OBJECTIVES: To compare injury patterns resulting from explosions in the open air versus within confined spaces.
METHODS: Medical charts of 297 victims of four bombing events were analyzed. Two explosions occurred in the open air and two inside buses. Similar explosive devices were applied in all four incidents. The incidence of primary blast injuries, significant penetrating trauma (Abbreviated Injury Scale score > or = 2), burns, Injury Severity Score, Revised Trauma Score, and mortality were compared between the two populations.
RESULTS: A total of 204 casualties were involved in open-air

OBJECTIVES: Due to the absence of clinical blast data, automotive injury data using the abbreviated injury score (AIS) has been extrapolated to define current North Atlantic Treaty Organisation (NATO) injury thresholds for anti-vehicle mine tests. We hypothesized that AIS, being a marker of fatality rather than disability, would be a worse predictor of poor clinical outcome compared with the lower limb-specific foot and ankle severity score (FASS).
METHODS: Using a prospectively collected trauma database, we identified UK Service Personnel sustaining lower leg injuries from under-vehicle expl

Blast-related traumatic brain injury (bTBI) is a significant cause of wartime morbidity and mortality. In recent decades, thermobaric explosives have emerged as particularly devastating weapons associated with bTBI. With recent documentation of the use of these weapons in the war in Ukraine, clinicians and laypersons alike could benefit from an improved understanding behind the dynamic interplay between explosive weaponry, its potential for bTBI, and the subsequent long-term consequences of these injuries. Therefore, we provide a general overview of the history and mechanism of action of therm

Explosive blast is a frequent cause of traumatic brain injury (TBI) among personnel deployed to Afghanistan and Iraq. Functional magnetic resonance imaging (fMRI) with an event-related stimulus-response compatibility task was used to compare 15 subjects with mild, chronic blast-related TBI with 15 subjects who had not experienced a TBI or blast exposure during deployment. Six TBI subjects reported multiple injuries. Relative to the control group, TBI subjects had slightly slower responses during fMRI and increased somatic complaints and symptoms of post-traumatic stress disorder (PTSD) and dep

Little is known about the utilization of central nervous system (CNS) and musculoskeletal (MS) medications in Operation Iraqi Freedom/Operation Enduring Freedom (OIF/OEF) veterans with blast-related injuries (BRIs). We followed prescription drug use among a cohort of 133 OIF/OEF veterans with BRIs by using the Joint Theatre Trauma Registry, the Tampa Polytrauma Registry, and electronic medical records. We extracted 12 months of national medication records from the Veterans Health Administration Decision Support System and analyzed them with descriptive statistics. Over the 12-month period (fis

To investigate the dynamic response characteristics of personnel injuries caused by blast shock waves under actual working conditions, experimental studies on blast-induced injuries in rats, behavioral experiments post-injury, and pathological examinations were conducted, followed by injury assessments. The results showed that all rats died when the peak overpressure was 0.427 MPa. At a peak overpressure of 0.192 MPa, the rats were assessed as moderately injured. Behavioral tests at 7 and 28 days revealed reduced motor ability and decreased autonomous exploration, with statistically significan

INTRODUCTION: Blast injuries are common during wartime but are also related to various holiday festivals worldwide. Despite strict legislations, these devices still cause serious injuries leaving permanent disabilities. Children are affected in numerous cases with the hand being the most affected body region. The aim of our study was to profile the blast related hand injuries and to review treatment modalities that we used.
METHODS: A retrospective study of patients treated for blast-devices related hand injuries between 2006 and 2015 was performed.
RESULTS: 112 patient with blast related ha

The application of lessons learned on the battlefield for timely surgical control of lower extremity hemorrhage secondary to blast injuries to the civilian practice for similar wounding patterns from industrial accidents or terrorist activities is imperative. Although simple cut-down procedures are commonly sufficient for the control of blood vessels for distal extremity traumatic amputations, high-thigh or disarticulation wounding patterns often require more complex surgical methods. The following details both the decision-making process and operative techniques for controlling hemorrhage fro

BACKGROUND: Unstable, severely comminuted fractures of the metacarpophalangeal (MCP) joint are difficult to treat. Closed treatment and casting of these fractures often fail to maintain proper alignment and impede wound care where concomitant open injuries such as gunshot wounds are present. Conventional pinning or plating techniques are not feasible if extensive bone loss and comminution are present. A distraction pinning technique represents a potential alternative, but results with this approach, to our knowledge, have not been reported.
QUESTIONS/PURPOSES: The purposes of this study were

BACKGROUND: One of the important causes of death after blast injuries is reduced blood volume, which typically results from hemorrhage but may also result from nonhemorrhagic causes. Hemoconcentration is one such alternate cause of reduced blood volume, but its mechanism is unclear. Because blood is condensed after chest-abdomen blast injuries in rabbits, a series of experiments was conducted to clarify this phenomenon.
METHODS: Chest-abdomen blast injuries from different distances (10 cm, 15 cm, 20 cm, and 30 cm) were induced in male rabbits. ¹²⁵I-albumin was injected into the blood, and its

The physiology and management of blast injuries and underwater blast injuries are described in the literature; however, cases of underwater blast injuries in pediatric patients are not discussed. We describe a case of a 10-year-old girl presenting to the pediatric emergency department who sustained injury from a firework discharged while underwater and then developed respiratory failure as a result of pulmonary hemorrhage and required endotracheal intubation.

Blunt traumatic aortic injury (BTAI) is a rare and often fatal. We present the case of a 19-year-old male soldier with BTAI and left subclavian artery (LSA) dissection after a blast injury from a rocket-propelled grenade. Despite the absence of other significant primary blast injuries, imaging revealed a grade III pseudoaneurysm at the aortic isthmus and LSA dissection. Emergent thoracic endovascular aortic repair with LSA stenting and thrombectomy was performed successfully. This report is the first of an isolated BTAI after a combat blast injury. It underscores the need for heightened clinic

INTRODUCTION AND IMPORTANCE: Gunshot injury has many medical aspects, and the blast effect is a rare one that needs a precise first emergency examination.
CASE PRESENTATION: A 16-year-old male who was injured by a gunshot. Upon arrival at the emergency department, he was treated as a trauma call. There was only one bullet that entered from the outer edge of the right ASIS and exited inside the right buttock approximately 10 cm lower tangentially and no bullet entered the abdomen. Initial assessments and imaging revealed no underlying pathology findings, and the patient's vital signs were stab

Dismounted complex blast injury (DCBI) has been one of the most severe forms of trauma sustained in recent conflicts. This injury has been partially attributed to limb flail; however, the full causative mechanism has not yet been fully determined. Soil ejecta has been hypothesized as a significant contributor to the injury but remains untested. In this study, a small-animal model of gas-gun mediated high velocity sand blast was used to investigate this mechanism. The results demonstrated a correlation between increasing sand blast velocity and injury patterns of worsening severity across the t
