Do laboratory fear extinction, avoidance and generalization predict exposure outcomes for fear of spiders?

This study assessed the predictive potential of laboratory conditioning processes (extinction, avoidance, and generalization) for exposure therapy outcomes in 112 spider-fearful women. Three aversive conditioning tasks were followed by a baseline assessment of spider fear, virtual reality exposure training, a post assessment after one week and a three-month follow-up assessment. Initial analyses indicated significant positive correlations between reduced extinction, increased avoidance of a conditioned threat stimulus, and overgeneralization, particularly evident in verbal measures. In subsequ

Biofeedback, cognitive-behavioral methods, and hypnosis in dermatology: is it all in your mind?

Biofeedback can improve cutaneous problems that have an autonomic nervous system component. Examples include biofeedback of galvanic skin resistance (GSR) for hyperhidrosis and biofeedback of skin temperature for Raynaud's disease. Hypnosis may enhance the effects obtained by biofeedback. Cognitive-behavioral methods may resolve dysfunctional thought patterns (cognitive) or actions (behavioral) that damage the skin or interfere with dermatologic therapy. Responsive diseases include acne excoriée, atopic dermatitis, factitious cheilitis, hyperhidrosis, lichen simplex chronicus, needle phobia, n

Startle responses in Duchenne muscular dystrophy: a novel biomarker of brain dystrophin deficiency.

Duchenne muscular dystrophy (DMD) is characterized by loss of dystrophin in muscle, however patients also have variable degree of intellectual disability and neurobehavioural co-morbidities. In contrast to muscle, in which a single full-length dystrophin isoform (Dp427) is produced, multiple isoforms are produced in the brain, and their deficiency accounts for the variability of CNS manifestations, with increased risk of comorbidities in patients carrying mutations affecting the 3' end of the gene, which disrupt expression of shorter Dp140 and Dp71 isoforms. A mouse model (mdx mouse) lacks Dp4

The PAP-NAP one decade later: patient risk factors, indications, and clinically relevant emotional and motivational influences on PAP use.

STUDY OBJECTIVES: Positive airway pressure (PAP) noncompliance remains a major barrier in obstructive sleep apnea (OSA) treatment. The PAP-NAP, a daytime PAP "test-drive," has been associated with increased PAP use, but risk factors and indications for the procedure remain unspecified. METHODS: A retrospective chart review was conducted on 139 patients who (1) were diagnosed with OSA (n = 116) or upper airway resistance syndrome (UARS) (n = 23); (2) adamantly refused to proceed with either a full-night attended titration or in-home trial of PAP; and (3) elected to proceed with a PAP-NAP inste

The Intricacies of the Histopathology of Eosinophilic Esophagitis: A Critical Review.

Eosinophilic esophagitis is a chronic, immune-mediated disorder that afflicts both adults and children. Dysphagia and food impaction are common symptoms in adults, whereas children experience failure to thrive, vomiting, and food aversion. On endoscopy, mucosal exudates, plaques, and furrows correlate with histologic inflammation, whereas rings and strictures are possibly related to a remodeled hyperplastic muscularis mucosae. The overreliance on intramucosal eosinophil counts for diagnosis and for the evaluation of response to therapy is a misguided oversimplification. A judicious analysis of

Metabolic state and value-based decision-making in acute and recovered female patients with anorexia nervosa.

BACKGROUND: Patients with anorexia nervosa forgo eating despite emaciation and severe health consequences. Such dysfunctional decision-making might be explained by an excessive level of self-control, alterations in homeostatic and hedonic regulation, or an interplay between these processes. We aimed to understand value-based decision-making in anorexia nervosa and its association with the gut hormone ghrelin. Besides its homeostatic function, ghrelin has been implicated in the hedonic regulation of appetite and reward via the modulation of phasic dopamine signalling. METHODS: In a cross-secti

A pilot study of closed-loop neuromodulation for treatment-resistant post-traumatic stress disorder.

The neurophysiological mechanisms in the human amygdala that underlie post-traumatic stress disorder (PTSD) remain poorly understood. In a first-of-its-kind pilot study, we recorded intracranial electroencephalographic data longitudinally (over one year) in two male individuals with amygdala electrodes implanted for the management of treatment-resistant PTSD (TR-PTSD) under clinical trial NCT04152993. To determine electrophysiological signatures related to emotionally aversive and clinically relevant states (trial primary endpoint), we characterized neural activity during unpleasant portions o

Distinct neuronal populations in the basolateral and central amygdala are activated with acute pain, conditioned fear, and fear-conditioned analgesia.

Fear-conditioned analgesia (FCA) is modulated by brain areas involved in the descending inhibitory pain pathway such as the basolateral (BLA) and central amygdala (CEA). The BLA contains Ca2+/calmodulin-dependent protein kinase II (CaMKII) and parvalbumin (PV) neurons. CEA neurons are primarily inhibitory (GABAergic) that comprise enkephalin (ENK) interneurons and corticotropin-releasing factor (CRF) - neurons that project to the periaqueductal grey. The purpose of our experiment was to determine the pattern of activation of CaMKII/PV and ENK/CRF neurons following the expression of acute pain,

Prolonged Suppression of Neuropathic Hypersensitivity upon Neurostimulation of the Posterior Insula in Mice.

Neurostimulation-based therapeutic approaches are emerging as alternatives to pharmacological drugs, but need further development to optimize efficacy and reduce variability. Despite its key relevance to pain, the insular cortex has not been explored in cortical neurostimulation approaches. Here, we developed an approach to perform repetitive transcranial direct current stimulation of the posterior insula (PI tDCS) and studied its impact on sensory and aversive components of neuropathic pain and pain-related anxiety and the underlying neural circuitry in mice using behavioral methods, pharmaco

Extinction and beyond: an expanded framework for exposure and response prevention for obsessive-compulsive disorder.

Exposure therapy is a first-line, empirically validated treatment for anxiety, obsessive-compulsive, and trauma-related disorders. Extinction learning is the predominant theoretical framework for exposure therapy, whereby repeated disconfirmation of a feared outcome yields fear reduction over time. Although this framework has strong empirical support and substantial translational utility, extinction learning is unlikely to be the sole process underlying the therapeutic effects of exposure therapy. In our clinic, we commonly treat obsessive-compulsive disorder (OCD) patients successfully with e

Chronic incretin-based therapy in cystic fibrosis-related diabetes: A tale of 3 patients treated with sitagliptin for over 5 years.

Cystic fibrosis-related diabetes (CFRD) affects 40-50% of adult patients with cystic fibrosis. Insulin therapy is recommended but there are therapeutic challenges, particularly risk of hypoglycemia and aversion of some patients to injectables. An oral incretin-based therapy using a DPP-4i (dipeptidyl peptidase-4 inhibitor), may be a reasonable option, especially in the early stage of the disease. The effect of chronic incretin-based therapy on CFRD is unknown. Here is a report of 3 cases of CFRD patients treated with sitagliptin and the response to therapy over a period of 5-10 years. An effec

[Female sexuality and its disorders from the psychological viewpoint].

Female sexuality is determined to a far greater extent by the woman's emotional receptiveness to sexual feelings than by her physiological reactions. Concerning sexual symptoms, female patients suffer much more from a lack of sexual desire and from a deficit in sexually stimulating feelings and sexual pleasure than from insufficient genital functions. The following sexual symptoms are presented: sexual arousal disorders, orgasm disorders, vaginismus, dyspareunia, sexual desire disorders, sexual aversion/sexual phobia. Sex consultation and sex therapy are recommended as preferential treatment o

Gastrointestinal motility and sensory abnormalities may contribute to food refusal in medically fragile toddlers.

BACKGROUND: In chronically ill children who refuse to eat, surgery to correct anatomic problems and behavioral treatments to overcome oral aversion often succeed. A few patients fail with standard treatments. The aims of the study were to: 1) investigate motility and gastric sensory abnormalities and 2) describe treatment that was individualized based on pathophysiology in children who failed surgery and behavioral treatments. METHODS: We studied 14 patients (age 1.5-6; mean 2.5; M/F: 7/7). All had a lifelong history of food aversion and retching or vomiting persisting after feeding therapy a

Priapism in teenage boys following depot testosterone.

Priapism is rare in children and may result in erectile dysfunction and sexual aversion behaviours. Testosterone therapy is commonly regarded as safe in children and is widely used in constitutional delay of growth and puberty, hypogonadism, hypospadias and micropenis. We report two cases of priapism in teenage boys with constitutional delay of growth and puberty after a change in the formulation of depot testosterone. One case required surgical intervention and the other was preceded by stuttering priapism. These cases illustrate the importance of patient and/or parent counselling before test

One receptor, two opposite approaches: efficacy and tolerability of GIPR agonism and antagonism in obesity pharmacotherapy.

Glucagon-like peptide-1 receptor (GLP-1R) agonists have transformed obesity pharmacotherapy, producing clinically meaningful weight loss and robust improvements in glycemic control. Yet their efficacy remains constrained by dose-limiting gastrointestinal adverse effects, including nausea and vomiting, that reduce adherence and limit escalation to maximally effective doses. Thus, the next-generation of obesity therapeutics must not only enhance weight loss but also expand the therapeutic window by dissociating metabolic efficacy from aversive side effects. The recent clinical success of dual GL

[Extinction of emotional response as a novel approach of pharmacotherapy of anxiety disorders].

Studies on the neurobiological background of anxiety indicate that the patogenesis of anxiety may be related to the process of an extinction of aversive memories. It has been suggested that disruption of selective attention for emotional stimuli may confer the risk for mental disorders, such as phobias and post-traumatic stress disorder, PTSD. Differences in the effects of local neuronal and hormonal activities of the HPA axis on emotional memory formation, might underlie individual differences in the emotional reactivity. Studies on molecular and cellular mechanisms responsible for individual

Protective role of hemokinin-1 against age-dependent spatial memory and attention decline in mice.

Age-related cognitive decline is a major social and medical challenge, but its mechanisms remain poorly understood; prevention and therapy are unsatisfactory. The tachykinin hemokinin-1 (HK-1), encoded by the Tac4 gene, is expressed in brain regions involved in learning and memory and has been implicated in neuroimmune regulation, but its contribution to age-dependent cognitive impairment remains unclear. Here, we investigated whether endogenous HK-1 modulates cognitive performance during aging. Young (3-5 months), middle-aged (12-15 months), and old (18-20 months) male and female C57BL/6 J wi

Aripiprazole relieves motivational anhedonia in rats.

BACKGROUND: Anhedonia is considered a relevant feature in depression and psychosis, characterized by poor treatment outcome, and associated with deficits in mesolimbic dopaminergic responsiveness. Clinical studies suggest the potential utility of aripiprazole as adjunctive therapy for resistant depression. Since aripiprazole can stabilize the dopaminergic system, in search of tailored therapeutic strategies for reward dysfunctions, we investigated whether the drug restored motivation toward positive stimuli in a rat model. METHODS: Anhedonia is modeled in non food-deprived 9-week old male Spr

Contextual fear cues inhibit eating in food-deprived male and female rats.

Previously we have shown that food-deprived male and female rats inhibit food consumption when presented with a discrete conditioned stimulus that signals danger. Here, in a series of three experiments, we examined whether contextual conditioned stimuli can exert the same effect. Experiment 1 paired a distinct context with footshocks, to examine food intake in food-deprived rats upon re-exposure to the context. Experiment 2 used a discrimination protocol with two alternating contexts; rats were given food in one, and received footshocks in the other. This protocol allowed us to monitor food co
