High-Flow Nasal Cannula and COVID-19: A Clinical Review.

During the coronavirus disease 2019 (COVID-19) pandemic, noninvasive respiratory support has played a central role in managing patients affected by moderate-to-severe acute hypoxemic respiratory failure, despite inadequate scientific evidence to support its usage. High-flow nasal cannula (HFNC) treatment has gained popularity because of its effectiveness in delivering a high fraction of humidified oxygen, which improves ventilatory efficiency and the respiratory pattern, as well as its reported high tolerability, ease of use, and application outside of ICUs. Nevertheless, the risk of infection

Optimal cannula positioning of HeartMate 3 left ventricular assist device.

Cannula position in HeartMate II and HeartWare left ventricular assist devices (LVADs) is associated with clinical outcome. This study aimed to investigate the clinical implication of the device positioning in HeartMate 3 LVAD cohort. Consecutive patients who underwent HeartMate 3 LVAD implantation were followed for one year from index discharge. At index discharge, chest X-ray parameters were measured: (a) cannula coronal angle, (b) height of pump bottom, (c) cannula sagittal angle, and (d) cannula lumen area. The association of each measurement of cannula position with one-year clinical outc

Physiologic Effects of High-Flow Nasal Cannula in Healthy Subjects.

BACKGROUND: High-flow nasal cannula (HFNC) is increasingly used in the management of acute and chronic respiratory failure. Little is known about the optimal settings for HFNC. This study was designed to assess the dose effect of HFNC on respiratory effort indexes and respiratory patterns in spontaneously breathing adults. METHODS: A randomized controlled crossover study was conducted in 10 healthy subjects. Five experimental conditions were evaluated: baseline with no therapy; 5 L/min with conventional nasal prongs; and HFNC at 20, 40, and 60 L/min. The primary outcomes were the indexes of r

Aerosol Therapy During Noninvasive Ventilation or High-Flow Nasal Cannula.

Noninvasive ventilation (NIV) and high-flow nasal cannula (HFNC) are increasingly used for patients with acute respiratory failure. Some patients receiving these therapies might also benefit from inhaled drug delivery. Thus, it is attractive to combine aerosol therapy with NIV or HFNC. The purpose of this paper is to review the available evidence related to the use of inhaled aerosols with NIV or HFNC. Available evidence supports the delivery of aerosols during NIV. Inhaled bronchodilator response might be improved with the use of NIV in acute asthma, but the evidence is not sufficiently matur

Effort of breathing in children receiving high-flow nasal cannula.

OBJECTIVE: High-flow humidified nasal cannula is often used to provide noninvasive respiratory support in children. The effect of high-flow humidified nasal cannula on effort of breathing in children has not been objectively studied, and the mechanism by which respiratory support is provided remains unclear. This study uses an objective measure of effort of breathing (Pressure. Rate Product) to evaluate high-flow humidified nasal cannula in critically ill children. DESIGN: Prospective cohort study. SETTING: Quaternary care free-standing academic children's hospital. PATIENTS: ICU patients y

Defunctioning cannula ileostomy after lower anterior resection of rectal cancer.

BACKGROUND AND OBJECTIVE: Most surgeons suggest the use of fecal diversion in patients undergoing low anterior resections of rectal tumors at high risk for anastomotic leakage. We describe an exploratory study to evaluate the efficacy and safety of a new diversion method called a spontaneously closing cannula ileostomy, which was designed to protect rectal anastomoses in patients at high risk for anastomotic leakage. The outcomes of patients treated with cannula ileostomy were compared to those of patients treated with loop ileostomy. MAIN OUTCOME MEASURES: Outcomes included the rates of anas

High flow nasal cannula for respiratory support in preterm infants.

BACKGROUND: High flow nasal cannulae (HFNC) are small, thin, tapered cannulae used to deliver oxygen or blended oxygen and air at flow rates of > 1 L/min. HFNC can be used to provide high concentrations of oxygen and may deliver positive end-expiratory pressure. OBJECTIVES: To compare the safety and efficacy of HFNC with other forms of non-invasive respiratory support in preterm infants. SEARCH STRATEGY: The strategy included searches of the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library 2010), MEDLINE, CINAHL, EMBASE and abstracts from conference proceedings.

Cannula selection and cannulation techniques for nonpulsatile mechanical ventricular assistance.

The successful use of mechanical ventricular assistance is, in large part, dependent upon easy insertion of well designed inflow and outflow cannulas. This manuscript describes a family of cannulas specifically designed for use in a nonpulsatile ventricular assist device (VAD) circuit. Although a variety of commercially available cannulas can be employed in a nonpulsatile VAD circuit, the cannulas described in this manuscript possess a number of unique design features. The reinforced thin-walled design and end hole construction optimize fluid flow characteristics in both the inflow and outflow

Thirty seven eyes with congenital cataract in with there a lot of transparent cortex were treated with McIntyre co-axial cannula. Primary posterior capsuloctomy after removing the cortex was performed on patients aged less 3 years; for patients over 3 years, their posterior capsule remained intact. After the operation, all cases got transparent pupillary space more than 5 mm in diameter. The visual acuities improved in 32 eyes and remained unchanged in 5 eyes. This procedure not only received satisfactory results, but also avoided side effects caused by capsuloctomy and reduced the possibility

Ultrasmall Gauge Biopsy of Small Choroidal Melanomas with a Subretinal Cannula.

PURPOSE: To report a novel technique for biopsying small choroidal tumors for molecular prognostication utilizing a 38-gauge subretinal cannula. METHODS: Through a transvitreal approach, a subretinal cannula enters the eye and penetrates the tumor in an area devoid of blood vessels. Manual aspiration is conducted with the aid of a surgical assistant. The cannula's transparency allows for visualization of biopsy depth and tissue aspiration. The cannula is withdrawn, and the sample is manually injected into the specimen vial. RESULTS: Sixteen patients underwent biopsy using this technique for

Cannula tip separation and configuration as determinants of recirculation in venovenous ECMO.

BACKGROUND: Recirculation during venovenous extracorporeal membrane oxygenation (VV ECMO) is an important phenomenon that limits the efficiency of support. Femoro-jugular (FJ) cannulation may be associated with lower recirculation than femoro-femoral (FF) configurations. Local practice is to routinely overlap cannula in the FJ configuration; this approach remains poorly described, and its impact on recirculation is not understood. METHODS: The saturation-based formula was used to assess recirculation fraction amongst 24 patients receiving VV ECMO between July 2018 and March 2025 in this singl

Prevalence and impact of ECMO cannula colonization: a single center study.

During ECMO therapy, infections are common and have substantial impact on treatment outcomes. However, data on the prevalence and clinical impact of microbial ECMO cannula colonization are scarce. Between October 2020 and May 2022, we evaluated 112 ECMO cannulas from 58 patients for microbial colonization using sonication fluid culture & 16S-rRNA-PCR (n = 105), or roll plate method (n = 7). 38 (33.9%) cannulas from 30 (51.7%) patients showed evidence for bacterial colonization with the predominance of coagulase-negative staphylococci (67.4%).
