Ravulizumab for relapse prevention in AQP4-IgG-positive neuromyelitis optica spectrum disorder: a 2-year follow-up case report.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune disease of the central nervous system, distinct from multiple sclerosis, characterized by severe inflammatory attacks targeting the optic nerves, spinal cord, and brainstem. Treatment has evolved from broad immunosuppressants to targeted biologics, such as complement inhibitors (ravulizumab/eculizumab), supported by strong evidence, including the CHAMPION-NMOSD trial for ravulizumab in AQP4-Ab+ve NMOSD patients. This case highlights the importance of prompt diagnosis and innovative treatment approaches in the manag

Activation of brain stem neurons by irritant chemical stimulation of the throat assessed by c-fos immunohistochemistry.

The method of c-fos immunohistochemistry was used to identify the brain stem distribution of neurons activated following irritant chemical stimulation of the laryngopharyngeal mucosa. In pentobarbital-anesthetized rats, either water (control), nicotine (600 mM, 1 ml) or capsaicin (330 microM, 1 ml) was applied to the pharynx via a cannula placed posterior to the soft palate. Following nicotine and capsaicin, there was a significant increase in fos-like immunoreactivity (FLI) compared with controls in the following areas: nucleus of the solitary tract from the level of the pyramidal decussation

Pediatric paraneoplastic neuromyelitis optica spectrum disorder associated with ovarian teratoma.

Neuromyelitis optica spectrum disorder is an inflammatory condition of the central nervous system typically manifesting as myelitis, optic neuritis, and/or area postrema syndrome. Here, we present a pediatric patient who developed symptoms consistent with area postrema syndrome with positive anti-aquaporin-4 (AQP4) antibodies who was also found to have an ovarian teratoma. Pathological specimens revealed the presence of aquaporin-4. This was felt to be the antigenic trigger that led to the patient's condition. She suffered no further clinical attacks and seroconverted to negative AQP4 status u

Evolution of longitudinally extensive transverse myelitis in an aquaporin-4 IgG-positive patient.

A 36-year-old woman presented with hemiplegia and loss of pain and temperature sensation 1 day after the subacute onset of intractable vomiting. Paresthesia followed. She was tetraplegic at day 12. Initial MRI revealed a lesion in the medulla oblongata, involving primarily the area postrema (figure). The lesion progressively extended into the upper cervical cord. Forebrain MRI was normal. CSF contained 11 leukocytes/mm3. Brainstem tumor and multiple sclerosis were early diagnostic considerations. Aquaporin-4 immunoglobulin (Ig)G was detected in serum. This case supports the concept that fenest

Sensory circumventricular organs in health and disease.

Circumventricular organs (CVOs) are specialized brain structures located around the third and fourth ventricles. They differ from the rest of the brain parenchyma in that they are highly vascularised areas that lack a blood-brain barrier. These neurohaemal organs are classified as "sensory", when they contain neurons that can receive chemical inputs from the bloodstream. This review focuses on the sensory CVOs to describe their unique structure, and their functional roles in the maintenance of body fluid homeostasis and cardiovascular regulation, and in the generation of central acute immune a

Neuromyelitis Optica Spectrum Disorder Presenting With Organizing Pneumonia: A Case Report and Literature Review.

Neuromyelitis optica spectrum disorder (NMOSD) is an inflammatory disease of the central nervous system (CNS) that can result in severe neurological sequelae. Aquaporin-4, a cause of complement-mediated inflammation in NMOSD, is expressed on alveolar epithelial cells and in the CNS, and organizing pneumonia (OP) is recognized as an extra-CNS symptom in NMOSD. We herein present a case of NMOSD following OP. The patient initially presented with area postrema syndrome, and the coexistence of OP made the diagnosis of NMOSD challenging. A review of previously reported cases of NMOSD with OP suggest

Digitalis: neurally mediated arrhythmogenic and coronary vasoconstrictor properties.

Studies of digitalis have revealed an appreciable role of the autonomic nervous system in the modulation of the cardiovascular toxicity of digitalis. Neuraxis transection experiments and studies using digitalis compounds that fail to enter the central nervous system suggest that centers located in the floor of the fourth cerebral ventricle play a substantial part in the neural arrhythmogenic effects of the digitalis glycosides. Electrical stimulation of the area postrema in partially digitalized animals, not otherwise manifesting ventricular arrhythmias, elicits ventricular tachycardia. Neurax

Inhibition of food intake.

Over 100 publications, principally from five groups, describe an effect of amylin and amylin analogs in inhibition of food intake in animals and humans. The major groups contributing to this area are those of the following: Chance and Balasubramaniam (Balasubramaniam et al., 1991a,b; Chance et al., 1991a,b, 1992a,b, 1993). Morley, Flood, and Edwards (Edwards and Morley, 1992; Flood and Morley, 1992; Macintosh et al., 2000; Morley and Flood, 1991, 1994; Morley et al., 1992, 1993, 1994, 1995, 1996, 1997). Acute oxycodone induces the pro-emetic pica response in rats.

Oxycodone, a semisynthetic opioid analgesic, is frequently prescribed for the management of pain. Side effects of nausea and emesis affect patient compliance and limit its therapeutic use. The present study established that an antinociceptive dose of oxycodone (15 mg/kg; oral) induces the pica response. We found sex differences in the temporal course of pica, with females having a longer duration. Opioid receptors mediated the pica response, as 1.0 mg/kg naloxone transiently attenuated and 2.0 mg/kg naloxone blocked pica. A κ-selective antagonist failed to block the response, suggesting mediat

Amylin activates distributed CNS nuclei to control energy balance.

Amylin is a pancreas-derived neuropeptide that acts in the central nervous system (CNS) to reduce food intake. Much of the literature describing the anorectic effects of amylin are focused on amylin's actions in the area postrema, a hindbrain circumventricular structure. Although the area postrema is certainly an important site that mediates the intake-suppressive effects of amylin, several pieces of evidence indicate that amylin may also promote negative energy balance through action in additional CNS nuclei, including hypothalamic and mesolimbic structures. Therefore, this review highlights

A survey of occurrence of about seventeen circumventricular organs in brains of various vertebrates with special reference to lower groups.

The occurrence of about 17 circumventricular organs is described in 31 species belonging to various groups of vertebrates from cyclostomes to mammals and the phylogeny of each organ is discussed briefly. The neurohypophysis (median eminence and neural lobe) and the subcommissural organ are observed in all species studied. The pineal is also ubiquitous except for the hagfish (Eptatretus) and the caiman (Caiman). These four circumventricular organs are phylogenetically the oldest organs. The subfornical organ and the area postrema may be phylogenetically the youngest organs, although the subforn

[The relationship of the content of AVP and the expression of V1b receptors in some brain areas with the sexual difference in the susceptibility of motion sickness in rats].

OBJECTIVE: To measure the content of arginine vasopressin (AVP) and V1b receptor expression in the brain areas in rats of both genders and after rotatory stimulation and thereby, to identify the involvement of AVP in the mechanisms of motion sickness. METHODS: SD rats were rotated about a horizontal axis for 30 min, the content of AVP and the expression of V1b receptors in some brain areas were then measured with radioimmunological analysis and immunofluorescent method respectively. RESULTS: We proved that: (1) In female rats, the content of AVP in each area we measured in rotation group did

Changes in sodium appetite evoked by lesions of the commissural nucleus of the tractus solitarius.

Ablation of the area postrema/caudal nucleus of the tractus solitarius (NTS) complex increases sodium intake, but the effect of selective lesions of the caudal NTS is not known. We measured depletion-induced sodium intake in rats with electrolytic lesions of the commissural NTS that spared the area postrema. One day after the lesion, rats were depleted of sodium with furosemide (10 mg/kg body weight, sc) and then had access to water and a sodium-deficient diet for 24 h when 1.8% NaCl was offered. Water and saline intakes were measured for 2 h. Saline intake was higher in lesioned than in sham-

Differential release of vasopressin and oxytocin in response to abdominal vagal afferent stimulation or apomorphine in the ferret.

The aim of this study was to investigate whether direct afferent stimulation of the abdominal vagus could promote release of the neurohypophyseal hormones. The nucleus of the solitary tract is the major recipient of vagal afferent information, and this region of the brainstem may also be activated by stimulation of the area postrema. For this reason apomorphine, a D2 dopaminergic agonist which acts on the area postrema, and can evoke vasopressin secretion in man, was also investigated for its effect on vasopressin and oxytocin release. Our results show that vasopressin, but not oxytocin is rel

Intractable hiccups, nausea, and vomiting, an unnerving cause of vision loss.

Acute optic neuritis (ON) is caused by variety of complex disorders that can be differentiated with the help of history, radiology, and serology. Identification of nonneurological symptoms that occur before the demyelinating event aids in timely diagnosis and prevention of further neurological attacks. We describe a case of unilateral ON with a history of intractable hiccups, nausea, and vomiting, wherein the possibility of area postrema syndrome (APS) was overlooked until the development of visual symptoms. APS recently identified as a hallmark of neuromyelitis optica spectrum disorder is a r

Intractable Vomiting and Hiccups: An Atypical Presentation of Neuromyelitis Optica Spectrum Disorder.

A 17-year-old male patient presented with frequent hiccups, nausea, and vomiting. He was initially treated for a hiatus hernia and gastritis with partial relief; however, symptoms recurred and worsened, necessitating a thorough assessment that included upper gastrointestinal endoscopy. Neuroimaging was ordered to look for a central cause of vomiting. The results showed a demyelinating lesion in the area postrema (AP), indicating neuromyelitis optica spectrum disorder (NMOSD). It typically affects the optic nerves and causes inflammatory demyelination of the spinal cord. Anti-aquaporin 4 (AQP4)

Neurokinin-1 Antagonists for Postoperative Nausea and Vomiting.

Postoperative nausea and vomiting (PONV) are the second most frequent adverse events after surgery second only to postoperative pain. Despite the advances in antiemetics and implementation of multimodal prophylactic interventions, the clinical management of PONV remains problematic. Neurokinin-1 (NK-1) receptor is a tachykinin receptor found throughout the central and peripheral nervous systems, with a particular affinity towards substance P. NK-1 receptors interact with several parts of the neuronal pathway for nausea and vomiting. This includes the chemoreceptor trigger zone, the gastrointes
