Anesthetic Management Using Epidural Anesthesia and Continuous Intravenous Remifentanil in a Cesarean Section With Tethered Cord Syndrome: A Case Report.

Spinal anesthesia is contraindicated in parturients with tethered cord syndrome undergoing cesarean sections. The epidural approach is an attractive anesthetic alternative. However, sacral complications can compromise their efficacy. A 33-year-old parturient with a history of spina bifida and tethered cord syndrome was admitted for a cesarean section. Since imaging studies showed no abnormalities in the epidural space at the lumbar level, the anesthesiologist proposed epidural anesthesia. Continuous intravenous remifentanil provided additional analgesia. During uterine incision closure, the pa

Spontaneous and oxytocin-induced contractility of gravid human myometrium with exposure to intravenous anesthetic agents: an ex vivo laboratory study.

PURPOSE: The objective of this study was to investigate the ex vivo effects of various intravenous anesthetic drugs on the contractility of gravid human myometrium. We hypothesized that these drugs would cause a concentration-dependent decrease in spontaneous uterine contractility and oxytocin coadministration would modify their contractile response. METHODS: We conducted an ex vivo laboratory study with individual myometrial strips obtained from patients undergoing elective Cesarean delivery. We subjected the myometrial strips to concentration-response testing in organ bath chambers with int

Intravenous lidocaine reduces the propofol EC50 for loss of consciousness and intraoperative anesthetic consumption in gynecological laparoscopy: A randomized controlled trial.

BACKGROUND: Intravenous lidocaine reduces propofol requirements and procedure-related adverse events. OBJECTIVES: The study aimed to test whether intravenous lidocaine would reduce the effect-site concentration of propofol required to achieve loss of consciousness and decrease propofol consumption during total intravenous anesthesia in gynecological laparoscopy. METHODS: This was a prospective, randomized, double-blind, placebo-controlled trial. Sixty patients were randomly allocated to receive either intravenous lidocaine (1.5 mg·kg-¹ bolus) followed by continuous infusion or an equal volum

Anesthetic-sparing effect of dexmedetomidine during total intravenous anesthesia for children undergoing dental surgery: A randomized controlled trial.

BACKGROUND: Dexmedetomidine, an α2-adrenergic agonist, reduces propofol and remifentanil requirements when used as an adjunct to total intravenous anesthesia in adults, but studies in a pediatric population are sparse. This study investigates the magnitude of dose-sparing effects of a postinduction dexmedetomidine bolus on propofol and remifentanil requirements during pediatric surgery. METHODS: In this randomized, double-blind, controlled trial, children aged 2-10 years undergoing elective dental surgery were assigned to one of four groups: placebo, 0.25 mcg/kg dexmedetomidine, 0.5 mcg/kg de

Mild hypothermia is associated with altered volume kinetic parameters of an intravenous crystalloid fluid bolus in healthy isoflurane-anesthetized cats.

OBJECTIVE: To assess the impact of mild hypothermia on the distribution and elimination of an IV crystalloid fluid bolus in healthy anesthetized cats using volume kinetic (VK) analysis. METHODS: 10 adult cats were anesthetized and included in a prospective, randomized, cross-over study. The subjects were maintained either normothermic (38.3 ± -16.9 °C) or mildly hypothermic (35 ± -16.9 °C), with a 7-day washout period between anesthetic episodes. All cats received 20 mL/kg of a balanced isotonic solution (Normosol-R) IV over 20 minutes, following the achievement and stabilization of target te

Intravenous anesthetic ketamine attenuates complete Freund's adjuvant-induced arthritis in rats via modulation of MAPKs/NF-κB.

BACKGROUND: The current study was intended to investigate the effect of ketamine (KET) on complete Freund's adjuvant (CFA)-induced arthritis in rats. METHODS: The CFA was administered in the hind paw of the rats for the induction of adjuvant-induced arthritis. The paw swelling of experimental animals was measured as hind paw volume. Hematoxylin and eosin staining was estimated and pathological changes in the joint tissues were observed under a light microscope. Furthermore, the bicinchoninic acid assay was used for protein quantification. The antibody-reactive bands were visualized using enha

[Anesthetic management of a child with myotubular myopathy who underwent two operations using only total intravenous anesthesia (TIVA)].

We present a case of anesthetic management in a child with myotubular myopathy. A 3-month-old, 3.0 kg, male patient, who had been suspected of a congenital myopathy, was scheduled for the muscle biopsy. He was intubated at birth in NICU. Anesthesia was induced with propofol and remifentanil, and maintained with propofol and remifentanil. The results of biopsy and gene analysis led to the diagnosis of myotubular myopathy. Five months later, this 8-month-old, 4.0 kg, patient was scheduled for the tracheostomy. Anesthesia was induced with propofol, fentanyl and rocuronium bromide, and maintained

Tryptophan and Cysteine Mutations in M1 Helices of α1β3γ2L γ-Aminobutyric Acid Type A Receptors Indicate Distinct Intersubunit Sites for Four Intravenous Anesthetics and One Orphan Site.

BACKGROUND: γ-Aminobutyric acid type A (GABAA) receptors mediate important effects of intravenous general anesthetics. Photolabel derivatives of etomidate, propofol, barbiturates, and a neurosteroid get incorporated in GABAA receptor transmembrane helices M1 and M3 adjacent to intersubunit pockets. However, photolabels have not been consistently targeted at heteromeric αβγ receptors and do not form adducts with all contact residues. Complementary approaches may further define anesthetic sites in typical GABAA receptors. METHODS: Two mutation-based strategies, substituted tryptophan sensitivit

Gastric vagal efferent inhibition evoked by intravenous CRF is unrelated to simultaneously recorded vagal afferent activity in urethane-anesthetized rats.

Corticotropin-releasing factor (CRF) injected peripherally or released in response to stressful challenges to the organism reduces gastric tone and contractility, in part by vagal pathways. However, information on the changes in gastric vagal impulse activity evoked by peripheral CRF administration is entirely lacking. Using a novel "dual recording" method in urethane-anesthetized rats, vagal efferent (VE) and afferent (VA) impulse activities were recorded simultaneously from separate, fine bundles dissected from the ventral gastric vagus nerve branch innervating the glandular stomach. Intravenous and Intratracheal Thyrotropin Releasing Hormone and Its Analog Taltirelin Reverse Opioid-Induced Respiratory Depression in Isoflurane Anesthetized Rats.

Thyrotropin releasing hormone (TRH) is a tripeptide hormone and a neurotransmitter widely expressed in the central nervous system that regulates thyroid function and maintains physiologic homeostasis. Following injection in rodents, TRH has multiple effects including increased blood pressure and breathing. We tested the hypothesis that TRH and its long-acting analog, taltirelin, will reverse morphine-induced respiratory depression in anesthetized rats following intravenous or intratracheal (IT) administration. TRH (1 mg/kg plus 5 mg/kg/h, i.v.) and talitrelin (1 mg/kg, i.v.), when administered

Cerebral function and muscle afferent activity following intravenous succinylcholine in dogs anesthetized with halothane: the effects of pretreatment with a defasciculating dose of pancuronium.

The effects of iv succinylcholine (SCh) on cerebral blood flow (CBF), muscle afferent activity (MAA), electromyographic activity (EMG), visible fasciculations, and PaCO2 were tested in 12 dogs anesthetized with 0.87% end-expired halothane (1 MAC). Six dogs (group I) received treatments of both SCh 1.0 mg/kg iv and saline placebo 3.0 ml iv. Fasciculations and substantial increases in EMG activity were observed in all six dogs given SCh. At the onset of fasciculations, there were parallel increases in MAA and CBF to peak values of 466% +/- 77% of control (mean +/- SE) and 136% +/- 5% of control,

Apparent Reversal of a Successful Peripheral Neural Blockade With Intravenous Lipid Emulsion After Treatment for Local Anesthetic Systemic Toxicity: A Case Report.

The use of local anesthetics for improved pain management is well established. However, significant morbidity may be caused by local anesthetic systemic toxicity (LAST) from inadvertent intravascular injection or excessive dosing of local anesthetics. Despite incomplete understanding of the mechanism of action of intravenous lipid emulsions (ILE), their use has become a first-line therapy for treating LAST. We present a case report of LAST, successfully treated with ILE with a secondary effect of complete reversal of a successful peripheral nerve block as quickly as the LAST symptoms resolved.

Total intravenous anesthesia vs inhaled anesthetic for intraoperative visualization during endoscopic sinus surgery: a double blind randomized controlled trial.

BACKGROUND: Bleeding during endoscopic sinus surgery (ESS) can impair visualization and delay surgical progress. The role that anesthetic technique may have on the quality of surgical field during ESS has been previously studied. However, meta-analyses have deemed the current literature inconclusive and lacking methodological consistency. This study was designed with these critiques in mind to assess the effect of total intravenous anesthesia (TIVA) vs inhaled anesthetic on the quality of the surgical field during ESS. METHODS: This study was a double-blind, randomized, controlled trial of 30

[On the toxicity of CT-1341 evoked by long-term administration. I. Subacute toxicity of the intravenous anesthetic, CT-1341 in rats (author's transl)].

CT-1341, an intravenous anesthetic was given in various daily doses in rats for a period of one month to test the subacute toxicity. The drug was administered intraperitoneally in rats. Rats tolerated to daily administration of CT-1341 at doses of less than 1.8 ml/kg without showing other particular toxic signs than anesthesia. Main pathological findings were swelling of cells in the liver and renal tubules, and perivascular cuffing in lungs. No severe patho-histological changes were observed in any organs. Mortal cases were seen in the group of rats, in which CT-1341 was given in a daily dose
