Bladder versus renal bacteriuria during pregnancy: recurrence after treatment.

Localization of bacteriuria has been shown to correlate with the pattern of recurrence after treatment. The immunofluorescent technique was used to localize infection in 233 pregnant women with asymptomatic bacteriuria to determine whether this would identify those who were at greater risk for recurrence. In both an indigent and a military population, the incidence of renal bacteriuria was 42%. Regardless of the site of infection, after one course of short-term (10-day) or long-term (21-day) antimicrobial therapy, almost two thirds of these women were abacteriuric for the remainder of gestatio

Advances in the Treatment of Urinary Tract Infection and Bacteriuria in Pregnancy.

Understanding the management of asymptomatic bacteriuria (ASB) and urinary tract infection (UTI) during pregnancy is important, given the prevalence of these conditions, associated risks to the mother and fetus /newborn, and potential requirement for antibiotic treatment. Screening for ASB with a urine culture is required during pregnancy, and a positive culture is treated for 5 to 7 days with culture-specific antibiotics. Recent research on non-antibiotic prevention and shortened duration of antibiotic treatment may offer a slightly different approach to ASB and UTI in pregnancy.

Outcome of febrile neutropenic patients treated for bacteriuria in hematology.

INTRODUCTION: Positive urine sample is a frequent finding in post-chemotherapy febrile neutropenia (FN) and can lead to prolonged antibiotic therapy. The aim of this study was to assess the outcome of bacteriuria episodes in FN patients receiving targeted antibiotic therapy. MATERIALS AND METHODS: A multi-centric retrospective study was conducted over a four-year period (2014-2019) on systematic urinalysis. All consecutive first bacteriuria episodes (≤ 2 bacteria with at least ≥ 103 CFU/mL) during FN in hospitalized adult patients for hematological malignancies were included. Relapse and recu

ASYMPTOMATIC BACTERIURIA AS A PREDICTOR OF PRE-ECLAMPSIA: A CASECONTROLLED STUDY.

BACKGROUND: Pre-eclampsia, an important cause of maternal and perinatal morbidity and mortality world-wide has been linked to subclinical infections, with maternal infection and inflammation postulated in its aetio-pathogenesis including asymptomatic bacteriuria which is common in pregnancy. The Obejctive of the study is to determine the relationship of asymptomatic bacteriuria as a risk factor for pre-eclampsia. METHODOLOGY: A hospital-based case-control study among 28 pre-eclamptic pregnant women (cases) and 56 healthy pregnant women (controls) at gestational age of at least 28 weeks at the

Clinico-microbiological Profile and Outcomes of Asymptomatic Bacteriuria in Pregnancy.

BACKGROUND: Asymptomatic bacteriuria (ASB) during pregnancy can lead to symptomatic urinary tract infection (UTI), with increased fetal and maternal morbidity and mortality. We evaluated the incidence, clinical and microbiological profile, and outcome of ASB in pregnant women attending our antenatal clinic. MATERIALS AND METHODS: This prospective study was conducted on 3769 pregnant women in a routine antenatal clinic at a tertiary care center. Participants were divided into two groups, ASB and non-bacteriuria. Data were collected in a standard proforma and analyzed using the software Statist

Antibiotic use for asymptomatic bacteriuria in children with neurogenic bladder.

PURPOSE: Patients with neurogenic bladder (NB) often perform clean intermittent catheterization (CIC) and are predisposed to bladder colonization. Antibiotics are not routinely indicated in those with asymptomatic bacteriuria (ASB). The original purpose of this study was to compare patients that received antibiotics for ASB and those that did not. However, because the non-antibiotic group was very small, the final analysis evaluated treatment patterns of ASB in children with NB. METHODS: A retrospective chart review was completed, including patients who presented with urinary tract infection

Is asymptomatic bacteriuria a risk factor for prosthetic joint infection?

BACKGROUND: Infection is a major complication after total joint arthroplasty. The urinary tract is a possible source of surgical site contamination, but the role of asymptomatic bacteriuria (ASB) before elective surgery and the subsequent risk of infection is poorly understood. METHODS: Candidates for total hip or total knee arthroplasty were reviewed in a multicenter cohort study. A urine sample was cultured in all patients, and those with ASB were identified. Preoperative antibiotic treatment was decided on an individual basis, and it was not mandatory or randomized. The primary outcome was

Not all nosocomial Escherichia coli bacteriurias are catheter associated.

We prospectively determined what proportion of nosocomial E. coli bacteriurias are associated with urinary catheters. Only 46% (95% CI 37–56%) of nosocomial E. coli bacteriurias were catheter-associated. Compared to bacteriuric patients with catheters, non-catheterized patients were less likely to be male and have renal insufficiency or a recent urogenital procedure.

Delayed Salmonella bacteriuria in a patient infected with Schistosoma haematobium.

The authors report a case of Schistosoma haematobium infection with delayed occurrence of Salmonella bacteriuria following treatment of schistosomiasis. Standard models of interaction between these two pathogens may not be fully satisfactory in such a case of co-infection. The role played by a decreased host immune response following schistosomiasis may thus be highlighted to explain a delayed or prolonged infection with Salmonella.

Bacterial adhesion as an indicator of renal involvement in bacteriuria of pregnancy.

The association of bacterial virulence with the host response to bacteriuria was evaluated in 70 pregnant women with acute pyelonephritis or bacteriuria detected at screening. Patients infected with Escherichia coli attaching to Gal alpha 1----4Gal beta-containing receptors, had significantly higher levels of C-reactive protein and lower renal concentrating capacity than patients infected with strains lacking this specificity. The renal concentrating capacity ranged from 419-1151 mOsm/kg in the women with bacteriuria on screening. 5/11 women with a renal concentrating capacity less than or equ

Urethral meatal colonization in the pathogenesis of catheter-associated bacteriuria.

Urethral meatal colonization with potential urinary tract pathogens was studied before bacteriuria in 31 consecutive acute spinal cord injury patients on an open ward (137 cultures) and 18 renal homotransplant patients in reverse isolation (80 cultures). All patients required indwelling urethral catheterization and no antimicrobials were given. Daily quantitative cultures of the urethral meatus, and bladder and drainage bag urine were obtained. The urethra was the source for 11 of 31 episodes of bacteriuria (35 per cent). Urethral meatal colonization with the organism responsible for bacteriur

Significant bacteriuria in the puerperium: a prospective study of the risk factors.

The rate of significant bacteriuria in the puerperium was studied prospectively in 3554 women who were delivered at the State Maternity Hospital. The influence of catheterisation during labour and before Caesarean section, urinary tract infection during pregnancy, puerperal endometritis and impaired glucose metabolism on the rate of significant postpartum bacteriuria was studied. Catheterisation significantly increased the rate of significant bacteriuria in patients who were delivered vaginally. A single catheterisation before Caesarean section did not increase the incidence of significant bac

Bacteriuria in the puerperium: an evaluation of methods for collecting urine specimens.

Five hundred and eighteen puerperal patients were screened for bacteriuria by examination of clean-catch, midstream urine specimens using the dip-slide cultivation technique. Bladder urine from 105 patients showing 10(4) or more bacteria per milliliter of midstrean urine was obtained by suprapubic bladder aspiration and cultivated, along with two consecutive midstream specimens. The bacteriologic results in these specimen categories are compared. Of 23 patients showing growth in the midstream samples compatible with a true bacteriuria, 18 had sterile puncture urine specimens. Consecutive conta
