Salmonella paratyphi B mycotic aneurysm of the abdominal aorta in an HIV-infected patient: a case report.

An HIV-infected 49-year-old man was admitted with polyuria, fever, chills and a dull left lumbar pain. Laboratory tests showed increased C-reactive protein while urine analysis and abdomen ultrasound scan were negative. Blood cultures revealed a Salmonella paratyphi B, identified through MALDI-TOF mass spectrometry. Targeted antibiotic therapy with intravenous piperacillin/tazobactam was started and a multi-phase contrast-enhanced abdomen CT-scan was performed at 24 hours from admission showing a saccular aneurysm of the abdominal aorta with a 1 cm penetrating aortic ulcer on posterior wall. [Extra-anatomic bypass as therapy of infected bacterial (mycotic) infrarenal aortic aneurysm. A comparative, literature supported analysis].

Based on a review of the literature we compared the results of various operative procedures for infrarenal bacterial infected (mycotic) aortic aneurysms. We conclude that the preferred therapy involves the placement of an extraanatomic bypass, resection of the aneurysm and thorough debridement of infected tissue. Obligatory is appropriate systemic antibiotic therapy. The mortality rate has been substantially reduced since the introduction of this therapeutic concept.

Infective native arterial aneurysms and inflammatory abdominal aortic aneurysms: An overview with a focus on emergency settings.

Infective native arterial aneurysms and inflammatory aortic aneurysms are rare but morbid pathologies seen by vascular surgeons in the emergency setting. Presentation is not always clear, and a full workup must be obtained before adopting a management strategy. Treatment is multidisciplinary and is tailored to every case based on workup findings. Imaging with computed tomography, magnetic resonance, or with fluorodeoxyglucose-positron emission tomography aids in diagnosis and in monitoring response to treatment. Open surgery is traditionally performed for definitive management. Endovascular su

Abdominal aortic aneurysm and virus infection: A potential causative role for cytomegalovirus infection?

An abdominal aortic aneurysm (AAA) is a multifactorial disease with a variety of genetic and environmental risk factors, but the exact mechanism of AAA formation and progression is still not well understood. The present study investigated the frequency of cytomegalovirus (CMV), Epstein-Barr virus (EBV), and papillomavirus types 6 and 11 (HPV6 and HPV11), their impact on clinical manifestations of cardiovascular diseases, and their possible association with inflammation in patients with AAA and healthy volunteers. Genotyping of CMV UL75, EBV LMP-1, and HPV6, and HPV11 E6 was performed by polyme

Management of Infective Endocarditis and Tibioperoneal Mycotic Aneurysm Following COVID-19 Infection.

INTRODUCTION: Infective endocarditis and mycotic tibioperoneal aneurysms are rare complications of COVID-19 infection. Medical therapy may not always be sufficient to reduce the high morbidity and mortality associated with these cardiovascular complications. Surgical treatment may need to be considered in such patients. REPORT: A 56 year old male patient with diabetes, hypertension, and hyperlipidaemia developed severe pneumonia from COVID-19 infection. He was admitted to the intensive care unit (ICU) at another facility where he was ventilated for a period of six weeks. Blood culture isolate

Single-stage multidisciplinary management of contiguous infection involving infected abdominal aortic aneurysm and lumbar spondylodiscitis following infective endocarditis.

Infective endocarditis can cause septic embolization and metastatic infections, including infected aortic aneurysm and spondylodiscitis. We report the case of a 77-year-old man with contiguous infection who was successfully treated using a single-stage multidisciplinary approach. Following antibiotic therapy that controlled the endocarditis, rapid expansion of an abdominal aortic aneurysm indicated impending rupture. Simultaneous surgery included in situ reconstruction using a rifampicin-soaked graft, infection-avoiding graft routing, spinal debridement, autologous bone reconstruction, and ome

Complicated mycotic saccular aneurysm of the infra-renal abdominal aorta with infected retroperitoneal hematoma: a clinical case report.

BACKGROUND: Rupture of an abdominal aortic aneurysm (AAA) is a life-threatening emergency, with untreated cases nearing a 100% mortality rate. This case presents a rare complication of AAA rupture with an infected retroperitoneal hematoma, emphasizing the importance of timely diagnosis and a multidisciplinary approach. CASE PRESENTATION: A 59-year-old male presented with lower back pain, fever, and difficulty moving, persisting for three weeks. Imaging revealed a saccular infra-renal AAA rupture with an infected retroperitoneal hematoma. Emergency surgery included extracorporeal subclavian-fe

An ex-situ arterial reconstruction for the treatment of an infected suprarenal abdominal aortic aneurysm involving visceral vessels.

A 66-year-old man, who had complaints of back pain, high fever, and constipation, with a suprarenal infected abdominal aortic aneurysm involving visceral vessels, was successfully treated by an ex situ arterial reconstruction. Blood culture revealed bacteroides melaninogenicus. After establishing a temporary bypass from right axillary to right external iliac artery, both renal arteries--superior mesenteric artery and common hepatic artery--were preliminary bypassed from external iliac arteries. Then aortic trunk reconstruction was performed with the body twisting left side to separate the graf

Mycotic Aneurysm in Salmonella enteritidis Infection: A Fatal Complication.

Mycotic aneurysms are rare but severe complications that can arise from systemic bacterial infections, including those caused by Salmonella species. These aneurysms can progress rapidly and are associated with high mortality. A 62-year-old man with poorly controlled type 2 diabetes mellitus presented to the hospital in septic shock. The blood culture grew Salmonella enteritidis. A computed tomography angiogram showed a ruptured mycotic aneurysm of the infrarenal abdominal aorta. Despite antibiotic therapy and supportive care, the patient's condition rapidly deteriorated, resulting in his death

Multiple mycotic aneurysms with infective endocarditis: A case report.

Mycotic aneurysms are sometimes seen in patients with infective endocarditis. We report a case of infective endocarditis with multiple mycotic aneurysms. Although antibiotics were effective, mycotic aneurysms appeared in the cerebral, hepatic, and gastroepiploic arteries. A 55-year-old man presented with mitral valve endocarditis due to Streptococcus oralis. Surgical treatment was deferred because of cerebral hemorrhage. After antibiotic initiation, his fever and C-reactive protein levels declined, and blood culture was negative. However, he experienced repeated cerebral hemorrhage and the num
