Slow-transit constipation: evaluation and treatment.

Slow-transit constipation is characterized by delay in transit of stool through the colon, caused by either myopathy or neuropathy. The severity of constipation is highly variable, but may be severe enough to result in complete cessation of spontaneous bowel motions. Diagnostic tests to assess colonic transit include radiopaque marker or radioisotope studies, and intraluminal tests (colonic and small bowel manometry). Most patients with functional constipation respond to laxatives, but a small proportion are resistant to this treatment. In some patients biofeedback is helpful although the mech

Use of stabilometric platform and visual feedback in rehabilitation of patients after the brain injury.

Rehabilitation of patients after the brain injury requires employing of all available mechanisms of neuroplasticity. To achieve it, the voluntary activation of brain systems that are involved in the signal processing, represents the most effective tool. The control of balance is a complex neuronal mechanism based on unconditioned and conditioned reflexes, as well as on the actual cognitive processes. As it requires participation of several brain regions, training of the posture support mechanisms can provide a highly effective tool for rehabilitation. The aim of the study was to develop method

Biofeedback and/or sphincter exercises for the treatment of faecal incontinence in adults.

BACKGROUND: Faecal incontinence is a particularly embarrassing and distressing condition with significant medical, social and economic implications. Sphincter exercises and biofeedback therapy have been used to treat the symptoms of people with faecal incontinence. However, standards of treatment are still lacking and the magnitude of alleged benefits has yet to be established. OBJECTIVES: To determine the effects of biofeedback and/or anal sphincter exercises/pelvic floor muscle training for the treatment of faecal incontinence in adults. SEARCH STRATEGY: We searched the Cochrane Incontinen

Clinical management of constipation.

First, it is important to find out whether the patient is complaining of infrequent defaecation, excessive straining at defaecation, abdominal pain or bloating, a general sense of malaise attributed to constipation, soiling, or a combination of more than one symptom. Second, one must decide if there is a definable abnormality as a cause of the symptom(s). Is the colon apparently normal or is its lumen widened (megacolon)? Is the upper gut normal or is there evidence of neuropathy or myopathy? Is the ano-rectum normal or is there evidence of a weak pelvic floor, mucosal prolapse, major rectocel

Biofeedback and behavioral medicine: an overview.

Biofeedback is a behavioral method of achieving or enhancing voluntary control of physiological processes. Basic studies indicate that a variety of autonomic nervous system and other internal bodily changes can be modified with the method. It has been shown that specific responses and patterns of responses can be controlled. Biofeedback appears to be a promising method of altering symptoms of psychophysiological and medical disorders, e.g., high blood pressure, vascular changes in migraine, neuromuscular abnormalities, and cardiac arrhythmias. This research has stimulated a renewed interest in

[Functional and motility gastrointestinal disorders].

As in previous years, a huge number of studies were presented at the Congress of the American Gastroenterology Association (Digestive Diseases Week [DDW]), some of which were better than others. The present article attempts to extract and summarize the most interesting findings reported. In general terms, certain technological advances have been consolidated, with full incorporation into clinical practice, such as impedancemetry and high-resolution manometry. New physiopathological data are coming to light that increasingly indicate the inextricable link between organic and psychological facto

Behavioral and prophylactic pharmacological intervention studies of pediatric migraine: an exploratory meta-analysis.

In this review, the effectiveness of behavioral and pharmacological treatments for pediatric migraine was quantitatively summarized following the meta-analytic approach outlined by Hedges and Olkin (1985). A first meta-analysis based on treatment outcome within treatment conditions revealed that thermal biofeedback and interventions combining biofeedback and progressive muscle relaxation seem to be significantly more efficacious than other behavioral treatment modalities, psychological and drug placebo, and the more commonly used prophylactic drug regimens. Though there is some evidence sugges

Structure and function in neurobiology: a conceptual framework and the localization of functions.

A framework of concepts on structure and function of the nervous system is presented, in which we attempt to formulate the intended meanings of some ambiguous statements on structure and function in neurobiology in a set of strictly defined concepts. Function. The word "function" as generally used has different meanings. Two meanings of the question "What is the function of B?" are particularly relevant for this subject: (1) "What does B do?" and (2) "Why did B evolve?" It is shown that question 1 must be answered before question 2. Function of brain region S. The question "What does brain reg

Attention deficit hyperactivity disorder across the lifespan: the child, adolescent, and adult.

Management of a child, adolescent, college student, or adult with ADD/ADHD (ADHD) is reviewed with emphasis on pharmacologic approaches in the adult. Psychological treatment includes psychotherapy, cognitive-behavior therapy, support groups, parent training, biofeedback, meditation, and social skills training. Medications are reviewed that research has revealed can improve the core symptomatology of a child or adolescent with ADHD. These medications include stimulants (psychostimulants), antidepressants, alpha-2 agonists, and a norepinephrine reuptake inhibitor. Psychopharmacology approved and

The Intersection of Psychology and Gynecology: A Comprehensive Review of Pain Management Strategies in Women's Health.

Chronic gynecological pain conditions, including dysmenorrhea, endometriosis, vulvodynia, and chronic pelvic pain syndrome (CPPS), present significant challenges in women's health. These conditions result in significant physical discomfort and are closely associated with psychological comorbidities such as anxiety, depression, and increased central pain sensitization. Traditional pharmacological and surgical interventions often provide limited relief, as they fail to address the complex interplay between biological, neurological, and psychological factors underlying pain perception. This revie

Psychobiological factors of resilience and depression in late life.

In contrast to traditional perspectives of resilience as a stable, trait-like characteristic, resilience is now recognized as a multidimentional, dynamic capacity influenced by life-long interactions between internal and environmental resources. We review psychosocial and neurobiological factors associated with resilience to late-life depression (LLD). Recent research has identified both psychosocial characteristics associated with elevated LLD risk (e.g., insecure attachment, neuroticism) and psychosocial processes that may be useful intervention targets (e.g., self-efficacy, sense of purpose

Habit training versus habit training with direct visual biofeedback in adults with chronic constipation: study protocol for a randomised controlled trial.

BACKGROUND: Constipation affects up to 20% of adults. Chronic constipation (CC) affects 1-2% of adults. Patient dissatisfaction is high; nearly 80% feel that laxative therapy is unsatisfactory and symptoms have significant impact on quality of life. There is uncertainty about the value of specialist investigations and whether equipment-intensive therapies using biofeedback confer additional benefit when compared with specialist conservative advice. METHODS/DESIGN: A three-arm, parallel-group, multicentre randomised controlled trial. OBJECTIVES: to determine whether standardised specialist-le

Stress-Reducing Psychological Interventions as Adjuvant Therapies for Diabetic Chronic Wounds.

BACKGROUND: Diabetic foot ulcers (DFUs) are a major complication of diabetes mellitus and a leading cause of lower limb amputation. Interventions to reduce psychological stress may have the potential to improve self-care and greatly reduce the morbidity and mortality associated with DFU. This review is focused on the consequences of psychological stress in wound healing and reflects on the effects of currently used psychological stress-reducing interventions in patients with DFU, proposing new applications for currently used stress-reduction interventions. RESULTS: Stress is a natural and fun

Evaluating a brief smartphone-based stress management intervention with heart rate biofeedback from built-in sensors in a three arm randomized controlled trial.

Perceived stress is prevalent in industrial societies, negatively impacting mental health. Smartphone-based stress management interventions provide accessible alternatives to traditional methods, but their efficacy remains modest, potentially due to limited integration of smartphone sensor technology. The primary aim of this study was to evaluate the efficacy of an 18-day smartphone-based stress management intervention, MT-StressLess with integrated heart rate (HR)-based biofeedback using built-in accelerometer sensors, compared to a waitlist control (WLC) condition. Secondary outcomes include

Behavioral treatment of dizziness secondary to benign positional vertigo following head trauma.

Benign positional vertigo (BPV) represents a challenge to rehabilitation due to the subjective nature of the complaint of dizziness, frequent failure of pharmacologic intervention, and complicating psychologic factors. Behavioral therapy was used to treat a 26-year-old woman who complained of debilitating dizzy spells after mild head injury sustained in a motor vehicle accident. During a three-week baseline period before treatment, the patient reported a weekly average of 48 dizzy spells, which prevented participation in independent activities and kept her homebound and psychologically distres
