Inhalation versus intravenous anesthesia for adults undergoing heart valve surgery: a systematic review and meta-analysis.

INTRODUCTION: Many studies have found that volatile anesthetics are associated with improved clinical outcomes for adults undergoing coronary artery bypass grafting. However, the effect of volatile anesthetics for adults after heart valve surgery has been unclear. So we conducted a meta-analysis of randomized controlled trials (RCTs) to explore whether the choice of an anesthetic regimen might influence patients' outcomes after valve surgery. EVIDENCE ACQUISITION: PubMed, Embase, and Cochrane Library were searched from inception to June 2018. We included eligible research comparing inhalation

[A critical patient relieved from status asthmaticus with isoflurane inhalation therapy].

In cases of life-threatening status asthmaticus which are refractory to drug therapy, the administration of inhalation anesthetics can be life-saving as they help alleviate bronchial spasm. We had an 11-year-old female patient suffering from status asthmaticus who was moribund from severe CO2 narcosis and was not responding to any of the conventional therapies. She finally fell into ventricular fibrillation. After cardiopulmonary resuscitation, we administered 2.0% isoflurane in oxygen. Within half an hour, her high inspiratory pressure was dramatically decreased, and then the isoflurane conce

Effect of diazepam treatment on hepatic microsomal anesthetic defluorinase activity.

Different dosing regimens with diazepam were evaluated in male Fischer 344 rats for their ability to enhance the metabolism of several inhalation anesthetics. In vitro metabolism was assessed by the rate of microsomal anesthetic defluorination as measured by the appearance of inorganic fluoride ion (i.e., defluorinase activity). Single daily doses of diazepam, 40 mg/kg, were delivered by gavage for either 7 or 14 days; more continuous dosing was achieved by administration of diazepam at 0.03% or 0.10% in food pellets. Hepatic microsomes isolated from treated rats showed significant dose-relate

Anesthesia and sedation of foals.

Physiologic similarities and differences between foals and adult horses, including response to pain, cardiopulmonary function, ability to compensate for dehydration and hemorrhage, and response to anesthetic drugs, are considered in this article. Preanesthetic evaluation, choice of anesthetic drugs, technique, and monitoring support requirements through to anesthetic recovery are based on these physiologic considerations. Anesthetic techniques discussed include drugs for premedication, parenteral or inhalational anesthetic induction, and maintenance using inhalational and parenteral anesthesia

Effects of propofol on Ca2+ regulation by malignant hyperthermia-susceptible muscle membranes.

BACKGROUND: The effects of inhalation anesthetics on Ca2+ regulation in malignant hyperthermia-susceptible skeletal muscle are considered to be responsible for triggering malignant hyperthermia. The intravenous anesthetic propofol does not trigger malignant hyperthermia in susceptible patients or experimental animals, suggesting that there are important differences between the effects of propofol and the effects of inhalation anesthetics on Ca2+ regulation in malignant hyperthermia-susceptible muscle. Understanding these differences may help to clarify the mechanisms responsible for triggering

Aerosol propellant interference with clinical mass spectrometers.

Metered dose inhalers containing halogenated propellants may interfere with mass spectrometer quantitation of halogenated inhalation anesthetics. We identify the propellant(s) in a commercially available metered dose inhaler that caused erroneous mass spectrometer readings. In addition, we identify the causes of different types of interference in different mass spectrometers.

Therapeutic applications and uses of inhalational anesthesia in the pediatric intensive care unit.

OBJECTIVE: To review the physical properties, end-organ effects, therapeutic applications, and delivery techniques of inhalational anesthetic agents in the pediatric intensive care unit. DATA SOURCE: A computerized, bibliographic search regarding intensive care unit applications of inhalational anesthetic agents. MAIN RESULTS: Although the end-organ effects of inhalational anesthetic agents vary depending on the agent, general effects include a dose-related depression of ventilatory and cardiovascular function. With increasing anesthetic depth, there is a decrease in alveolar ventilation wit

The association between intraoperative anesthesia methods used during gastric cancer surgery and long-term mortality: A retrospective observational study using a Japanese claims database.

PURPOSE: Various basic and clinical studies have investigated the association between the types of anesthetic agents and prognosis. However, the results have varied among studies and remain controversial. In the present study, we aimed to investigate whether the risk of all-cause mortality differs between inhaled or intravenous anesthetics in patients with gastric cancer undergoing gastrectomy. METHODS: Using a Japanese nationwide insurance claims database, we analyzed patients who underwent gastrectomy under general anesthesia for gastric cancer between January 2005 and September 2019. Ventilatory effects of the nonimmobilizer 1,2-dichlorohexafluorocyclobutane (2N) in swine.

UNLABELLED: Nonimmobilizers (inhaled compounds that do not suppress movement in response to a noxious stimulus) resemble anesthetics in their capacity to suppress memory, but unlike anesthetics, they can cause convulsions. Higher concentrations of nonimmobilizers may cause death, even with apparent suppression of convulsions by the concurrent administration of conventional inhaled anesthetics. We hypothesized that nonimmobilizers can depress ventilation and can cause death by adding to the depression of ventilation produced by conventional anesthetics. To test these hypotheses, we administered

Alterations in the course of acid-induced lung injury in rats after general anesthesia: volatile anesthetics versus ketamine.

UNLABELLED: Pulmonary aspiration of gastric acid is a complication that occurs during anesthesia. The effects of the often used anesthetics on the inflammatory response after aspiration of acid are not known. We examined the effects of three different inhaled anesthetics--halothane, enflurane, and isoflurane--as well as parenteral ketamine, on the associated immediate mortality, alveolar protein leakage, and morphometric changes after intrapulmonary instillation of acidic solution in rats. Animals in deep state of anesthesia had a higher mortality after the instillation of acidic solutions tha

Studies on the mechanism of general anesthesia.

Inhaled anesthetics are a chemically diverse collection of hydrophobic molecules that robustly activate TWIK-related K+ channels (TREK-1) and reversibly induce loss of consciousness. For 100 y, anesthetics were speculated to target cellular membranes, yet no plausible mechanism emerged to explain a membrane effect on ion channels. Here we show that inhaled anesthetics (chloroform and isoflurane) activate TREK-1 through disruption of phospholipase D2 (PLD2) localization to lipid rafts and subsequent production of signaling lipid phosphatidic acid (PA). Catalytically dead PLD2 robustly blocks an

[The biotransformation of inhalation anaesthetics and its relevance to clinical side effects (author's transl)].

The volatile anesthetics are not inert substances, but they are metabolised by means of a non-specific drug metabolizing enzyme system of the liver. Thus, in combined administration of several drugs, the knowledge of the metabolic pathways and the influence of the anesthetics on the transforming system is not only theoretically important, but represents a conditio sine qua non to the anesthesist as regards the handling of modern anesthetics. The occasionally observed liver and kidney injuries indicate a potential formation of toxic metabolites. Renal damage due to methoxyflurane may be caused

Ambulatory surgery and malignant hyperthermia.

PURPOSE OF REVIEW: It is timely to review recent publications that add to our knowledge of malignant hyperthermia because we can go to the web or to newspapers and read of recent tragic malignant hyperthermia deaths. RECENT FINDINGS: New methods were used to affirm some of the previous observations regarding the epidemiology of malignant hyperthermia, and the well known North American Malignant Hyperthermia Registry was used to describe characteristics of malignant hyperthermia deaths. Anesthesia workstations have been found to have a larger reservoir of potent inhalation anesthetics than did

Obstetric analgesia: pharmacokinetics and its relation to neonatal behavioral and adaptive functions.

The neonatal pharmacokinetic and neurobehavioral properties of certain agents used in obstetric analgesia are reviewed (local anesthetics, opiates, inhalation agents, benzodiazepines). Fetal and neonatal pharmacokinetic alterations partly explain the neurobehavioral differences observed between different drug groups and ways of drug administration. The most effective and safest method with fewest neonatal neurobehavioral effects appears to be regional epidural analgesia performed with plain bupivacaine. The use of epidural opiates remains problematic. Inhalation agents and parenteral pethidine

[Anesthesia for a patient with xeroderma pigmentosum].

The anesthetic management of a patient with xeroderma pigmentosum is described. A 17-year-old woman underwent tracheostomy because of progressive recurrent nerve palsy. The operation was performed uneventfully under general anesthesia using propofol and fentanyl. Xeroderma pigmentosum (XP) is an autosomal recessive disease that is characterized by hypersensitivity to sunlight with a high incidence of skin cancer and exhibits variable neurological abnormalities. Classical types of XP have a defect in nucleotide excision repair (NER). It has been reported that volatile anesthetics such as haloth

Malignant hyperthermia.

Malignant hyperthermia is a rare syndrome that occurs in genetically susceptible individuals who are exposed to frequently used inhalation anesthetics. The disorder is most common in children and young adults. It is triggered through a defect in the ability of skeletal muscles to concentrate and release calcium. Signs of malignant hyperthermia include hypercarbia, muscle rigidity and tachycardia. Temperature elevation is often a late sign of the syndrome. Treatment begins with stopping all inhaled anesthetics at the earliest sign of the syndrome. The use of dantrolene has significantly reduced

Masseter muscle spasm in children: implications of continuing the triggering anesthetic.

This retrospective study was undertaken to examine the management and outcome of children who developed isolated masseter muscle spasm (MMS) after the administration of intravenous succinylcholine during anesthetic induction. The inhalation anesthetics used for induction were continued in all of these cases. The medical records of 68 patients (male/female ratio, 1.7:1), identified from approximately 42,000 anesthetics given during the period 1980-1989, were reviewed. Fifty-seven children (2.3-12 yr old) were diagnosed as having isolated MMS, i.e., MMS without spasm of other muscles; 11 experie

[Lung protection with anesthetics].

Common inhaled and intravenous anesthetics except the barbiturate are recommended for the patients with bronco-constrictive lung disease because of their bronco-dilating property. However there are pharmacological potency differences between individual anesthetics. The halogenated inhaled anesthetics and propofol exert antiinflammatory effect on acute lung injury and acute respiratory distress syndrome in the laboratory level, but further study is required for future clinical application. These anesthetics also have an organ protective effect on the ischemia reperfusion lung injury, and their

Inhalational anesthesia: basic pharmacology, end organ effects, and applications in the treatment of status asthmaticus.

The potent inhalational anesthetic agents are used on a daily basis to provide intraoperative anesthesia. Given their beneficial effects on airway tone and reactivity, they also have a role in the treatment of status asthmaticus that is refractory to standard therapy. Although generally not of clinical significance, these agents can affect various physiological functions. The potent inhalational anesthetic agents decrease mean arterial pressure and myocardial contractility. The decrease in mean arterial pressure reduces renal and hepatic blood flow. Secondary effects on end-organ function may

Management of anesthesia in the foal.

Several unique pharmacologic and physiologic factors must be considered when attempting to anesthetize premature or neonatal foals. Juvenile foals are similar to adults in their physiology and metabolism. Anesthetic drug and protocol selection should reflect the differences between these two age groups. Neonates are best anesthetized using an inhalation technique, whereas older foals can be safely anesthetized with either parenteral or inhalation anesthetic agents. Careful monitoring is absolutely essential when anesthetizing foals. The clinician should plan to routinely administer fluids and
