Femoral lengthening in achondroplasia: magnitude of lengthening in relation to patterns of callus, stiffness of adjacent joints and fracture.

Extensive limb lengthening may be indicated in achondroplastic patients who wish to achieve a height within the normal range for their population. However, increasing the magnitude of lengthening is associated with further complications particularly adjacent joint stiffness and fractures. We studied the relationship between the magnitude of femoral lengthening and callus pattern, adjacent joint stiffness and fracture of the regenerate bone in 40 femoral lengthenings in 20 achondroplastic patients. They were divided into two groups; group A had lengthening of less than 50% and group B of more t

Factors influencing bone regenerate healing in distraction osteogenesis.

BACKGROUND: The dynamics and effectiveness of new bone formation in the Ilizarov method varies according to different factors. Some of them may result in substantial prolongation of treatment time. The lengthening index (LI), defined as the number of months in a frame necessary to achieve 1 cm lengthening, is the main indicator of bone healing potential. The purpose of the study was to assess the effectiveness of distraction osteogenesis by analysing the lengthening index (LI) in relation to: age, limb length discrepancy, aetiology, bone segment involved, axial correction and lengthening achie

The reverse planning method for lengthening of the lower limb using a straight intramedullary nail with or without deformity correction. A new method.

OBJECTIVE: Lengthening of the lower limb with or without correction of angular deformity using an intramedullary nail with new correction strategies to restore or maintain a normal mechanical axis and orientation of the joints compared to traditional strategies using external fixation. Postsurgical lengthening will occur along the axis of the straight nail. PLANNING TECHNIQUE: The "reverse planning method" starts with the "ideal" final result of the projected correction and goes back to the preexistent deformity. The mechanical axis of the lower limb as well as the bone and joint orientation

[Upper limb lengthening. 47 clinical cases].

Between 1989 and 1993, the authors treated 39 patients by progressive bone lengthening in a total of 47 hands (17 post-traumatic and 30 congenital deformities). The Hoffman distractor was used in 34 cases and the Orthofix device was used in 13 cases. The percentage of lengthening obtained, compared to the initial bone length, was 87% for the hand skeleton and 47% for the forearm bones in congenital malformations while only 51% was obtained in post traumatic cases due to their older age and frequently associated skin problems. In 4 (8.5%) of these post-traumatic cases, the skin conditions had t

Extensive limb lengthening in Ollier's disease: 25-year follow-up.

A case of extensive lower limb lengthening (32 cm) in a 14-year-old male patient with Ollier's disease is reported. A varus deformity of the femur and a valgus deformity of the tibia were evident. The femur was successfully lengthened 22 cm by metaphyseal distraction, and the tibia was lengthened 10 cm by two-stage distraction-compression method with a cylindrical bone allograft. Ilizarov's distraction device was used. Radiologically, a good bone regenerate was formed. Host bone has incorporated (like sarcophagi) the allograft of tibia. No evidence of vascular or neural disturbances was found.

Limb lengthening combined with deformity correction in children with the Taylor Spatial Frame.

It is difficult to replicate the regular rate and rhythm described by Ilizarov while lengthening with the Taylor Spatial Frame. The purpose of the study was to examine whether this breach of Ilizarov's principles has any deleterious effect on the ability of children to make healthy regenerate bone. A retrospective case-control study was performed comparing pediatric patients undergoing primarily lengthening with Taylor Spatial Frame rings and struts, and patients undergoing lengthening with Taylor Spatial Frame rings and Ilizarov clickers. Fifteen patients had primarily lengthening with Taylor

Bifocal Lengthening with Monorail External Frame for Severe Shortening of the Femur: A Case Report.

AIM AND BACKGROUND: Femoral lengthening using a monorail external frame often involves creating two simultaneous distraction sites. This approach allows for a better distribution of the osteogenesis process and can reduce the overall treatment duration, particularly in cases of severe shortening. However, the literature on bifocal femoral lengthening is limited, with reported cases rarely exceeding 10 cm of lengthening. This case report aims to describe the treatment and outcome of a patient who underwent bifocal lengthening with a monorail external frame for severe femoral shortening. CASE D

Skin behavior during leg lengthening in patients with achondroplasia and hypochondroplasia: a short-term observation during leg lengthening.

BACKGROUND: Although distraction osteogenesis is a recently established method of limb lengthening, it is not well known how soft tissues behave during bone lengthening. Because skin is tissue that can be easily and directly observed, we analyzed skin extension during limb lengthening to study the condition of soft tissues. METHOD: We examined three achondroplastic and two hypochondroplastic patients who underwent bilateral tibial lengthening with unilateral bone lengtheners. To analyze skin extension during leg lengthening, the bilateral skin surface was marked in a latticework pattern, star

Reconstruction of Bony Defects With Motorized Intramedullary Nails After Tumor Resection.

Motorized intramedullary lengthening nails allow for transport of a bone segment for limb lengthening, deformity correction, healing of nonunion, and intercalary distraction osteogenesis. Resection of tumors involving the bone can result in substantial defects that require reconstruction. Use of these nails allows for a biologic reconstruction with the incorporation of allograft or by distraction osteogenesis. Limb lengthening after an internal hemipelvectomy where the hip joint is resected can be performed to improve gait, decrease pain, and prevent the need for a custom shoe or shoe lift. Us

Distraction osteogenesis in combined sequential use of external fixation and nailing (lengthening and then nailing): An experimental study in rabbits.

Limb lengthening relies on the process of distraction osteogenesis. The active periosteal bone formation has been detected in clinical practice with a lengthening and then nail (LATN) technique but has not been confirmed by experimental studies to date. The aim of this study is to compare the tissue regeneration of the distraction regenerate during tibial lengthening in rabbits using a LATN technique. This study was performed on 54 mature rabbits of the Soviet Chinchilla breed, which were divided into three groups of 18 animals. In group 1 (control), the tibia was lengthened in an external fix

The use of flexible intramedullary nails in limb lengthening.

Lower and upper limb length discrepancy and deformity, congenital or acquired, are amongst the most common conditions in children for orthopaedic referral. Over the last twenty years, surgical techniques have evolved in an attempt to decrease Healing Index and minimise these complications. Areas covered: The flexible intramedullary nailing (FIN) is a minimally invasive intramedullary osteosynthesis. In combination with an external fixator, it is appropriated for pediatric bone lengthenings and lengthening of bones of small diameter in adults. In the study the Scopus and/or PubMed indexed publi

[Pédiatrie - Internal lengthening nails: a novel method of lower limb reconstruction in children].

Intramedullary lengthening nails for lower limb reconstruction in children are a rapidly growing field and represent an alternative to external fixators. Today, two types of intramedullary lengthening nails are used: motorized (FITBONE) and magnetically driven (PRECISE) bone lengthening nails. Indications for limb lengthening nails are: 1) Significant lower limb length discrepancy; 2) Short stature and deformities due to musculoskeletal or dysplastic syndromes; 3) Constitutional short stature. Intramedullary lengthening surgery is challenging and represents a safe and reliable mode of treatmen
