Idiopathic Capital Femoral Chondrolysis: A Case Report.

Idiopathic capital femoral chondrolysis is a rare condition most commonly seen in African-American pre-adolescent females. The primary symptoms are hip stiffness and pain, which are accompanied by limping. Physical examinations typically reveal an external rotation contracture, flexion contracture, and abduction contracture. There is also immobility at the hip secondary to muscle spasms. Here, we present and discuss the follow-up case of a 10-year-old female patient who was diagnosed with idiopathic chondrolysis of capital femoral epiphysis, right side, and underwent right hip arthroscopic eva

Arthroscopic Treatment of Capitate Nonunion: A New Technique.

Isolated fractures of the capitate are very rare which means that their nonunion is even rarer. Repair techniques have always been performed by open surgery which may result in loss of mobility due to the capsulotomy. The typically used surgical techniques such as arthrodesis of the carpal or wrist bones can have irreversible consequences on range of motion. No arthroscopic technique has ever been described for this condition. Here, we describe a case of isolated capitate nonunion without ligament tears or other carpal fractures in a 21-year-old male, following a capitate fracture 10 years pri

The Role of Hyperinsulinemia in Slipped Capital Femoral Epiphysis.

BACKGROUND: Obesity in the prepuberal stage has been directly associated with slipped capital femoral epiphysis (SCFE). Serum insulin level increases in the prepuberal and adolescence stage, to a greater extent in the obese population. The main objective of this article was to analyze the relationship between insulin levels and SCFE. METHODS: A case-control study was conducted between January 2018 and April 2019. The study group was formed with patients with SCFE and the control group with patients from the pediatric obesity clinic of our hospital selected during their initial evaluation. Non

[Slipped capital femoral epiphysis-etiology and pathogenesis].

BACKGROUND: Slipped capital femoral epiphysis (SCFE) is the most significant hip disease in adolescence. Because of its typical occurrence at the time of the pubertal growth spurt and some particular clinical symptoms, it has been a topic of special interest in orthopedic research for a long time. A large number of etiological factors have been described, however their meaning in detail is still controversial. OBJECTIVES: After a short summary of the structure and function of the epiphysis, an overview of concepts in the pathogenesis of SCFE is given. MATERIALS AND METHODS: A selective revie

Mapping and conceptualizing recovery capital of recovered gamblers.

Recovery capital (RC) is an emerging conceptual framework in the addiction field that pinpoints the internal and external (i.e., human, community, cultural, social, financial) resources individuals can draw on throughout the recovery process. The concept of RC was developed and applied primarily to individuals who have recovered from substance addiction. The aim of this study was to extend the RC conceptual framework to gambling disorder (GD)-which is classified in the Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM-5; American Psychiatric Association, 2013) as a behavioral

Carpal synovitis with capitate bone tuberculosis in a child.

We present a 10-year-old boy with 2-month duration non-traumatic wrist pain and inflammatory signs. Due to elevated inflammatory markers on blood tests, with an increase in radiocarpal and intercarpal joints synovial fluid and no bony lesions, the patient was submitted to wrist arthrocentesis for the suspicion of septic arthritis. The patient did not improve on conventional treatment, however. An MRI showed synovitis around the carpus and a lytic lesion of the capitate bone due to osteomyelitis. A biopsy was able to identify the causative agent as Mycobacterium tuberculosis, and the patient wa

Ultrasonographic Diagnosis of Slipped Capital Femoral Epiphysis.

BACKGROUND: Slipped capital femoral epiphysis (SCFE), a fracture through the physis with resultant slip of the epiphysis, is the most common hip abnormality in adolescents and is a major cause of early osteoarthritis. Plain radiograph is the initial modality used to evaluate patients with painful hip joints. Ultrasonography and magnetic resonance imaging (MRI), which do not involve radiation exposure, have also been used. This case report supports the view that ultrasound can be used as an initial, cost-effective and radiation-free modality for the evaluation of suspected SCFE. CASE REPORT: A

Slipped Capital Femoral Epiphysis: Diagnosis and Management.

Slipped capital femoral epiphysis (SCFE) is the most common hip disorder in adolescents, occurring in 10.8 per 100,000 children. SCFE usually occurs in those eight to 15 years of age and is one of the most commonly missed diagnoses in children. SCFE is classified as stable or unstable based on the stability of the physis. It is associated with obesity, growth spurts, and (occasionally) endocrine abnormalities such as hypothyroidism, growth hormone supplementation, hypogonadism, and panhypopituitarism. Patients with SCFE usually present with limping and poorly localized pain in the hip, groin,

Hamate body and capitate fracture in punch injury.

Hamate fractures represent only 2% to 4% of all carpal bone fractures because they require a large degree of force to the hand or wrist. This is a case report of a patient with hamate and capitate fractures after a punch injury. It details the minute change seen in routine 3-view radiographic imagery and indications for computed tomography. A 29-year-old African American man with professional boxing training presented to the emergency department with pain and swelling in the hand and wrist after striking a refrigerator with his right hand. Enough force generated along the axial plane of the fo

Chondroblastoma with secondary aneurysmal bone cyst of the capitate.

Chondroblastoma is a benign tumor that typically arises in the epiphysis of a long bone. There have been only 2 reported cases of chondroblastoma involving the capitate. This is the first report of chondroblastoma with secondary aneurysmal bone cyst involving the capitate. A 33-year-old man presented with a 3-year history of pain and swelling of the right wrist. Radiography as well as computed tomography showed a radiolucent area and no matrix calcification within the capitate. Magnetic resonance imaging revealed a homogeneous signal that was low on T1-weighted images and high on T2-weighted i

Surgery for slipped capital femoral epiphysis in adolescents.

The treatment of slipped capital femoral epiphysis (SCFE) in adolescents remains controversial. The goal of initial treatment is to prevent further slippage of the epiphysis. In mild forms, both stable and unstable, in situ fixation is widely accepted as the reference treatment. In contrast, several techniques are available for stable moderate-to-severe SCFE. In unstable moderate-to-severe SCFE, emergent reduction with decompression and internal fixation is currently the preferred method. Selection of the surgical technique rests on an appraisal of advantages versus drawbacks. The goal of this

Delayed-onset of slipped capital femoral epiphysis.

BACKGROUND: Slipped capital femoral epiphysis in adults is uncommon. The purpose of this study was to report our own four cases (six hips) of slipped capital femoral epiphysis (SCFE) in adulthood and to review the cases reported in the literature. The authors attempted to investigate the various causative underlying disorders and clinical characteristics, treatment, outcome, and complications. METHODS: We searched for all 22 reported cases of SCFE in adults age 18 years and older via MEDLINE and Google Scholar from 1963 to 2012 without any exception. In doing so, we documented our own four ca

Slipped capital femoral epiphysis: what's new?

Slipped capital femoral epiphysis (SCFE) is a common hip disorder among adolescents, whereby the epiphysis is displaced posteriorly and inferiorly to the metaphysis. Treatment modalities aim to stabilize the epiphysis, prevent further slippage, and avoid complications associated with long-term morbidity, such as osteonecrosis and chondrolysis. Controversy exists with SCFE regarding prophylactic fixation of the contralateral, painless, normal hip, the role of femoroacetabular impingement with SCFE, and whether in situ fixation is the best treatment method for SCFE. This article presents and dis

Is the acetabulum retroverted in slipped capital femoral epiphysis?

BACKGROUND: Recent biplanar radiographic studies have demonstrated acetabular retroversion and increased superolateral femoral head coverage in hips with slipped capital femoral epiphysis (SCFE), seemingly divergent from earlier CT-based studies suggesting normal acetabular version. QUESTION/PURPOSES: We therefore asked: Are there differences in (1) acetabular version at the superior ¼ of the acetabular dome (AV(sup)), (2) acetabular version at the center of the femoral head (AV(cen)), and (3) superolateral femoral head coverage (lateral center-edge angle [LCEA]) among affected SCFE hips, una

Acetabulotrochanteric distance in slipped capital femoral epiphysis.

BACKGROUND: A pathognomonic finding of slipped capital femoral epiphysis (SCFE) is a combination of the decrease in the height of the slipped epiphysis and the position of external rotation and flexion of hip. We believe that decrease in the acetabulotrochanteric distance (ATD) on an anteroposterior (AP) radiograph represents this finding. MATERIAL AND METHODS: In a retrospective study of 25 consecutive cases of unilateral SCFE, we defined ATD as the distance between a line connecting the superolateral margins of the acetabulae with a second line, parallel to the first line, which goes throug

[Isolated fractures of the capitate: treatment of delayed union].

Delayed and non-union of isolated fractures of the capitate are rare injuries. Initial plain radiographs do not always show the fractures. Especially fractures through the neck of the capitate can lead to delayed union. We report on a patient with persistent localised pain over the capitate 5 months after a fall on the hand. The CT examination showed a delayed union of the capitate, which was treated with an iliac cancellous bone graft and screw osteosynthesis. Bone healing was verified 3 months after operation. Operative therapy is recommended in cases of delayed union or non-union of the cap

The epidemiology of slipped capital femoral epiphysis: an update.

The Kids' Inpatient Database, reflecting 6.70 million pediatric discharges in 1997 and 7.30 million in 2000, was coupled with the US Census Bureau data and was used to elicit the epidemiology of idiopathic slipped capital femoral epiphysis (SCFE) that occurred in children 9-16 years. It was found that the overall incidence of SCFE in the United States for these years was 10.80 cases/100,000 children. The relative incidence of SCFE was 3.94 times higher in black children and 2.53 times higher in Hispanic children than in white children. The incidence rate was significantly higher in boys (13.35
