Spasticity after spinal cord injury (SCI) encompasses increased muscle tone, exaggerated tendon reflexes, muscle spasms, and involuntary rhythmic contractions (clonus) elicited by rapid muscle stretch. It develops in about two thirds of all patients within the first-year post-injury and often causes severe functional impairments. In rodent models, however, post-SCI spasticity period is frequently subtle or overlooked due to overlapping with central neuropathic pain symptoms such as reduced locomotor activity and paresis. Here we describe a simple and reproducible method to reveal latent spasti

There are currently no therapies for the staggering disability and public health costs of chronic low back pain (LBP). Innervation of the degenerating intervertebral disc (IVD) is suspected to cause discogenic LBP, but the mechanisms that orchestrate the IVD's neo-innervation and subsequent symptoms of LBP remain unknown. We hypothesize that Vascular Endothelial Growth Factor-A (VEGFA) critically mediates the neurite invasion in the IVD and contributes to prolonged LBP. Initiating IVD degeneration through a mechanical injury, we evaluated the progression of neurovascular features into the IVD,

Meckel's diverticulum (MD) is a frequently silent congenital anomaly of the gastrointestinal tract. While complications such as bleeding and obstruction are more common, traumatic perforation remains extremely rare, particularly from penetrating injuries. To date, isolated perforation of MD following self-inflicted abdominal stab wounds has been reported only sporadically. We present the case of a 39-year-old male with a long-standing history of schizophrenia and epilepsy who arrived at the emergency department following his fifth self-inflicted abdominal stab injury over the past decade. Cognitive impairments have frequently been reported in individuals with spinal cord injury (SCI) across different domains such as working memory, attention, and executive function. The mechanism of cognitive impairment after SCI is not well understood due to the heterogeneity of SCI sample populations, and may possibly be due to factors such as cardiovascular dysfunction, concomitant traumatic brain injury (TBI), hypoxia, sleep disorders, and body temperature dysregulation. In this study, we implement the Neuropsychiatric Unit Cognitive Assessment Tool (NUCOG) to assess cognitive differences b

Developing a greater understanding of the social and environmental factors that are related to differential outcomes for individuals who experience persistent pain and disability is important for achieving health equity. In this study, we aimed to develop insights into the role of the social determinants of health (SDH) in care experiences and health status for socio-economically disadvantaged adults who experience persistent low back pain or persistent pain following spinal cord injury. Our objectives were to investigate 1) relationships between the SDH and health outcomes, 2) care experience

Abdominal oblique muscle injury is characterized by acute pain and localized tenderness over the lateral trunk. This injury is particularly common among throwing athletes, and usually presents as anterolateral abdominal wall pain. Imaging evidence is scarce in regard to whether oblique muscle injury at its junction with the thoracolumbar fascia can instead present with low back pain. A high school baseball player with unilateral low back pain was referred to us with a different diagnosis. Careful palpation and magnetic resonance imaging guided our care, and the patient returned to high-level c

BACKGROUNDS AND OBJECTIVE: Spinal cord injury remains to be a challenge to clinicians and it is attractive to employ autologous adult stem cell transplantation in its treatment, however, how to harvest cells with therapeutic potential easily and how to get enough number of cells for transplantation are challenging issues. In the present study, we aimed to isolate skin-derived precursors (SKPs) and dermal multipotent stem cells (dMSCs) simultaneously from single human skin samples from patients with paraplegia.
METHODS: Dissociated cells were initially generated from the dermal layer of skin s

DESCRIPTION: The American College of Physicians (ACP) and American Academy of Family Physicians (AAFP) developed this guideline to provide clinical recommendations on nonpharmacologic and pharmacologic management of acute pain from non-low back, musculoskeletal injuries in adults in the outpatient setting. The guidance is based on current best available evidence about benefits and harms, taken in the context of costs and patient values and preferences. This guideline does not address noninvasive treatment of low back pain, which is covered by a separate ACP guideline that has also been endorse

Accumulating evidence suggests that spinal intervertebral disk damage is associated with activation of the adaptive immune system. Key unknown features of this response are the mechanism and time course for the generation of pronociceptive antibodies and the antibody classes involved. This study used the lumbar spinal disk puncture (DP) model in male wildtype (WT) and muMT mice (lacking mature B cells) to evaluate pronociceptive immune responses. Sera collected up to 33 weeks after DP were purified for immunoglobulin (Ig)M and IgG intrathecal injections and antigen binding experiments. Spinal

STUDY DESIGN: Longitudinal case-controlled animal study.
OBJECTIVE: To investigate putative cellular mechanisms to explain structural changes in muscle and adipose and connective tissues of the back muscles after intervertebral disc (IVD) injury.
SUMMARY OF BACKGROUND DATA: Structural back muscle changes are ubiquitous with back pain/injury and considered relevant for outcome, but their exact nature, time course, and cellular mechanisms remain elusive. We used an animal model that produces phenotypic back muscle changes after IVD injury to study these issues at the cellular/molecular level.

BACKGROUND: There is growing awareness that back pain in adolescent soccer (European football) players is not always related to local tissue damage. Approaches taking into consideration lifestyle factors are needed. This study aimed to investigate the association between psychological distress, skipped meals, and insufficient sleep, and the occurrence of back pain among adolescent female soccer players.
HYPOTHESIS: Psychological distress, skipped meals, and insufficient sleep are associated with the occurrence of back pain.
STUDY DESIGN: Cohort study.
LEVEL OF EVIDENCE: Level 3.

BACKGROUND: Spinal cord stimulation (SCS) is a minimally invasive neuromodulation treatment modality primarily indicated for failed back surgery syndrome (FBSS). When FBSS occurs in the setting of spinal cord injury (SCI) it can often be refractory to treatment with opioids and anticonvulsants; in such cases, SCS has demonstrated promising results. Here, we present a case series of 2 patients with FBSS in the setting of SCI who received pain relief with SCS therapy.
CASE REPORT: Our first patient is a 49-year-old man with a history of SCI (T10-11) with resulting paraplegia, who presented afte

This study investigated the psycho-social factors among injured workers and the influence of their psycho-social profile on the readiness of return to work. Sixty-four subjects with injuries on the upper limbs or lower limbs, low back pain, or cumulative trauma disorder were recruited. The workers' profiles (self-perceived physical health, mental health, and work readiness) together with the impact and types of work injury on the workers' perceived wellness were measured. The regression model of work readiness was constructed to develop a theoretical background for predicting work readiness ba

OBJECTIVE: To analyze trends in low back pain presentations to US emergency departments (EDs) and identify associated risk factors and barriers to care.
STUDY DESIGN: Retrospective analysis using data from the National Electronic Injury Surveillance System (NEISS) from 2014 to 2023.
SUMMARY OF BACKGROUND DATA: Low back pain is one of the most common reasons for seeking medical care in the United States. Post-COVID-19, many older adults seem to defer care. Understanding trends in low back pain incidence can highlight potential improvements in prevention and gaps in health care access.
