Antegrade balloon occlusion of inferior vena cava during thrombectomy for renal cell carcinoma.

Nephrectomy with inferior vena cava (IVC) thrombectomy for advanced renal cell carcinoma (RCC) is a challenging and morbid surgical case. We describe the use of a simple endoluminal technique to occlude the suprahepatic IVC during thrombectomy. A 60-year-old male presented with a large right-sided RCC and IVC tumour thrombus. The tip of the thrombus, which was non-adherent to the caval wall, extended to the level of the hepatic veins. After complete dissection of the kidney, we obtained suprahepatic control of the IVC by a large compliant balloon, introduced through the right internal jugular

Endovascular balloon occlusion for catheter-induced large artery perforation in the catheterization laboratory.

BACKGROUND: Vessel perforation is a complication that cannot be completely avoided in the setting of endovascular procedures. When a large noncompressible artery is disrupted, uncontrolled bleeding may lead to hemodynamic collapse. Endovascular occlusion may provide rapid control of hemorrhage and facilitate definitive therapy; yet, occlusion balloons are not commonly utilized in the cardiac catheterization laboratory. METHODS: We describe our experience with the use of dedicated occlusion balloons for the management of catheter-induced major bleeding events. Endovascular occlusion was utiliz

Elective use of aortic balloon occlusion in cesarean hysterectomy for placenta previa percreta.

BACKGROUND: Only few reports are available on the use of aortic balloon catheter for cesarean hysterectomy in placenta previa percreta. CASE: A 32-year-old woman with placenta previa percreta underwent cesarean hysterectomy at 34 weeks of gestation. Before starting the surgery, an aortic occlusion balloon catheter (30 mm balloon, 5 Fr) was inserted. For total hysterectomy, the aortic balloon catheter was inflated and there was a sudden and dramatic reduction in blood loss, and the surgery was completed safely. An aortic occlusion was sustained for 80 min, with blood loss estimated at 3,200 ml

Total laparoscopic iliac artery aneurysm repair using endoscopic techniques and endovascular balloon occlusion.

UNLABELLED: We present a novel total laparoscopic technique to treat patients with iliac and aorto iliac aneurysms. The laparoscopic procedure does not require clamping of the iliac arteries because of a hybrid approach. REPORT: Laparoscopic exposure of the aorta is performed using transperitoneal left retrorenal access. A transfemorally placed balloon catheter blocks the external iliac artery. Two haemostatic sheaths are inserted directly through the skin into the abdominal cavity. Balloons are passed through these sheaths to block the common iliac artery and the hypogastric artery, allowing

Temporary balloon occlusion and ethanol injection for preoperative embolization of carotid-body tumor.

We report on the preoperative embolization of a carotid-body paraganglioma by temporary balloon occlusion and ethanol injection. Complete devascularization was achieved without complication. Resection after a short postembolization interval required artery sacrifice. Histologic evaluation revealed that the tumor contained diffuse ethanol-induced microemboli. Compared with unembolized and polyvinyl-alcohol-embolized carotid-body paragangliomas, our technique resulted in no greater adverse effects on the tumor-vessel interface. This procedure is an effective and promising method of preoperative

Fetoscopic temporary tracheal occlusion by means of detachable balloon for congenital diaphragmatic hernia.

Occlusion of the fetal trachea blocks the egress of fetal lung fluid and stimulates the growth of hypoplastic lungs in fetuses with diaphragmatic hernia. Accomplishing temporary and reversible occlusion of the fetal trachea has proven difficult without invasive fetal surgery. Using simultaneous real-time ultrasonography and fetal bronchoscopy through a single uterine port, we placed a detachable balloon in the trachea of 2 fetuses with severe diaphragmatic hernia. In both fetuses the fetal lung subsequently enlarged, allowing survival after birth.

Combined percutaneous atrial septal defect occlusion and pulmonary balloon valvuloplasty in adult patients.

Severe pulmonary stenosis in association with a large atrial septal defect is uncommon. When these 2 conditions are present, significant left-to-right shunt is often prevented by the outflow obstruction, which protects the pulmonary bed until adulthood. This report shows our initial experience of percutaneous treatment of both congenital malformations, either staged or combined in the same procedure, in 2 adult patients whose treatments yielded effective atrial septal defect occlusion and right ventricular pressure relief that persisted at mid-term follow-up. Although these opposite procedures

[Intra-aortic balloon occlusion of the descending thoracic aorta for intra-abdominal hemorrhage].

Intra-aortic balloon occlusion (IABO) of the descending aorta is beneficial to rescue patients in shock following traumatic intra-abdominal hemorrhage. The authors experienced four such patients who had received IABO of the descending thoracic aorta to maintain hemodynamics prior to operation. After hemostasis, the balloon was deflated gradually. Releasing the balloon in the operating room was, however, abandoned in two patients because of severe deoxygenation and life threatening arrhythmia. In the other two patients, the balloon was successfully deflated during the operations; one of them di

[Open distal anastomosis or aortic balloon occlusion technique during complete aortic arch replacement].

The technique of open distal anastomosis or application of aortic balloon occlusion catheter designed to occlude the descending thoracic aorta have been used in 33 and 19 patients, respectively, to control bleeding during the procedure of distal anastomosis for complete aortic arch replacement with a prosthetic graft. These two techniques allowed us a simple approach to the lesion and the avoidance of clamp injury to the fragile aortic tissue. Open distal anastomosis was applied for 91% patients of operated aortic dissection and all emergent cases, it's duration ranged from 10 to 110 minutes w

Closure of short, wide patent ductus arteriosus with cardiopulmonary bypass and balloon occlusion.

The wide, short patent ductus arteriosus in adults and older adolescents poses an extreme hazard with standard closed ligation techniques. The method of transpulmonary balloon catheter occlusion and repair of pediatric ductus arteriosus is herein reported in older patients using a Foley catheter and normothermic bypass. Transesophageal echocardiography is crucial in assessing the size of the ductus and confirming adequacy of repair. The technique is simple and safe even in the presence of a wide, short ductus.

Computed tomography in a primate stroke model using selective balloon catheter arterial occlusion.

Ten heparinized nonhuman primates (rhesus monkeys and Papio maryumaya baboons) were subjected to temporary occlusion of an insular branch of the middle cerebral artery with a balloon catheter. An infarct was created. The lesions were monitored in vivo by computed tomography performed at intervals for 1 year. Animals were sacrificed and the lesions examined histologically. The technique obviates the disadvantages of surgical reaction, intravascular thrombosis, and gross cerebral edema observed in prior animal stroke models.

Evaluation of Magnetic Resonance Imaging Safety and Imaging Issues Associated with the Occlusion Balloon Used during Fetoscopic Endoluminal Tracheal Occlusion.

INTRODUCTION: Congenital diaphragmatic hernias can be successfully treated by fetoscopic tracheal occlusion (FETO), a minimally invasive procedure that may improve postnatal survival. The endoluminal balloon utilized for FETO contains a metallic component that may pose possible risks for the fetus and mother related to the use of magnetic resonance imaging (MRI). The objective of this study is to evaluate MRI-related imaging and safety issues (magnetic field interactions, heating, and artifacts) for the occlusion balloon used in FETO. MATERIALS AND METHODS: Using well-established techniques,

Successful Utilization of Resuscitative Endovascular Balloon Occlusion of the Aorta in a Pediatric Patient.

In the United States, pediatric trauma resulting in traumatic brain injury (TBI) and massive hemorrhage is the leading cause of death. Although use of Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA) continues to gain favor, limited data exists on use and efficacy in pediatric patients. We describe a case using REBOA in a pediatric patient with blunt abdominal injury causing hemorrhagic shock. A 14-year-old female presented via air to a level 1 trauma center post motor vehicle collision with prolonged extraction. At landing, she was hemodynamically unstable with GCS and vitals

Outcomes of balloon occlusion in the University of California Morbidly Adherent Placenta Registry.

BACKGROUND: Morbidly adherent placenta, also known as placenta accreta spectrum, is associated with severe maternal morbidity and mortality. Multiple adjunctive procedures have been proposed to improve outcomes, and at many institutions, interventional radiologists will play a role in assisting obstetricians in these cases. OBJECTIVE: The objective of the study was to evaluate the outcomes of women with morbidly adherent placenta who underwent cesarean hysterectomy with aortic balloon occlusion or internal iliac artery balloon occlusion catheters, compared with cesarean hysterectomy with surg

Nationwide Analysis of Resuscitative Endovascular Balloon Occlusion of the Aorta in Civilian Trauma.

IMPORTANCE: The need for improved methods of hemorrhage control and resuscitation has resulted in a reappraisal of resuscitative endovascular balloon occlusion of the aorta (REBOA). However, there is a paucity of data regarding the use of REBOA on a multi-institutional level in the United States. OBJECTIVE: To evaluate the outcomes in trauma patients after REBOA placement. DESIGN, SETTING, AND PARTICIPANTS: A case-control retrospective analysis was performed of the 2015-2016 American College of Surgeons Trauma Quality Improvement Program data set, a national multi-institutional database of t

Innovative technologies for SFA occlusions: drug coated balloons in SFA lesions.

The concept of using a balloon catheter to directly deliver an antiproliferative drug at the site of injury has become one of the most interesting technological developments in endovascular therapy. There have been important advances in knowledge concerning balloon-based drug delivery technologies during the last years, and different methods have been developed by different companies to coat the balloon with the antiproliferative agent. Currently there is a rapidly increasing clinical study program using drug coated balloons (DCB) in different locations and indications. There are four already

Bedside use of a dual aortic balloon occlusion for the treatment of cerebral vasospasm.

BACKGROUND: Delayed ischemic neurological deficits (DIND) due to cerebral vasospasm remains a major cause of morbidity and mortality following aneurysmal subarachnoid hemorrhage (aSAH). Methods to prevent DIND remain limited both in safety and efficacy. A novel intra-aortic dual balloon catheter (NeuroFlo™: CoAxia, Maple Grove, MN) is under investigation for treatment of ischemic stroke, including DIND. Because this technique does not require cerebral artery navigation, it may be useful as a bedside procedure, outside of the conventional angiography suite. We report the first case of ultrasoun
