Which Safe Zone Is Safe in Total Hip Arthroplasty? The Effect of Bony Impingement.

"Safe zones" for cup position are currently being investigated in total hip arthroplasty (THA). This study aimed to evaluate the impact of bony impingement on the safe zone and provide recommendations for cup position in THA. CT scans were performed on 123 patients who underwent a cementless THA. Using the implant data and bone morphology from the CT scans, an impingement detection algorithm simulating range of motion (ROM) determined the presence of prosthetic and/or bony impingement. An impingement-free zone of motion was determined for each patient. These zones were then compared across all

A new technique for morphologic measurement of the femur. Its application for Japanese patients with osteoarthrosis of the hip.

The cementless femoral prostheses developed in Western countries have commonly been used for Japanese patients, but it has been noted that it is difficult to achieve a good fit between the cementless femoral prostheses and some femora. In Japan, there are many patients with secondary osteoarthrosis (OA) of the hip, but Western designed femoral prostheses seem to be mainly for use with primary OA of the hip. To design femoral prostheses better suited to Japanese patients, a new method to study the morphometry of the femur is required--one which can determine the particular features of the femur

Age-Dependent Susceptibility to Mechanical Unloading-Induced Femoral Morphological Changes and Patellar Instability in a Rat Model.

Abnormal bone morphology and joint malalignment are key risk factors for several musculoskeletal disorders, including patellar instability and lateral patellar dislocation (LPD). Insufficient weight loading can affect bone formation in immature joints; however, its impact on mature joints remains unclear. The aim of this study was to compare the effects of hindlimb suspension (HS) on femur morphology and knee alignment associated with patellar instability and LPD in rats with different levels of musculoskeletal maturity. Immature (4-week-old) and mature (12-week-old) female rats underwent 8 we

How much change in pelvic sagittal tilt can result in hip dislocation due to prosthetic impingement? A computer simulation study.

Developing spinal pathologies and spinal fusion after total hip arthroplasty (THA) can result in increased pelvic retroversion (e.g., flat back deformity) or increased anterior pelvic tilt (caused by spinal stenosis, spinal fusion or other pathologies) while bending forward. This change in sagittal pelvic tilt (SPT) can result in prosthetic impingement and dislocation. Our aim was to determine the magnitude of SPT change that could lead to prosthetic impingement. We hypothesized that the magnitude of SPT change that could lead to THA dislocation is less than 10° and it varies for different hip

The association between anterior femoroacetabular impingement and femoral neck fractures: An observational study.

The impact between acetabulum and femoral neck is another possible mechanism of femoral neck fracture.Direct trauma of the greater trochanter may not be able to fully explain the mechanism underlying femoral neck fracture. In this study, we sought to investigate whether anterior femoroacetabular impingement are associated with femoral neck fractures.A total of 36 patients with femoral neck fracture who had undergone total hip arthroplasty or hemiarthroplasty were included in this study. These patients were divided into 2 groups: labrum tear group and normal labrum group. Patients' age, gender,

Analysis of posterior hip joint impingement about developmental dysplasia of the hip after total hip arthroplasty.

BACKGROUND: Total hip arthroplasty is the preferred treatment for advanced hip osteoarthritis, yet complications like hip dislocation (0.2 %-10 %) persist due to factors such as implant design, positioning, surgical technique, and patient-specific conditions. Impingement between prosthetic components or the acetabulum and proximal femur is a primary cause of instability. METHODS: A retrospective analysis of 120 patients (130 hips) undergoing total hip arthroplasty at XX Hospital (2011-2023) categorized patients by lateral center-edge angle: developmental dysplasia of the hip, borderline devel

Anterior total hip arthroplasty using a metaphyseal bone-sparing stem: component alignment and early complications.

BACKGROUND: Limited-incision total hip arthroplasty (THA) preserves hip abductors, posterior capsule, and external rotators potentially diminishing dislocation risk. However, potential complications also exist, such as component malposition. Specific implants have been manufactured that enhance compatibility with this technique, while preserving metaphyseal bone; however, little data exists documenting early complications and component position. The purpose was to evaluate primary THA using a curved, bone-sparing stem inserted through the anterior approach with respect to component alignment a

Effect of opening-wedge high tibial osteotomy on the three-dimensional kinematics of the knee.

Although it is clear that opening-wedge high tibial osteotomy (HTO) changes alignment in the coronal plane, which is its objective, it is not clear how this procedure affects knee kinematics throughout the range of joint movement and in other planes. Our research question was: how does opening-wedge HTO change three-dimensional tibiofemoral and patellofemoral kinematics in loaded flexion in patients with varus deformity?Three-dimensional kinematics were assessed over 0° to 60° of loaded flexion using an MRI method before and after opening-wedge HTO in a cohort of 13 men (14 knees). Segmental femoral fracture malunion: evidence and prognostic analysis of medical intervention in the third century BC.

We examined the remains of an individual who was unearthed from the Tuchengzi site and was believed to be from the Warring States period in China. The remains exhibited segmental femoral fracture. We aimed to deduce the cause of fracture, medical interventions, healing process, and motion behavior after fracture healing using several techniques, including macroscopic observation, computed tomography (CT), and finite element analysis. Based on the morphology of the long bones, it appeared that the individual was male. The fractures resulted in an adduction angle of 5.47° and an anterior flexion

Radiological evaluation of patellofemoral instability and possible causes of assessment errors.

Patellofemoral instability (PI) is the disruption of the patella's relationship with the trochlear groove as a result of abnormal movement of the patella. To identify the presence of PI, conventional radiographs (anteroposterior, lateral, and axial or skyline views), magnetic resonance imaging, and computed tomography are used. In this study, we examined four main instability factors: Trochlear dysplasia, patella alta, tibial tuberosity-trochlear groove distance, and patellar tilt. We also briefly review some of the other assessment methods used in the quantitative and qualitative assessment o

Total hip arthroplasty in fused hips with spine stiffness in ankylosing spondylitis.

Ankylosing spondylitis (AS) is characterized by involvement of the spine and hip joints with progressive stiffness and loss of function. Functional impairment is significant, with spine and hip involvement, and is predominantly seen in the younger age group. Total hip arthroplasty (THA) for fused hips with stiff spines in AS results in considerable improvement of mobility and function. Spine stiffness associated with AS needs evaluation before THA. Preoperative assessment with lateral spine radiographs shows loss of lumbar lordosis. Spinopelvic mobility is reduced with change in sacral slope f

[Musculoskeletal pathology in children with infantile cerebral palsy: a new classification system].

BACKGROUND: Cerebral palsy (CP) is one of the most common causes of physical disability in childhood. While the Gross Motor Function Classification System (GMFCS) describes motor abilities, a unified classification for musculoskeletal pathologies was lacking. The newly proposed four-stage system-based on the Mercer Rang model-describes the progression of lower limb pathology and supports diagnosis, treatment planning, and research. STAGE 1: Hypertonia: From birth to about 6 years, spasticity and delayed motor development predominate; contractures are rare. Early intervention and spasticity man

Hindlimb Suspension and Subsequent Weight-Loading on Femoral Morphology, Knee Joint Alignment, and Patellar Tracking in Growing Rats.

OBJECTIVES: Reduced weight-loading on the lower extremities during growth induces abnormal bone morphology and malalignment, but the effects of reloading on these abnormalities remain unknown. We aimed to determine the specific timeframes in which hindlimb suspension (HS) during growth induces abnormal femoral morphologies and knee malalignments present at skeletal maturity in rats. METHODS: Four-week-old female rats were subjected to HS for one, two, or four weeks using the tail suspension method, followed by reloading. Age-matched rats were used as controls. At 12 weeks of age, femoral morp

Optimization of Revision Hip Arthroplasty Workflow by Means of Detailed Pre-Surgical Planning Using Computed Tomography Data, Open-Source Software and Three-Dimensional-Printed Models.

BACKGROUND: In revision hip arthroplasty (RHA), establishing the center of rotation (COR) can be technically challenging due to the acetabular bone destruction that is usually present, particularly in severe cases such as Paprosky type II and III defects. The aim of this study was to demonstrate the use of open-source medical image reconstruction software and low-cost 3D anatomical models in pre-surgical planning of RHA. METHODS: A total of 10 patients, underwent RHA and were included in the study. Computed tomography (CT) scans were performed for all cases, before surgery and approximately 1
