Plasma FGF23 is associated with left atrial remodeling in children on hemodialysis.

BACKGROUND: FGF23 mediates cardiac fibrosis through the activation of pro-fibrotic factors in in vitro models and is markedly elevated in kidney disease. Left atrial global longitudinal strain (LA GLS) derived by echocardiographic speckle-tracking measures longitudinal shortening of the LA walls, quantifies atrial performance and may enable detection of early LA remodeling in the setting of normal ventricular function. We hypothesized that LA GLS is abnormal in children on hemodialysis (HD) compared to healthy controls of comparable age/sex distribution and that, among HD patients, greater FGF

Left Atrial Reservoir Strain as a Marker of Carotid Vascular Remodeling: Insights Into the Atrial-Vascular Aging Continuum.

BACKGROUND: Left atrial reservoir strain (LASr) has emerged as a sensitive marker of subclinical cardiac dysfunction and has been proposed as an early indicator of cardiovascular aging. However, whether impaired LASr reflects the progressive spectrum of carotid vascular aging has not been fully established. AIMS: To evaluate the association between LASr and different stages of carotid vascular aging and to determine its ability to identify advanced carotid vascular disease in adults undergoing comprehensive cardiovascular imaging. DESIGN: Retrospective, single-center, observational cross-sec

Radiofrequency Ablation of Persistent Atrial Fibrillation: Diagnosis-to-Ablation Time, Markers of Pathways of Atrial Remodeling, and Outcomes.

BACKGROUND: Various ablation strategies of persistent atrial fibrillation (PersAF) have had disappointing outcomes, despite concerted clinical and research efforts, which could reflect progressive atrial fibrillation-related atrial remodeling. METHODS AND RESULTS: Two-year outcomes were assessed in 1241 consecutive patients undergoing first-time ablation of PersAF (2005-2012). The time intervals between the first diagnosis of PersAF and the ablation procedures were determined. Patients had echocardiograms and measures of B-type natriuretic peptide and C-reactive protein before the procedures.

Repetitive electrical remodeling by paroxysms of atrial fibrillation in the goat: no cumulative effect on inducibility or stability of atrial fibrillation.

INTRODUCTION: Episodes of atrial fibrillation (AF) are known to cause both a rapid reduction in atrial refractoriness (atrial electrical remodeling) and a more delayed increase in AF stability in the chronic goat model. The aims of this study were to examine (1) the hypothesis that an AF-induced increase in AF stability might be due to a mechanism with a longer onset and offset than that of changes in refractoriness and (2) the possibility that repeated paroxysms of maintained AF might cause a cumulative increase in AF stability independent of changes in atrial refractoriness. METHODS AND RES

Left Atrial Remodeling Identification and Catheter Ablation Outcomes With 18F-Fluorodeoxyglucose Positron Emission Tomography in Persistent Atrial Fibrillation.

BACKGROUND: Left atrial structural remodeling contributes to the persistence of atrial fibrillation (AF) and influences the outcomes of catheter ablation (CA). We investigated the usefulness of 18F-fluorodeoxyglucose-positron emission tomography in detecting low atrial glucose uptake (LGU) as a potential marker of fibrosis and its predictive value for CA success in persistent AF. METHODS: Thirty-six patients without diabetes with persistent AF scheduled for CA underwent nicotinic acid-stimulated 18F-fluorodeoxyglucose-positron emission tomography to assess global and segmental LGU before CA.

Left Atrial Mechanics and Remodeling in Paroxysmal Atrial Fibrillation: Introducing the EASE Score for Pre-Ablation Risk Prediction.

Background: Paroxysmal atrial fibrillation (PAF) is a common arrhythmia often treated with catheter ablation, particularly pulmonary vein isolation (PVI). However, recurrence remains frequent and is often linked to unrecognized structural and functional remodeling of the left atrium. Methods: We introduce the Echocardiographic Atrial Strain and conduction Evaluation (EASE) score as a theoretical, noninvasive model to stratify recurrence risk in patients undergoing catheter ablation for PAF. The score is based on the hypothesis that integrated echocardiographic parameters can reflect the extent

Left atrial remodeling and voltage-guided ablation outcome in persistent atrial fibrillation patients according to CHA2DS2-VASc score.

BACKGROUND: CHA2DS2-VASc score-related differences have been reported in atrial fibrotic remodeling and prognosis of atrial fibrillation (AF) patients after ablation. There are currently no data on the efficacy of low voltage zone (LVZ)-guided ablation in persistent AF patients according to CHA2DS2-VASc score. We assessed in a cohort of persistent AF patients the extent of LVZ, the regional distribution of LA voltage and the outcome of LA voltage-guided substrate ablation in addition to PVI according to CHA2DS2-VASc score. METHODS: 138 consecutive persistent AF patients undergoing a first vol

Macrophage Inflammatory Protein-1 Alpha, a Potential Biomarker for Predicting Left Atrial Remodeling in Patients With Atrial Fibrillation.

Objectives: Left atrial (LA) remodeling itself is an independent risk factor for ischemic stroke and mortality, with or without atrial fibrillation (AF). Macrophage inflammatory protein-1 alpha (MIP-1α) has been reported to be involved in the induction of autoimmune myocarditis and dilated cardiomyopathy. Little is known about whether MIP-1α can be used to predict LA remodeling, especially in patients with AF. Methods: We prospectively enrolled 78 patients who had received a cardiac implantable electronic device due to sick sinus syndrome in order to define AF accurately. AF was diagnosed clin

Severe and uniform bi-atrial remodeling measured by dominant frequency analysis in persistent atrial fibrillation unresponsive to ablation.

BACKGROUND: High values of ECG and intracardiac dominant frequency (DF) are indicative of significant atrial remodeling in persistent atrial fibrillation (peAF). We hypothesized that patients with peAF unresponsive to ablation display higher ECG and intracardiac DFs than those remaining in sinus rhythm (SR) on the long term. METHODS: Forty consecutive patients underwent stepwise ablation for peAF (sustained duration 19 ± 11 months). Electrograms were recorded before ablation at 13 left atrium (LA) sites and at the right atrial appendage (RAA) and coronary sinus (CS) synchronously to the ECG.

Persistent left atrial remodeling after catheter ablation for non-paroxysmal atrial fibrillation is associated with very late recurrence.

BACKGROUND: This study aimed to evaluate the association between left atrial (LA) structural remodeling and very late recurrence [VLR; initial recurrence >12 months after catheter ablation (CA)] after successful CA for non-paroxysmal atrial fibrillation (AF). METHODS: We retrospectively evaluated 63 patients who underwent initial, single ablation for drug-refractory persistent or long-standing persistent AF and those who had no recurrence in the first year after CA. We followed patients for a mean of 3.2±1.5 years and divided them into VLR and no-recurrence (NR) groups. Before and 3 months af

Effects of verapamil and metoprolol on recovery from atrial electrical remodeling after cardioversion of long-lasting atrial fibrillation.

The aim of this prospective, randomized study was to investigate the effect of pretreatment with two different intracellular calcium-lowering drugs (verapamil and metoprolol) on recovery from atrial effective refractory period (AERP) shortening after internal electrical cardioversion (EC) of persistent atrial fibrillation (AF) in patients on amiodarone. Twenty-one patients on amiodarone for at least 30 days were referred to our hospital for internal EC of a persistent AF refractory to external EC. They were randomized to receive only amiodarone (group AMI, n=7), or amiodarone and verapamil 240

The risk factors of atrial substrate remodeling in the patients of paroxysmal atrial fibrillation following pulmonary vein isolation.

BACKGROUND: Atrial substrate remodeling (ASR) was emerged as a critical determinant of very late recurrence (VLR) in paroxysmal atrial fibrillation (PaAF). However, the multifaceted risk factors driving ASR progression and their interplay with clinical outcomes remain incompletely characterized. This study aimed to identify clinical, electrophysiological, and structural risk factors associated with ASR in PaAF patients experiencing VLR after catheter ablation (CA). METHODS: A total of 1786 consecutive patients with PaAF who underwent catheter ablation at Xinhua Hospital Affiliated to Shanghai

Altered left atrial 4D flow characteristics in patients with paroxysmal atrial fibrillation in the absence of apparent remodeling.

The pathophysiology behind thrombus formation in paroxysmal atrial fibrillation (AF) patients is very complex. This can be due to left atrial (LA) flow changes, remodeling, or both. We investigated differences for cardiovascular magnetic resonance (CMR)-derived LA 4D flow and remodeling characteristics between paroxysmal AF patients and patients without cardiac disease. In this proof-of-concept study, the 4D flow data were acquired in 10 patients with paroxysmal AF (age = 61 ± 8 years) and 5 age/gender matched controls (age = 56 ± 1 years) during sinus rhythm. The following LA and LA appendage
