Autonomic Dysreflexia in Spinal Cord Injury: Mechanisms and Prospective Therapeutic Targets.

High-level spinal cord injury (SCI) often results in cardiovascular dysfunction, especially the development of autonomic dysreflexia. This disorder, characterized as an episode of hypertension accompanied by bradycardia in response to visceral or somatic stimuli, causes substantial discomfort and potentially life-threatening symptoms. The neural mechanisms underlying this dysautonomia include a loss of supraspinal control to spinal sympathetic neurons, maladaptive plasticity of sensory inputs and propriospinal interneurons, and excessive discharge of sympathetic preganglionic neurons. While ne

Intracerebral haemorrhage following uncontrolled autonomic dysreflexia post suprapubic catheter placement surgery.

INTRODUCTION: We describe a case of intracerebral haemorrhage (ICH) following uncontrolled episodes of autonomic dysreflexia (AD) within 24 h of a minor urological procedure. CASE PRESENTATION: A 33-year-old active paraplegic patient T1 Association Impairment Scale A underwent an elective suprapubic catheter (SPC) placement for bladder management. The surgery was done under general anaesthesia and was uneventful. Four hours after surgery, he developed haematuria and multiple blood clots in the urine, which eventually caused blockage of the SPC and resulted in symptomatic AD. The clots and blo

[Acute aortic dissection induced by autonomic dysreflexia; report of a case].

Autonomic dysreflexia can occur in patients with spinal cord injury and lead to excessive hypertension. A 61-year-old man who had been treated for spinal cord injury 28 years before in an accidental fall, was admitted with an acute aortic dissection and underwent replacement of ascending aorta. He had been injured the 6th cervical neurologic level and he had not been given regular medical care. His incident of aortic dissection during defecation might related to the elevation of blood pressure due to autonomic dysreflexia.

Autonomic dysreflexia: an important cardiovascular complication in spinal cord injury patients.

Autonomic dysreflexia (AD) is a life-threatening complication of spinal cord injury (SCI) at T6 or above that results in an uncontrolled sympathetic discharge in response to noxious stimuli. It is a symptom complex characterized by a lethal rise in blood pressure with dangerous consequences. Autonomic dysreflexia is often secondary to urological, gastrointestinal, or gynecological problems or manipulations. Early recognition and prompt treatment of AD is vital to prevent complications, including death. Its management starts primarily with its prevention. Easy measures can avoid this high risk

Gabapentin for spasticity and autonomic dysreflexia after severe spinal cord injury.

STUDY DESIGN: Using a complete transection spinal cord injury (SCI) model at the fourth thoracic vertebral level in adult rats, we evaluated whether blocking noxious stimuli below the injury diminishes abnormal somatic and autonomic motor reflexes, manifested in muscular spasticity and hypertensive autonomic dysreflexia, respectively. Gabapentin (GBP) is well tolerated and currently used to manage neuropathic pain in the SCI population; evidence suggests that it acts to decrease presynaptic glutamate release. As clinical evidence indicates that GBP may suppress muscular spasticity in the chron

Early autonomic dysreflexia in acute paraplegia following anterior spinal artery thrombosis.

This report discusses the diagnosis and management of autonomic dysreflexia. A 19-year-old man presented with an acute quadriplegia secondary to anterior spinal artery thrombosis. He required admission to the intensive care unit for ventilatory support and developed autonomic dysreflexia within 72 hours of his first symptoms, due to paralytic ileus with distension of the bowel. He was successfully treated by sublingual nifedipine and bowel decompression. Awareness of the potential for autonomic dysreflexia to occur in the acute phase of spinal cord injury will assist to direct management appro

Autonomic dysreflexia: one more way EMS can positively affect patient survival.

Autonomic dysreflexia is a life-threatening medical condition that affects people with spinal cord injuries above T6. Caused by the division of the autonomic nervous system, it can result in disastrous hypertension. Although complicated in nature, AD can be quickly treated and reversed by prehospital providers. The prompt emptying of a patient's bladder and/or bowels will resolve most occurrences. Other factors that can't be resolved in the prehospital setting may cause AD. In these situations, quickly transport the patient to a definitive care facility and consider the use of antihypertensive

Sacral bladder denervation for treatment of detrusor hyperreflexia and autonomic dysreflexia.

OBJECTIVES: Detrusor hyperreflexia after spinal cord injury may cause urinary incontinence and chronic renal failure. In patients refractory to conservative treatment and not eligible for ventral sacral root stimulation for electrically induced micturition, we investigated the therapeutic value of sacral bladder denervation as a stand-alone procedure. METHODS: Nine patients (8 men and 1 woman) between 21 and 58 years old (mean 30.2) with traumatic suprasacral spinal cord lesions underwent sacral bladder denervation for treatment of detrusor hyperreflexia and/or autonomic dysreflexia. Autonomic dysreflexia and detrusor-sphincter dyssynergia in spinal cord injury patients.

This study reevaluates the significance of our previously reported blood pressure monitoring during cystometrographic studies in spinal cord injury patients who had detrusor-sphincter dyssynergia. We have now evaluated 26 spinal cord injury patients (21 tetraplegics and five high paraplegics) with complex urodynamic studies both before and after transurethral modified sphincterotomy (TURS). In these patients, mean systolic rise prior to TURS was 32.4 mm Hg (S.D. +/- 16.3) and diastolic rise was 14.3 mm Hg (S.D. +/- 9.3). Following TURS, mean systolic rise was 15.5 +/- 0.1 and diastolic rise wa

Silent autonomic dysreflexia during voiding in men with spinal cord injuries.

PURPOSE: We determined whether symptoms of autonomic dysreflexia correlated with elevations in blood pressure in men with spinal cord injuries. MATERIALS AND METHODS: During a routine yearly urodynamic evaluation 45 consecutive men with complete spinal cord injuries above T6 underwent simultaneous monitoring of blood pressure and symptoms of autonomic dysreflexia. Those with systolic blood pressure of greater than 160 mm. Hg or diastolic blood pressure of greater than 90 mm. Hg during voiding were assigned to the hypertensive group. During voiding 35 men (78%) had significant hypertension. R

Metastatic melanoma to the bladder presenting as autonomic dysreflexia in a patient with paraplegia.

We report a case of a 66-year-old male with T2 American Spinal Injury Association Impairment Scale (AIS) A paraplegia who presented to Urology with worsening autonomic dysreflexia. Work-up identified a bladder mass treated by transurethral resection and pathologically confirmed as melanoma. Additional work-up revealed metastatic melanoma to the back and brain. The patient completely recovered with no evidence of disease more than two years after completing therapy. In this report, we review the presentation of metastatic bladder melanoma presenting with autonomic dysreflexia, which has never b

Autonomic dysreflexia in a case of radiation myelopathy and cisplatin-induced polyneuropathy.

INTRODUCTION: While autonomic dysreflexia caused by severe spinal cord lesions can be life-threatening, relevant reports on non-traumatic spinal lesions are rare. Furthermore, modes of innervation of the supraspinal inhibitory pathways at each spinal sympathetic segment remain unknown. Herein, I report the case of a patient with autonomic dysreflexia and radiation myelopathy. The laterality of autonomic dysreflexia was investigated with special reference to the sudomotor function. CASE PRESENTATION: A 51-year-old man with a history of epipharynx carcinoma, radiotherapy, and cisplatin chemothe

Recognition and management of autonomic dysreflexia in patients with a spinal cord injury.

Autonomic dysreflexia is a potentially life-threatening condition that affects patients with a spinal cord injury at the level of T6 or above. It is characterised by uncontrolled elevation of systolic blood pressure of more than 20mmHg, which may occur alongside bradycardia. This article explains the pathophysiology of autonomic dysreflexia, alongside its causes, signs and symptoms. It also details the pharmacological and non-pharmacological management interventions that should be promptly initiated in patients who present to the emergency department with autonomic dysreflexia, to alleviate th

Autonomic Dysreflexia in the Peripartum Patient: A Multidisciplinary and Interprofessional Simulation Scenario.

This case is one of an eight-case multidisciplinary curriculum designed and implemented at the University of Ottawa by simulation educators with specialty training in obstetrics and gynecology (ob/gyn) and anesthesiology. Consultation with a nurse educator maintained quality and relevance of objectives for nursing participants. The curriculum was prepared to train ob/gyn and anesthesiology residents and nurses to hone crisis resource management skills and to recognize and manage rare/critical medical events in an obstetrical setting. Obstetricians, anesthesiologists, and nurses often work toge

Autonomic dysreflexia triggered by an unstable lumbar spine in a quadriplegic patient.

A 40 year-old man with C5 complete quadriplegia, had L2-L3 pyogenic spondylitis treated with debridement and fusion of the lumbar spine with left iliac bone graft. Three months later he developed symptoms of autonomic dysreflexia, including headache, cold sweating, and hypertension whenever he was in an upright position. These symptoms resolved after lying down. Roentgenograms of the lumbar spine revealed absorption of the bone graft at the L2-L3 level. A spinal stabilization procedure was done after eight weeks of antibiotic therapy under the impression of unstable spine caused by pyogenic sp

Long-term follow-up of sphincterotomy in the treatment of autonomic dysreflexia.

Autonomic dysreflexia (AD) is a syndrome affecting a majority of patients with high spinal cord lesions. An association between AD and detrusor-sphincter dyssynergia (DSD) is often seen. Between 1980 and 1988 we performed 51 transurethral sphincterotomies in 38 patients. Twelve of these patients underwent procedures primarily for the prevention of AD related to bladder distension. This study presents their long-term follow-up. One patient was lost to follow-up secondary to unrelated death; the remaining 11 patients were followed on average 117.5 months (94-154) after the initial operative proc
