Effects of moguisteine, a peripheral nonnarcotic antitussive agent, on airway inflammation in guinea-pigs in vivo.

Cough is a common symptom of respiratory diseases associated with irritation or inflammation of the airways, and symptomatic antitussive drugs are frequently prescribed to control an abnormal cough reflex. Our aim was to evaluate the effects of moguisteine, a novel, peripheral, nonnarcotic antitussive agent, on airway inflammation induced in guinea-pigs with a variety of stimuli. These stimuli included exposure to tobacco smoke for 10 min, to elicit airway hyperreactivity, eosinophil recruitment in bronchoalveolar lavage (BAL), airway epithelial damage and plasma exudation; graded platelet-act

[Air burden and respiratory and vascular diseases].

Three atmospheric pollutants are discussed: Sulfur dioxide (SO2) acts as irritant gas on upper airways, trachea and large bronchi. Bronchoconstriction by SO2 is enhanced during work. Dose-response correlation may be observed with SO2 concentrations and bronchial hyperreactivity. Deaths and morbidity rates of patients with COPD parallel peaks of SO2 concentration such as occurred in the 1956 London smog. The mechanisms involved seem to be the same in cross sectional as in long term SO2 effects on human airways. Ozone (O3) is a major irritant pollutant. O3 penetrates deeply into the small airway

Forced expiration through the nose is a stimulus for NANIPER but not for controls.

OBJECTIVE: Is forced expiration through the nose a mechanical stimulus to which patients with nasal hyperreactivity react? Do parameters, such as peak nasal expiratory flow rate (PNEF), influence nasal airway resistance (NAR) in these patients? METHOD: NAR, mucus production and sneezing were measured on 2 occasions two weeks apart. Measurements were conducted before and during a period of 10 minutes after 3 repeated PNEFs in 15 non-allergic non-infectious perennial rhinitis (NANIPER) patients suffering from nasal hyperreactivity, and in 15 controls. RESULTS: In NANIPER versus controls PNEF m

Diagnosis of asthma - new theories.

INTRODUCTION: Recent studies have shown a remarkably high frequency of poorly controlled asthma. Several reasons for this treatment failure have been discussed, however, the basic question of whether the diagnosis is always correct has not been considered. Follow-up studies have shown that in many patients asthma cannot be verified despite ongoing symptoms. Mechanisms other than bronchial obstruction may therefore be responsible. The current definition of asthma may also include symptoms that are related to mechanisms other than bronchial obstruction, the clinical hallmark of asthma. AIM: Bas

Eosinophil-associated inflammation in bronchial asthma: a connection to the nervous system.

Evidence exists that eosinophil cationic proteins damage respiratory epithelium in bronchial asthma. Furthermore, the degree of eosinophilia in the blood and the lung is related to bronchial hyperreactivity. The eosinophil might increase airway irritability by increasing vagal responsiveness. Sensitized challenged guinea pigs develop M2 muscarinic receptor cholinergic dysfunction which is abolished by injection of heparin or polyglutamate and both the eosinophil granule major basic protein and the eosinophil peroxidase act as allosteric M2 receptor antagonists. Thus, eosinophil-associated pulm

Nonallergic rhinitis with eosinophilia syndrome a precursor of the triad: nasal polyposis, intrinsic asthma, and intolerance to aspirin.

Fifty-two cases of perennial rhinitis were studied, leading to the diagnosis of seven cases of nonallergic rhinitis with eosinophilia syndrome (NARES) a frequency of 13.5%. Symptoms of nasal hyperreactivity involving sneezing, rhinorrhea, nasal obstruction and pruritus were more severe than in other types of rhinitis. The frequency of hyposmia was very specific to NARES. Nasal endoscopy and sinus CT revealed an evolution towards nasal polyposis in four patients. The nasal challenge to house dust mites showed the absence of any increase in local eosinophilia. Bronchial hyperreactivity to carbac

Guidelines for the diagnosis of occupational asthma. Subcommittee on 'Occupational Allergy' of the European Academy of Allergology and Clinical Immunology.

The 'Guidelines for the diagnosis of Occupational Asthma' have been written by the Subcommittee on Occupational Allergy of the European Academy of Allergology and Clinical Immunology to give common diagnostic criteria in the evaluation of individual patients with suspected occupational asthma. The suggested diagnostic procedure includes five steps. 1. History suggestive of occupational asthma. 2. Confirmation of bronchial asthma, with demonstration of reversibility of bronchial obstruction, of non-specific bronchial hyperreactivity and of increased diurnal variability of peak expiratory flow r

Identification of a new defective SERPINA1 allele (PI*Zla palma) encoding an alpha-1-antitrypsin with altered glycosylation pattern.

BACKGROUND: Alpha-1-antitrypsin (AAT) deficiency is a genetic condition that arises from mutations in the SERPINA1 gene and predisposes to develop pulmonary emphysema and, less frequently, liver disease. Occasionally, new defective SERPINA1 alleles are detected as an outcome of targeted-screening programs or case-findings. METHODS: This study began with a female patient showing bronchial hyperreactivity. Serum level and phenotype for AAT was analysed by immunonephelometry and isoelectric focusing electrophoresis. The SERPINA1 gene of the proband was genotyped by PCR amplification and DNA sequ

Differences in lung function, bronchial hyperresponsiveness and respiratory health between elite athletes competing in different sports.

The aim of this study was to examine lung function, bronchial hyperresponsiveness (BHR) and exercise-induced respiratory symptoms in elite athletes performing different sports. Norwegian national-team athletes (30 swimmers, 32 cross-country skiers, 16 speed-skaters, 11 rowers/paddlers, 17 handball players and 23 soccer players) completed a validated questionnaire, measured exhaled nitric oxide (FENO), spirometry, methacholine provocation (PD20met) and skin prick test. Three cut-off levels defined BHR; i.e. PD20met ≤2 µmol, ≤4 µmol and ≤8 µmol. Mean forced vital capacity (FVC) was highest in sw

Pneumocystis carinii pneumonia presenting as asthma: increased bronchial hyperresponsiveness in Pneumocystis carinii pneumonia.

Two male patients presented with clinical and laboratory findings consistent with typical bronchial asthma and subsequently developed Pneumocystis carinii pneumonia (PCP). Only on subsequent questioning did both admit to homosexuality and behavior associated with a high risk of HIV-infection. In order to determine how frequently reversible airway obstruction is seen in patients with PCP, we measured peak expiratory flow rates (PEFR) before and after bronchodilator administration in 37 of these patients. Initial PEFR measurements revealed a significant decrease in PEFR (< 80% predicted) in 84%,

The role of bronchial epithelial cells in airway hyperresponsiveness.

It is commonly accepted that airway hyperresponsiveness (AHR) is a chronic airway inflammation although the exact mechanism of its pathogenesis is still unclear. In the past ten years, an epithelial defect hypothesis has gradually gained supports from the main stream. Airway epithelium is no longer considered only as a simple mechanic barrier but an active interface between the inner and outer environment. Bronchial epithelial cells play a critical role in maintenance of homeostasis in the airway local microenvironment through a wide range of physiologic functions including anti-oxidation, exo

Respiratory syncytial virus pneumonia in an AIDS patient.

Pneumonia caused by respiratory syncytial virus in an AIDS patient is reported. A co-infection with cytomegalovirus was also demonstrated. Treatment with ribavirin and foscarnet produced good clinical response. The patient, known to have serious obstructive lung disease, suffered from bronchial hyperreactivity for some time afterwards. The dilemma of antiviral therapy is discussed.

[Interpretation of standard technical specifications for methacholine chloride (Methacholine) bronchial challenge test (2023)].

Bronchial challenge tests are important clinical examinations for detecting airway hyperresponsiveness, with methacholine being a commonly used provocative agent. However, the previous guidelines had not been updated in a timely manner, leading to some problems in clinical application. The "Standard technical specifications for methacholine chloride (Methacholine) bronchial challenge test (2023)" is an updated and refined version of 2014 "Guidelines for Pulmonary Function Testing (Part Ⅲ)-Histamine and Methacholine Bronchial Challenge Tests". In the 2023 version, the scope of MCT applicability

Mast cells are essential in the development of exposure-associated exercise-induced bronchoconstriction in a mouse model.

BACKGROUND: Cold air and air pollution are known triggers to induce symptoms in exercise-induced bronchoconstriction (EIB). Mast cells are hypothesized to be a key player in the pathogenesis of EIB. This study aims to investigate the role of mast cells using mast cell deficient (Cpa3cre/+) mice and with mast cell stabilizers (Doxantrazole) in an exposure-associated mouse model of EIB. METHODS: Male Cpa3cre/+ mice and wild type littermates or BALB/c mice were exposed to a submaximal running protocol in cold environment (4°C) or resting period (room temperature) 5 days for 3 weeks after orophar

[Rhinitis and sinusitis: influence on asthma and COPD].

Basing on epidemiology, pathophysiology and clinical manifestations we can show an important correlation between rhinitis and bronchial asthma. Prevalence of allergic rhinitis in childhood is an early manifestation of atopy. It is estimated that allergic rhinitis occurs in about 60% of patients before the onset or simultaneously with bronchial asthma. The epidemiologic, etiopathogenetic and clinical data demonstrate that the upper and lower airways are one system, which, in the case of allergic inflammation induces common mechanisms intensifying and maintaining this process. There is no doubt

Sublingual immunotherapy in children with allergic rhinoconjunctivitis mono-sensitized to house-dust-mites: a double-blind-placebo-controlled randomised trial.

BACKGROUND: Although sublingual immunotherapy (SLIT) has been demonstrated to be a safe and efficient treatment in children with seasonal allergic rhinitis (AR), there is little evidence on the efficacy of SLIT with house-dust-mite (HDM) extract in children with isolated perennial AR. OBJECTIVES: We sought to assess the clinical efficacy and safety of HDM-SLIT in children with isolated allergic rhinitis-conjunctivitis mono-sensitized to HDM without asthma symptoms. METHODS: Twenty-two children (aged 5-10 years) with perennial AR and conjunctivitis symptoms mono-sensitized to Dermatophagoides

Acute effects of low levels of ambient ozone on peak expiratory flow rate in a cohort of Australian children.

BACKGROUND: We enrolled a cohort of primary schoolchildren with a history of wheeze (n = 148) in an 11-month longitudinal study to examine the relationship between ambient ozone concentrations and peak expiratory flow rate. METHODS: Enrolled children recorded peak expiratory flow rates (PEFR) twice daily. We obtained air pollution, meteorological and pollen data. In all, 125 children remained in the final analysis. RESULTS: We found a significant negative association between daily mean deviation in PEFR and same-day mean daytime ozone concentration (beta-coefficient = 0.88; P = 0.04) after a

The role of atopy in potroom workers' asthma.

A study was performed to clarify a possible role of atopy in the occurrence of acute bronchoconstrictive impairment observed in some workers in a plant using the Alu-Swiss process for the electrolytic extraction of aluminum. Of 227 workers examined the percentage of those with a history of atopy and positive skin tests for common allergens was within the expected range. Six had positive patch test with 2% NaF. Among seven workers with paroxysmal wheezing and dyspnea, of whom three were only light smokers, three had positive skin test with common allergens but only one of them had an increased
