Immediate Recognition and Surgical Treatment of Iatrogenic Acute Type A Aortic Dissection Is Associated with Low Hospital Mortality and High Intermediate-Term Survival.

BACKGROUND: To investigate short- and intermediate-term outcomes of immediate (on table) recognition and surgical treatment of iatrogenic acute type A aortic dissection (ATAD) that occurred during the course of the cardiac surgical procedures. METHODS: Of 23,143 adult patients undergoing cardiac surgical procedures at our institution from January 2016 to December 2020, 21 (0.09%) suffered from intraoperative iatrogenic ATAD and underwent immediate aortic repair. Their clinical characteristics, in-hospital outcomes and follow-up results were analyzed. RESULTS: Among the 21 patients, 13 (61.9%

Comprehensive Review of Coronary Artery Anatomy Relevant to Cardiac Surgery.

In order to perform safe cardiac surgery, a knowledge of applied coronary artery anatomy and its variants is essential for cardiac surgeons. In normal individuals, the right and the left coronary arteries arise from the corresponding sinuses of Valsalva within the aortic root. From the cardiac surgical perspective, the coronary artery is divided into the left main coronary artery, its branches (the left anterior descending artery and the circumflex artery), and the right coronary artery. With high-risk cardiac surgeries, including redo procedures, becoming increasingly performed, abnormal cour

Recent Developments in Catheter-based Cardiac Procedures.

Since the first transcatheter aortic valve implantation in 2002, catheter-based cardiac procedures have increased significantly. This review provides an overview of CE approved devices, updated guidelines, and the key clinical findings for TAVR as well as for AV Valve interventions. Recent studies compare balloon-expanding and self-expanding valves, highlighting differences in prosthesis mismatch and paravalvular leaks. Long-term trials confirm TAVR's efficacy, especially in low-risk patients. Challenges include paravalvular regurgitation, vascular complications, and coronary obstruction, with

Transcatheter Interventions in Adult Congenital Heart Disease.

Congenital heart disease (CHD) is the most common congenital birth defect with an incidence of 1 in 100. Current survival to adulthood is expected in 9 out of 10 children with severe CHD as the diagnostic, interventional, and surgical success improves. The adult CHD (ACHD) population is increasingly diverse, reflecting the broad spectrum of CHD and evolution of surgical techniques to improve survival. Similarly, transcatheter interventions have seen exponential growth and creativity to reduce the need for repeat sternotomies. This article focuses on newer data and evolving techniques for trans

A Review of the Surgical Management of Aorto-ventricular Tunnels.

We present a synthesis of 95 published investigations of the exceedingly rare tunnels that can exist between the aortic root and the left or right ventricles. From the 220 suitable cases included in these investigations, we reviewed the clinical presentations, modalities used for diagnosis, surgical approaches, and outcomes. Diagnostic information was provided by clinical presentation, radiographic findings, saline contrast echocardiography, computed tomographic angiocardiography, magnetic resonance imaging, cardiac catheterization, and angiocardiography. These techniques elucidated the corona

[The best of congenital heart disease in 2005].

The end of 2004 and the year 2005 have seen several publications on congenital heart disease, with studies performed in the fetus, infant, child and adult. In the fetus, cardiac malformations as well as rhythm or conduction defects have been revealed, cardiomyopathies with diverse origins. In infants affected by congenital heart disease, respiratory complications due to respiratory syncitial virus can be serious, and vaccination is therefore recommended for poorly tolerated cardiopathies. Myocardial non-compaction has been the subject of several publications with an excellent review by Freedom

Favorable cardiac remodeling in response to treatment in obstructive hypertrophic cardiomyopathy: a current appraisal.

Hypertrophic cardiomyopathy is a heterogenous genetic condition caused by myocyte disarray with varying degrees of interstitial fibrosis. Traditional management strategies focused on symptom mitigation and sudden cardiac death prevention among high-risk patients. The only disease-modifying treatments have historically been septal reduction therapy or heart transplantation. However, cardiac myosin inhibitors have emerged as promising novel pharmacotherapies. Emerging evidence indicates that cardiac myosin inhibitors as well as surgical myectomy result in not only symptomatic benefit and reducti

Variations in current clinical practice of postoperative pericardial effusion: a questionnaire study.

OBJECTIVE: Postoperative pericardial effusion (PPE) occurs frequently after cardiac surgery, potentially leading to life-threatening cardiac tamponade. Specific treatment guidelines are currently lacking, possibly leading to variations in clinical practice. Our goal was to assess clinical PPE management and evaluate variation between centres and clinicians. METHODS: A nationwide survey was sent to all interventional cardiologists and cardiothoracic surgeons in the Netherlands, regarding their preferred diagnostic and treatment modality of PPE. Clinical preferences were explored utilising four

[Current respective role of surgery and permanent endocavitary stimulation in the treatment of hypertrophic cardiomyopathies].

Surgery and cardiac pacing are the two main non-drug treatments for hypertrophic cardiomyopathy. Various surgical techniques have been proposed over the last 35 years: myotomy, myotomy-septal myomectomy, isolated mitral valve replacement, heart transplantation. Patients eligible for surgery are those with severe symptoms (NYHA stage III or IV) and refractory or no longer responding to drug treatment. The choice between the various techniques is based on morphological and haemodynamic criteria (significant subaortic gradient associated with increased septal thickness, severe and/or organic mitr

The anesthesiologist's response to a low-output state after cardiopulmonary bypass: etiologies and remedies.

Artifacts or mechanical problems may cause data which suggest poor myocardial performance during emergence from cardiopulmonary bypass (CPB). Transducer and monitoring equipment malfunctions, damping of the arterial blood pressure tracing, effects of drugs, hypercarbia, inordinately high intrathoracic pressure, cardiac tamponade, and others are all possible culprits. It is important to have a systematic plan for evaluating and interpreting the signs and data that are evident. Causes of hypotension after CPB include low hematocrit, hypercarbia, sympathetic inhibition, vasodilator action, anaphy
