Trends in Complication Rates Following Ankle Arthrodesis and Total Ankle Replacement.

BACKGROUND: This study provides an updated comparison of the reoperation rates following primary ankle arthrodesis and total ankle replacement on the basis of observational, population-based data from California. We previously reported data from 1995 to 2004, and our current study includes new data from 2005 to 2010. Our hypothesis was that total ankle replacement would demonstrate increasing utilization and lower complication rates given advances in implant design and growth in surgeon experience. METHODS: California's hospital discharge database was used to identify patients who had undergo

Dynamic ankle control in athletes with ankle instability during sports maneuvers.

BACKGROUND: Ankle sprain is a common sports injury. While the effects of static constraints in stabilizing the ankle joint are relatively well understood, those of dynamic constraints are less clear and require further investigation. PURPOSE: This study was undertaken to evaluate the dynamic stability of the ankle joint during the landing phase of running and stop-jump maneuvers in athletes with and without chronic ankle instability (CAI). STUDY DESIGN: Controlled laboratory study. METHODS: Fifteen athletes with CAI and 15 age-matched athletes without CAI performed running and stop-jump lan

Ankle performance after ankle fracture: a randomized study of early mobilization.

In a prospective, randomized study, 30 patients were evaluated after ankle fracture treated by means of open reduction and internal fixation. The patients were randomized to either postoperative immobilization in a plaster cast for 6 weeks or early mobilization (1-2 weeks after surgery) in an ankle brace. Both regimens allowed weightbearing. Evaluation after 10 weeks and after 12 months included clinical assessment and isokinetic muscle strength measurements. Patients with impaired ankle function, as shown by means of an ankle score at 12 months, were followed for 3 years. At 10 weeks, impaire

A Simple Ankle Value to Differentiate Copers and Patients Likely to Develop Chronic Ankle Instability After Lateral Ankle Sprain.

BACKGROUND: Lateral ankle sprain (LAS) is the most common injury in sports, and only 30% of patients will fully recover (copers). Currently, no single objective tool exists to help clinicians decide on return to sport between LAS copers and patients likely to develop chronic ankle instability (CAI). The Simple Ankle Value (SAV) is a score developed recently to evaluate function among patients with ankle injury. OBJECTIVE: To evaluate the ability of the SAV to distinguish between LAS copers and patients likely to develop CAI. SETTING: Clinic. DESIGN: Cross-sectional study. PATIENTS OR OTHER

Ankle laxity affects ankle kinematics during a side-cutting task in male collegiate soccer athletes without perceived ankle instability.

OBJECTIVES: To investigate whether ankle joint laxity alone influences lower limb kinematics during a side-cutting task. DESIGN: A cross-sectional study. SETTING: Sports medicine research laboratory. PARTICIPANTS: In total, 66 male collegiate soccer players with history of ankle sprains with no perceived ankle instability were categorised into three groups: no-laxity copers (n = 26), laxity copers (n = 23), and severe-laxity copers (n = 17). MAIN OUTCOME MEASURES: The hip, knee, ankle, rearfoot, midfoot, and forefoot kinematic data during the stance phase (0%-100% indicated initial contact

Expecting ankle tilts and wearing an ankle brace influence joint control in an imitated ankle sprain mechanism during walking.

A thorough understanding of the functional aspects of ankle joint control is essential to developing effective injury prevention. It is of special interest to understand how neuromuscular control mechanisms and mechanical constraints stabilize the ankle joint. Therefore, the aim of the present study was to determine how expecting ankle tilts and the application of an ankle brace influence ankle joint control when imitating the ankle sprain mechanism during walking. Ankle kinematics and muscle activity were assessed in 17 healthy men. During gait rapid perturbations were applied using a trapdoo

Knee Kinetics and Kinematics in Patients With Ankle Arthroplasty and Ankle Arthrodesis.

Background: Previous literature suggests that patients treated with total ankle arthroplasty (TAA) versus ankle arthrodesis (AA) may have better function and lower risk for adjacent joint arthritis in the foot. Little is known on how these interventions affect proximal joints such as the knee. Questions: We sought to assess whether patients with TAA and AA exhibited altered biomechanics linked to the onset and progression of knee osteoarthritis (KOA). We used the knee adduction moment (KAM), a surrogate measure for the mechanical load experienced at the medial tibiofemoral compartment, because

Effect of ankle taping on mechanical laxity in chronic ankle instability.

BACKGROUND: The high percentage of re-injury after an initial ankle sprain necessitates examination of preventative methods. The purpose of this project was to investigate the effect ankle tape has on mechanical laxity in subjects with CAI. MATERIALS AND METHODS: Twenty subjects with unilateral CAI (seven males and 13 females; age, 20.6 +/- 1.8 years; mass, 69.9 +/- 13.7 kg; height, 172.3 +/- 9.3 cm) and 20 healthy subjects (seven males and 13 females; age, 21.9 +/- 4.3 years; mass, 72.0 +/- 14.6 kg; height, 171.1 +/- 6.7 cm) participated in the study. Both ankles of each subject were tested

Ankle impingement: combined anterior and posterior impingement syndrome of the ankle.

BACKGROUND: Simultaneous anterior and posterior ankle impingement has not been previously reported. We identified 62 patients with both anterior and posterior impingement and report the results of anterior arthroscopic and posterior open treatment. METHODS: Between January, 1990, and December, 2003, 62 consecutive patients with symptoms and signs of both anterior and posterior impingement of the ankle were identified. Fifty-eight recorded a single injury or multiple ankle sprains. The most common mechanism of injury was inversion or plantarflexion. Initial conservative treatment failed in all

The effect of ankle braces on the prevention of dynamic forced ankle inversion.

BACKGROUND: Athletes often employ prophylactic braces to reduce the risk of ankle injuries. HYPOTHESIS: Ankle braces do not significantly decrease the risk of forced inversion on a standardized one-footed jump landing. STUDY DESIGN: Controlled laboratory study. METHODS: Fourteen healthy men with a mean age of 25.1 years were tested. Three braces, two semirigid (Aircast and Bledsoe) and one lace-up (Swede-O), were fitted to each subject. Forced dynamic ankle inversion of 24 degrees was to be resisted as the subjects landed on one foot with a force of two body weights on a stimulus presented

The effect of recurrent ankle inversion sprain and taping on proprioception at the ankle.

PURPOSE: A common explanation for the tendency for ankle inversion sprains to recur is that proprioception is impaired by the initial sprain. It is further hypothesized that the application of tape prevents further sprains by enhancing proprioceptive acuity. The purpose of this study was to determine whether proprioception was impaired in people with recurrent ankle inversion sprain, and whether taping the ankle provided enhanced proprioceptive ability. METHODS: We compared the ability to perceive passive plantarflexion and dorsiflexion movements imposed at the relaxed ankle in 25 subjects wi

Stability of the ankle joint. Analysis of the function and traumatology of the ankle ligaments.

The purpose of this study was to elucidate the function of the ligamentous structures of the ankle joint, the traumas in which they may rupture, and the types of instability caused by such ruptures. Most previous experimental investigations on the function of ankle ligaments have been performed on osteoligamentous preparations either by forcing a movement in the joint into more or less well-defined directions and observing the resulting injuries or else by cutting ligaments in various combinations and describing the resulting instability. As a rule, this has been done without inducing the incr

Acute Ankle Sprains.

Understanding the types of ankle sprains is essential in determining the most appropriate treatment and preventing substantial missed time from sports. Commonly known and recognized is an acute lateral ankle sprain, however, a differentiation should also be made to understand high (syndesmotic) ankle sprains as the mechanism of injury and recovery periods differ between these two types.

Posterior ankle impingement.

Posterior ankle impingement is a common cause of chronic ankle pain and results from compression of bony or soft tissue structures during ankle plantar flexion. Bony impingement is most commonly related to an os trigonum or prominent trigonal process. Posteromedial soft tissue impingement generally arises from an inversion injury, with compression of the posterior tibiotalar ligament between the medial malleolus and talus. Posterolateral soft tissue impingement is caused by an accessory ligament, the posterior intermalleolar ligament, which spans the posterior ankle between the posterior tibio
