This paper reviews the relevant pediatric literature about blood pressure (BP) hyperreactivity to stress as a possible precursor of hypertension and coronary heart disease. Two prospective studies of children indicate that BP hyperreactivity predicts later development of hypertension. Several studies have identified correlates of BP reactivity. Race, obesity, Type A and family history of hypertension appear to be associated with BP reactivity in children. There appear to be both genetic and environmental influences, but relatively few psychosocial variables have been studied. There are signifi

Essential trace elements such as zinc, iron, and copper participate in various enzyme reactions directly related to the regulation of blood pressure and indirectly related to generation of oxidative metabolic energy, alterations in blood lipid levels, and alterations in taste acuity. The toxicological action of several heavy metal ions including cadmium, lead, mercury, and thallium can cause hypertension by affecting hormone metabolism, vasoconstriction, and renal tubular function. We conclude, however, that neither deficiencies of essential elements nor the presence of toxic heavy metals are

Thiazides produce an average loss of 2 L of extracellular fluid volume, of which about 300 ml is plasma volume, during the first 48 hours of administration, and this reduction is maintained without further loss for as long as the drug is given. Do thiazides lower blood pressure by volume depletion or by direct vasodilator action? As evidence against the vasodilator theory, these facts may be adduced: (1) thiazides do not lower blood pressure in patients (with renal disease) who cannot obtain a diuresis, and (2) other diuretics and diets very low in sodium also reduce blood pressure and volume

Blood pressure response to hemodialysis was investigated in 15 patients with end-stage kidney disease; mean arterial pressure was unchanged in five (Group 1) and reduced 10 mm Hg in 10 (Group 2). The two groups did not differ significantly with regard to either biochemical values or hemodynamic indices before dialysis, and both sustained comparable reduction in body weight, total blood volume, and cardiac output following dialysis. Heart rate remained unchanged in both. The only significant difference between the two was the response of total peripheral resistance (TPR) to fluid depletion. TPR

BACKGROUND: There have been few studies evaluating the control of hypertension (HT) in children. This study aimed to assess the control of HT using ambulatory blood pressure monitoring (ABPM) and to compare the parameters between the uncontrolled HT and controlled HT groups.
METHODS: Hypertensive patients aged ≥ 5 years who underwent ABPM to assess the control of HT were enrolled. Demographics, office blood pressure (BP), ABPM, and echocardiographic data were collected. Controlled HT was defined using a BP goal recommended by the 2016 European Society of Hypertension guidelines.
RESULTS: The

PURPOSE: This study aimed to evaluate the association of retinal vascular occlusion, including retinal vein occlusion (RVO) and retinal artery occlusion (RAO), with stages of hypertension.
DESIGN: Nationwide, population-based retrospective cohort study.
METHODS: Based on baseline blood pressure (BP) as defined by the 2017 American College of Cardiology/American Heart Association guideline, participants were categorized into 4 BP groups. For the BP change measurement, BP groups were defined based on the combination of baseline and follow-up BP categories. The composite retinal vascular occlus

BACKGROUND: Post-transplant hypertension is a major modifiable risk factor for graft and patient survival. The aim of this study was to establish independent predictors for achieving systolic, diastolic and overall blood pressure target [Kidney Disease Outcome Quality Initiative (K/DOQI) target blood pressure <130/80 mm Hg] in renal-transplant recipients (RTRs) and to consider whether current management strategies are adequate to achieve this aim.
METHODS: The most recent office blood pressure readings were collected for 513 RTRs at least 6 months after transplantation. In addition, demograph

BACKGROUND: Determining the agreement between ambulatory blood pressure (BP) monitoring (ABPM) and home BP monitoring (HBPM) can inform their use.
METHODS: Participants ≥65 years of age taking antihypertensive medication in the AMBROSIA-HOME study completed 24 hours of ABPM and 7 days of HBPM. Controlled office and awake BP was defined as mean systolic BP (SBP)/diastolic BP (DBP) <130/<80 mm Hg and uncontrolled office and awake BP as mean SBP or DBP ≥130/80 mm Hg. Masked uncontrolled hypertension was defined as controlled clinic BP with uncontrolled BP on HBPM or ABPM, and white coat effect a

BACKGROUND: Cuffless blood pressure (BP) devices have demonstrated potential in estimating daytime BP but have not been able to predict nocturnal BP precisely, creating difficulties when the intended use is ambulatory BP monitoring (ABPM). We evaluated the ability of a photoplethysmography-based cuffless BP device, with the intended use 24-hour ABPM, to estimate nocturnal BP compared to a cuff-based oscillometric device.
METHODS: Participants underwent 24-hour ABPM with the cuffless device placed on the non-dominant upper arm and a validated cuff-based oscillometric device on the dominant arm

BACKGROUND: The increasing accessibility of high altitudes due to modern lifts has led to rise in individuals reaching high elevations without acclimatization or medical screening. Acute exposure to hypobaric hypoxia induces hemodynamic and metabolic stress, increasing blood pressure (BP) and heart rate while reducing oxygen saturation. These effects are particularly concerning for individuals with preexisting cardiovascular diseases.
METHODS: We conducted an observational cross-sectional study at the base station (1300 m) of the Skyway Monte Bianco cable car, evaluating the demographic and c

INTRODUCTION: Adherence to blood pressure (BP) evaluation 7-10 days after pregnancy complicated by hypertension is poor. Current guidelines for postpartum BP control are more liberal than those for nonpregnant individuals. Our objective was to determine whether a comprehensive BP management program and tight BP control improve BP outcomes at 7-10 days postpartum compared with clinician-directed management and standard targets.
METHODS: Postpartum patients with chronic or gestational hypertension, preeclampsia/eclampsia, or HELLP (hemolysis, elevated liver enzymes, low platelets) syndrome were

The values used to define white-coat and masked blood pressure (BP) effects are usually arbitrary. This study aimed at investigating the accuracy of various cutoffs based on the differences (ΔBP) between office BP (OBP) and 24h-ambulatory BP monitoring (ABPM) to identify white-coat (WCH) and masked (MH) hypertension, which are phenotypes coupled with adverse prognosis. This cross-sectional study included 11,350 [Derivation cohort; 45% men, mean age = 55.1 ± 14.1 years, OBP = 132.1 ± 17.6/83.9 ± 12.5 mmHg, 24 h-ABPM = 121.6 ± 11.4/76.1 ± 9.6 mmHg, 25% using antihypertensive medications (AH)] an

OBJECTIVE: To compare blood pressure (BP) in tonometric radial artery recordings during passive head-up tilt with ambulatory recordings and evaluate possible laboratory cutoff values for hypertension.
METHODS: Laboratory BP and ambulatory BP were recorded in normotensive (n = 69), unmedicated hypertensive (n = 190), and medicated hypertensive (n = 151) subjects.
RESULTS: Mean age was 50.2 years, BMI 27.7 kg/m 2 , ambulatory daytime BP 139/87 mmHg, and 276 were male (65%). As supine-to-upright changes in SBP ranged from -52 to +30 mmHg, and in DBP from -21 to +32 mmHg, the mean values of BP s

Hypertension is a major risk factor for cardiovascular diseases, and behavior modification has been shown to improve blood pressure (BP). We investigated whether daily self-monitoring of systemic BP and other factors related to cardiovascular events decreased BP in hypertensive participants. In this prospective, randomized, open, blinded-endpoint trial, we assigned 161 participants with hypertension to monitor their BP daily (BP-measurement group) or, in addition to BP, monitor their body fat, sleeping time, and daily step count (multiple-measurement group) or no self-monitoring (control group

Ambulatory blood pressure monitoring (ABPM) correlates more closely to organ damages than clinic blood pressure (BP). In the current study we aimed to investigate the association between micro- and macrovascular complications of diabetes and both diurnal and nocturnal variability in BP.A total of 192 patients with type 2 diabetes (T2DM) who had complete data on ABPM were selected. BP categories were defined based on 2017 ACC/American Heart Association BP guideline. The cross-sectional association between different BP phenotypes and diabetes complications including cardiovascular disease (CVD),

BACKGROUND: Cardiovascular morbidity and mortality was reduced by 25% when blood pressure (BP) was targeted to 120 mm Hg systolic compared with 140 mm Hg systolic in Systolic Blood Pressure Intervention Trial (SPRINT); however, BP was measured using a research technique. SPRINT specified 5 minutes of seated rest in a quiet room followed by 3 oscillometric measurements without an observer in the room. The relationship of this research-grade methodology to routine BP measurements is not known.
METHODS AND RESULTS: Among 275 people with chronic kidney disease who had BP <140/90 mm Hg when they c
