Tethering technique using bone screws and wire for chronic mandibular dislocation: a preliminary study of refractory cases.

A retrospective study was performed to determine the efficacy of a tethering procedure developed to achieve a more rigid fixation and more reliable outcome in patients with refractory dislocation of the temporomandibular joint. The cases of eight patients with dementia and systemic diseases who underwent this technique were reviewed. In these eight patients, the condyles of 13 joints were ligated using wire between screws placed in the eminence and condylar head. Additional screw-wire ligations were applied to reinforce the restraint of movement in five of the 13 joints with suspected uncontro

Aspects of internal fixation of fractures in porotic bone. Principles, technologies and procedures using locked plate screws.

Fractures of the bones of elderly people occur more often and have a more important effect because of a generally diminished ability to coordinate stance and walking. These fractures occur at a lower level of load because of lack of strength of the porotic bone. Prompt recovery of skeletal support function is essential to avoid respiratory and circulatory complications in the elderly. To prevent elderly people from the risks of being bedridden, demanding internal fixation of fractures is required. The weak porotic bone and the high level of uncontrolled loading after internal fixation pose com

Allograft Strut Fixation to Humeral Bone: A Biomechanical Comparison of Suture-Tape Cerclage, Wire Cerclage, and Screws.

BACKGROUND: Shoulder arthroplasty is increasingly utilized to treat shoulder arthritis, cuff tear arthropathy and proximal humerus fractures. Onlay allograft cortical strut augmentation is a common technique used in the management of humeral bone loss, periprosthetic fractures, and during revision surgeries to enhance stability and potentially reconstitute bone. This study investigated the biomechanical performance of three commonly used strut fixation methods used to secure an allograft strut to host diaphyseal bone: suture-tape cerclage, wire cerclage and lag screw fixation. METHODS: Eight

S3 sacral-alar iliac screws application in partial sacral resection for an aneurysmal bone cyst: illustrative case.

BACKGROUND: Sacropelvic fixation is used in the management of deformity, neoplasms, and fractures. S2-alar-iliac (S2AI) screws have evolved as a technique for sacropelvic fixation to address disadvantages associated with traditional iliac screws. In select patients, S2AI screw placement is not feasible due to patient anatomy, trauma, or osteolytic lesions. OBSERVATIONS: A 15-year-old girl presented with a 3-month history of right S1-2 radicular pain and right dorsiflexion and plantar flexion weakness. Imaging revealed a right sacral alar aneurysmal bone cyst. The patient underwent right S1-3

Fixation stability comparison of bone screws based on thread design: buttress thread, triangle thread, and square thread.

BACKGROUND: The influence of thread profile on the fixation stability of bone screws remains unclear. This study aimed to compare the fixation stability of screws with different thread profiles under several loading conditions. METHODS: Bone screws that differed in thread profile (buttress, triangle, and square thread) only were made of stainless steel. Their fixation stabilities were evaluated individually by the axial pullout test and lateral migration test, besides, they were also evaluated in pairs together with a dynamic compression plate and a locking plate in polyurethane foam blocks u

Arthrodesis Using Bilateral Dual Iliac Screws with Autologous Iliac Bone Transfer for the Treatment of Pyogenic Sacroiliitis.

Pyogenic sacroiliitis is a relatively rare condition that often leads to surgical treatment, including debridement and arthrodesis. Here we introduce a new surgical technique using bilateral dual iliac screws to secure early ambulation and maximal fusion success rate for the treatment of pyogenic sacroiliitis. We retrospectively reported a case and technical reports of pyogenic sacroiliitis treated by a new bilateral dual iliac screw fixation arthrodesis technique using radiologic outcomes, including plain X-rays and MRI scans, as well as outcomes based on the visual analogue scale for pain me

Reconstruction of iliac crest defect after autogenous harvest with bone cement and screws reduces donor site pain.

BACKGROUND: The iliac crest is the most common autogenous bone graft donor site, although associated with postoperative pain, functional disability, cosmesis, morphology and surgical satisfaction. We assessed each aspect above by comparing iliac crest reconstruction with bone cement and screws following harvest with no reconstruction. METHODS: We evaluated patients who underwent large iliac crest harvesting, including ten patients who underwent iliac crest defect reconstruction with bone cement and cancellous screws (R group) and ten randomly matched patients without reconstruction (NR group)

Cardiac Perforation Caused by Bone Cements as a Complication of Cement Augmented Pedicle Screw Fixation Using the Fenestrated Pedicle Screw: A Case Report.

Cement-augmented fenestrated pedicle screw fixation is becoming more popular for osteoporotic patients. Although several reports have been published on leakage-related problems with bone cement, no cases of cardiac perforation after cement-augmented pedicle screw fixation have been reported. We present a case of cardiac perforation after cement-augmented fenestrated pedicle screw fixation. A 67-year-old female was admitted to our hospital with complaints of dyspnea and chest pain after lumbar surgery. She had been treated with L4-5 lumbar interbody fusion and percutaneous pedicle screw fixatio

Optimized Screw Trajectory for Lumbar Cortical Bone Trajectory Pedicle Screws Based on Clinical Outcome: Evidence Favoring the Buttress Effect Theory.

BACKGROUND: Cortical bone trajectory (CBT) is a relatively new technique for pedicle screw insertion in the field of spine surgery. Previous studies have demonstrated the significantly better pullout and toggle characteristics the new method offers, and it appears to have certain advantages over the widely used traditional trajectory. The mechanism of the pullout and toggle characteristics still remains unknown. PURPOSE: To report the medium- to long-term follow-up findings of patients who underwent posterior lumbar interbody fusion or posterior lumbar fusion (PLIF/PLF) at our institution and

Mechanical strength and optimal site of placement of a threaded bone screw assessed on the basis of the screw breakage for non-union of the scaphoid: a biomechanical study.

We have performed the surgical procedure on the patients with the fracture of the scaphoid using mini-acutrak screws (ACUMED, Inc.). A 25-year-old man was injured in a fall. Surgical treatment for non-union of the scaphoid using a mini-acutrak screw was performed. However, this screw was broken, despite the absence of any obvious trauma mechanism. A structural examination was performed with a metal microscope and a hardness test was performed. A hardness evaluation was conducted on this broken screw and similar screws. Cannulated screws having diameters of 2.7 and 3.5 mm were also evaluated in

Placing a bone graft more posteriorly may reduce the risk of pedicle screw breakage: analysis of an unexpected case of pedicle screw breakage.

Telemeterized internal spinal fixation devices were implanted in a patient with degenerative instability and a narrow spinal canal in order to measure the fixator loads during daily activities. Anterior interbody fusion was performed three weeks later. During walking, the typical maximum flexion bending moments were 10 N m in the left and 5 N m in the right fixator. On removal of the implants three months later, a fatigue fracture was found not on the high loaded left side but in the upper right pedicle screw. The crack started on the caudal side of the cross-sectional area and progressed cran

Biomechanical comparison of suture and screw fixation in PCL avulsion with variable bone quality.

Various fixation techniques, including screws and suture-based methods, are used for posterior cruciate ligament (PCL) avulsion fractures. This study compared the biomechanical effects of different suture configurations in simulated PCL avulsion fractures. Twenty cadaveric knees were assigned to four fixation groups: antegrade screw (A), 1 suture tape with 2 tunnels (B), 2 suture tapes with 2 tunnels (C), and 2 suture tapes with 3 tunnels (D). Biomechanical testing included cyclic loading and load-to-failure tests, with elongation, stiffness, yield load, and failure modes recorded. No signific

The Application of an Allogenic Bone Screw for Stabilization of a Modified Chevron Osteotomy: A Prospective Analysis.

Background: Distal first metatarsal osteotomies are commonly performed operative procedures for hallux valgus deformity, and usually involve fixation with a metal screw. However, various bioabsorbable osteosynthesis materials have been in use for a number of years. One recent innovation is the Shark Screw®, a human cortical bone allograft. This study aimed to evaluate the efficacy and safety of this allogeneic screw in the stabilization of Reversed L-Shaped osteotomy, a modified Chevron osteotomy. Methods: In a prospective study, 15 patients underwent a Reversed L-Shaped osteotomy stabilized w

Biodegradable magnesium fixation screw for barrier membranes used in guided bone regeneration.

An ideal fixation system for guided bone (GBR) regeneration in oral surgery must fulfil several criteria that includes the provision of adequate mechanical fixation, complete resorption when no longer needed, complete replacement by bone, as well as be biocompatible and have a good clinical manageability. For the first time, a biodegradable magnesium fixation screw made of the magnesium alloy WZM211 with a MgF2 coating has been designed and tested to fulfill these criteria. Adequate mechanical fixation was shown for the magnesium fixation screw in several benchtop tests that directly compared

Correlation of bone density to screw loosening in dynamic stabilization: an analysis of 176 patients.

Although osteoporosis has negative impacts on lumbar fusion, its effects on screw loosening in dynamic stabilization remain elusive. We aimed to correlate bone mineral density (BMD) with screw loosening in Dynesys dynamic stabilization (DDS). Consecutive patients who underwent 2- or 3-level DDS for spondylosis, recurrent disc herniations, or low-grade spondylolisthesis at L3-5 were retrospectively reviewed. BMD was assessed by the Hounsfield Unit (HU) in vertebral bodies (VB) and pedicles with and without cortical bone (CB) on pre-operative computed tomography (CT). Screw loosening was assesse
