BACKGROUND: The bladder exstrophy-epispadias complex represents a spectrum of urogenital anomalies in which part or all of the distal urinary tract fail to close and are exposed on the outer abdominal wall. Previous studies are suggestive of an underlying multifactorial mode of inheritance. However, no genetic or nongenetic factor has been identified so far. In this study, we sought risk loci by parametric and nonparametric linkage analysis, searching for homozygous segments, and more complex inherited loci, respectively.
METHODS: Two pedigrees, Spanish and German, each comprising two members

The authors report a rare variant of the exstrophy-epispadias complex in which the patient has two bladders and complete duplication of müllerian structures. We also discuss the treatment of the patient's orthopedic and genitourinary conditions. The patient's prognosis is excellent, and she is expected to be fully continent, able to void spontaneously, and conceive children.

OBJECTIVE: Many changes have occurred in the treatment of bladder exstrophy over the last few years and many repairs are now offered. The purpose of this study was to evaluate long-term outcomes in a select group of patients in whom modern staged repair (MSRE) was undertaken.
PATIENTS AND METHODS: From an institutionally approved database were extracted 189 patients who had undergone primary closure between 1988 and 2004. The records of 131 patients (95 males) who underwent MSRE with a modified Cantwell-Ransley repair by a single surgeon in 1988-2004 were reviewed with a minimum 5-year follow

PURPOSE: In our hands complete primary repair (CPR) of bladder exstrophy results in hypospadias in two-thirds of boys. To our knowledge hypospadias repair following CPR of bladder exstrophy has not been reported previously. We report our experience with hypospadias repair following CPR of bladder exstrophy.
MATERIALS AND METHODS: A total of 22 boys with bladder exstrophy underwent CPR using Mitchell's technique between November 1998 and January 2002. The procedure resulted in hypospadias in 15 boys (68%). The site of the meatus was distal penile in 5 patients, mid penile in 3 and proximal pen

PURPOSE: Functional reconstruction of children born with bladder exstrophy results in a successful outcome in the majority. However, failed bladder neck repair, especially after multiple surgical attempts, compromises the quality of the tissues and, thus, limits surgical options. In these cases conventional sphincteroplasty and other continence procedures, such as rectus sheath sling or artificial urinary sphincters, are ill-advised due to ischemia and fibrosis of the bladder neck.
MATERIALS AND METHODS: We transferred the gracilis muscle as a vascularized pedicle flap around the bladder neck

OBJECTIVES: To describe a unique approach to the management of duplicate bladder exstrophy combining initial bladder closure and epispadias repair. Bladder exstrophy has been successfully managed by staged surgical repair with early bladder closure, subsequent epispadias repair, and, finally, bladder neck reconstruction. Duplicate bladder exstrophy is a rare variant of the exstrophy complex with fewer than 20 cases reported.
METHODS: A male newborn presented with the appearance of both bladder exstrophy and a ruptured omphalocele. Repair of the omphalocele and bilateral orchiopexy was perform

A 13-year-old boy had a complete double bladder and urethra associated with an inferior abdominal lesion, which on histological examination proved to be a colonic remnant with no communication to the underlying bowel. This entity is one of the exstrophy/epispadias complex variants known as covered exstrophy with sequestered bowel. Although 4 cases of this anomaly have been reported in the literature, to my knowledge this is the first such case with a double bladder. Excision of the lesion and the incontinent bladder was performed, and the ureter was reimplanted in the contralateral continent b

One hundred thirty-four cases of classical bladder exstrophy, managed at our institution, were reviewed. Fifty-six percent of the boys and 15% of the girls developed inguinal hernias over an average follow-up time-span of 10 years. Thirty-one percent of the patients with hernias underwent repair at the time of initial bladder closure. Forty-six percent of the patients who developed a hernia were diagnosed during the first year following their initial procedure. More than 50% of the individuals in the latter category presented with an incarcerated hernia, and required emergent management. Boys

OBJECTIVE: To assess the role of osteotomy at the time of bladder neck reconstruction (BNR) for continence in classic bladder exstrophy, in which closure of the pelvic ring and reconstitution of the pelvic diaphragm may affect eventual continence.
PATIENTS AND METHODS: The results of using osteotomy at the time of BNR in 29 children were reviewed. The mean bladder capacity before BNR was 76 mL. The indications for osteotomy were a wide pubic diastasis and a soft intersymphyseal bar. After osteotomy, all children were maintained in external fixation and lower-extremity traction for 6-8 weeks.

Young-Dees' method of bladder neck reconstruction is described. A material of 17 cases with complete or almost complete incontinence on account on a deficient sphincter who were submitted to this operative method is presented. It is concluded that the method can be recommended as the primary operation for incontinence, particularly in patients with exstrophy of the bladder or epispadias as satisfactory continence could be obtained in 60%. No deterioration of renal function was observed and the patients functioned well both socially and sexually.

BACKGROUND: Bladder exstrophy is a rare congenital anomaly with long-term physical and psychosocial implications. While surgical management has improved, little is known about the psychological and sexual outcomes of affected individuals and their caregivers.
METHODS: We retrospectively evaluated 27 patients (aged 12-33 years) who underwent bladder exstrophy closure and augmentation. Psychological and sexual well-being were assessed using validated psychometric tools, including the Arizona Sexual Experience Scale (ASES), Beck Depression Inventory (BDI), Maudsley Obsessive-Compulsive Inventory

Duplicate bladder exstrophy, a rare variant of the bladder exstrophy-epispadias complex, was diagnosed in a 5-month-old female infant presenting with a 4 × 4 cm lower abdominal mass. Physical examination revealed absent umbilical scar, wide symphysis pubis, and anteriorly displaced genitalia and anus. Imaging ruled out classic bladder exstrophy and vesicoureteral reflux. Surgical exploration confirmed a suprapubic exstrophic mucosal plaque without communication to a functional bladder. Management involved excision of the exstrophic plaque and primary abdominal wall closure. Post-operative cour

INTRODUCTION: Achieving urinary continence is a key goal in children born with the bladder exstrophy-epispadias complex (BEEC). Unfortunately, this goal is only moderately achieved despite sometimes extensive surgical treatment. Undergoing repeated hospitalization and operations may consequently have a negative impact on quality of life. We therefore believe that other, conservative treatment options should be explored in an earlier stage of incontinence treatment in BEEC patients. As part of this, an intensive urotherapy program based on was offered to patients with persistent incontinence af

BACKGROUND: Exstrophy variants are well described urologic anomalies. They are characterized by atypical anatomical and physical findings than those found in patients with classic bladder exstrophy and epispadias malformation. The combination of these anomalies with duplicated phallus is a rare occurrence. Here we present a neonate with a rare form of exstrophy variant associated with penile duplication.
CASE SUMMARY: One day old male neonate who was born at term was admitted to our neonatal intensive care unit. He had lower abdominal wall defect and open bladder plate with no visible ureteri

Bladder exstrophy (BE) is a devastating congenital abnormality of the urinary tract in which infants are born with the urinary bladder extruded through their abdominal wall, a urethra that is open dorsally, and significant secondary abnormalities of the genitalia. We present a male patient with BE who suffered from infertility and after a long course of failed attempts with assisted reproductive technology ultimately benefitted from a synchronous testicular sperm extraction. We review the literature surrounding assisted reproductive techniques in the setting of bladder exstrophy and comment on

Bladder exstrophy is a rare and complex malformation. Often associated with diverse deformations, an accurate diagnosis is a must for adequate management. In the African setting, especially in a remote area, delivery rarely occurs within a healthcare structure, thus reducing the chance of an early diagnosis. Due to a low density of specialists, people in rural areas refer to traditional healers for healthcare problems, the thing that delays the time to diagnosis. We report, according to the CARE guidelines (https://www.equator-network.org/reporting-guidelines/care/), a case of an 18-year-old m
