Atropine (0.05%) for rapid progressive childhood myopia (ARM study).

PURPOSE: This study aims to assess the effectiveness of atropine 0.05% for myopia control among children exhibiting (documented) rapid myopia progression (>0.75D/year). METHODS: This prospective interventional single-arm clinical trial included children aged between 6-12 years, spherical equivalent refractive (SER) error between - 2 and - 6D, and having documented myopia progression of >0.75D in the preceding year. All participants were administered atropine 0.05% in both eyes once at bedtime for 1 year. The primary outcome measure was a change in the rate of myopia progression (D/year) and c

Comparison of cycloplegia with atropine 1% versus cyclopentolate 1.

PURPOSE: Cycloplegic refraction is mandatory for children to know the eye's refractive status. In this study, we compared cycloplegia induced by cyclopentolate 1% to that induced by atropine 1% by means of retinoscopy. METHODS: In this parallel-designed interventional study, we included 67 children aged between 4 and 17 years. After the initial retinoscopy under cyclopentolate 1% (used twice in each eye), we repeated it a week later under atropine ointment 1% (used twice a day for 3 days); both were done by the same trained optometrist masked to the drug. Each eye's refraction was converted t

Transient symptomatic worsening by atropine in opsoclonus-myoclonus syndrome.

Opsoclonus-myoclonus syndrome (OMS) is characterized by abnormal eye and systemic involuntary movements, as well as cerebellar ataxia. Some sedatives and anesthetics worsen movements associated with OMS, while there is no known report of a negative effect of atropine. We report on sedation in two patients with OMS. Involuntary movements were transiently worsened after using atropine with midazolam or thiamylal in both, but were not seen when atropine was not used. We speculated that atropine has the potential to exacerbate involuntary movements in OMS due to vulnerability to this agent via unk

Is prophylactic atropine necessary during ketamine sedation in children?

AIM: To quantify clinically significant hypersalivation and other adverse events requiring intervention, with and without the use of atropine during ketamine use, using a consensus-based, standardised terminology. METHODS: This was a retrospective study based on paediatric patients who received ketamine for procedures done at the children's emergency department from July 2010 to September 2010. Patients who were given atropine were compared with patients who were not given atropine with regard to clinically significant hypersalivation. All other side effects of ketamine (airway, respiratory,

Impurity profiling of atropine sulfate by microemulsion electrokinetic chromatography.

An oil-in-water microemulsion electrokinetic chromatography (MEEKC) method has been developed and validated for the determination of atropine, its major degradation products (tropic acid, apoatropine and atropic acid) and related substances from plants material (noratropine, 6-hydroxyhyoscyamine, 7-hydroxyhyoscyamine, hyoscine and littorine). Separation of atropine and all impurities was optimized by varying the voltage, the nature of the oil droplet and the buffer, as well as the organic modifier (methanol, 2-propanol or acetonitrile) and the surfactant type and concentration. The optimum O/W

Plasma atropine concentrations via intravenous, endotracheal, and intraosseous administration.

To date, there have been limited studies on the pharmacokinetics of intravenous atropine and no pharmacokinetic studies on the endotracheal or intraosseous administration of atropine. This study examines the time to peak plasma concentration of atropine following intravenous, endotracheal, and intraosseous administration in anesthetized monkeys using a triple crossover design. Plasma atropine was assayed by a radioreceptor method. The time to peak plasma concentration of atropine was shortest with intravenous administration; and longest with endotracheal administration. The mean plasma concent

Midazolam versus atropine plus pethidine as premedication in children.

The effects of oral midazolam or intramuscular atropine and pethidine used as premedication in two groups of 35 children over 5 years of age were studied. There was some evidence that the anxiolytic effect of midazolam was rather better than that of atropine plus pethidine, but, in other respects, subjective assessments in the two patient groups were similar. Intramuscular atropine caused tachycardia and subjective side-effects, nevertheless children appear to require anticholinergics during premedication because of excessive salivary secretion, especially during extubation. Oral midazolam is

Long Q-T syndrome precipitated by atropine and hypokalemia.

In a small child successfully treated with atropine sulfate and other drugs because of pesticide poisoning, a prolongation of the Q-T interval, extrasystolic ventricular tachyarrhythmia complicated by ventricular fibrillation and cardiac syncope, was observed. At the time of this incident the child was hypokalemic. After compensation of hypokalemia, reduction of atropine dose and subsequent discontinuation of the drug, the changes in the patient's follow-up ECG records disappeared. Thus, the use of atropine in the presence of hypokalemia should be included in the list of factors releasing the

Use of atropine for brady-asystolic prehospital cardiac arrest.

The efficacy of atropine in treating prehospital cardiac arrest patients developing asystole slow pulseless idioventricular rhythms (PIVR) was evaluated in a controlled, prospective study. Twenty-one prehospital cardiac-arrested patients developing asystole or PIVR (less than 40) were divided into atropine-treated or non-atropine (control) groups. Control group patients received treatment including bicarbonate, epinephrine, calcium, isoproterenol, dexamethasone, and transthoracic pacing. Atropine-treated patients received 1 mg atropine intravenously with a repeat dose at one minute if no rhyth

Refraction of 1-year-old children after atropine cycloplegia.

The refractions of 1648 children aged 11 to 13 months are reported. Atropine 1% was used for cycloplegia. 11.83% of the children had bilateral hypermetropia of +2.00 or more D. 13.23% of them had +1.50 or more D astigmatism in one or both eyes, and 6.5% had anisometropia. Anisometropia was significantly (P=0.000 001%) associated with bilateral hypermetropia, but even more significantly (P=0.000 000 4%) associated with astigmatism of +1.50 or more D in one or both eyes.

Using low-dose atropine in control of intermittent exotropia.

CLINICAL RELEVANCE: There are a variety of non-surgical methods to control and correct intermittent exotropia. Identifying the effectiveness of an alternative method that carries minimal risk and ocular symptoms can guide clinicians in optimising patient care. BACKGROUND: The aim of this work is to evaluate the efficacy of 0.05% atropine eye drops on exotropia control in children with intermittent exotropia. METHODS: This parallel-group, controlled, randomised clinical trial was conducted on children aged 2-8 years with intermittent exotropia who had been admitted at the Paediatric Ophthalmo

Atropine Eye Drops in Childhood Myopia Control: A Review.

Myopia is a pandemic problem in this world. Per 2019 report of the World Health Organization in their first-ever report on vision, ~27% world's population are suffering from myopia. This trend is expected to increase to ~50% by 2050. Unfortunately, no cure method is available so far due to its complications of the mechanistic actions. Therefore, control of myopia is the focus clinically. Currently, it is suggested that 0.05% atropine is optimal, balanced in efficacy and rebound in controlling myopia initiation and progression. In such a control process, measurement of reduction in spherical eq

Topical review: Potential mechanisms of atropine for myopia control.

Atropine is effective at slowing myopia progression in children, but the mechanism of action by which it controls myopia remains unclear. This article is an evidenced-based review of potential receptor-based mechanisms by which atropine may act to slow the progression of myopia. The rising number of individuals with myopia worldwide and the association between myopia and vision-threatening ocular pathologies have made myopia control treatments one of the fastest growing areas of ophthalmic research. High-concentration atropine (1%) is the most effective treatment for slowing myopia progression

Comparisons of atropine versus cyclopentolate cycloplegia in myopic children.

CLINICAL RELEVANCE: In clinical practice, 1% atropine and 1% cyclopentolate are used as cycloplegia agents to diagnose refractive error. The influence of 1% atropine on ocular biometry is obscure, and the impact of 1% cyclopentolate remains controversial. BACKGROUND: This study aims to compare the effects of atropine versus cyclopentolate cycloplegia on ocular biometry in myopic children and to determine the sites of action for atropine. METHODS: A total of 207 myopic children aged 6-12-years were included in the analysis. All participants underwent comprehensive eye examinations before and
