Variability of arterial blood pressure in normal and hypertensive pregnancy.

In normal pregnancy the circadian blood pressure rhythm is similar to that in the non-pregnant state, with the highest blood pressure values in the morning and the lowest at midnight. This rhythm is lost in patients with pre-eclampsia. Women with severe pre-eclampsia show a reversed circadian rhythm, with a nocturnal increase in blood pressure during the sleeping phase. Although the reasons for this nocturnal hypertension in severe pre-eclampsia are poorly understood, the results suggest that pre-eclamptic women are endangered by hypertensive emergencies, mostly at night. Therefore blood press

Mean arterial pressure readings: variations with positions and transducer level.

The purpose of this study was to examine the relationship between mean arterial pressure (MAP) readings, measured at two reference points, the right atrium (RA) and the catheter site (CS), and in two body positions, supine and semi-Fowler's. Four MAP readings were taken on 29 CCU patients, 17 males and 12 females, with ages ranging from 33 to 93 (mean = 57.8; SD = 14.3). MAPs were obtained in the semi-Fowler's position-RA reference point (mean = 82.6), semi-Fowler's-CS reference point (mean = 90.3), supine-RA reference point (means = 82.2), and supine-CS reference point (mean = 84.03). Exercise alveolar-arterial oxygen pressure difference in interstitial lung disease.

Abnormality of gas exchange is best evaluated by the exercise alveolar-arterial oxygen pressure difference, P(A-a)O2. We studied the P(A-a)O2 in 168 patients with sarcoidosis, desquamative interstitial pneumonia (DIP), usual interstitial pneumonia (UIP), berylliosis, and asbestosis who were seen for clinical and disability consultations. The increase of P(A-a)O2 with exercise was greatest in UIP (mean 16 mm Hg), least in sarcoidosis (mean 1 mm Hg), and intermediate in DIP, berylliosis, and asbestosis (means 9, 9, and 7 mm Hg, respectively). The increase was best predicted by the single breath

Action of propranolol on arterial pressure and on cardiovascular reflexes.

In the rabbit, administration of 500 mug (+/-) propranolol into the lateral cerebral ventricle resulted in rapid leakage of drug into the blood. The drug produced a small reduction in blood pressure at 2 and 4 hours after injection but because of the rapid leakage, this could have been due to its peripheral actions. To investigate possible central mechanisms of action which were independent of peripheral effects, we tested the action of intravenously administered propranolol on the reflex rise in TPR evoked in rabbits by graded Valsalva-like manoeuvres. This was performed in unanaesthetised ra

Systemic arterial blood pressure and its relevance to optometric practice.

The measurement of systemic blood pressure is a very significant detection test that can provide a great deal of information for the optometrist. The blood pressure readings can be critical in aiding the practitioner's evaluation of both the patient's visual health and related systemic condition and in indicating the possible need for referral. This paper presents a general survey of the benefits blood pressure measurements can provide in the optometric practice.

The indirect measurement of arterial blood pressure in the horse.

An accurate modified auscultatory technique for the indirect measurement of arterial pressure in the horse is described. Limitations of the method were sensitivity to external motion in nervous, conscious horses, and the failure to detect Korotkoff sounds in shocked, anaesthetized surgical cases. The apparatus required for the method is expensive and is probably impractical for routine monitoring during anaesthesia.

The arterial stiffness and pulse pressure are predictors of intraocular pressure in a middle-aged population.

The vascular theory posits that glaucoma is associated to ocular blood flow alterations. However, there is uncertainty regarding the association of arterial stiffness index (ASI) and pulse pressure (PP) to intraocular pressure (IOP). This study investigates the relationship between ASI and PP, and IOP, using the UK Biobank population. Among 89,930 participants of the UK Biobank population, ASI and PP were assessed, and their correlations with IOP were estimated using multiple linear and logistic regressions adjusted for age, sex, BMI, diabetes, dyslipidemia, mean blood pressure, heart rate, to

Blood pressure variability in normotensive perimenopausal women: Non-dipping status, maximum blood pressure and arterial stiffness.

BACKGROUND: Postmenopausal women are more likely to have uncontrolled hypertension and are at higher risk of cardiovascular disease compared with age-matched men. Blood pressure variability is emerging as a predictor of adverse cardiovascular outcomes and may be implicated in the relationship between menopause and worsened vascular health in women. We conducted an observational study, BRAVE (Blood pRessure And Vascular hEalth around menopause) to study this relationship. METHOD: Normotensive perimenopausal women were recruited. Blood pressure variability was measured through 24-h blood pressu

Pulmonary capillary wedge pressure and pulmonary arterial pressure in heart failure patients with sleep-disordered breathing.

BACKGROUND: There is a high prevalence of central sleep apnea (CSA) in patients with chronic heart failure (CHF). The present study investigates the hypotheses that CSA in CHF patients reflects heart failure severity as measured by cardiac index (CI), pulmonary artery pressure (PAP) and pulmonary capillary wedge pressure (PCWP). METHODS: In 105 patients with stable CHF (NYHAII, LV-EF40%) cardiorespiratory polygraphy and simultaneous right and left heart catheterization was performed. RESULTS: CSA was present in 58% and obstructive sleep apnea (OSA) in 23% of patients. In CSA patients, PAP an

Assessment of dynamic cerebral autoregulation based on spontaneous fluctuations in arterial blood pressure and intracranial pressure.

Assessments of dynamic cerebral autoregulation usually measure the cerebral blood flow velocity (CBFV) response to changes in arterial blood pressure (ABP). We studied the effect of substituting ABP by cerebral perfusion pressure (CPP), expressed as the difference between ABP and intracranial pressure (ICP), in estimates of dynamic autoregulation obtained by transfer function analysis. CBFV, ABP and ICP were recorded during periods of physiological stability in 30 patients with severe head injury. Transfer function analysis was performed using the following combinations of input-output variabl

[Ambulatory registration of arterial pressure during treatment with perindopril. Effects on systolic pressure and hemodynamic implications].

The effects of perindopril on the 24-hour arterial pressure levels were evaluated by ambulatory recording in 21 patients (mean age 48 +/- 2 years) with mild to moderate hypertension. At the end of a 3 months treatment with perindopril (4 to 8 mg per day in one dose), comparison by variance analysis of the mean values of arterial pressure over 24 hours before and after treatment showed a significant decrease of SAP (from 144 +/- 3 to 133 +/- 3 mmHg, p less than 0.01) and DAP (from 95 +/- 2 to 87 +/- 2 mmHg, p less than 0.01). The fall in arterial pressures was more pronounced during day-time (7

Influence of bolus doses of phenoperidine on intracranial pressure and systemic arterial pressure in traumatic coma.

The effects of bolus doses of phenoperidine 1-2 mg i.v. either alone, or combined with pancuronium 2-4 mg, were investigated in seven patients in traumatic coma. Phenoperidine alone significantly reduced mean arterial pressure (MAP) by a mean (+/- SEM) of 13.2 (+/- 2.8) mm Hg. Overall there was no significant change in intracranial pressure (ICP) despite the decreases in MAP and, consequently, cerebral perfusion pressure (CPP) decreased (14.0 +/- 2.4 mm Hg) on all but one occasion. In some instances these decreases were considerable (maximum 38 mm Hg). Similar results were obtained when phenop

Dynamic Arterial Elastance to Predict Mean Arterial Pressure Decrease after Reduction of Vasopressor in Septic Shock Patients.

After fluid status optimization, norepinephrine infusion represents the cornerstone of septic shock treatment. De-escalation of vasopressors should be considered with caution, as hypotension increases the risk of mortality. In this prospective observational study including 42 patients, we assess the role of dynamic elastance (EaDyn), i.e., the ratio between pulse pressure variation and stroke volume variation, which can be measured noninvasively by the MostCare monitoring system, to predict a mean arterial pressure (MAP) drop > 10% 30 min after norepinephrine reduction. Patients were divided i

Age-related differences in blood pressure, ultrasound-derived arterial diameters and arterial wall stiffness parameters in horses.

BACKGROUND: Arterial rupture mainly affects older horses. The reason why older horses are more prone to arterial rupture and which underlying vascular changes predispose older horses to aortic rupture is still unclear. OBJECTIVES: To investigate the effect of ageing on the equine arterial wall and blood pressure. STUDY DESIGN: Cohort study. METHODS: Non-invasive blood pressure measurement using a tail cuff and vascular ultrasound from aorta, common carotid artery and external iliac artery was performed in 50 healthy young (3-7 years) and 50 healthy old Warmblood horses (>18 years). Non-invasive Estimation of the Intracranial Pressure Waveform from the Central Arterial Blood Pressure Waveform in Idiopathic Normal Pressure Hydrocephalus Patients.

This study explored the hypothesis that the central aortic blood pressure (BP) waveform may be used for non-invasive estimation of the intracranial pressure (ICP) waveform. Simultaneous invasive ICP and radial artery BP waveforms were measured in 29 individuals with idiopathic normal pressure hydrocephalus (iNPH). The central aortic BP waveforms were estimated from the radial artery BP waveforms using the SphygmoCor system. For each individual, a transfer function estimate between the central aortic BP and the invasive ICP waveforms was found (Intra-patient approach). Thereafter, the transfer

Effect of torasemide on intracranial pressure, mean systemic arterial pressure and cerebral perfusion pressure in experimental brain edema of the rat.

The study was performed to establish whether a lipophilic loop diuretic, torasemide could modify intracranial pressure and cytotoxic brain edema. Brain edema was induced by water intoxication in nephrectomized rats. Following intravenous injection of 100 mg torasemide/kg body weight at 50, 60, 70, 90 and 120 min, a significant decrease of intracranial pressure was observed.

Arterial site selection for measurement of mean arterial pressure in septic shock patients on high-dose norepinephrine.

BACKGROUND: The guidelines of the Surviving Sepsis Campaign suggest using invasive blood pressure (IBP) measurement in septic shock patients, without specifying for a preferred arterial site for accuracy in relation to the severity of septic shock. The objective of this study was to determine the mean arterial pressure (MAP) gradient between the femoral and radial artery sites in septic shock patients. METHOD: This prospective study was carried out at a 20-bed ICU in a university hospital. Simultaneous MAP measurements at femoral and radial arterial sites were obtained in septic shock patient
