Characterizing opioid agonist treatment discontinuation trends in British Columbia, Canada, 2012-2018.

BACKGROUND: Given the elevated risk of mortality immediately following opioid agonist treatment (OAT) discontinuation, determining the frequency and timing of OAT discontinuation can help guide the planning of services to facilitate uninterrupted OAT. We sought to describe weekly and monthly trends in OAT episode discontinuations in British Columbia to determine the potential resource needs for implementing support services. METHODS: This population-based retrospective study utilized a provincial-level linkage of health administrative databases to identify all people with opioid use disorder

Opioid agonist treatment uptake within provincial correctional facilities in British Columbia, Canada.

BACKGROUND AND AIMS: Multiple interventions and policy changes related to opioid agonist treatment (OAT) have been introduced in British Columbia, Canada to increase engagement and retention in OAT. We aimed to estimate the impact of policy changes and the announcement of the opioid overdose-related public health emergency on the use of OAT for incarcerated individuals with opioid use disorder. DESIGN: Interrupted time-series analysis. Events of interest included the expansion of buprenorphine/naloxone into provincial health-care insurance coverage in October 2015 and the public health emerge

Antiretroviral drug costs and prescription patterns in British Columbia, Canada: 1996-2011.

INTRODUCTION: Treatment options and therapeutic guidelines have evolved substantially since highly active antiretroviral treatment (HAART) became the standard of HIV care in 1996. We conducted the present population-based analysis to characterize the determinants of direct costs of HAART over time in British Columbia, Canada. METHODS: We considered individuals ever receiving HAART in British Columbia from 1996 to 2011. Linear mixed-effects regression models were constructed to determine the effects of demographic indicators, clinical stage, and treatment characteristics on quarterly costs of

Cohort profile: Mothers who use substances and their children in British Columbia, Canada.

INTRODUCTION: Perinatal substance use is a growing public health challenge in the province of BC. A population-based cohort was established using linked health administrative data to underpin three research studies that will evaluate: 1) the effectiveness of specialized acute care for pregnant people who use substances; 2) the comparative effectiveness of different medication regimens for the treatment of perinatal opioid use disorder (OUD); and 3) the longitudinal association between maternal substance use and child health outcomes. METHODS: The population-based cohort includes all individua

Pediatric Meningoencephalitis Cluster Caused by Snowshoe Hare Virus, Whistler, British Columbia, Canada, 2024.

Snowshoe hare virus (SSHV) is an arbovirus in the California serogroup known to circulate throughout Canada and northern latitudes of the United States. The clinical spectrum of SSHV infection ranges from asymptomatic or mild febrile illness to neuroinvasive disease; neuroinvasive disease occurs more often in children and young adults. We describe a cluster of confirmed and probable SSHV meningoencephalitis cases in 3 children from Whistler, British Columbia, Canada, in the summer of 2024. We highlight the shared epidemiology, clinical manifestations, serologic diagnostic methods, and outcomes

Criminalizing public space through a decriminalization framework: The paradox of British Columbia, Canada.

This commentary explores a recent shift in British Columbia's drug policy under a novel drug "decriminalization" framework. We focus on the province's move toward "recriminalization" under this framework. In short, recriminalization was a shift in BC's drug decriminalization framework to only apply in private residences, and be removed from essentially all outdoor spaces. This policy change was completed through an agreement with the federal government amid a public health emergency. Since 2016, BC has faced a severe crisis of drug-related overdoses and poisonings, driven by a toxic and unregu

Fetal alcohol syndrome and population level health care usage in British Columbia, Canada.

BACKGROUND: The literature indicates that youth with Fetal Alcohol Syndrome (FAS) may experience high rates of both physical and mental health issues compared to youth without FAS. However, there is little population level health data available for youth with FAS, particularly for youth transitioning from pediatric to adult healthcare services. OBJECTIVE: The objective of this study was to compare health care usage of youth with Fetal Alcohol Syndrome to youth without any intellectual/developmental disabilities (IDD). METHODS: This study used a retrospective cohort design and population-leve

Trends in antihypertensive drug utilization in British Columbia, 2004-2019: a descriptive study.

BACKGROUND: Clinical guidelines for hypertension were updated with lower blood pressure targets following new studies in 2015; the real-world impact of these changes on antihypertensive drug use is unknown. We aimed to describe trends in antihypertensive drug utilization from 2004 to 2019 in British Columbia. METHODS: We conducted a longitudinal study to describe the annual prevalence and incidence rate of use of 5 antihypertensive drug classes (thiazides, angiotensin-converting enzyme [ACE] inhibitors, angiotensin II receptor blockers [ARBs], calcium channel blockers and β-blockers) among BC

Known fentanyl use among clients of harm reduction sites in British Columbia, Canada.

BACKGROUND: North America is in the midst of an opioid overdose epidemic and it is commonly suggested that exposure to fentanyl is unknown. Using a provincial survey of harm reduction site clients, we aimed to characterize known and unknown fentanyl use and their correlates among people who use drugs in British Columbia, Canada. METHODS: We recruited 486 clients who were >18 years old and 316 agreed to provide a urine sample for substance use testing. Reported known fentanyl use was defined as a three-level categorical variable assessing recent (i.e., in the previous three days) fentanyl expo

Treatment outcomes for isoniazid-resistant tuberculosis under program conditions in British Columbia, Canada.

BACKGROUND: Every year, over 1 million people develop isoniazid (INH) resistant tuberculosis (TB). Yet, the optimal treatment regimen remains unclear. Given increasing prevalence, the clinical efficacy of regimens used by physicians is of interest. This study aims to examine treatment outcomes of INH resistant TB patients, treated under programmatic conditions in British Columbia, Canada. METHODS: Medical charts were retrospectively reviewed for cases of culture-confirmed INH mono-resistant TB reported to the BC Centre for Disease Control (BCCDC) from 2002 to 2014. Treatment regimens, patient

Evaluating the Timeliness of Enteric Disease Surveillance in British Columbia, Canada, 2012-13.

Timely surveillance of enteric diseases is necessary to identify and control cases and outbreaks. Our objective was to evaluate the timeliness of enteric disease surveillance in British Columbia, Canada, compare these results to other settings, and recommend improvements. In 2012 and 2013, information was collected from case report forms and laboratory information systems on 2615 Salmonella, shigatoxin-producing E. coli, Shigella, and Listeria infections. Twelve date variables representing the surveillance process from onset of symptoms to case interview and final laboratory results were colle

Determinants of trends in prescription opioid use in British Columbia, Canada, 2005-2013.

PURPOSE: To explore the determinants of total opioid consumption in a Canadian province, and to examine patterns of opioid dispensations by sex, age, and income quintile. METHODS: We used population-based administrative data on prescription drug dispensations in British Columbia (BC; population ~4 million). We apply an index-based approach to examine how changes in population exposure, type of opioids used, and intensity of use contributed to changes in total morphine equivalents dispensed per 1000 population. RESULTS: Between 2005 and 2013 in BC, opioid consumption increased by 31%, driven

Exposure-based traffic crash injury rates by mode of travel in British Columbia.

BACKGROUND: Traffic-related trauma is an important contributor to morbidity and mortality in Canada, especially among children and young adults. Comparing exposure-based injury rates between travel modes and jurisdictions is a valuable tool towards improving safety. METHODS: We used injury data from the British Columbia Motor Vehicle Branch, trip diary data from the Metro Vancouver transportation authority, and population and provincial travel data from the Census to calculate crude fatality and injury rates for motor vehicle occupants, bicyclists, and pedestrians. We used three different den

Hepatitis C virus and risk of non-Hodgkin lymphoma in British Columbia, Canada.

We investigated Hepatitis C virus (HCV) seropositivity and the risk of non-Hodgkin lymphoma (NHL) in a population-based case-control study in British Columbia, Canada. Cases were aged 20-79, diagnosed between March 2000 and February 2004, and resident in greater Vancouver or Victoria. Cases with HIV or a prior transplant were excluded. Controls were chosen from the Client Registry of the British Columbia (BC) Ministry of Health, and were age/sex/region frequency matched to cases. Antibodies for HCV were measured in 795 cases and 697 control subjects. HCV seropositivity was 2.4% in cases and 0.

Prevalence of activity limitation among persons living with HIV/AIDS in British Columbia.

BACKGROUND: As antiretrovirals increase the life expectancy of persons living with HIV, quality of life issues become more important. Little research has examined the types and levels of activity limitations among HIV-positive populations. The objective of this report is to compare the levels of limitations among HIV-positive persons to the general population. METHODS: The BC Persons With AIDS (BCPWA) Society consists of approximately 3,500 HIV-positive members. A recent survey conducted among BCPWA members included a section assessing activity limitations. Prevalence of limitations in this g

Hepatitis A among residents of First Nations Reserves in British Columbia, 1991-1996.

BACKGROUND: Hepatitis A spreads by the fecal-oral route. We hypothesize that it is more common in Aboriginal communities because of poverty, crowded housing and inadequate or substandard water and sewage systems. METHODS: We tabulated on-reserve cases reported to First Nations and Inuit Health Branch, Health Canada. We obtained information on community water supply, sewage disposal and mean population per housing unit, from site inspections in a 1994 survey. RESULTS: Crude incidence on-reserve was 31 per 100,000 persons per year (95% CI: 25 to 37), twice as high as in the general population
