This study was carried out to examine the incidence, morphology and clinical implication of antral septa. Out of 265 maxillary sinuses, 65 sinuses in atrophic maxillae were examined clinically during sinus floor elevation and 200 sinuses examined radiographically (CT), the latter being further subdivided into non-atrophic/dentate and atrophic/edentulous maxillary segments. Eighteen (27.7%) out of 65 clinically-examined maxillae and 32 (16%) out of the 200 non-preselected CT-examined maxillary sinuses showed antral septa. CT-topogram subclassification revealed 21 septa (13.2%) in 159 non-atroph

BACKGROUND: Implant therapy in the advanced atrophic jaw remains challenging in oral and maxillofacial surgery. Hence, a plethora of different augmentation procedures to increase bone volume in the maxilla and mandible have been published. Horizontal vascular-stalked split osteotomy (HVSO) represents a safe and effective approach for the three-dimensional jaw augmentation since it combines maximum vascularization through lingual or palatinal periosteal stalking with reduced grafting morbidity.
OBJECTIVE: To analyze the efficacy of HVSO for implantation therapy in atrophic jaws by assessing ve

BACKGROUND: Oral rehabilitation of atrophic maxillae features high complexity, for which there are several therapeutic modalities reported on scientific literature. Zygomatic implant placement is a viable option that features low morbidity and allows immediate prosthetic loading. The purpose of the present study was to determine the methodological quality of systematic reviews that assessed the effectiveness of zygomatic implants placed in atrophic maxillae.
MATERIAL AND METHODS: Searches were conducted on Medline via Pubmed, LILACS, Dare Cochrane, Scopus, and Sigle via Open Grey up to June 2

Edentulous patients with an atrophic maxilla associated with lip-palate fissures have unpredictable results after undergoing grafting procedures. In situations where the atrophic maxilla does not adequately allow reconstruction, the use of zygomatic implants has been indicated, and probably these implants can be indicated for the rehabilitation of patients with lip-palate fissures. This case report describes the oral rehabilitation treatment of a patient with a lip-palate cleft treated with zygomatic implants and implant-supported fixed prosthesis with two years of follow-up. A 65-year-old fem

Effectively restoring a grossly atrophic maxilla can be difficult for the implant surgeon. The placement of dental implants in patients who are edentulous in the posterior maxilla can present difficulties because of deficient posterior alveolar ridge and increased pneumatization of the maxillary sinus, resulting in a minimal hard tissue bed. Implant placement requires adequate quality and quantity of bone, especially in the posterior maxilla. Insufficient bone height and width in this area of the maxilla, because of expansion of the maxillary sinus and atrophic reduction of the alveolar ridge,

Implant rehabilitation of the posterior maxilla can entail difficulties due to reduced bone quantity and poor bone quality, especially after long-term edentulism. In some patients, multiple surgeries are necessary, which may lead to higher patient morbidity and longer treatment times before a prosthetic rehabilitation can be achieved. This article presents an immediate fixed prosthesis in a posterior atrophic maxilla supported by 1 anterior axial implant and 1 posterior tilted fixture placed with an intrasinus insertion. Additionally, a classification scheme for immediate treatment for the pos

A technique for maxillary bone grafting to augment the atrophic ridge is presented, and the results from 15 cases followed from three to ten years are briefly described. Loss of postoperative ridge height ranged from 10-20% in this sample. It appears that the tendencies to postoperative resorption that occur when autogenous particulate cancellous bone grafts are employed to restore atrophic mandibles are not operative in the maxilla.

Restoration of the atrophic edentulous maxilla and mandible with implant retained prostheses has involved the use of axially placed implants in regions of the maxilla and mandible based on the adequate availability of bone, often using a staged surgical approaches. Anatomic limitations including pneumatized maxillary sinus, proximity of the inferior alveolar nerve and lack of available native bone have many clinicians performing traditional grafting procedure prior to implant placement. Utilization of the "All-on-4" concept has overcome these anatomic restrictions by allowing placement of 2 ve

The present article discusses an atrophic maxilla reconstruction with iliac crest bone block and particulate grafts and dental implants. Onlay block grafts were used to restore bone volume of the anterior maxilla, whereas bilateral sinus floor augmentation was performed using a particulate graft. Ten months after the grafting surgery, 9 dental implants were placed to rehabilitate the case. Results of a 7-year follow-up were obtained clinically and by cone beam computed tomographic images.

This report describes the case of an edentulous patient with an atrophic maxilla and severe class III malocclusion. Prosthetic rehabilitation was performed using CAD/CAM techniques for manufacturing an implant-supported overdenture with horizontal insertion. A vestibulo-lingual insertion overdenture is a precision prosthesis with a fixation system affording a good fit between the primary and secondary structure. Both structures exhibit passive horizontal adjustment. This treatment option requires the same number of implants as implant-supported fixed dentures. The horizontal assembly system pr

Combination syndrome (CS) is a pathologic condition of the stomatognathic system characterized by an edentulous maxilla with an atrophic anterior ridge, maxillary papillary hyperplasia, hypertrophic maxillary tuberosities, super-erupted mandibular anterior teeth, as well as atrophic mandibular posterior ridges. This condition can present a challenging situation to the clinician who needs to assemble evidence from all aspects of dentistry to formulate an appropriate treatment plan. One of the possible treatment choices for the fully edentulous patient with a CS is a full-arch implant-supported

The aim of this retrospective study was to evaluate the outcomes of simultaneous LeFort I osteotomy and zygomatic/dental implant placement for oral rehabilitation of patients with extremely atrophic/dysmorphic edentulous maxilla. Simultaneous LeFort I osteotomy and zygomatic/dental implant placement was performed with patient-specific anatomical models and surgical guides produced through three-dimensional virtual planning methods. All patients received their final prosthesis, with immediate loading, on the day after surgery. The primary outcome variables were the implant survival rate, and th

Various techniques have been proposed to regenerate deficient ridges after tooth removal, including guided bone regeneration, block grafting, distraction osteogenesis, and ridge splitting. However, these procedures are technique-sensitive and often present complications which prevent reconstruction of the deficient ridge and implant placement. In an atrophic anterior or posterior maxilla, these techniques often fail to produce satisfactory long-term outcomes due to the poor bone quality, pneumatization of the maxillary sinus, and the highly cosmetic patient demands. The customized alveolar rid
