health activities: surveillance and response, applied research, infrastructure and training, and prevention and control. The plan targets high-priority emerging infectious diseas

What determines survival and morbidity at very early gestation? Alaska's experience.

The charts of all infants (N = 46) 22-25 weeks gestation admitted to the Newborn Intensive Care Unit at Providence Alaska Medical Center from 1991-1994 were reviewed to determine factors associated with mortality and severe morbidity. Survival rates at 22 & 23 weeks gestation was poor (25% and 14% respectively). Survival rates of 24 and 25 week gestation infants were similar (53% and 62% respectively). Infants with "poor" outcomes (death or survival with Grade 3 or Grade 4 intraventricular hemorrhage or periventricular leukomalacia) were more likely to have suboptimal intrapartum care which pr

HISTOPLASMIN SENSITIVITY OF YOUNG MEN IN ALASKA, HAWAII, THE PHILIPPINES AND PUERTO RICO.

The author describes results of a continuing study of skin sensitivity to histoplasmin among United States Navy recruits from Alaska, Hawaii, the Philippine Islands and Puerto Rico. For comparison, material is also included for several groups of recruits from different states of mainland United States. The evidence is interpreted as indicating that the frequent large reactions in Puerto Rican and Filipino recruits represent specific infection with Histoplasma, whereas the small reactions in recruits from Hawaii and the Philippines represent cross-reactions due to sensitization with a presently

Elimination of Hepatitis B virus transmission in an endemic area in Western Alaska.

OBJECTIVES: In the US, hepatitis B virus (HBV) is only endemic in western Alaska, where 90% of the population are Alaska Native (AN) peoples; in the 1970s, the incidence of hepatocellular carcinoma (HCC) in children was the highest in the world. In the 1980s, we screened 53,860 AN peoples for HBV infection, administered HBV vaccine to 43,618 HBV seronegative persons, and initiated universal newborn vaccination. In this study, we examine the impact of this effort 40 years later. METHODS: This is a population-based outcome study. We used vaccine and electronic health records to examine rates of

Body Size and Risk of Death During the 1918 Influenza Pandemic in Alaska.

In the aftermath of the 2009 H1N1 influenza A pandemic, research revealed the relationship between body size and the severity of influenza outcomes. However, there is little data available on body size in historical populations; therefore, the relationship between body size and 1918 influenza pandemic outcomes is virtually unknown. Alaskan death records from the Alaska Bureau of Vital Statistics with recorded height and mass at death (n = 2724) were analyzed to illuminate this relationship during both the pandemic (1918-20) and a non-pandemic period (1917, 1921-25). Binomial logistic regressio

Prenatal vitamin D levels and postpartum hemorrhage in a rural Alaska cohort.

OBJECTIVE: To examine the relationship between prenatal vitamin D serum levels and postpartum bleeding. METHODS: We conducted a retrospective electronic health record cohort study of pregnant Alaska Native and American Indian (ANAI) people in southwestern Alaska who had at least one prenatal vitamin D level measurement recorded and had a vaginal birth between January 2017 and December 2019. Prenatal vitamin D level was the independent variable; postpartum abnormal bleeding and hemorrhage occurrence after a vaginal birth were the primary outcomes. RESULTS: A total of 1343 index pregnancies we

Addressing the Impacts of Racism on American Indian and Alaska Native Child Health.

The legacy of racism toward Native Americans is far-reaching. We will review the topic using the conceptual model of racism as a form of violence as it is inherent in racism, as are prejudice and power. Using the basic frameworks of racism as internalized, interpersonal, institutional, and structural, we will discuss the many types of racism affecting Native Americans today. Racism is the bedrock of generations of trauma experienced in Native communities. The generational/historical trauma of racism has led to epigenetic-level changes affecting Native American people today. We will cover the h

Avian Cholera Causes Marine Bird Mortality in the Bering Sea of Alaska.

The first known avian cholera outbreak among wild birds in Alaska occurred during November 2013. Liver, intestinal, and splenic necrosis consistent with avian cholera was noted, and Pasteurella multocida serotype 1 was isolated from liver and lung or spleen in Crested Auklets (Aethia cristatella), Thick-billed Murres (Uria lomvia), Common Eider (Somateria mollissima), Northern Fulmars (Fulmarus glacialis), and gulls (Larus spp.).

Bartonella spp. exposure in northern and southern sea otters in Alaska and California.

Since 2002, an increased number of northern sea otters (Enhydra lutris kenyoni) from southcentral Alaska have been reported to be dying due to endocarditis and/or septicemia with infection by Streptococcus infantarius subsp. coli. Bartonella spp. DNA was also detected in northern sea otters as part of mortality investigations during this unusual mortality event (UME) in Kachemak Bay, Alaska. To evaluate the extent of exposure to Bartonella spp. in sea otters, sera collected from necropsied and live-captured northern sea otters, as well as necropsied southern sea otters (Enhydra lutris nereis)

Serologic survey for canine distemper and infectious canine hepatitis in wolves in Alaska.

Sera from 57 wolves (Canis lupus) in three areas of Alaska were evaluated for evidence of previous exposure to infectious canine hepatitis virus (ICHV) and canine distemper virus (CDV). Fifty-four sera (94.7%) were positive for ICHV exposure and four (7%) were positive for CDV exposure. All four CDV-reacting wolves also had titres to ICHV. The relatively common occurrence of ICHV exposure may be due to the greater resistance of ICHV to chemical and physical agents and its transmissibility via the urine of infected animals. The ICHV titres observed could indicate enzootic pathogenic ICHV, or ex

Potentially preventable hospitalizations among American Indian and Alaska Native adults, 2016-2021.

OBJECTIVE: Potentially preventable hospitalizations are inpatient admissions for a standard set of selected acute illnesses and chronic conditions that might have been avoided with preventive care or outpatient management. During 2010-2012, Alaska Native adults had higher rates of potentially preventable hospitalizations compared to other adults in Alaska. We evaluated potentially preventable hospitalizations among American Indian/Alaska Native (AI/AN) adults in the United States during 2016-2021. METHODS: We used hospital discharge data from the Indian Health Service National Patient Informa

Misinformation and digital health literacy among American Indian and Alaska Native people.

BACKGROUND: American Indian and Alaska Native (AI/AN) communities in the United States experience significant health disparities, compounded by socioeconomic challenges, systemic racism, limited health information access, and mistrust of health research, mainstream healthcare, and federal health organizations. Misinformation on digital media and variable digital health literacy further hinder informed decision-making. OBJECTIVE: Explore digital health literacy, digital technology use for health information-seeking, and the perceived influence of misinformation on health decision-making among

Serological Detection of Marine Origin Brucella Exposure in Two Alaska Beluga Stocks.

Among emerging threats to the Arctic is the introduction, spread, or resurgence of disease. Marine brucellosis is an emerging disease concern among free-ranging cetaceans and is less well-studied than terrestrial forms. To investigate marine-origin Brucella sp. exposure in two beluga stocks in Alaska, USA, this study used serological status as well as real-time polymerase chain reaction (rtPCR) and bacterial culture. In total, 55 live-captured-released belugas were tested for Brucella exposure in Bristol Bay (2008-2016) and 112 (8 live-captured; 104 subsistence-harvested) whales were tested in

What Should Physicians Consider About American Indian/Alaska Native Women's Reproductive Freedom?

Historically, American Indians and Alaska Natives (AI/AN) have been subjected to a lack of control over various aspects of their lives, including their reproductive health. In discussions of family planning with AI/AN patients, clinicians must consider past violations of reproductive rights and the need for transparent consent. This article explores the following questions: What were historical violations of AI/AN women's reproductive rights? How should physicians express respect for this history and for the autonomy of AI/AN female patients regarding surgical sterilization procedures today?

Direct OPTOS Nerve Size Determination of Prevalent Optic Nerve Hypoplasia in Alaska.

BACKGROUND: Optic nerve hypoplasia (ONH), one of the most common causes of pediatric blindness in developed countries, has been difficult to directly quantify. We sought to measure optic nerve size in Alaskan pediatric patients with optic nerve hypoplasia using ultra-widefield fundus imaging. METHODS: Adult and pediatric patients underwent conventional ultra widefield fundus imaging (OPTOS, Dunfermline, Scotland) with manual image processing to determine optic nerve size validated against refractive error and nystagmus and compared to optical spectral domain tomography. De-identified cases we

Risk-Based Prenatal Hepatitis C Testing Practices and Results, Alaska 2013-2016.

Hepatitis C virus (HCV) infection in pregnant women is of concern as it presents a health threat not only to the mother, but also to her infant. A retrospective analysis was performed to evaluate HCV testing and exposure in women who delivered infants between 2013 and 2016 at a referral hospital in Alaska. Multiple risk behaviors were evaluated, including drug dependency or abuse (drug abuse), tobacco use, alcohol dependency or abuse, and late presentation to prenatal care. Of the 2856 women who delivered between 2013 and 2016, 470 (16.5%) were tested for HCV during pregnancy and 1356 (47.5%)

Comparative measles incidence among exposed military and nonmilitary persons in Anchorage, Alaska.

An outbreak of measles that occurred in Anchorage, Alaska, in 1998 resulted in 33 diagnosed cases: 26 were laboratory confirmed and 7 were clinically confirmed. Twenty-nine (88%) of 33 cases occurred in individuals who had not been immunized with at least two measles-mumps-rubella (MMR) vaccinations; 25 (76%) of 33 occurred in school-age children, 0 to 19 years of age. This study identifies the difference in the incidence of measles between the civilian school-age population, who was not completely immunized (two MMR vaccinations given at least 30 days apart), and the military dependent popula
