Short interpregnancy interval increases the risk of preterm premature rupture of membranes and early delivery.

OBJECTIVES: Preterm premature rupture of membranes (PPROM) is a major contributor to overall preterm birth (PTB) rates. A short interpregnancy interval (IPI) is a well-known risk factor for PTB. It is unknown if a short IPI specifically affects the risk of developing PPROM in a subsequent pregnancy. We sought to determine the association between IPI and the risk of PPROM in a subsequent pregnancy. METHODS: A retrospective cohort study using the Missouri birth certificate database of singleton births from 2003 to 2013 was conducted. A short IPI (delivery of the prior pregnancy to conception of

Induction of labour vs. spontaneous vaginal delivery in twin pregnancy after 36 weeks of gestation.

Time for delivery and delivery pathway in twin pregnancies are still in great debate. Our study goal was to compare the characteristics of delivery and maternal-foetal outcome in uncomplicated near-term twin pregnancies undergoing labour induction and those with spontaneous labour. We found no statistical differences in patients with twin pregnancies who underwent labour induction and those with spontaneous labour regarding the history of previous caesarean delivery, parity, pregnancy achieved by assisted reproductive techniques (ART), chorionicity and cervical dilation at the admission as wel

Birth weight and the effects of birth spacing and breastfeeding on infant mortality.

Analyses previously reported, based on data from the World Fertility Survey (WFS), are replicated here with data from the Malaysian Family Life Survey. Comparison of results, when data limitations inherent in the World Fertility Surveys are reproduced or relaxed, suggests that these limitations cause little distortion, and thus bolsters confidence in the validity of results based on WFS data in which these limitations are inescapable. Generalizations based on the present investigation and on the body of previous work that it tends to validate are presented. Most significantly, these include th

Short interpregnancy intervals and adverse pregnancy outcomes by maternal age in the United States.

PURPOSE: The purpose of the article was to examine the association between short interpregnancy intervals and adverse outcomes by maternal age among U.S. women. METHODS: Using publicly available natality files for 2013-2016 singleton births, we compared the risks of preterm birth, gestational diabetes, gestational hypertension, and maternal morbidity (delivery-related complications) for less than 6-month, 6 to 11-month, and 12 to 17-month to 18- to 23-month interpregnancy intervals, overall and by maternal age. Models adjusted for maternal demographics, conditions, and behaviors. RESULTS: Am

Redefining neonatal thrombocytopenia: an evidence-based approach, improved by the refineR algorithm.

OBJECTIVE: A common definition of neonatal thrombocytopenia is a platelet count <150,000/µL. The international hematologic consensus designation of cytopenia is a blood count <2.5th percentile lower reference interval for an appropriate comparison population. We hypothesized that a new reference interval modeling algorithm, refineR, would generate more accurate platelet reference intervals and, therefore, more accurate and evidence-based definitions of neonatal thrombocytopenia. STUDY DESIGN: We utilized refineR to analyze platelet counts from large clinical databases to generate new neonatal

Childhood mortality and associated factors in Migori County, Kenya: evidence from a cross-sectional survey.

OBJECTIVES: The under-five mortality (U5M) rate in Kenya (41 per 1000 live births) remains significantly above international goals (25 per 1000 live births). This is further exacerbated by regional inequalities in mortality. We aimed to describe U5M in Migori County, Kenya, and identify associated factors that can serve as programming targets. DESIGN: Cross-sectional observational survey. SETTING: Areas served by the Lwala Community Alliance and control areas in Migori County, Kenya. PARTICIPANTS: This study included 15 199 children born to respondents during the 18 years preceding the surv

Epidemiological study of congenital talipes calcaneovalgus.

Among 671,494 children born in South American hospitals participating in the Collaborative Latin American Study of Congenital Malformations (ECLAMC), 284 had talipes calcaneovalgus, corresponding to an incidence of 4.2 per 10,000 live births. Genetic and environmental risk factors were investigated in the sample of malformed newborns with talipes calcaneovalgus and in matched control babies. An excess number of females was found among the newborns with talipes calcaneovalgus. Talipes calcaneovalgus patients did not differ from the matched controls in terms of ethnic groups, inbreeding coeffici

Interpregnancy Interval and Adverse Pregnancy Outcomes: An Analysis of Successive Pregnancies.

OBJECTIVE: To examine the association between interpregnancy interval and maternal-neonate health when matching women to their successive pregnancies to control for differences in maternal risk factors and compare these results with traditional unmatched designs. METHODS: We conducted a retrospective cohort study of 38,178 women with three or more deliveries (two or greater interpregnancy intervals) between 2000 and 2015 in British Columbia, Canada. We examined interpregnancy interval (0-5, 6-11, 12-17, 18-23 [reference], 24-59, and 60 months or greater) in relation to neonatal outcomes (pret

Interpregnancy interval and risk of preterm labor.

In 1977-1980, over 12,000 pregnant women being followed at the Boston Hospital for Women were interviewed and had their medical records reviewed. The effect of interpregnancy interval on the risk of preterm labor was estimated in 4,467 of these women whose previous pregnancy had resulted in a full-term live birth. The rate of preterm birth after the spontaneous onset of labor in this cohort was 3.8 percent. While the possibility of an increased risk of preterm labor for interpregnancy intervals of 3 months or less cannot be definitely excluded (adjusted odds ratio = 2.0, 95 percent confidence

Advanced Maternal Age and the Risk of Low Birth Weight and Preterm Delivery: a Within-Family Analysis Using Finnish Population Registers.

Advanced maternal age at birth is considered a major risk factor for birth outcomes. It is unclear to what extent this association is confounded by maternal characteristics. To test whether advanced maternal age at birth independently increases the risk of low birth weight (<2,500 g) and preterm birth (<37 weeks' gestation), we compared between-family models (children born to different mothers at different ages) with within-family models (children born to the same mother at different ages). The latter procedure reduces confounding by unobserved parental characteristics that are shared by sibli

Interpregnancy Interval and Risk of Adverse Pregnancy Outcomes: A Register-Based Study of 328,577 Pregnancies in Denmark 1994-2010.

Objectives To investigate the associations of interpregnancy interval (IPI) with miscarriage, stillbirth, preterm delivery and small for gestational age delivery. Methods The study population comprised all women who had a live birth and at least one subsequent pregnancy in Denmark during the period from 1994 to 2010 (N = 328,577). Linear regression was used to estimate risk differences for miscarriage, stillbirth, preterm delivery and small for gestational age delivery according to IPI. Results The results were heterogeneous: the risk of miscarriage increased monotonically with the length of t

[Slight sibling age difference--a risk factor in the genesis of schizoid disorders?].

615 neurotic patients from two outpatient Clinics were divided into three groups: only children, siblings with minimal age difference group, siblings not of minimal age difference group. The following hypothesis were to be examined: The younger of two siblings with minimal age difference are at greater risk to develop schizoid disorders. The hypothesis was proved correct. Amongst our patients the younger of two siblings of minimal age difference were at a particularly high risk in becoming schizoid. In addition we found the following risiko configuration in the early developmental period. Freq

Short birth spacing and its association with maternal educational status, contraceptive use, and duration of breastfeeding in Ethiopia. A systematic review and meta-analysis.

BACKGROUND: Closely spaced birth increases the risk of adverse maternal and child health outcomes. In Ethiopia, the prevalence of short birth spacing was highly variable across studies. Besides, contraceptive use, educational status, and duration of breastfeeding were frequently mentioned factors affecting short birth spacing. Therefore, this meta-analysis aimed to estimate the pooled prevalence of short birth spacing and its association with contraceptive use, educational status, and duration of breastfeeding among reproductive-age women in Ethiopia. METHODS: International databases: Google

Interpregnancy interval and risk of autistic disorder.

BACKGROUND: A recent California study reported increased risk of autistic disorder in children conceived within a year after the birth of a sibling. METHODS: We assessed the association between interpregnancy interval and risk of autistic disorder using nationwide registry data on pairs of singleton full siblings born in Norway. We defined interpregnancy interval as the time from birth of the first-born child to conception of the second-born child in a sibship. The outcome of interest was autistic disorder in the second-born child. Analyses were restricted to sibships in which the second-born

Diarrhoeal disease--epidemiology and intervention.

The causes of the complex triad of gastro-enteritis, other related infections and malnutrition have at their roots poverty, lack of political will, overpopulation and destruction of the environment. Intervention involves multiple sectors of the economy and not only the health services of a country, and should take place as early as possible during the natural history of the disease.

Nutrition before conception and the outcome of pregnancy.

Close birth spacing increases the risk of miscarriage, congenital malformation and perinatal death because it does not give a mother time to recover from the nutritional depletion caused by the first pregnancy or time to recover a normal hormonal profile before the beginning of the next pregnancy. The placental vitamin pump causes maternal vitamin depletion. Nutritional deficits do more damage to reproduction around the time of conception than later during pregnancy. Vitamin and mineral deficiencies can cause not only direct damage to embryonic development but damage to both male and female ge

Reconsidering childhood undernutrition: can birth spacing make a difference? An analysis of the 2002-2003 El Salvador National Family Health Survey.

It is well understood that undernutrition underpins much of child morbidity and mortality in less developed countries, but the causes of undernutrition are complex and interrelated, requiring a multipronged approach for intervention. This paper uses a subsample of 3853 children under age 5 from the most recent family health survey in El Salvador to examine the relationship between birth spacing and childhood undernutrition (stunting and underweight). While recent research and guidance suggest that birth spacing of three to five years contributes to lower levels of infant and childhood mortalit
