Observational study of the efficacy of supraclavicular brachial plexus block for arteriovenous fistula creation.

BACKGROUND AND AIMS: Arteriovenous (AV) fistula surgery is commonly performed for AV access for hemodialysis. However the ideal anaesthetic technique of choice remains debated. We aimed to assess operative conditions, vascular patency, and complication rate following AV fistula surgery with isolated brachial plexus block among end-stage renal disease (ESRD) patients. METHODS: This prospective, observational study included 214 patients undergoing AV fistula surgery under isolated supraclavicular brachial plexus block between January and December 2017. The diameters of the vessels both before a

Addition of clonidine or lignocaine to ropivacaine for supraclavicular brachial plexus block: a comparative study.

INTRODUCTION: Clonidine is used with local anaesthetics to improve analgesia. However, the improvement conferred when clonidine is used together with ropivacaine is controversial. Thus, the present study aimed to evaluate the improvement in analgesia when clonidine is used together with ropivacaine for supraclavicular brachial plexus block. METHODS: This was a prospective, randomised, double-blind controlled study. A total of 75 patients who were scheduled to undergo supraclavicular block were randomly assigned into three groups (i.e. clonidine, lignocaine and control groups) of 25. Patients

A retrospective study of the incidence of neurological injury after axillary brachial plexus block.

BACKGROUND: It has been suggested that performing a nerve block under general anesthesia, as customary in pediatric population, may predispose to nerve injury. However, few clinical data exist to either support or refute this assertion. METHODS: We retrospectively reviewed data on all patients who received an axillary block for upper extremity surgery in our institution during an eight-year period. The blocks were performed under sedation or general anesthesia, without using a nerve stimulator. Perioperative records from the Hand Surgery Unit Clinic were reviewed for postoperative complaints

Interscalene brachial plexus block for shoulder surgery in a patient with arthrogryposis multiplex congenita.

A five-year-old child with severe arthrogryposis multiplex congenita and malnutrition underwent surgery for chronic osteomyelitis of the head of the left humerus. The child had typical features of arthrogryposis multiplex congenita, including a difficult airway. Propofol was used for induction and maintenance. Spontaneous respiration was maintained with a nasal airway. Analgesia was provided with an interscalene brachial plexus block placed using a nerve stimulator. No opioid was given. The child had an uneventful recovery with good postoperative analgesia. The anaesthetic implications of arth

[Percutaneous catheterization of the newborn: control of pain and easy insertion with brachial plexus block].

The increasingly prolonged survival of extremely premature infants who required long-term venous access means that this is often a major problem in modern neonatal unit. The insertion of central venous catheters has become an established practice and the development of silastic catheters inserted by newer percutaneous techniques through a peripheral vein is, now, the choice technique in awake non sedated critically-ill infants. Such technique has an high percentage of failure. We have undertaken a retrospective study to determine whether the brachial plexus block performed via the axillary app

A new axillary approach for continuous brachial plexus block. A clinical and anatomic study.

Catheter insertion in the neurovascular space by axillary approach allows a continuous brachial plexus block and/or postoperative analgesia. We developed a perivenous technique whereby the approach to the neurovascular sheath is guided under fluoroscopy by a preopacified axillary vein. A randomized study compared this technique to the technique of Selander in ASA grade I-II patients scheduled for surgery or painful physiotherapy of the hand. The study was performed in 36 patients randomly divided into two groups. In Group 1 (n = 18), the catheter was placed according to the technique described

Efficacy and Safety of Phrenic-Nerve Sparing Blocks vs. Interscalene Brachial Plexus Block for Shoulder Surgery.

PURPOSE OF REVIEW: Shoulder arthroplasty is often associated with significant postoperative pain, making effective analgesia a crucial part of surgery day. The interscalene brachial plexus nerve block (ISB) is the gold standard for regional anesthesia in otherwise healthy patients. However, its use is limited by a high incidence of ipsilateral phrenic nerve paralysis, which may result in respiratory compromise, particularly in patients with chronic obstructive pulmonary disease (COPD), reduced pulmonary reserve, or preexisting diaphragmatic dysfunction. The incidence of hemidiaphragmatic paral

A patient with a history of syncope experiencing cardiac arrest after brachial plexus block.

A 65-year-old male with a history of hypertension and diabetes, experienced syncope and cardiac arrest during shoulder arthroscopy after receiving a brachial plexus block. The patient, who was sedated with dexmedetomidine, developed bradycardia after being positioned in the beach chair position, which lead to cardiac arrest. Prompt cardiopulmonary resuscitation restored the patient's heart rate and consciousness. This report discusses the possible causes, including the vasovagal reflex, dexmedetomidine effects, and Bezold- Jarisch reflex, and proposes preventive measures. Recognising high-risk

Bilateral brachial plexus blocks in a patient of hypertrophic obstructive cardiomyopathy with hypertensive crisis.

Hypertrophic obstructive cardiomyopathy (HOCM) is a challenge to anesthesiologists due to the complex pathophysiology involved and various perioperative complications associated with it. We present a 50-year-old man, a known case of HOCM, who successfully underwent emergency haemostasis, and debridement of the traumatically amputated right upper limb and the contused lacerated wound on the left forearm under bilateral brachial plexus blocks. His co-morbidities included hypertension (in hypertensive crisis) and diabetes mellitus. He was full stomach and also had an anticipated difficult airway.

Effect of addition of fentanyl to local anesthetic in brachial plexus block on duration of analgesia.

INTRODUCTION/CONTEXT: A comparative study was carried out to evaluate the analgesic efficacy and side effects of addition of fentanyl to local anesthetic undergoing surgeries on forearm and elbow. All patients were hemodynamically stable, and there were no serious side effects in any of the patients in both the groups. The difference in the mean duration of analgesia between the groups was statistically significant (P<0.01). So we can conclude- Addition of Fentanyl to local anaesthetic in brachial plexus block increases duration of analgesia. MATERIALS AND METHODS: Patients were randomly divi

Vertical infraclavicular block of the brachial plexus: effects on hemidiaphragmatic movement and ventilatory function.

BACKGROUND AND OBJECTIVES: Several case reports have suggested that block of the brachial plexus by the vertical infraclavicular approach influences hemidiaphragmatic movement and ventilatory function. These effects have not been evaluated in a prospective study. METHODS: Thirty-five consecutive patients scheduled for elective surgery under brachial plexus anesthesia were included. A vertical infraclavicular block was performed with ropivacaine 0.75%, 0.5 mL/kg. Ipsilateral hemidiaphragmatic movement was measured by ultrasonography at maximal forced inspiration and sniff. Forced vital capacit
