Transcortical sensory aphasia following left frontal infarction.

Two right-handed patients who exhibited language disability after left frontal infarction are described. The patients spoke fluently and exhibited excellent repetition ability from the onset of infarction without exhibiting any oral apraxia, but had deficits in auditory comprehension, naming, reading and writing. In both patients, brain magnetic resonance imaging (MRI) revealed infarction in the left inferior frontal gyrus, the middle frontal gyrus and the anterior part of the lower precentral gyrus. Single photon emission computed tomography (SPECT) revealed decreased blood flow in the same r

Communicative responsibility and semantic task in aphasia and "schizophasia".

A probe technique requiring convergent and divergent semantic behavior and representing five levels of communicative responsibility served as the research tool. Stimuli were presented to 29 asphasic adults (13 Broca's, 7 Wernicke's, and 9 anomic), 26 adults with chronic undifferentiated schizophrenia, and 32 normal elderly control subjects. Within each group significant differences were observed on the semantic task (convergent and divergent) and on level of communicative responsibility. Among subjects with aphasia, differences appeared to relate more to severity than type. Differences between

Recovery of language function in Korean-Japanese crossed bilingual aphasia following right basal ganglia hemorrhage.

Few studies have investigated language recovery patterns and the mechanisms of crossed bilingual aphasia following a subcortical stroke. In particular, Korean-Japanese crossed bilingual aphasia has not been reported. A 47-year-old, right-handed man was diagnosed with an extensive right basal ganglia hemorrhage. He was bilingual, fluent in both Korean and Japanese. After his stroke, the patient presented with crossed aphasia. We investigated changes in the Korean (L1) and Japanese (L2) language recovery patterns. Both Korean and Japanese versions of the Western Aphasia Battery (WAB) were comple

Altered patterns of word associations in dementia and aphasia.

The word associations of 38 demented, 17 aphasic, and 22 normal subjects were studied. Both normal and brain-injured subjects appear to make judgments about the grammatical class of the word stimulus. Certain stimulus words (especially nouns and adjectives) elicit paradigmatic responses whereas other words (especially verbs and adverbs) elicit syntagmatic responses. The mechanism producing syntagmatic responses seems relatively resistant to deterioration in dementia or aphasia. However, in dementia the mechanism that generates paradigmatic responses becomes progressively less efficient (possib

Neural connectivity underlying core language functions.

INTRODUCTION: Although many white matter tracts underlying language functions have been identified, even in aggregate they do not provide a sufficiently detailed and expansive picture to enable us to fully understand the computational processes that might underly language production and comprehension. We employed diffusion tensor tractography (DTT) with a tensor distribution model to more extensively explore the white matter tracts supporting core language functions. Our study was guided by hypotheses stemming largely from the aphasia literature. METHODS: We employed high angular resolution d

Effective music therapy techniques in the treatment of nonfluent aphasia.

In music therapy for nonfluent aphasia patients who have difficulty producing meaningful words, phrases, and sentences, various benefits of singing have been identified: strengthened breathing and vocal ability, improved articulation and prosody of speech, and increased verbal and nonverbal communicative behaviors. This paper will introduce these various techniques used in clinical music therapy, and summarize findings based on our recent study to illustrate the strength of different techniques emphasizing rhythm, pitch, memory, and vocal/oral motor components dealing with different symptoms.

Profiling agrammatic spoken language: towards a government and binding framework.

An analytical procedure for describing agrammatic spoken language based on government and binding theory is used to characterise the spoken grammar of an aphasic adult (JR) with severe agrammatism. Analysis of JR's spoken language revealed that he produced a greater number of single-word utterances (single lexical items) in comparison with lexical and/or functional projections. both morphological and syntactic evidence suggested that JR retained a differentiated set of lexical categories. No violations of word order were found. Target determiners and target inflection phrases were not always c

Construction of a Prediction Model for Naming Recovery in Subacute Poststroke Aphasia Based on Multivariate Analysis: Evaluation of Accuracy and Reliability.

PURPOSE: Anomia, a common dysfunction in poststroke aphasia (PSA), impacts daily communication, and its prognosis remains challenging. This study aimed to develop a model to predict naming rehabilitation in patients with subacute PSA. METHOD: Data of PSA were collected retrospectively. Logistic regression (LR) analyses by a nested fivefold cross-validation were performed to identify predictors and construct a predictive model. The efficacy of the model was evaluated using receiver operating characteristic (ROC) curves, calibration curves, and decision curve analysis (DCA). Additionally, the n

Clinical and Radiological Features of Aphasia Caused by Brainstem Infarction: Two Case Reports.

Cognitive impairment resulting from cerebellar damage, known as cerebellar cognitive affective syndrome (CCAS), can cause language impairment and aphasia. The disruption of the cerebro-cerebellar pathway is a possible mechanism for CCAS. Consequently, aphasia may also arise from brainstem lesions. However, the precise mechanisms of brainstem-induced aphasia are not fully understood owing to the scarcity of published reports describing this condition. Herein, we investigated characteristics of patients with aphasia attributed to the brainstem infarction. We retrospectively analyzed medical reco

The third alexia.

A variety of alexia has been demonstrated that can be distinguished from the two classically recognized types of alexia. This reading disorder is seen in patients with a dominant frontal lobe pathologic condition that has produced a motor language disturbance (Broca aphasia) and is sufficiently disabling to deserve consideration as a variety of alexia. Functionally, the frontal reading disturbance differs from the other two types in that the patient comprehends meaningful content words better than relational or syntactic structures. Four factors that may underly frontal alexia have been noted:

[A case of subarachnoid hemorrhage presenting with supplementary motor aphasia as an initial symptom].

Supplementary motor aphasia results from impairment of the supplementary motor area in the left mesial frontal cortex. We report a rare case of subarachnoid hemorrhage presenting with supplementary motor aphasia as an initial symptom. A 52-year-old woman was brought to our hospital by ambulance due to sudden severe headache and supplementary motor aphasia. CT demonstrated subarachnoid hemorrhage that appeared to be particularly thick in the pericallosal cistern. She had undergone neck clipping of a left vertebral artery aneurysm for subarachnoid hemorrhage 14 years earlier. At that time, she u

Grammatism.

Findings from the literature on language development, dyslexia, and adult sentence processing provide a vehicle for comparing two models of the symptom complex associated with agrammatism. One model contends that agrammatism represents a deficit in linguistic structures. The other model maintains that the linguistic behavior associated with agrammatism is the result of a limitation in language processing. To adjudicate between the models, the present paper examines one linguistic construction, the restrictive relative clause. The results of experimental investigations across several subject po

The effects of modified melodic intonation therapy on nonfluent aphasia: a pilot study.

OBJECTIVE: Positive results have been reported with melodic intonation therapy (MIT) in nonfluent aphasia patients with damage to their left-brain speech processes, using the patient's intact ability to sing to promote functional language. This pilot study sought to determine the immediate effects of introducing modified melodic intonation therapy (MMIT), a modification of MIT, as an early intervention in stroke patients presenting with Broca's aphasia. METHOD: After a randomized controlled single-blind design, 30 acute stroke survivors with nonfluent aphasia were randomly assigned to receive

Verb processing during sentence comprehension in aphasia.

This study examines verb processing during on-line sentence comprehension in aphasia. We describe two experiments that explore whether a group of Broca's aphasics, who were agrammatic in comprehension as well as speech, a group of fluent aphasics, and a group of normal controls are sensitive to the argument structure arrangements of verbs. Subjects had to perform a complex secondary task both in the immediate vicinity of the verb and also at a point well past the verb while listening to sentences for meaning. Reaction times to this secondary task show that both normal controls and agrammatic B

Hemispheric alpha asymmetries in fluent and dysfluent aphasics during linguistic and resting conditions.

The hemispheric alpha asymmetries of a group of normal males, normal females, fluent aphasic males and dysfluent aphasic males were investigated during the auditory presentation of sentences, a short story and a resting condition in the absence of linguistic stimuli. No differences were found for the groups' mean alpha ratio scores during the resting condition. Fluent aphasics were found to demonstrate greater right hemispheric alpha suppression than the other groups for the sentence and story conditions. The dysfluent aphasics showed greater right hemispheric alpha suppression for the sentenc

Transcranial magnetic stimulation combined with speech and language training in early aphasia rehabilitation: a randomized double-blind controlled pilot study.

BACKGROUND: Functional neuroimaging studies with poststroke aphasia patients have shown increased activation of the unaffected hemisphere, which hypothetically reflects a maladaptive strategy of brain reorganization. OBJECTIVE: We investigated whether repetitive transcranial magnetic stimulation (rTMS) inhibiting the right-hemisphere homologue of Broca's area improves language restitution if combined with speech/language therapy. METHODS: Forty aphasic patients during the subacute phase of ischemic stroke were randomized to a 3-week aphasia rehabilitation protocol in combination with real or

Grammatical Parallelism in Aphasia: A Lesion-Symptom Mapping Study.

Sentence structure, or syntax, is potentially a uniquely creative aspect of the human mind. Neuropsychological experiments in the 1970s suggested parallel syntactic production and comprehension deficits in agrammatic Broca's aphasia, thought to result from damage to syntactic mechanisms in Broca's area in the left frontal lobe. This hypothesis was sometimes termed overarching agrammatism, converging with developments in linguistic theory concerning central syntactic mechanisms supporting language production and comprehension. However, the evidence supporting an association among receptive synt

Alexia and agraphia: contrasting perspectives of J.-M. Charcot and J. Hughlings Jackson.

OBJECTIVE: To evaluate 19th-century concepts of cerebral localization for complex mental activities, focusing on alexia and agraphia in published writings of Jean-Martin Charcot (1825-1893) and John Hughlings Jackson (1835-1911). BACKGROUND: In the early 1860 s, Broca's reports on a special role for the left frontal lobe in articulate language ignited frenetic interest in cerebral localization. Disorders of written language (alexia and agraphia) were enmeshed in ensuing discussions of how the brain was organized for language and other complex behaviors. DESIGN/METHODS: Focused review and ana
