Bacterial Infections of the Central Nervous System.

Bacterial infections of the central nervous system (CNS) continue to be an important cause of morbidity and mortality in children. The spectrum of bacterial infection of CNS includes; focal or multifocal infections like brain abscesses or subdural empyema; or more generalized or diffuse infections like pyogenic meningitis or ventriculitis. Focal and generalized infections may co-exist in an individual patient. Prompt and adequate antibiotic therapy and occasionally neurosurgical interventions are the cornerstone of effective management. The recent emergence of several multidrug-resistant bacte

Management of bacterial central nervous system infections.

Bacterial infections of the central nervous system present as a medical emergency, thus requiring rapid diagnosis and immediate treatment. The most prevalent bacterial infections seen in the intensive care unit can be summarized as acute bacterial meningitis, subdural empyema, intracerebral abscess, and ventriculitis, which all commonly involve the brain parenchyma. The infections can either be community-acquired or hospital-acquired, e.g., after neurosurgical intervention, as a complication of severe neurotrauma or related to indwelling cerebrospinal fluid drains. Community-acquired bacterial

Empiric Antibiotic Therapy of Nosocomial Bacterial Infections.

Broad-spectrum antibiotics are commonly used by physicians to treat various infections. The source of infection and causative organisms are not always apparent during the initial evaluation of the patient, and antibiotics are often given empirically to patients with suspected sepsis. Fear of attempting cephalosporins and carbapenems in penicillin-allergic septic patients may result in significant decrease in the spectrum of antimicrobial coverage. Empiric antibiotic therapy should sufficiently cover all the suspected pathogens, guided by the bacteriologic susceptibilities of the medical center

Bacterial Infections of the Central Nervous System.

PURPOSE OF REVIEW: Bacterial infections of the central nervous system are neurologic emergencies. Prompt recognition and treatment are essential not only to prevent mortality, but also to decrease neurologic sequelae. This article focuses on the two most common central nervous system bacterial infections, bacterial meningitis and spinal epidural abscess. RECENT FINDINGS: Two outbreaks of serogroup B meningococcal disease have occurred on US college campuses. The meningococcal vaccine given to young adults does not contain serogroup B. SUMMARY: In bacterial meningitis and in bacterial spinal

Combating multidrug-resistant Gram-negative bacterial infections.

INTRODUCTION: Multidrug-resistant (MDR) bacterial infections, especially those caused by Gram-negative pathogens, have emerged as one of the world's greatest health threats. The development of novel antibiotics to treat MDR Gram-negative bacteria has, however, stagnated over the last half century. AREAS COVERED: This review provides an overview of recent R&D activities in the search for novel antibiotics against MDR Gram-negatives. It provides emphasis in three key areas. First, the article looks at new analogs of existing antibiotic molecules such as β-lactams, tetracyclines, and aminoglycos

The changing panorama of bacterial enteric infections.

We studied the age-specific population-based incidence of bacterial enteric infections caused by Shigella, Salmonella and Campylobacter, in Jerusalem. During 1990-2008, 32,408 cases were reported (incidence rate 232.1/100,000 per annum). The patterns of Shigella (47.4% of cases), Salmonella (34.4%) and Campylobacter (18.2%) infections evolved noticeably. Campylobacter rates increased from 15.0 to 110.8/100,000 per annum. Salmonella rates increased from 74.2 to 199.6/100,000 in 1995 then decreased to 39.4/100,000. Shigella showed an endemic/epidemic pattern ranging between 19.7 and 252.8/100,00

Molecular diagnostics of medically important bacterial infections.

Infectious diseases are common diseases all over the world. A recent World Health Organization report indicated that infectious diseases are now the world's biggest killer of children and young adults. Infectious diseases in non-industrialized countries caused 45% in all and 63% of death in early childhood. In developed countries, the emergence of new, rare or already-forgotten infectious diseases, such as HIV/AIDS, Lyme disease and tuberculosis, has stimulated public interest and inspired commitments to surveillance and control. Recently, it is reported that infectious diseases are responsibl

Bacterial infections of pulp and periodontal origin.

The anatomical and structural characteristics of the pulp make this structure prone to altering as a result of, for instance, periodontal conditions (proximity), iatrogenic alterations, infections and involvement of vascular and nerve structures (it is surrounded by hard tissues that prevent expansion), to name just a few. Pulpitis is a process that courses with pain of varying intensity that allows us to determine the location of the lesion in clinical terms. Its evolution varies and may even progress to pulpar necrosis that in turn, produces neuritis-like pain. Diagnosis is established by me

[Impact of bacterial resistance on severe infections].

Since many years, the antimicrobial resistance increases as well as for community-acquired as for nosocomial infections. Antibiotic-resistant pneumococci are neither more nor less virulent susceptible strains. Except for immunocompromised patients, the outcome of penicillin-resistant pneumococcal infections have been similar to those in patients who are infected by susceptible ones. Current levels of S. pneumoniae resistance to penicillin and cephalosporin are not associated to an increase in mortality in children with meningitis if adequate doses of antibiotics are given. Because empiric ther

Childhood visceral leishmaniasis complicated by bacterial infections.

Bacterial superinfection is one of the major complications leading to death in patients with visceral leishmaniasis. We studied the frequency and type of bacterial infection in 54 patients admitted to hospital with visceral leishmaniasis. The patients were children who ranged in age from 3 1/2 months to 7 years. Bacterial infections were found in 22 (41%) of the patients. Bacteria were isolated in patients with pneumonia, septicaemia, otitis media, urinary tract infections and skin infections. Enterobacteriaceae were the most common bacterial agents isolated. In infants with visceral leishmani

Canine and feline primary ocular bacterial infections.

Bacterial infections are frequently associated with diseases of the eyelids, cornea, and conjunctiva. Animals sustaining KCS commonly have bacterial infections of the external eye owing to a lack of antimicrobial properties present in the normal tearfilm. Infection can occur in the nasolacrimal duct or lacrimal sac, which is referred to as dacryocystitis. Severe corneal ulcers are frequently infected with bacteria, especially Pseudomonas sp. Three new topical ophthalmic antibiotics have recently become commercially available: ciprofloxacin, norfloxacin, and ofloxacin.

Bacterial and fungal brain infections in AIDS.

Many bacterial and fungal pathogens can infect the central nervous system (CNS) in patients also infected with human immunodeficiency virus (HIV). Most of these organisms cause meningitis, and this is sometimes accompanied by focal parenchymal infection. More virulent pathogens, such as Mycobacterium tuberculosis hominis, can cause disease in individuals with mild degrees of HIV-associated immunosuppression. Less virulent pathogens, such as Cryptococcus neoformans, tend to affect individuals with more advanced immunosuppression. This review focuses on the clinical manifestations, diagnosis, an

[Bacterial infections in felines in zoological gardens].

The significance of selected bacterial infections of felines in zoological gardens is discussed. Anthrax and malleus are in Germany of historical interest only, tuberculosis went down very strongly. Isolated cases of pseudotuberculosis must be expected in future. Infections with bacteria of the genera Chlamydia, Pasteurella and of the family Enterobacteriaceae are also considered. The significance of Clostridium perfringens in the so called tiger disease is discussed.

[Intractable bacterial infections in the nervous system].

It is not exaggeration to say that infectious meningitis is most often seen among infections of the nervous system caused by various microorganisms, in which viruses are main causative pathogens. Infectious meningitis due to bacteria includes purulent one with acute progress, tuberculous and fungal ones which are representative for subacute meningitis. They can not be easily treated. As brain abscess is similar to them, diagnosis and chemotherapy on these four diseases are briefly summarized here. It is needless to say that the prognosis is influenced by early diagnosis and how fast appropriat

[Conjugate vaccines against bacterial infections: typhoid fever].

Capsular polysaccharides have been studied as possible vaccines against infectious diseases. However, they are capable to induce only short-run protection because of their T-independent properties and they would not be protective against infection in high-risk populations. The alternative to face this problem is to develop methods to join covalently the polysaccharide and proteins to both increase the immunogenicity of and to confer the property of T-dependence to this antigen. In order to obtain a conjugate vaccine against typhoid fever, in our laboratory we have tried to synthesize a conjuga

Bacterial infections in suspected cutaneous leishmaniasis lesions.

In Iran, microscopic examination of skin scrapings from 2202 individuals with clinically diagnosed cutaneous leishmaniasis (CL) lesions revealed the presence of amastigotes in 1123 cases (51.0%). Bacteriological examinations of the lesions indicated that 788 individuals (35.7%) were also infected with one or more pathogenic bacteria, including coagulase-positive staphylococci (27.8%), beta-haemolytic streptococci (10.6%), and other opportunist pathogenic bacteria (total, 2.5%). The prevalence of bacterial infections in lesions in which leishmania parasites were detected was 26.5%, while for le

[Bacterial infections and early Plasmodium falciparum malaria].

Two cases of Plasmodium falciparum malaria associated with bacterial infections--streptococcus A septicemia and Legionnaires' disease--are described. The association of these two infections is probably not incidental, and the hypothesis of Plasmodium falciparum induced immuno-deficiency is discussed.

Secondary bacterial infections in herpes simplex keratitis.

In nine patients who had herpetic corneal disease, secondary bacterial infections developed. Five of the patients had the ulcers in their own cornea, whereas four had them in corneal transplants in which the graft had been performed because of herpes. All of the patients with grafts were receiving topical steroids when the ulcer developed. Four of the eyes had active herpetic disease at the time of onset, four had no known active disease, and in one it was unknown. The infections respond well to topical treatment except in eyes that had been grafted because of herpes. Staphylococcus was the mo
