Don't Miss This! Red Flags in the Pediatric Eye Exam: Pupils.

The pupillary exam in the pediatric population is a vital part of any clinician's workup. In the right clinical setting, pupillary abnormalities such as anisocoria, light-near dissociation, an afferent pupillary defect, and paradoxic pupillary constriction in the dark can be red flags that trigger further examination and workup. Through both careful physical examination and detailed history-taking and observation, potentially vision- and life-threatening conditions can be detected.

Neurosyphilis Masquerading as an Acute Adie's Tonic Pupil: Report of a Case.

We describe the case of a male patient who presented with anisocoria, and was initially diagnosed with an acute Adie's tonic pupil. On subsequent laboratory testing, he was found to have neurosyphilis. Magnetic resonance imaging demonstrated enhancement of the right oculomotor nerve. This case underscores the importance of considering this diagnosis in patients with acute onset internal ophthalmoplegia and hypersensitivity to dilute pilocarpine, even in the absence of other oculomotor nerve findings.

[Neurological deterioration following a marathon - atypical manifestation of basilar artery occlusion].

We report on a 27-year-old female patient who developed a progressive neurological disorder (anisocoria, dysarthria and facial nerve paresis) after participating in a marathon race. She was admitted to a maximum care level hospital after endotracheal intubation with a provisional diagnosis of intracranial bleeding. Imaging studies (computed tomography and computed tomographic angiography) revealed basilar artery occlusion. A successful outcome was achieved following thrombolysis within two hours of the initial onset of symptoms.

Orbital apex syndrome associated with herpes zoster ophthalmicus.

We report our findings for a patient with orbital apex syndrome associated with herpes zoster ophthalmicus. Our patient was initially admitted to a neighborhood hospital because of nausea and loss of appetite of 10 days' duration. The day after hospitalization, she developed skin vesicles along the first division of the trigeminal nerve, with severe lid swelling and conjunctival injection. On suspicion of meningoencephalitis caused by varicella zoster virus, antiviral therapy with vidarabine and betamethasone was started. Seventeen days later, complete ptosis and ophthalmoplegia developed in t

Friederich Nietzsche and the seduction of Occam's razor.

Friedrich Nietzsche developed dementia at the age of 44 years. It is generally assumed that the cause of his dementia was neurosyphilis or general pareisis of the insane (GPI). Others have proposed frontal-based meningioma as the underlying cause. We have reviewed Nietzsche's medical history and evaluated the evidence from the medical examinations he underwent by various physicians. We have viewed the possible diagnosis of GPI or meningioma in light of present neuro-ophthalmic understanding and found that Nietzsche did not have the neurological or neuro-ophthalmic symptoms consistent with a di

Concomitant ectatic posterior communicating artery and tentorial meningioma as a source of oculomotor palsy: case report.

OBJECTIVE AND IMPORTANCE: Although non-aneurysmal vascular compression of the oculomotor nerve is rare, it should be considered in the evaluation of unilateral oculomotor palsy. CLINICAL PRESENTATION: A 36-year-old non-diabetic man presented with two months of intermittent retro-orbital headache and third nerve paresis caused by compression of the oculomotor nerve between an ectatic, atherosclerotic posterior communicating artery (PComA) and a small tentorial meningioma. At operation, the subarachnoid portion of the nerve, prevented from migrating posteriorly and laterally by the meningioma,

[Pupillometry in Menière's disease].

The autonomic dysfunction is one of the etiopathogenetic hypothesis of Ménière's disease. We have already described the presence of an anomalous pupillary contraction, induced by methacholine, during the attack stage of Ménière's disease, without cardio-vascular reflexes involving. We have now employed, in a larger number of cases, a new digital equipment with infrared lighting system, built by us. The pupillary area was measured in darkness by a personal computer in basal condition (time 0) and 10, 20, 30, 45, 60 minutes after methacholine instillation. We have observed 16 cases of Ménière's

[Strabologic and neuro-ophthamologic aspects of childhood cancer].

Pediatric oncology has achieved major progress by continuous optimization of diagnostic and therapeutic approaches. In the interdisciplinary team, the ophthalmologist plays an important role. In the field of strabismus and neuro-ophthalmology clinical symptoms as strabismus, visual loss, anisocoria, visual field defects, and involuntary eye movements may be key indicators of childhood cancer. The appropriate diagnostic workup as well as the diagnostic and therapeutic steps during the course of the disease are important often requiring individually tailored approaches.

A-beta-lipoproteinaemia. Ocular involvement in a Danish case.

A-beta-lipoproteinaemia in a Danish woman is reported. The ocular involvement comprised pigmentary retinopathy, anisocoria and reduced corneal sensibility. Serum concentrations of vitamin A and retinol-binding protein (RBP) were low, but were normalized after vitamin treatment. Serum concentration of carotene was low, and not altered by treatment. The retinopathy progressed over a 2 1/2 year period in spite of controlled serum vitamin A and RBP concentrations. The possible role of vitamin A in the development of retinopathy is discussed.

Peduncular hallucinosis associated with ruptured basilar-superior cerebellar artery aneurysm--case report.

A 65-year-old female developed peduncular hallucinosis 3 days after rupture of a basilar-superior cerebellar artery aneurysm. There were no neurological deficits except slight anisocoria when she first complained of hallucinations. Vasospasm of the perforating arteries to the upper brainstem, rather than direct brainstem damage caused by the bleeding, was probably the cause. Peduncular hallucinosis is possibly the only localizing sign of ruptured upper posterior circulation aneurysm.

Brimonidine eye drops reveal diminished sympathetic pupillary tone in comatose patients with brain injury.

BACKGROUND: There is an urgent need for easy-to-perform bedside measures to detect residual consciousness in clinically unresponsive patients with acute brain injury. Interestingly, the sympathetic control of pupil size is thought to be lost in states of unconsciousness. We therefore hypothesized that administration of brimonidine (an alpha-2-adrenergic agonist) eye drops into one eye should produce a pharmacologic Horner's syndrome if the clinically unresponsive patient is conscious, but not if the patient is unconscious. Here, in a first step to explore this hypothesis, we investigated the p

[Usefulness of apraclonidine in the diagnosis of Horner syndrome].

CASE REPORTS: We present four cases: two males with Horner Syndrome, who were diagnosed by means of apraclonidine 0.5% and cocaine 4% tests respectively. Two children with anisocoria, one of whom had Horner Syndrome confirmed with apraclonidine 1% and the other, in whom an apraclonidine test ruled out this syndrome but in whom pilocarpine 0.125% confirmed Adie's pupil. DISCUSSION: Apraclonidine drops (1% and 0.5%) may aid in the diagnosis of Horner Syndrome and are easier to obtain than cocaine.

Congenital varicella syndrome.

The authors are reporting a typical case of congenital varicella syndrome following maternal varicella during the 17th week of pregnancy. At birth, the newborn showed necrotic bullae on the skin that healed later with characteristic scars. Other typical anomalies, i.e. hypoplastic limbs with muscular atrophy and clubfoot, intrauterine atrophy, dysphagia and anisocoria were also found. In view of the risk of serious malformations following intrauterine varicella infection attempts should be made to prevent varicella zoster virus infection during pregnancy.

Isolated Internal Ophthalmoplegia Secondary to Herpes Zoster Ophthalmicus: A Rare Case Report.

Herpes zoster becomes latent in the sensory ganglia after the primary infection and may reactivate many years later to produce shingles in adults. Involvement of the ophthalmic branch of the trigeminal nerve with this virus is called Herpes zoster ophthalmicus (HZO). The eyelid skin, cornea, uvea, retina, optic nerve and other cranial nerves can be affected by HZO. Isolated internal ophthalmoplegia can rarely be seen. Clinicians should be aware of this condition and HZO must be kept in mind in the differential diagnosis of anisocoria.

Neuro-ophthalmological emergencies: which ocular signs or symptoms for which diseases?

There are five possible ocular signs or complaints of a life or sight threatening neuro-ophthalmological condition: diplopia, isolated anisocoria, transient visual loss, severe pain in head or neck (with or without photophobia) and oscillopsia/nystagmus. In this review, the ocular signs and symptoms of neuro-ophthalmological emergencies are described together with their practical work-up and the risks associated with missing the diagnosis. Concerning diplopia, the associated signs pointing to a possible threatening condition are emphasized. Six focus points resuming core messages are displayed

But it's not all there.

A 44-year-old man presented with acute onset headache and isolated anisocoria. Dilation lag was noted in the smaller pupil and Horner syndrome was suspected despite the lack of eyelid ptosis. Cocaine testing confirmed the clinical diagnosis, and urgent neuroimaging found an internal carotid artery dissection. The patient was managed with systemic anticoagulation with a good final outcome. Horner syndrome should be suspected in any patient with aniscoria and dilation lag, despite the absence of eyelid ptosis or other associated findings.

Acute subdural hematoma following disseminated intravascular coagulation associated with an obstetric catastrophe.

The clinical, laboratory, radiological and operative recordings of a patient with disseminated intravascular coagulation (DIC) related to therapeutic abortion is reported. During a comatose state following respiratory arrest, anisocoria with right dilated unreactive pupil and decerebration signs appeared. A brain CT scan showed a right frontotemporal hemorrhage. On surgical intervention a subdural hematoma was found and removed. The patient remained in a persistent vegetative state. The rarity of subdural hematoma complicating DIC is presented and the causes of intracranial bleeding in obstetr

Side effects of topical atropine 0.05% compared to 0.01% for myopia control in German school children: a pilot study.

PURPOSE: Based on findings of the Asian low-concentration atropine for myopia progression study, a concentration of 0.05% has been proposed as a good compromise between safety and efficacy for myopia control. However, no data on side effects have been published so far in Caucasian children receiving this dose. METHODS: Prior to commencement of bilateral atropine treatment with 0.05% atropine, 19 myopic children aged 5 to 15 years were treated in only one eye at bedtime leaving the other eye as a control. Pupil size, accommodation amplitude and near visual acuity were measured at 10:00 a.m. th

[Pathogenesis and management of secondary neural damage in head trauma patients: analysis of patients who talk and deteriorate "fulminantly"].

To clarify the pathogenesis and management of secondary neural damage after head trauma, we characterized the clinical features and CT findings in nine patients who talked and deteriorated "fulminantly". The patient who talks and deteriorates "fulminantly" is defined as a patient who talks (verbal score of Glasgow Coma Scale > or = 4) on admission, and subsequently shows a rapid neurological deterioration to develop an anisocoria and/or a decerebrated posture within 24 hours after trauma. Nine (8%) out of 118 patients with severe head trauma satisfied the definition mentioned above. The mechan
