Treatment of pathological fractures of the humeral shaft due to bone metastases: a comparison of intramedullary locking nail and plate osteosynthesis with adjunctive bone cement.

In a retrospective study 37 patients were surgically treated for 38 fractures (27 actual and 11 impending) caused by metastatic lesions of the humerus shaft. A comparison between plate osteosynthesis with bone cement (20 cases) and intramedullary nailing (18 cases) was made. There was no mortality related to the surgical procedure. The patients' survival rate was 61% after 3 months and 44% after 6 months; six were alive after 1 year. Overall, a subjective and objective relief of pain was achieved in 92% and 79%, respectively. Restoration of arm function was improved in 95%. The operative cours

Nail-to-bicondylar ratio, but not CPAK classification, is associated with arthrosis and deformity progression after retrograde fixator-assisted nailing for distal femoral deformity in metabolic bone disease.

BACKGROUND: Distal femoral deformities are common in metabolic bone disease and often require surgical correction. Retrograde Fixator-Assisted Nailing(r-FAN) stabilizes the femoral axis, yet the influence of nail-to-bicondylar ratio (NBR) and coronal-plane knee alignment patterns(CPAK) on deformity and osteoarthritis(OA) progression remains unclear. METHODS: We retrospectively evaluated 30 limbs from 17 patients with distal femoral deformities treated with r-FAN, with a mean follow-up of 10.5 ± 4.6 years. Pre- and postoperative radiographs were analyzed for mechanical lateral distal femoral a

Locking retrograde nail, non-locking retrograde nail and plate fixation in the treatment of distal third femoral shaft fractures: radiographic, bone densitometry and clinical outcomes.

BACKGROUND: Distal third femoral shaft fractures are characterized by increasing incidence and complexity and are still considered a challenging problem (high morbidity and mortality). No consensus on best surgical option has been achieved. This study aims to investigate radiographic, mineral bone densitometry and clinical outcomes of locking retrograde intramedullary (LRN) nailing, non-locking retrograde intramedullary nailing and anatomical locking plate to surgically treat distal third femoral shaft fractures in young adults. Our hypothesis was that there is no significant statistical diffe

Bone Union and Safety of the Long Chimaera Nail in Extracapsular Femoral Fractures: A Retrospective Multicenter Study.

BACKGROUND: Proximal femoral fractures (PFFs) are a major source of morbidity and mortality in the elderly. Although cephalomedullary nailing (CMN) systems are widely used for the treatment of extracapsular/trochanteric PFFs, the optimal implant choice remains debated - in particular, in the case of stable trochanteric fracture patterns. ChimaeraTM (Orthofix, Italy) is a novel CMN system designed to enhance fracture compression and rotational stability through a telescoping lag screw and optional anti-rotational element. This retrospective, multicenter, post-market study evaluated the clinical

Wedge fragments: no significant impact on bone healing in femoral shaft fractures treated with intramedullary nailing.

INTRODUCTION: Intramedullary nailing (IMN) is the treatment of choice for femoral shaft fractures with wedge-shaped fragments. However, the management of wedged fragments remains controversial. This study investigated whether wedged fragments affect bone union rates in patients treated with IMN. The primary hypothesis was that wedged fragments would not significantly affect bone union when proper IMN principles were followed. MATERIALS AND METHODS: Between March 2014 and June 2023, 162 patients were diagnosed with femoral shaft fractures (AO/OTA 32-B type) accompanied by wedge fractures. The

Nail manifestations reflecting abnormalities of bone and soft tissues in interphalangeal joints affected by psoriatic arthritis.

INTRODUCTION: Psoriatic arthritis (PsA) presents various imaging abnormalities in joints and diverse nail changes, but the link between nail and joint findings remains unclear. This study aimed to gain a better understanding of the relationships between nail manifestations and abnormal articular architectures by investigating the associations between nail abnormalities and articular findings in PsA-affected interphalangeal joints. METHODS: A total of 106 nails adjacent to PsA-affected distal interphalangeal joints were examined in 29 patients who underwent comprehensive systemic evaluation an

Influence of Bone Cement Augmentation on Complications in Cephalomedullary Nail Fixation of Geriatric Intertrochanteric Hip Fractures.

INTRODUCTION: The purpose of this study was to determine if augmentation of the helical blade with polymethylmethacrylate bone cement decreases the rates of varus cut-out and medial perforation in geriatric intertrochanteric hip fracture fixation. METHODS: This was a retrospective comparative cohort study at two urban Level I trauma centers. Patients with an intertrochanteric hip fracture (classified as AO 31A1-3) who were treated with the TFN-Advanced Proximal Femoral Nailing System (TFNA) from 2018 to 2021 were eligible for the study. Medical records and post-operative radiographs were revi

Finite element analysis of proximal femur bionic nail for treating intertrochanteric fractures in osteoporotic bone.

This study compared the biomechanical characteristics of proximal femur bionic nail (PFBN) and proximal femoral nail antirotation (PFNA) in treating osteoporotic femoral intertrochanteric fractures using finite element analysis. Under similar bone density, the PFBN outperforms the PFNA in maximum femoral displacement, internal fixation displacement, stress distribution in the femoral head and internal fixation components, and femoral neck varus angle. As the bone density decreases, the PFBN's biomechanical advantages over PFNA become more pronounced. This finding suggests that the PFBN is supe

Induced membrane technique combined with a retrograde intramedullary nail for the treatment of infected bone defects of the ankle.

In this study, we treated infected ankle bone defects with the induced membrane two-stage technique. The ankle was fused with a retrograde intramedullary nail in the second stage, and the aim of this study was to observe the clinical effect. We retrospectively enrolled patients with infected bone defects of the ankle admitted to our hospital between July 2016 and July 2018. In the first stage, the ankle was temporarily stabilized with a locking plate, and antibiotic bone cement was used to fill the defects after debridement. In the second stage, the plate and cement were removed, the ankle was

Aseptic femoral nonunion treated with exchange locked nailing with intramedullary augmentation cancellous bone graft.

BACKGROUND: Closed reamed locked intramedullary nailing has been the treatment of choice for most of femoral shaft fractures. A high union rate with a low complication rate is generally predictable. For an aseptic femoral shaft nonunion with a prior inserted intramedullary nail, exchange nailing is one of favored surgical techniques for treatment. However, a greatly varied success rate of 72-100% has been reported. To improve the success rate of exchange femur nailing, a modified bone grafting technique was developed. The purpose of this retrospective study intended to evaluate outcomes of suc

Is the compressive nail method capable of accelerating bone union in patients with femoral distal diaphysis fracture?

OBJECTIVES: This study was conducted to evaluate clinically and radiologically the patients for whom compressive and non-compressive intramedullary nails were applied and to analyse the compression effectiveness in adult femur distal diaphysis fractures. HYPOTHESIS: The compressive locked nails shortened the union period in the distal diaphysis fractures, and it was found to be an effective method, as the pseudoarthrosis rate was lower. MATERIAL AND METHODS: A retrospective study was conducted among 59 patients for whom the compressive and non-compressive IMNs were applied to treat femur dis

Efficacy of a tip of the big toe remodeling in the distal nail embedding with bone overgrowth of the distal phalanx.

INTRODUCTION: Distal nail embedding due to hyponychium hypertrophy can be caused by traumatic or surgical avulsion of the nail. As a consequence of these changes, the nail plate is blocked through the deformed tip of the toe. Changes that occur at the tip of the big toe are due to bone growth on the dorsal surface of the distal end of the distal phalanx. This study aimed to present a surgical technique for the treatment of hypertrophy of the tip of the toe and evaluate its effectiveness. MATERIAL AND METHODS: The surgical technique involved remodeling of the tip of the big toe, with removal o

Open Intramedullary Nailing for Segmental Long Bone Fractures: An Effective Alternative in a Resource-restricted Environment.

BACKGROUND: Closed, locked intramedullary nailing has been accepted as the gold standard in the care of femoral fractures, with reported union rates as high as 98-100%. Closed, locked intramedullary nailing often requires expensive equipment which is a challenge in developing countries. Segmental long bone fractures are often a result of high-energy trauma and hence often associated with a lot of injuries to the surrounding soft tissues. This consequently results in higher rates of delayed or nonunion. This study was proposed to review the outcome of management of segmental fractures with lock

Outcome of Distal Both Bone Leg Fractures Fixed by Intramedulary Nail for Fibula & MIPPO in Tibia.

BACKGROUND: Fractures of the distal third of the tibia are mostly associated with a fibular fracture that often requires fixation. The preferred treatment of distal tibial fracture is the minimally invasive percutaneous plate osteosynthesis (MIPPO) procedure. However, there are no clear cut guidelines on fixation of the fibular fracture and currently most orthopedic surgeons use a plate osteosynthesis for the fibula as well. A common complication associated with dual plating is an increased chance of soft tissue necrosis, infection, and in some cases resulting in an exposed implant. We conduct

Treatment of femoral and tibial diaphyseal nonunions using reamed intramedullary nailing without bone graft.

INTRODUCTION: Intramedullary nailing has long been used successfully in the treatment of aseptic nonunions of the femur and tibia. However, recently the efficacy of reamed intramedullary nailing in the treatment of nonunions of the femur has been questioned by some publications reporting unfavourable results. The purpose of this study is to evaluate the treatment results of femoral and tibial diaphyseal nonunions with intramedullary nailing. PATIENTS AND METHODS: We retrospectively reviewed thirty-two patients with femoral or tibial diaphyseal nonunions who were treated with reamed intramedul
