Effects of resistive breathing on exercise capacity and diaphragm function in patients with ischaemic heart disease.

BACKGROUND: Muscle weakness has been suggested to result from the deconditioning that accompanies decreased activity levels in chronic cardiopulmonary diseases. The benefits of standard exercise programmes on exercise capacity and muscular strength in disease and health are well documented and exercise capacity is a significant predictor of survival in patients with chronic heart failure (CHF). Selective respiratory muscle training has been shown to improve exercise tolerance in CHF and such observations have been cited to support the suggestion that respiratory muscle weakness contributes to

The breathing route dependence of ventilatory responses to hypercapnia and exercise is modulated by upper airway resistance.

OBJECTIVE: The ventilatory response to hypercapnia is greater breathing orally than nasally. METHODOLOGY: We hypothesize that this is due to higher nasal resistance to airflow compared with oral resistance. Seven normal male subjects were studied during both progressive hyperoxic hypercapnia (HC) and exercise (EX) until ventilation exceeded 40 L/min. Under each condition, subjects breathed via the nose only or the mouth only. For each breathing route, ventilation and pathway resistance were calculated simultaneously at the highest common exercise workload (140 +/- 20 watt; mean +/- SE) or the

Augmented hyperventilation via normoxic helium breathing does not prevent exercise-induced hypoxemia.

The purpose of this study was to determine if augmented hyperventilation produced via normoxic helium breathing would reduce exercise-induced hypoxemia (EIH). Seven highly trained endurance athletes with a mean maximum oxygen uptake of 65 ml.kg-1.min-1, performed two cycle ergometer tests to volitional exhaustion. During one of the tests the subjects breathed ambient air, while during the other they breathed normoxic helium (21% O2, 79% He). Mean maximum expired ventilation significantly (p < .05) increased from 139 L.min-1 during the ambient trial to 168 L.min-1 while breathing normoxic heliu

Pulmonary gas exchange and breathing pattern during and after exercise in highly trained athletes.

Highly trained athletes (HT) have been found to show arterial hypoxaemia during strenuous exercise. A lack of compensatory hyperpnoea and/or a limitation of pulmonary diffusion by pulmonary interstitial oedema have been suggested as causes, but the exact role of each is not clear. It is known, however, that interstitial pulmonary oedema may result in rapid shallow breathing (RSB). The purpose of this study was therefore twofold: firstly, to determine the exact role of a lack of compensatory hyperpnoea versus a widened in ideal alveolar minus arterial oxygen partial pressure difference [PA(i)-a

Oral and oronasal breathing during continuous exercise produce similar responses to ozone inhalation.

Breathing route has a profound effect on sulfur dioxide-induced pulmonary function response in human subjects. There is comparatively little evidence of the effects of oral, nasal, and oronasal breathing on ozone (O3)-induced responses in humans. In this study, six young adult males were exposed on five occasions to 0.40 parts per million (ppm) O3 while exercising continuously at one of two workloads (minute ventilation, VE, of approximately 30 and 75 l/min). The VE exposure time product was similar for all protocols. Four exposures were delivered randomly with a Hans-Rudolph respiratory valve

Effect of the oronasal breathing route on sulfur dioxide-induced bronchoconstriction in exercising asthmatic subjects.

We undertook a study to determine how the oronasal breathing route affects the bronchoconstrictor response to sulfur dioxide (SO2) inhaled by asthmatic subjects during exercise. In 6 subjects, we compared the changes in specific airway resistance (SRaw) caused by breathing humidified air through a mouthpiece during 5 min of exercise on a bicycle ergometer (550 kpm/min) to the changes caused by breathing humidified air plus 0.5 ppm of SO2, (a) through a mouthpiece (oral breathing), (b) by facemask (oronasal breathing), and (c) by facemask with the mouth occluded (nasal breathing) during exercis

The impact of breathing route on pulmonary and sensory responses to ozone exposure during exercise in adults.

Ozone (O3) is a photochemical air pollutant known to reduce lung function and induce respiratory symptoms. This study aimed to determine whether breathing route influences O3-induced lung function changes during exercise. Thirteen recreationally active participants (6 females, V̇O2peak = 45.8 ± 5.3 mL/kg/min), with no history of cardiovascular or pulmonary disease or exercise-induced bronchoconstriction, completed a double-blind crossover study with four experimental visits. Participants performed 30 min of low-intensity cycling (90% of power at the first ventilatory threshold) while breathing

Analysis of the breathing pattern in patients with asthma during physical exercise: A cross-sectional study.

BACKGROUND: About 25 % of patients with asthma suffer from dysfunctional breathing, often leading to exercise induced dyspnoea (EID). EID, however, is often attributed to exercise induced bronchoconstriction, while the contribution of dysfunctional breathing to EID is overlooked. This leads to potential suboptimal treatment of patients with asthma. Therefore, this study investigates the breathing patterns of patients with asthma during exercise compared to healthy subjects and its influence on the level of EID. METHODS: This cross-sectional study investigated individual breathing patterns of

A multidimensional strategy to managing dysfunctional breathing and exercise-induced laryngeal obstruction in adolescent athletes.

BACKGROUND: Exercise induced laryngeal obstruction (EILO) causes inspiratory distress in the upper airway in many adolescent athletes. The nature of EILO is not fully understood, and effective management strategies are lacking. This study aimed to assess the effectiveness of a multidimensional individually tailored intervention, including Norwegian Psychomotor Physiotherapy (NPMP), elements of cognitive behavioural therapy and a rehabilitation plan, in reducing inspiratory distress and dysfunctional breathing in adolescent athletes with EILO. METHODS: A mixed methods design, which combined qu

Dysfunctional breathing diagnosed by cardiopulmonary exercise testing in 'long COVID' patients with persistent dyspnoea.

BACKGROUND: 'Long COVID'-associated dyspnoea may persist for months after SARS-CoV-2 infection. Among the causes of persistent dyspnoea, dysfunctional breathing (DB), defined as an erratic or inappropriate ventilation at rest or exercise, has been observed, but little is known about its occurrence and pathophysiology among individuals with 'long COVID'. We aimed to describe the occurrence and identify clinical predictors of DB among patients following SARS-CoV-2 infection. METHODS: Cardiopulmonary exercise testing (CPET) was performed in 51 SARS-CoV-2 patients (median age, 64 years (IQR, 15))

Analysis of Exercise-Induced Periodic Breathing Using an Autoregressive Model and the Hilbert-Huang Transform.

Evaluation of exercise-induced periodic breathing (PB) in cardiopulmonary exercise testing (CPET) is one of important diagnostic evidences to judge the prognosis of chronic heart failure cases. In this study, we propose a method for the quantitative analysis of measured ventilation signals from an exercise test. We used an autoregressive (AR) model to filter the breath-by-breath measurements of ventilation from exercise tests. Then, the signals before reaching the most ventilation were decomposed into intrinsic mode functions (IMF) by using the Hilbert-Huang transform (HHT). An IMF represents

Association of breathing sound spectra with glottal dimensions in exercise-induced vocal cord dysfunction.

The objective of this study was to evaluate associations between the breathing sound spectra and glottal dimensions in exercise-induced vocal cord dysfunction (EIVCD) during a bicycle ergometry test. Nineteen subjects (mean age 21.8 years and range 13-39 years) with suspected EIVCD were studied. Vocal folds were continuously imaged with videolaryngoscopy and breathing sounds were recorded during the bicycle exercise test. Twelve subjects showed paradoxical movement of the vocal folds during inspiration by the end of the exercise. In seven subjects, no abnormal reactions in vocal folds were fou

Hydrogen breath test as a simple noninvasive method for evaluation of carbohydrate malabsorption during exercise.

The aim of this study was to examine hydrogen (H2) production with the hydrogen breath test (HBT) after ingesting primarily digestible carbohydrate (CHO) during 3 h of 75% maximal oxygen consumption exercise. This was done to indicate CHO overflow in the colon which may occur when gastric emptying, intestinal transit and CHO absorption are not matched and CHO accumulates in the colon where it is subject to bacterial degradation. Further, this study was designed to assess breath H2 production as a function of the type of CHO ingested and the type of exercise. A group of 32 male triathletes perf
