BACKGROUND: While health care needs assessments have been conducted among juveniles or adolescents by researchers in developed countries, assessments using an ethics framework particularly in developing countries are lacking. We analysed the health care needs among adolescents at the Nakambala Correctional Institution in Zambia, using the Beauchamp and Childress ethics framework. The ethics approach facilitated analysis of moral injustices or dilemmas triggered by health care needs at the individual (adolescent) level.
METHODS: The research team utilized 35 in-depth interviews with juveniles,

Cesarean delivery on maternal request presents an ethical challenge in obstetrics. This comprehensive ethical examination of cesarean delivery on maternal request incorporates emerging evidence on both medical and psychological factors while providing a framework for balancing respect for patient autonomy with professional beneficence-based obligations to optimize maternal and neonatal outcomes. We explore key factors influencing cesarean delivery on maternal request decisions, including cognitive biases, clinical considerations, and the impact of physician self-interest. Major obstetric organ

The increasing incidence of requests for medical services to support gender transition for children, adolescents, and adults has consequences for society, governmental institutions, schools, families, health-care professionals, and, of course, patients. The sociological momentum to recognize and accommodate to trans phenomena has posed ethical dilemmas for endocrinologists, mental health professionals, and sexual specialists as they experience within themselves the clash between respect for patient autonomy, beneficence, nonmaleficence, and informed consent. The larger ethical clashes are cult

This case from Saudi Arabia describes a 27-year-old woman at 39 weeks and 6 days of gestation in spontaneous labor, who had had two prior cesarean deliveries and no previous vaginal births, but declined cesarean delivery despite worsening fetal heart tracings and labor dystocia. Repeated counseling was provided, and an ethics consultation was done as the intrapartum refusal persisted. After approximately 12 hours of labor and no cervical changes, she consented to cesarean delivery. The neonate was born with metabolic acidosis related to three tight nuchal cords. This case illustrates the ethic

This discussion paper is drawn from a qualitative research project comparing the effect of special and ordinary schools on the lives of children, young people and their families. Special schools are recommended by health professionals who seldom know how ineffective these schools are. We question the beneficence and justice of health professionals' advice on education for children with disabilities and other difficulties. Cooperation with local education authorities (LEAs) plays a considerable part in the work of community paediatricians, clinical medical officers, therapists and other health

The decision to initiate extended intensive care in older patients is often challenging. The ethical principles of beneficence and non-maleficence, together with the patient's autonomous will, form the foundation of decision-making. Important prognostic factors include frailty, functional status, and comorbidities. If intensive care treatment is initiated, it should be started promptly, but with a clearly defined treatment goal and predefined limitations regarding interventions that should not be undertaken. If, during the course, the decision is made against curative intensive care, palliativ

The purpose of this article was to discuss the ethical issues relevant to inviting family members to witness brainstem testing in the intensive care unit (ICU). To highlight these issues, we begin by discussing a hypothetical clinical case of a patient admitted to the ICU following an out-of-hospital cardiac arrest. A brief review of the literature on current practice will be presented, followed by a balanced discussion on the practice of permitting relatives to witness brainstem testing under the ethical principles of beneficence, non-maleficence, autonomy and justice. Finally, we will presen

In the United States, women are over-represented and Blacks are under-represented as living kidney donors. A traditional bioethics approach would state that as long as living donors believe that the benefits of participation outweigh the risks and harms (beneficence) and they give a voluntary and informed consent, then the demographics reflect a mere difference in preferences. Such an analysis, however, ignores the social, economic and cultural determinants as well as various forms of structural discrimination (e.g., racism, sexism) that may imply that the distribution is less voluntary than m

Ethical dilemmas are becoming increasingly common in dentistry and periodontics. Clinicians, challenged with such dilemmas, may not know how to apply the appropriate moral reasoning needed to resolve these situations especially when any of the five fundamental principles of ethics that form the foundation of the American Dental Association Principles of Ethics and Code of Professional Conduct--patient autonomy, nonmaleficence, beneficence, justice, and veracity--come into conflict with each other. The author describes one clinical case that presented with an ethical dilemma. An analytic framew

Women now comprise the majority of dermatology residents, and residency training coincides with a childbearing timeline that residents do not control. Federal legal and national accreditation frameworks establish meaningful minimum standards for lactation accommodations. Yet compliance with these standards does not guarantee that accommodations function in practice. The central ethical failure is not scheduling conflicts that inevitably arise and interfere with the usage of such accommodations, but the absence of a functional process through which a resident can flag that an accommodation is n

Surgical techniques for correcting cleft lip and/or cleft palate (CL/P) are highly advanced, and global humanitarian organizations are increasingly expanding access for children in low- and middle-income countries (LMICs). However, these children and their families have minimal access to psychosocial care necessary for coping with surgeries and the secondary effects of CL/P, likely compromising health and quality of life outcomes. This article highlights one nongovernmental organization, Operation Smile's psychosocial care advocacy policy and practices, which integrate the principles of child

Artificial intelligence (AI) has emerged in orthopedics with the potential to improve diagnostic accuracy, optimize surgical workflows, and support personalized care. We conducted a narrative review exploring the bioethical considerations of AI use in the orthopedic clinical setting, focusing on 4 core principles-autonomy, beneficence, nonmaleficence, and justice-to provide orthopedists with a practical framework for AI's implementation. We utilized the Preferred Reporting Items for Systematic Reviews and Meta-Analyses framework to conduct a comprehensive PubMed search; 89 articles were evalua

Anaesthesia waste gases (AWGs) represent a significant yet often overlooked contributor to healthcare's environmental footprint, with implications extending beyond ecological concerns to encompass occupational hazards and complex ethical considerations. This article examines the intersection of AWGs and bioethics through the framework of environmental stewardship, occupational health principles, patient-centered care, and distributive justice. Contemporary volatile anaesthetics possess global warming potentials thousands of times greater than carbon dioxide, while implicated occupational expos

The practice of patient optimization seems to be straightforward, logical, and not controversial, but ethical issues may arise. This most commonly occurs when the desire for risk reduction competes with concerns regarding access to care. There is no uniformity in how patient optimization is defined or achieved, and thus the outcomes of optimization programs are highly variable. The ethical tenets of beneficence, nonmaleficence, autonomy, and justice can help to guide decision making. The framework proposed helps the surgeon to consider a potentially modifiable risk factor that involves three q

AIM: (1) To determine the repartition of criteria which can be considered as marks of lack of assent by the child with intellectual disabilities from the dentist's point of view and whether that influences the decision to examine the patient or not. (2) To explain the decision of practitioners and determine the ethical implications of these practices.
METHODS: An anonymous and structured questionnaire was distributed online using the scenario of a 9-year-old child with moderate cognitive impairment with eight different oppositional behaviours. The practitioners were asked about their percepti

In medicine, it is the physician's obligation to promote and protect the patient's interest. In obstetrics, the ethical principles of beneficence and autonomy provide the fundamental framework which guides the management of all pregnant patients. As there is the need for consideration of the fetus, autonomy can become a complex issue giving rise to what is sometimes called "maternal-fetal conflict." In this paper, we aim to discuss some scenarios we encounter in our day-to-day obstetric practice such as pre-eclampsia, fetal growth restriction and labour induction when the best interests of the

John Harris has previously proposed that there is a moral duty to participate in scientific research. This concept has recently been challenged by Iain Brassington, who asserts that the principles cited by Harris in support of the duty to research fail to establish its existence. In this paper we address these criticisms and provide new arguments for the existence of a moral obligation to research participation. This obligation, we argue, arises from two separate but related principles. The principle of fairness obliges us to support the social institutions which sustain us, of which research
