First experience in Italy with a new transcutaneous bone conduction implant.

Since 2011, transcutaneous bone-anchored auditory implants have been an alternative to the classic percutaneous implant (Baha) for bilateral conductive/mixed hearing loss that cannot be corrected by surgery. Recently, a new transcutaneous device has been approved for clinical use. Its internal component is made of the classic titanium Baha fixture, coupled to a 27 mm diameter subcutaneous circular magnet. The external component includes a second circular magnet 29 mm in diameter and a digital sound processor. To date, there are no reports describing the results of the application of this devic

[Recovery of bone conduction hearing level after stapes surgery in otosclerosis].

Although otosclerosis is essentially a middle ear disease, patients with this disease often exhibit mixed hearing loss. This discrepancy is thought to be explained by the following mechanism: the resonance of the ossicular chain is disturbed by the fixation of stapes, leading to the attenuation of inertial bone conduction. The bone-conduction hearing level usually recovers after stapes surgery. We herein studied the change in air- and bone-conduction hearing after stapes surgery in patients with otosclerosis. Six consecutive patients with otosclerosis who underwent stapes surgery in our depart

Ossicular chain reconstruction improves bone conduction threshold in chronic otitis media.

This study aimed to assess the elevation of bone conduction threshold in patients with chronic otitis media and to investigate the mechanism of this phenomenon. One hundred and six patients with unilateral chronic otitis media who had undergone a tympanomastoidectomy were reviewed retrospectively. The differences in the bone conduction thresholds between the diseased and normal sides were assessed and compared according to the duration of the disease and the presence of cholesteatoma. Post-operative changes in the bone conduction threshold were also assessed. The mean bone conduction threshold

[Late deterioration in bone conduction after stapes surgery: a retrospective analysis].

In spite of the well-consolidated technique that otosclerosis surgery has built up, there are several aspects that have yet to be satisfactorily explained. One of these is the greater long-term "vulnerability" that appears to characterize an ear that has been operated on compared to a healthy one. In searching for a feasible explanation of this phenomenon and its therapeutic implications, a retrospective analysis of 26 cases was carried out on patients who had been operated between 1966 and 1995 and had come to our attention between 1989 and 1999 due to a rapid, late deterioration in bone cond

Bone conduction experiments in animals - evidence for a non-osseous mechanism.

Bone conducted stimuli are used to differentiate between conductive and sensori-neural hearing loss. It has been thought that the main route for the transfer of vibratory energy from the point of application of the bone vibrator on the skull to the inner ear is completely osseous. An additional mechanism may play a prominent role. In rats, a bone vibrator was applied to the skull and also directly on the brain, after removing bone (a craniotomy), exposing the brain. Auditory nerve-brainstem evoked response (ABR) could be elicited not only with the vibrator on bone, but also with the vibrator d

Elevation of bone conduction threshold in children with middle ear effusion.

A retrospective study of children having otitis media with effusion revealed fluctuations in bone conduction thresholds as well as in air conduction thresholds. Previous investigations in this area presented both low- and high-tone bone conduction hearing loss which were reversible. We conducted a detailed study including complete otologic, audiologic and tympanometric evaluation of 27 (41 ears) children who had fluctuating bone conduction hearing loss. From these audiologic examinations, 3 types of bone conduction hearing loss could be classified: high-tone, low-tone and flat-type bone conduc

Evoked otoacoustic emissions (EOAE) and bone conduction stimulation. A preliminary report.

Otoacoustic emissions were evoked (EOAE) in 6 normal-hearing subjects by both bone (BCS) and air conduction stimulation (ACS). Four subjects affected by unilateral otosclerosis were also examined in order to determine the role of the ossicular chain in EOAE transmission to the eardrum. In normal-hearing subjects, EOAE by BCS showed the same characteristics as those evoked by ACS. The morphological features remained unchanged over a period of 4 months and their amplitude increased non-linearly with increasing stimulus intensity. In subjects with unilateral otosclerosis before surgery, no EOAE c

Bone-conduction force values for the sensorineural acuity level (SAL) test.

The sensorineural acuity level (SAL) test has the advantages of eliminating the questions of when to mask and how much masking to use in audiometry, greater efficiency in assessing cochlear sensitivity than conventional bone-conduction (bc) testing, and fewer and less serious errors in measuring cochlear sensitivity. However, a major limitation of the SAL test is that norms are generally determined using the time-consuming real-ear calibration method. A more satisfactory procedure is to establish normative force levels using an artificial mastoid. Normal-hearing young adults (N:18) were tested
