Comparative finite element analysis of fixation strategies for sports-related tibial tuberosity avulsion fractures in adolescents: biomechanical evidence for optimized clinical decision-making.

OBJECTIVE: This finite element study aimed to biomechanically compare screw fixation vs. K-wire fixation for adolescent tibial tubercle avulsion fractures (TTAF), investigating the differential biomechanical outcomes and physeal insult associated with each fixation method. METHODS: A finite element model of an adolescent tibia with a Modified Ogden type III fracture was developed. Three fixation types-(A) screws, (B) two non-crossing K-wires, and (C) four crossing K-wires-were tested both as standalone constructs and in their tension band-augmented counterparts. Simulations applied a 1,654 N

Spider plate fixation: no significant improvement in limited wrist arthrodesis.

Using the Spider plate fixation device, limited wrist arthrodesis was performed on 30 patients over a 2-year period. Developed as an alternative to Kirschner wires and compression bone screws in limited intercarpal fusions, its rigid fixation was expected to yield better results. Five hand surgeons treated 24 men and 6 women with an average age of 45 years. Diagnosis included the following: scaphoid nonunion advanced collapse, scapholunate advanced collapse, scapholunate dissociation, midcarpal arthrosis, and Kienböck disease. The 8-hole regular Spider and 6-hole mini-Spider plates were applie

Stress fracture of the fifth metatarsal base caused by tension band wiring: an isolated case report.

Tension band technique has been described as a successful method of treatment for the fractures of the proximal part of the fifth metatarsal bone. A high rate of union and a low rate of complications have made this mode of treatment popular, especially in patients with nonunion and high-level athletes with stress fracture of the proximal fifth metatarsal bone. The authors report a case of an avulsion fracture of the base of the fifth metatarsal bone treated with tension band wiring fixation, which resulted in a stress fracture 1 year after the tension band wiring. The authors recommend a K-wir

The effect of lateral hinge fracture with hinge hole and protective K-wire for medial opening-wedge high tibial osteotomy by compression testing and finite element analysis.

BACKGROUND: Medial open-wedge high tibial osteotomy (MOWHTO) is effective for treating medial-compartment knee osteoarthritis but carries a risk of lateral hinge fractures (LHF), compromising stability and outcomes. Hinge holes and protective K-wires reduce LHF by lowering stress and enhancing lateral support. However, their combined effect has not been evaluated. This study investigates whether using both techniques together can more effectively reduce lateral cortical bone stress and prevent LHF during MOWHTO. METHODS: This study combined finite element analysis (FEA) and in-vitro compressi

A New Technique for Placing Pedicle Screws Into Bone Cement-Strengthened Vertebrae Involving Drilling and Melting.

OBJECTIVE: Vertebroplasty with non-degradable polymethyl methacrylate bone cement is a common procedure in spine surgery. However, this bone cement reinforcement of the vertebral body could affect subsequent spinal surgeries, especially for pedicle screw insertion. This study proposes a novel method of inserting pedicle screws into bone cement-reinforced vertebral bodies through thermal softening via Kirschner wires (K-wires) drilling, and conducts preliminary in vitro experiments to assess its feasibility and safety. METHODS: This study includes bone cement block experiments and in vitro goa

Does Irrigating While Drilling Decrease Bone Damage?

BACKGROUND: Heat generated during bone drilling may be associated with thermal necrosis and direct damage, leading to complications after surgery. This preclinical study evaluates the in vivo effects of saline irrigation, drilling device type, and device sharpness on heat generation and bone damage in viable cortical bone. METHODS: Bicortical drilling of each tibial diaphysis from anesthetized research dogs was performed to evaluate temperature and bone damage using five different devices with or without saline irrigation. RESULTS: Saline irrigation and sharp drill bits were associated with

Treatment of closed unstable metacarpal fractures using percutaneous transverse fixation with Kirschner wires.

BACKGROUND: The vast majority of metacarpal bone fractures are stable and are treated conservatively; however, when surgical intervention is to be preferred, many different operative procedures have been described. The purpose of this prospective study was to assess the anatomic and functional outcome of percutaneous transverse fixation with Kirschner wires (K-wires) of closed metacarpal neck, shaft. and intra-articular fractures of the base of the fifth metacarpal. METHODS: Twenty-four consecutive patients with 25 closed metacarpal fractures were treated with this technique between 1999 and

Open Reduction and Intrafragmentary Compression Fixation with External Fixator (the Ichi-Fixator) Treatment of Distal Phalangeal Nonunion.

The nonunion of distal phalangeal communized fracture is relatively rare in hand fractures. However, if it occurred, the surgical treatment is quite difficult because of small piece of fragmentations. We developed a new fixation method that involves the insertion of two wires and external wire compression fixation using a metal clamp. The aim of this technique was to increase the compression force between fragments and rigidity of conventional percutaneous Kirschner wire fixation. Here, we present a patient with the nonunion of distal phalangeal communized fracture who was satisfactorily treat

Bacteriophage mediated killing of Staphylococcus aureus in vitro on orthopaedic K wires in presence of linezolid prevents implant colonization.

BACKGROUND: Infections of bone and joint tissues following arthroplasty surgeries remain a major challenge in orthopaedic settings. Methicillin resistant Staphylococcus aureus (MRSA) is recognised as an established pathogen in such infections. Combination therapy using linezolid and bacteriophage impregnated in biopolymer was investigated in the present study as an alternative strategy to prevent MRSA colonisation on the orthopaedic implant surface. METHODOLOGY: Coating of stainless steel orthopaedic grade K-wires was achieved using hydroxypropylmethlycellulose (HPMC) mixed with phage alone,

Hemi-hamate arthroplasty versus transarticular Kirschner wire fixation for unstable dorsal fracture-dislocation of the proximal interphalangeal joint in the hand.

Following the introduction of the hemi-hamate arthroplasty (HHA) technique to our unit, we sought to evaluate the early clinical outcomes achieved with this method of fixation and compare these with simple trans-articular Kirschner wire (K-wire) fixation for dorsal fracture dislocations (DFD) of the proximal interphalangeal joint (PIPJ). Ninteen patients underwent fixation of these injuries with either K-wire fixation (12/19) or hemi-hamate bone grafting (7/19) between 2005 and 2011. At a mean follow-up of 14 weeks median arc of movement at the PIPJ was 65° (range 31° to 108°) following HHA an

Slight Elongation of the Scaphoid and Cancellous Bone Graft Without Compression for Treatment of Scaphoid Nonunions.

In treating scaphoid nonunion, we have developed a technique of bone grafting and elongation of the scaphoid stabilizing the construct with K wires without compression. Bony union was achieved in the large majority of scaphoids as demonstrated on computed tomography (CT) scans. We advocate slight lengthening of the scaphoid with bone graft and K-wire fixation without compression of the grafted bone when treating scaphoid waist nonunion.

Method of bone fixation in reattachment of amputations in the upper extremities.

The object of secure bone fixation in the reattachment of amputated fingers is early motion. The use of specially designed screws and Kirschner wires is essential for this objective. This becomes even more important in the reattachment of fingers than the upper arm or forearm. If there should be non-union of the bone, an autologous bone graft should be applied as a secondary procedure.

An exploratory study of gold wire implantation at acupoints to accelerate ulnar fracture healing in rats.

By causing long-term stimulation, gold wire implantation at acupoints has been used empirically to prolong the effects of acupuncture. This study shows that subcutaneous gold wire implantation at acupoints has long-term effects on bone regeneration in the rat ulna bone defect model.

Comparative outcome analysis of internal screw fixation and Kirschner wire fixation in the treatment of scaphoid nonunion.

In cases with difficult scaphoid screw placement due to small, fragile bone fragments, or transplants and insufficient space, the internal Kirschner wire fixation is a fallback option; however, controversy remains regarding its stability, safety, and outcome. Between 2001 and 2011, 95 patients were treated in our center (n = 80 with cannulated compression screws and n = 15 with Kirschner wires), and retrospectively analyzed. The outcome measurements included the analysis of patient data, union rate and analysis of functional measures, and quality of life. Bony reconstructions were performed wi

Preliminary evaluation of the Sternum Screw: a novel method for improved sternal closure to prevent dehiscence.

Sternal dehiscence is a relatively rare but serious complication of sternal closure with an unacceptable mortality and morbidity rate. The 6 mm cannulated Sternum Screws are a novel approach that aims to prevent dehiscence. The screws are placed on either side of the sternotomy and wire threaded through the cannula and closed customarily. The Sternum Screws make the bone non-penetrable at the same time retaining the familiarity of conventional stainless steel wire. This novel technique was tested in sheep sterna to compare its efficacy to conventional wire closure. Using tensile testing in a t

Excessive Derotational K-Wire Angulation Decreases Compression by Headless Compression Screw.

Background  Scaphoid fracture is the most common carpal bone fracture. Open reduction internal fixation of scaphoid fractures typically undergo stabilization by a single headless compression screw (HCS). During screw insertion, a derotational Kirschner wire (K-wire) is often placed for rotational control of the near and far fragment. Questions/Purposes  The aim of this study was to determine if there is an angle of derotational K-wire placement in relation to the axis of a HCS that compromises the amount of compression generated at a fracture site by the HCS. We hypothesize that increased off-

Significantly lower femoral neck growth in screw fixation of the asymptomatic contralateral hip in unilateral slipped capital femoral epiphysis.

There is an ongoing debate on which fixation technique should be preferred for the prophylactic fixation of the asymptomatic contralateral hip in slipped capital femoral epiphysis (SCFE). In the case of Kirschner-wire (K-wire) fixation, there is a possibility of secondary loss of fixation because of longitudinal growth of the physis, whereas in screw fixation, physeal growth of the femoral neck might be impaired. The aim of this matched-pair study was to compare the longitudinal growth of the femoral neck in screw fixation versus K-wire fixation of the asymptomatic contralateral hip in SCFE. A
