[Noncirrhotic liver fibrosis after chronic arsenic poisoning].

A 67-year-old woman with portal hypertension, splenomegaly without portal vein thrombosis, leucopenia and thrombocytopenia of splenic origin had repeated episodes of life-threatening haemorrhage from esophageal varices. Since childhood she had suffered from psoriasis and had been treated over a period of 15 years with Fowler's solution (in all about 25 g of arsenic trioxide). She had the characteristic skin lesions of arsenical poisoning-palmar hyperkeratoses and two basal cell carcinomas on the trunk. Histological examination of a wedge biopsy from the liver showed definite structural changes

Arsenic poisoning. Ongoing diagnostic and social problem.

Arsenic, commonly found in insecticides, herbicides, and industrial materials, is involved in the majority of heavy metal poisonings reported in the United States. Accidental poisoning appears to be most common in the pediatric age-group, whereas intentional and covert poisonings predominate in adults. Diagnosis is often difficult. The clinical presentations of arsenic poisoning, both acute and chronic types, represent a wide spectrum, largely dependent on route of exposure, chemical form, and dose. Because the patient or others providing the history may suppress information on exposure and be

Levels of arsenic in Indian opium eaters.

Intake of opium is very common in India. The contraband material is generally contaminated with arsenic. Most often opium eaters present with neuropathy and hepatomegaly. Arsenic was estimated in serum, urine, nails and hair of opium eaters with and without neuropathy. Arsenic was also estimated in various opium samples. Arsenic was significantly higher in serum, urine, nails and hair of opium addicts when compared to controls. The opium samples analysed showed varyingly high amounts of arsenic.

Clinical and environmental aspects of arsenic toxicity.

Arsenic is widely distributed throughout the animal and plant kingdoms and our environment where sources can be natural or anthropogenic. Agricultural uses of arsenic have declined recently, but it still has well-defined roles in industry. Small amounts of arsenic are metabolized in a variety of ways and are largely rapidly methylated and excreted by man and animals. Poisoning can occur and may follow an acute or chronic course. Toxic manifestations in man occur at the cellular level and may appear in many organ systems. Specific effects can often be demonstrated in the skin and in the vascula

Carcinogenic, teratogenic, and mutagenic effects of arsenic.

This review outlines briefly the history and present status of the problem of carcinogenic, teratogenic and mutagenic effects of arsenic. Discrepancies between clinical observations and positive results of epidemiological studies and the experimental induction of cancer by arsenic are discussed. The present knowledge of the mechanism of teratogenic and mutagenic effects of arsenic is analyzed. The growing importance of arsenic as an environmental pollutant is demonstrated. Continuation of throughly organized epidemiological studies in regions with excessive arsenic exposure of the population a

D-penicillamine therapy of acute arsenic poisoning.

Severe poisoning resulting from single ingestions of rodenticides, herbicides, or insecticides containing arsenic have been frequently recognized. We record three cases of solubilized arsenic trioxide poisoning in Navajo Indian children and one case of sodium arsenate ingestion in an infant. One fatality occurred during dimercaprol therapy prior to initiation of therapy with D-penicillamine. Three survivors were treated with 2.3-dimercaprol intramuscularly and with oral D-penicillamine. The use of D-penicillamine in arsenic poisoning has not been generally appreciated. Excretion data from the

Arsenic and cancer: the still unanswered question.

For some 2,500 years, arsenic was considered an important element of pharmacopeias and was praised for its medicinal qualities. However, an incidental link with cancer was suggested during the nineteenth century, and since that time a controversy has ensued with regard to the compound's tumorigenicity. Several epidemiological surveys, for example, have suggested that arsenic induces lung, liver, or skin cancer in humans. On the other hand, all experimental attempts to reproduce such neoplasms in laboratory animals have consistently failed, thereby denying support to the human data. Recently ar

Subacute Arsenic Neuropathy: Clinical and Electrophysiological Observations.

We report a patient who developed subacute peripheral neuropathy following ingestion of a traditional medicine for obesity. A 9-year-old girl who had a residual equinus varus deformity and sphincter disturbance due to pelvic ganglioneuroma presented with subacute sensorimotor peripheral neuropathy of 2 weeks duration. Her symptoms started 3 weeks after she started taking a locally made traditional medicine for obesity. She had no other systemic features of arsenic toxicity. She had Mee's lines on her nails and high serum arsenic levels and 24-hour urine levels confirmed the diagnosis of arseni

Maternal arsenic exposure and nonsyndromic orofacial clefts.

BACKGROUND: Arsenic is widely distributed in the environment in both inorganic and organic forms. Evidence from animal studies suggests that maternal inorganic arsenic may lead to the development of orofacial clefts (OFC)s in offspring. This evidence, together with the limited epidemiologic data available, supports the need for a comprehensive examination of major sources of arsenic exposure and OFCs in humans. METHODS: Using interview data collected in the National Birth Defects Prevention Study, public and well water arsenic sampling data, and dietary arsenic estimates, we compared expert-r

Arsenic Toxicity in Male Reproduction and Development.

Arsenic is a toxic metalloid that exists ubiquitously in the environment, and affects global health problems due to its carcinogenicity. In most populations, the main source of arsenic exposure is the drinking water. In drinking water, chronic exposure to arsenic is associated with increased risks of various cancers including those of skin, lung, bladder, and liver, as well as numerous other non-cancer diseases including gastrointestinal and cardiovascular diseases, diabetes, and neurologic and cognitive problems. Recent emerging evidences suggest that arsenic exposure affects the reproductive

Arsenic and human health effects: A review.

Arsenic (As) is ubiquitous in nature and humans being exposed to arsenic via atmospheric air, ground water and food sources are certain. Major sources of arsenic contamination could be either through geological or via anthropogenic activities. In physiological individuals, organ system is described as group of organs that transact collectively and associate with other systems for conventional body functions. Arsenic has been associated with persuading a variety of complications in body organ systems: integumentary, nervous, respiratory, cardiovascular, hematopoietic, immune, endocrine, hepatic

Mitochondrial myopathy caused by arsenic trioxide therapy.

Arsenic trioxide (ATO) has been successfully used as a treatment for acute promyelocytic leukemia (APL) for more than a decade. Here we report a patient with APL who developed a mitochondrial myopathy after treatment with ATO. Three months after ATO therapy withdrawal, the patient was unable to walk without assistance and skeletal muscle studies showed a myopathy with abundant cytoplasmic lipid droplets, decreased activities of the mitochondrial respiratory chain complexes, multiple mitochondrial DNA (mtDNA) deletions, and increased muscle arsenic content. Six months after ATO treatment was in

Arsenic: toxicity, oxidative stress and human disease.

Arsenic (As) is a toxic metalloid element that is present in air, water and soil. Inorganic arsenic tends to be more toxic than organic arsenic. Examples of methylated organic arsenicals include monomethylarsonic acid [MMA(V)] and dimethylarsinic acid [DMA(V)]. Reactive oxygen species (ROS)-mediated oxidative damage is a common denominator in arsenic pathogenesis. In addition, arsenic induces morphological changes in the integrity of mitochondria. Cascade mechanisms of free radical formation derived from the superoxide radical, combined with glutathione-depleting agents, increase the sensitivi

Arsenic trioxide: safety issues and their management.

Arsenic trioxide (As2O3) has been used medicinally for thousands of years. Its therapeutic use in leukaemia was described a century ago. Recent rekindling in the interest of As2O3 is due to its high efficacy in acute promyelocytic leukaemia (APL). As2O3 has also been tested clinically in other blood and solid cancers. Most studies have used intravenous As2O3, although an oral As2O3 is equally efficacious. Side effects of As2O3 are usually minor, including skin reactions, gastrointestinal upset, and hepatitis. These respond to symptomatic treatment or temporary drug cessation, and do not compro

Percutaneous absorption of arsenic from environmental media.

Current knowledge of percutaneous absorption of arsenic is based on studies of rhesus monkeys using soluble arsenic in aqueous solution, and soluble arsenic mixed with soil (Wester et al., 1993). These studies produced mean dermal absorption rates in the range of 2.0-6.4% of the applied dose. Subsequently, questions arose as to whether these results represent arsenic absorption from environmental media. Factors such as chemical interactions, the presence of other metals, and the effects of weathering on environmental media all can affect the nature of arsenic and its potential for percutaneous

Arsenic speciation and toxicity in biological systems.

Although it is now commonly accepted that toxicity and bioavailability varies with As species, extensive research has been carried out on biological and environmental samples to assess toxicity and risk associated with As exposure based on total concentrations that may be in error. The health investigation guideline for the Australian environmental protection measure is 100 mg/kg (As(tot)), which would cause potential risk to human health if all the As present in a sample were bioavailable (ANZECC 1992). Similarly, the MPC for As in food is 1 mg/ kg (fresh weight), but this concentration may i
