It was observed that severe aortic displacement, narrowing, and obstruction can occur when thoracostomy tubes abut the mediastinum. To further investigate this previously undescribed phenomenon, aortography was performed with intraluminal pressure measurements in newborn puppies and aortograms obtained in two infant cadavers. Two patients with traumatic rupture of the mid-descending aorta successfully repaired are presented. Most clinical series of aortic tears do not include this entity. A review of the world literature reveals only 9 previous cases. In 6 of the 11 patients the diagnosis was either missed or delayed. Patients who require thoracic aortography for blunt decelerating chest trauma often sustain injury to other organ systems due to the magnitude and mechanism of injury. Hospital records and radiographs of 117 consecutive, injured patients studied with thoracic aortography were evaluated to assess the accuracy, value, and limitations of postaortographic abdominal plain radiography for detection of major genitourinary injury. We report three cases of patients with blunt thoracic trauma, who underwent transesophageal echocardiography (TEE) because of high index of clinical suspicion for acute traumatic aortic syndrome. Paradoxical embolization of venous thrombus into the abdominal aortic bifurcation has been diagnosed by means of aortography. Lower limb phlebothrombosis accounting for the embolus formation has been demonstrated by phlebography. Arterial embolectomy and venous thrombectomy were successfully performed. Recovery was complete without sequelae.

Little attention has been given to the sequential assessment and management of a cohort of patients with potential aortic disruption manifested by a widened mediastinum. These patients often require diagnostic peritoneal lavage (DPL), cranial computed tomography (CCT) scan, thoracic aortography, and multiple operations. The authors report two new cases of congenital quadricuspid aortic valve complicated by pure, severe, chronic, aortic regurgitation. This abnormality was diagnosed fortuitously at transoesophageal echocardiography in one case and at aortography in the other, and confirmed at surgery. This congenital abnormality was associated with superior displacement of the left coronary ostium.

The status of the celiac and mesenteric arteries in 124 patients with occlusive injuries to the abdominal aorta and iliac arteries has been studied by means of the antero-posterior and lateral translumbar abdominal aortography. OBJECTIVE: To describe the significance of aortic root distortion (AD) and/or aortic valve insufficiency (AI) during balloon angioplasty of the right ventricular outflow tract (RVOT) performed to rule out coronary artery compression prior to transcatheter pulmonary valve (TPV) implantation.

Two cases are presented illustrating the emergent and potentially lethal complications of neonatal umbilical artery catheterization that may confront the pediatric urologist. Immediate aortography via the umbilical artery catheter usually will be diagnostic. Prompt surgical intervention is advocated if uncontrollable renovascular hypertension or a false aneurysm is found.

Ascending aortic injury is seen in only 5% of patients who survive long enough to undergo imaging. Emergent aortography is commonly used to confirm and define suspected thoracic rupture. We describe a case of ascending aortic rupture at the root of the aorta following blunt trauma diagnosed with CT imaging.

Primary hydatid cyst of the mediastinum is rare. We report a patient with a primary mediastinal hydatid cyst which ruptured into the aorta, resulting in bilateral popliteal embolism. The contrast-enhanced computed tomographic features represent a pathognomonic sign of a communicating rupture of an echinococcal cyst into the aorta. Comparison is made with ultrasonography and aortography.

The case of a 41-year-old man with an embolus in the left popliteal artery following multiple injuries suffered in an auto accident is presented. Following popliteal embolectomy, a rupture of the descending thoracic aorta was demonstrated and repaired. A 50-year-old renal transplant recipient had Salmonella sepsis. When antibiotic therapy failed to eradicate the infection, abdominal aortography revealed an unsuspected mycotic aneurysm. After ligation and aneurysmectomy the patient's blood stream became sterile. Mycotic aneurysm should be considered when Salmonella sepsis is unresponsive to antibiotic therapy.
