Successful bridge therapy of gilteritinib to cord blood transplantation in relapsed acute myeloid leukemia after bone marrow transplantation.

The FMS-related tyrosine kinase 3 (FLT3) internal tandem duplication mutations (FLT3-ITD) positive acute myeloid leukemia (AML) is a disease with a dismal outcome. Gilteritinib is a second-generation FLT3 inhibitor with activity against ITD and high affinity toward the FLT3 receptor, thereby showing therapeutic potential for relapsed/refractory FLT3-mutated AML. Bone marrow transplantation (BMT) from a human leukocyte antigen (HLA) identical sibling donor was performed in a 38-year-old Japanese male with FLT3-ITD positive AML. Neutrophil engraftment (>0.5 × 109/L) was achieved on day 16, and b

Urgent-Start Peritoneal Dialysis as a Bridge to Definitive Chronic Renal Replacement Therapy: Short- and Long-Term Outcomes.

Background: The peritoneal dialysis (PD) urgent-start pathway, without typical 2-week break-in period, was meant for late-referral patients able and prone to join PD-first program, with its main advantages such as: keeping the vascular system intact, preserving their residual renal function and retaining life-style flexibility. We compared the short- and long-term outcomes of consecutive 35 patients after urgent- and 94 patients after the planned start of PD as the first choice. Methods: The study included all incident end-stage renal disease patients starting PD program between January 2005 a

Dialysis as a bridge therapy to renal transplantation: comparison of graft outcomes according to mode of dialysis treatment.

BACKGROUND: Renal transplantation is the ideal renal replacement therapy in patients with end-stage renal disease. It was unclear whether a difference in dialysis modality influences outcomes after kidney transplantation. Therefore, we evaluated the influence of dialysis modality. METHODS: We compared various clinical and laboratory parameters of 70 peritoneal dialysis (PD) and 180 hemodialysis (HD) patients (n=250), including 91 females and an overall age 36.7±9.7 years who underwent transplantation between 2000 and 2008 to evaluate factors affecting delayed graft function (DGF) and of trans

Venetoclax-Based Therapy as a Bridge to Hematopoietic Stem Cell Transplantation in Relapsed or Refractory Pediatric Acute Leukemia.

Relapsed and/or refractory (R/R) pediatric acute leukemia carries a dismal prognosis, largely driven by chemoresistance to conventional salvage therapy. The BH3-mimetic venetoclax, combined with chemotherapy or hypomethylating agents, has demonstrated efficacy in small clinical trials. However, data on its role as a bridge to hematopoietic stem cell transplantation (HSCT) and on re-challenge in previously exposed patients remain limited. We report outcomes from 21 children treated with 26 episodes of venetoclax-based regimens as a bridge to HSCT, including five after prior venetoclax exposure.

Hypomethylating agents plus venetoclax for high-risk MDS and CMML as bridge therapy to transplant: a GESMD study.

BACKGROUND: High-risk myelodysplastic syndromes (HR-MDS) and chronic myelomonocytic leukemia (CMML) remain therapeutic challenges with suboptimal outcomes. The only potentially curative treatment is allogeneic stem cell transplantation (allo-SCT). The most frequent pre-allo-SCT treatment is monotherapy with hypomethylating agents (HMA), but approximately 40% of patients cannot proceed to allo-SCT, mainly due to disease progression. Recent evidence suggests that combining HMA with venetoclax (HMA/VEN) could increase HMA efficacy in HR-MDS but it remains unclear if this combination could bridge

Mitral valve transcatheter edge-to-edge repair as bridge therapy in a middle-aged patient with Marfan syndrome.

UNLABELLED: A 45-year-old man was hospitalized at our institution due to congestive heart failure. He had a history of Marfan syndrome and four previous cardiovascular surgeries, including aortic valve replacement with a bioprosthetic valve 9 years previously. Transthoracic echocardiography revealed a myxomatous change of the mitral valve and severe mitral regurgitation (MR). After a thorough discussion, including the durability of a bioprosthetic valve, mitral valve transcatheter edge-to-edge repair (M-TEER) was performed as a bridge to the next cardiovascular surgeries. After the procedure,

FLAG with Bortezomib Salvage Therapy in Relapsed/Refractory Childhood Leukemia-A Reliable Bridge to Transplantation with Limited Toxicity.

Relapsed refractory leukemia represents a difficult-to-treat population of patients. The balance between perceived benefit and potential side effects along with the significant financial burden of managing multidrug-resistant sepsis are factors that determine the choice of salvage regimen. Here, we present our experience with the combination of fludarabine, cytarabine, granulocyte-colony stimulating factor with bortezomib. The morphological complete response rate was 58% with 50% of the patients achieving complete remission. With only three patients requiring intensive care unit admission duri

Stepwise Reversal of Immune Dysregulation Due to STAT1 Gain-of-Function Mutation Following Ruxolitinib Bridge Therapy and Transplantation.

PURPOSE: Patients with heterozygous gain-of-function (GOF) mutations in STAT1 frequently exhibit chronic mucocutaneous candidiasis (CMC), immunodeficiency and autoimmune manifestations. Several treatment options including targeted therapies and hematopoietic stem cell transplantation (HSCT) are available for STAT1 GOF patients but modalities and outcomes are not well established. Herein, we aimed to unravel the effect of ruxolitinib as a bridge therapy in a patient with sporadic STAT1 T385M mutation to manage infections and other disease manifestations. METHODS: Peripheral blood mononuclear c

[Usefulness of intracoronary therapy with progenitor cells in patients with dilated cardiomyopathy: Bridge or alternative to heart transplantation?].

INTRODUCTION AND OBJECTIVES: Some paediatric publications have recently raised the value of intracoronary therapy with autologous bone marrow-derived progenitor cells (APCs) in children with dilated cardiomyopathy (DCM) and heart failure. We describe the usefulness of this treatment in two infants with severe DCM and heart failure, who had been transferred to our hospital for cardiac transplant evaluation. PATIENTS AND METHODS: The first patient was a 3 months old male weighing 4 kg. The second was a 4 months old male weighing 5 kg. At the time of admission, both were in poor clinical conditi

Optimizing bridging radiotherapy prior to CAR-T cell therapy: An evidence-based approach.

Bridging radiotherapy has emerged as a valuable strategy to ensure optimal conditions for chimeric antigen receptor T-cell (CAR-T) therapy in relapsed or refractory hematologic malignancies. By controlling disease burden and maintaining clinical stability in the interval between leukapheresis and CAR-T infusion, it helps counteract the risk of tumor progression that can diminish treatment efficacy. Evidence from diffuse large B-cell lymphoma (DLBCL) and other lymphomas supports the use of hypofractionated radiotherapy to achieve rapid local control with minimal delays, while preliminary data i

Injectable hydrogel-based platforms for precision lung cancer therapy: bridging biomaterials and oncology.

Lung cancer is the leading cause of cancer-related deaths globally, with around 2.5 million new cases and 1.8 million fatalities reported each year, driven by late-stage diagnosis, aggressive tumor biology, and limited efficacy of systemic therapies due to poor tumor penetration and dose-limiting toxicities. These clinical challenges underscore the urgent need for localized, precision drug delivery strategies. This review critically examines injectable hydrogel-based platforms as emerging solutions for precision lung cancer therapy, focusing on their design principles, therapeutic mechanisms,

An NIR/GSH-responsive nanoplatform based on tetrasulfide bridging for targeted synergistic tumor therapy.

The efficient treatment of malignant tumors remains a major challenge in the biomedical field. To address the high toxicity and side effects of traditional chemotherapy, poor tumor targeting, and the limited effectiveness of single therapy, this study developed an intelligent nanoplatform (USMFC) that responds to both near-infrared (NIR) light and the tumor microenvironment by integrating upconversion nanoparticles (UCNPs), a tetrasulfide-bridged mesoporous silica (mSiO₂) shell, and folic acid (FA) for active tumor targeting, with polyethylene glycol (PEG) electrostatically adsorbed as the out

Liver Xenotransplantation: From Early Primate Trials to the First-in-Human Porcine Bridging Therapies.

Liver transplantation remains the definitive treatment for end-stage liver disease and acute liver failure, yet a critical and persistent shortage of donor organs results in thousands of preventable deaths annually worldwide. Xenotransplantation has emerged as a potential solution to this structural deficit. This narrative review traces the evolution of liver xenotransplantation, from early non-human primate trials in the 1960s through the application of CRISPR/Cas9-driven multi-gene editing platforms in contemporary porcine donors. The immunological barriers that drove the transition from pri

Psychedelic- and Substance-Assisted Therapies in Global Mental Health: Bridging Cultures, Evidence, and Access.

Psychedelic- and substance-assisted therapies, including MDMA, psilocybin, and ketamine, are gaining attention for conditions such as PTSD and depression, yet their development and implementation remain largely concentrated in high-income settings. This graphical abstract summarizes the central argument of the commentary: while these interventions may hold relevance for global mental health, particularly in conflict-affected and humanitarian contexts, their equitable use is constrained by cultural, ethical, regulatory, and resource-related challenges. Responsible implementation requires cultur

Multi-Omics approaches in gene therapy for vascular diseases: bridging genomics, transcriptomics, and epigenetics.

Vascular diseases such as atherosclerosis, aneurysms, and peripheral arterial disease remain leading causes of morbidity and mortality, with current treatments primarily managing symptoms rather than addressing underlying molecular drivers. Gene therapy offers a promising avenue for targeted intervention, and recent advances in multi-omics approaches-including genomics, transcriptomics, and epigenetics-are enhancing the precision and efficacy of these therapies. High-throughput sequencing and integrative omics analyses have facilitated the identification of causal genes, non-coding RNAs, and e

HER2 in gynecological malignancies: Bridging the gap between standardized pathological assessment and precision therapy.

HER2 is a transmembrane tyrosine kinase receptor involved in cell proliferation, survival, and differentiation. While its role in breast and gastric cancers is well established, increasing attention is being paid to HER2 expression in gynecologic malignancies. In endometrial cancer, HER2 overexpression is most commonly associated with serous endometrial carcinoma (SEC), where it occurs in approximately 30 % of cases and correlates with poor prognosis, platinum resistance, and TP53 mutations. HER2-low expression, defined by IHC scores of 1 + or 2 + without gene amplification, has also emerged a

Discovery of Novel Potent and Safe Immunoproteasome Inhibitors with Bridged Ring for Anti-Inflammatory Therapy.

Current clinical proteasome inhibitors suffer from nonselective pan-inhibition, leading to severe toxicity. Selective immunoproteasome inhibition offers a safer therapeutic strategy for autoimmune diseases. However, the inadequate LMP7/β5 selectivity of existing immunoproteasome inhibitors still lead to potential clinical side effects. To overcome this limitation, we designed and synthesized a series of novel immunoproteasome inhibitors featuring a bridged ring. The optimal compound A33 exhibited potent LMP7 inhibitory activity with >100-fold selectivity over β5 at the cellular level, along wi

Stem Cell-Based Therapies for Glaucoma Treatment: A Review Bridging the Gap in Veterinary Patients.

Retinal diseases are characterized by progressive damage to retinal cells, leading to irreversible vision loss. Among these, glaucoma stands out as a multifactorial neurodegenerative disease involving elevated intraocular pressure, retinal ganglion cell apoptosis, and optic nerve damage, ultimately resulting in blindness in both humans and dogs. Stem cell-based therapies have emerged as a promising therapeutic option for such conditions due to their regenerative and neuroprotective potential. These therapies, particularly those based on mesenchymal stem cells, offer the potential to repair and
