INTRODUCTION: Limited data are available regarding safety of catheter ablation of atrial fibrillation (AF) in patients using novel oral anticoagulants (NOAC) before and after pulmonary vein isolation. We aimed to assess the safety of a simple anticoagulation protocol in consecutive patients presenting for catheter ablation of AF.
METHODS: From November 2011 to December 2014, we prospectively included 234 patients referred for catheter ablation of AF who had already received NOAC treatment. NOAC was continued for a minimum of three months after ablation. We assessed procedure-related bleeding

As the population ages and treatment options for heart valve disease increase, the number of patients with intracardiac valve prostheses is growing rapidly. Although all devices have the potential to cause thrombus formation, the propensity depends on the type of prosthesis as well as risk of the individual patient. Mechanical valve prostheses carry the highest (and persistent) risk of thromboembolism, and these patients require anticoagulation with vitamin K antagonists (warfarin). Required international normalised ratio levels are dependent on the location of the valve (mitral>aortic), type

A 64-year-old female presented with severe respiratory failure secondary to a high-grade non-small lung cancer (IIIA NSCLC) causing extrinsic and intrinsic compression of the right main bronchus. She remained hypoxic despite 100% FiO2 delivery by high-flow nasal cannula and was considered at high risk of airway loss at intubation. Tumor debulking, histological diagnosis, and restoration of airway patency were facilitated with peri-procedural veno-venous extracorporeal membrane oxygen support (VVECMO). We describe a case of awake bifemoral VVECMO cannulation performed uneventfully as a bridge t

BACKGROUND: Cardiogenic shock in the setting of severe aortic stenosis or degenerated bioprosthetic valves is associated with high morbidity and mortality. Definitive valve intervention is often deferred due to profound hemodynamic instability and multiorgan dysfunction. Optimal strategies to stabilize these patients before transcatheter aortic valve replacement (TAVR) remain incompletely defined.
CASE SUMMARY: We present a case series of 3 patients with cardiogenic shock due to severe aortic valve disease who were managed with temporary mechanical circulatory support (MCS) as a bridge to TAV

AIM: Superior mesenteric vein thrombosis (SMVT) is an uncommon but potentially life-threatening postoperative complication of colorectal surgery. Risk factors and prognosis of SMVT have been poorly described and data to create gold standard criteria for diagnosis and management are lacking. SMVT has a wide spectrum of clinical presentation, hence, its early identification may be a diagnostic challenge.
CASE REPORT: 56 year old obese female patient with inherited prothrombotic condition underwent an open right hemicolectomy plus cholecystectomy; the immediate postoperative course was uneventfu

BACKGROUND: Perioperative airway management and oxygenation maintenance during central airway obstruction (CAO) treatment pose great challenges. While veno-venous extracorporeal membrane oxygenation (V-V ECMO) shows promise as a bridge therapy, optimal implementation and management strategies remain lacking. We present our experience with V-V ECMO in CAO management from a high-volume center.
METHODS: We retrospectively analyzed 29 consecutive patients who received V-V ECMO support for CAO between 2015 and 2023. Patient demographics, clinical characteristics, ECMO cannulation and operation par

Abnormal blood coagulation is a major health problem and natural anticoagulants from blood-feeding organisms have been investigated as novel therapeutics. NAPc2, a potent nematode-derived inhibitor of coagulation, has an unusual mode of action that requires coagulation factor Xa but does not inhibit it. Molecular dynamics simulations of NAPc2 and factor Xa were generated to better understand NAPc2. The simulations suggest that parts of NAPc2 become more rigid upon binding factor Xa and reveal that two highly conserved residues form an internal salt bridge that stabilises the bound conformation

Due to the complex polysaccharide structure, heparin can interact with a variety of bioactive molecules besides activated factor X and factor II, thus exerting multifaceted pharmacological effects beyond anticoagulation. In this light, anti-inflammatory illness has been noted as an important area regarding the expanded therapeutic potentials of heparin, through multiple targeting mechanisms against thrombo-inflammation. Herein this article sorts out a comprehensive clinical profile of major inflammatory disorders that can benefit from administration of heparin regimens, covering sepsis, pregna

Extracorporeal membrane oxygenation (ECMO) may offer life-saving treatment in severe pulmonary contusion or acute respiratory distress syndrome when conventional treatments have failed. However, because of the bleeding risk of systemic anticoagulation, ECMO should be performed only as a last resort in multiple trauma victims. Here, we report ECMO as a bridge for right main bronchus reconstruction and recovery of traumatic wet lung in a 31-year-old male multi-trauma patient with right main bronchial disruption, bilateral pulmonary contusion, cerebral contusion and long bone fracture. BACKGROUND: Patients with mechanical heart valves require lifelong oral anticoagulation with vitamin K antagonists such as warfarin to prevent valve-associated thromboembolism. Bleeding complications during this therapy pose significant clinical challenges and necessitate expert management. In particular, heavy uterine bleeding following miscarriage represents a high-risk and emergent situation. The management of uterine bleeding is challenging, due to the need to balance anticoagulation with hemostatic control. This report presents a case of severe and prolonged bleeding caused by retained pr

BACKGROUND: With advances in technology, temporary mechanical circulatory support (tMCS) devices have increasingly been used to treat patients in cardiogenic shock.
CASE SUMMARY: A 25-year-old G1P0 woman with known idiopathic dilated cardiomyopathy since infancy was hospitalized with acute on chronic decompensated heart failure (HF) at 30 weeks of gestation and bridged to delivery and heart transplantation with an Impella CP heart pump.
DISCUSSION: There is a dearth of experience in use of tMCS for patients experiencing peripartum HF reported in the literature and significant uncertainty ove

Skin necrosis is a rare albeit severe complication of warfarin use for anticoagulation, resulting in significant morbidity and mortality. Here, we present the case of a 58-year-old woman who developed erythema and pain in her left leg two weeks after initiation of warfarin therapy with concomitant early administration of heparin for a deep vein thrombosis and pulmonary embolism. Subsequently, the erythema progressed to skin necrosis, and a diagnosis of warfarin-induced skin necrosis (WISN) was suspected. Warfarin was discontinued, and unfractionated heparin was commenced. The simultaneous pres

The periprocedural management of patients receiving long-term oral anticoagulant therapy is a common but complex clinical problem. It is well established that maintaining oral anticoagulation is associated with an increased risk of bleeding in the periprocedural period while discontinuing anticoagulant therapy postoperatively leads to an elevated risk for thromboembolic events, especially in high risk patients. Nowadays there is growing evidence to maintain antiplatelet therapy with acetylsalicylic acid (ASS, Aspirin®) perioperatively in a setting of secondary prophylaxis. Beyond that the incr

A 66-year-old male with a past medical history of colon cancer came for elective filter placement in the inferior vena cava and the superior vena cava following repeated venous thromboses. The patient was undergoing chronic anticoagulation and was bridged to surgery with intravenous heparin. Two days postoperative, just prior to the date of discharge, the patient was found tachycardic and, despite all precautions, was diagnosed with an acute pulmonary embolism.

BACKGROUND: Patients with left ventricular assist devices (LVADs) require interruption of warfarin for invasive procedures, but parenteral bridging is associated with many complications. Four-factor prothrombin complex concentrate (4F-PCC) can temporarily restore hemostasis in patients undergoing anticoagulation with warfarin.
OBJECTIVES: This pilot study evaluated the strategy of using variable-dose 4F-PCC to immediately and temporarily reverse warfarin before invasive procedures without holding warfarin in patients with LVADs. The duration of effect of 4F-PCC on factor levels and time to re

Deep vein thrombosis (DVT) is an acute vascular condition commonly associated with immobility, hypercoagulability, and endothelial injury. We report the case of a 47-year-old woman presenting with progressive left lower extremity swelling and pain, ultimately diagnosed with extensive iliofemoral DVT secondary to extrinsic compression of the left common iliac vein by a markedly enlarged fibroid uterus. The patient was managed with therapeutic anticoagulation, blood transfusion, and intravenous iron therapy, followed by definitive hysterectomy approximately two months later, with guideline-direc
