Next-Generation Sequencing Identifies Key Targetable Mutations in the Treatment of Appendiceal Neoplasms.

BACKGROUND: Appendiceal neoplasms are a group of rare, heterogeneous tumors that exhibit varying malignant potential. Systemic treatment options for disseminated appendiceal cancer are limited. We sought to review rates of mutations that may indicate potential roles for routine next-generation sequencing to identify targetable therapies. METHODS: An analysis of 916 appendiceal tumor samples submitted to American Association for Cancer Research Genomics Evidence Neoplasia Information Exchange (GENIE) consortium was performed to compare patient demographics and the rates of mutations that may c

Low-grade appendiceal mucinous neoplasms combined with incidental gallbladder cancer: a case report.

BACKGROUND: Mucinous tumors of the appendix are relatively rare compared to other gastrointestinal tumors, but their incidence tends to gradually increase. Due to the lack of specific clinical manifestations, mucinous tumors of the appendix are prone to misdiagnosis and missed diagnosis. According to different pathological types, mucinous tumors of the appendix can be roughly divided into low-grade mucinous tumors and high-grade mucinous tumors, with significant differences in prognosis. Gallbladder cancer is the most common type of malignant tumor in the biliary system, with a high degree of

Laparoscopy-assisted Partial Cecum Resection Versus Ileocecal Resection for Non-malignant Appendiceal Neoplasms.

BACKGROUND/AIM: Preoperative pathological diagnosis of appendiceal tumors is difficult due to their anatomical nature. Although lymph node metastasis is rare in low-grade appendiceal mucinous neoplasms (LAMN), there is a risk of a positive surgical margin by appendectomy. Partial cecum resection is considered an option to obtain negative resection margins for LAMN; however, the benefits of this procedure via a laparoscopic approach remain unclear. PATIENTS AND METHODS: We retrospectively analyzed patients who underwent laparoscopy-assisted partial cecum resection (Lap-Ce) or ileocecum resecti

Genome-Wide Analysis of DNA Methylation in Pseudomyxoma Peritonei Originated from Appendiceal Neoplasms.

INTRODUCTION: Pseudomyxoma peritonei (PMP) is a disease characterized by progressive accumulation of intraperitoneal mucinous ascites produced by neoplasms in the abdominal cavity. Since the prognosis of patients with PMP remains unsatisfactory, the development of effective therapeutic drug(s) is a matter of pressing concern. Genetic analyses of PMP have clarified the frequent activation of GNAS and/or KRAS. However, the involvement of global epigenetic alterations in PMPs has not been reported. METHODS: To clarify the genetic background of the 15 PMP tumors, we performed genetic analysis usi

Predicting Underlying Neoplasms in Appendiceal Mucoceles at CT: Focal Versus Diffuse Luminal Dilatation.

OBJECTIVE. The purpose of this study was to determine whether a novel morphologic characteristic of appendiceal mucoceles at CT-focal distal appendiceal dilatation with a segment of morphologically normal appendix proximally-could predict an underlying neoplastic pathologic abnormality before surgery and histopathologic assessment. MATERIALS AND METHODS. A retrospective study was performed that assessed CT cases from 2012 through 2014. Cases showing morphologic features of a mucocele were identified and categorized into two subgroups: focal distal dilatation with a segment of normal appendix p

Connecting the dots: Low-grade appendiceal mucinous neoplasms and serrated polyps in the appendix.

OBJECTIVE: This study aimed to examine the relationship between low-grade appendiceal mucinous neoplasms (LAMNs) and serrated polyps (SPs) of the appendix, both characterized by KRAS mutations and overlapping morphologic features. METHODS: We analyzed 27 cases of LAMN and 24 cases of SP from archival records, reviewed pathology, and performed molecular analysis on select cases. Four cases initially diagnosed as LAMN were excluded for not meeting diagnostic criteria, and 1 SP case was reclassified as LAMN. RESULTS: Microscopic evaluation revealed serrated architecture in 8 (29.6%) of 27 LAMNs

Mucinous appendiceal neoplasms and pseudomyxoma peritonei: Morphological and clinical findings, differential diagnosis, and prognosis.

BACKGROUND: Low-grade appendiceal mucinous neoplasms (LAMN) of the appendix have bland cytological features and well-differentiated morphology. Despite this, they may cause a pseudomyxoma peritonei (PMP) disease characterized by mucinous deposits in the peritoneal cavity and may exhibit malignant behavior. AIMS AND OBJECTIVES: In this study, we evaluated the clinical course and histopathological findings of LAMN. The rate of progression to PMP, factors affecting its development, and the clinical course of cases with PMP were investigated. MATERIALS AND METHODS: Twelve thousand and forty-seve

Laparoscopic cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for perforated low-grade appendiceal mucinous neoplasms.

INTRODUCTION: Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) is an established treatment for pseudomyxoma peritonei (PMP) from perforated low-grade appendiceal mucinous neoplasms (LAMN II). In a selected group of LAMN II patients without established PMP, CRS/HIPEC can be performed laparoscopically (L-CRS/HIPEC); however the short-term benefits and safety of this approach have yet to be determined. This study aims to determine the short-term outcomes from a series of L-CRS/HIPEC LAMN II patients compared to those who have undergone a similar open operation (O-C

Enterocutaneous fistula resulting from the drainage of a contained appendiceal perforation secondary to a partially obstructing low-grade appendiceal mucinous neoplasm.

We report a case of an enterocutaneous fistula resulting from drainage of a contained appendiceal perforation secondary to an obstructing appendiceal mucinous neoplasm. A 66-year-old otherwise healthy female patient proceeded to laparoscopic appendicectomy with resection of the fistula tract. Histopathology demonstrated a mid-appendiceal mucinous neoplasm, with a benign enterocutaneous fistula tract communicating with the distal, non-malignant portion of the appendix. We provide an overview of the literature and highlight the need to consider appendiceal mucinous neoplasm as a differential in

A scoping review of postoperative surveillance strategies for localized, low-grade appendiceal mucinous neoplasms.

INTRODUCTION: There is a paucity of data to support surveillance protocols for localized, low-grade appendiceal mucinous neoplasms (LAMNs) after resection. METHODS: A search strategy was developed to identify postoperative surveillance strategies for LAMNs, in the context of rates of recurrence and disease-free survival, and applied to four literature databases. Resultant citations were subject to screening in duplicate, in three stages: title, abstract, and full-text. Data was extracted from the final set of included articles and subject to descriptive statistics. RESULTS: A total of 16 art

Appendiceal mucinous neoplasms: clinicopathologic study of 116 cases with analysis of factors predicting recurrence.

The classification and nomenclature of appendiceal mucinous neoplasms are controversial. To determine the outcome for patients with appendiceal mucinous neoplasms and further evaluate whether they can be stratified into groups that provide prognostic information, the clinicopathologic features of 116 patients (66 with clinical follow-up) with appendiceal mucinous neoplasms were studied. From a wide variety of histopathologic features assessed, the important predictors that emerged on univariate statistical analysis were presence of extra-appendiceal neoplastic epithelium (P=0.01), high-grade c

Low-Grade Appendiceal Mucinous Neoplasm: A Case Highlighting Diagnostic and Management Considerations.

Appendiceal mucoceles (AMs) are rare pathological entities characterized by the accumulation of mucin within the appendiceal lumen. They may arise from either non-neoplastic or neoplastic processes, including low-grade appendiceal mucinous neoplasms (LAMNs). Due to their variable clinical presentation and often incidental discovery, diagnosis can be challenging. We report the case of a 29-year-old female who presented with a 3-month history of constipation and intermittent abdominal pain. Physical examination revealed mild right lower quadrant tenderness without signs of peritonitis. Laborator
